An Air Canada Embraer ERJ 190-100 IGW (C-FHNL/ACA686) from Toronto/Lester B. Pearson International, ON (CYYZ) to St. John's International, NL (CYYT) that was on final approach for Runway 29, was pulled up by air traffic control (ATC) due to the preceding arrival, an American Airlines Boeing 767-323 (N350AN/AAL9648) from New York/John F. Kennedy International, NY (KJFK) to St. John's International, NL (CYYT), still being on the runway.

Incident at ST. JOHN'S INTL NL (CYYT)

An Air Canada Embraer ERJ 190-100 IGW (C-FLWE/ACA686) from Toronto/Lester B. Pearson Int'l, ON (CYYZ) to St. John's Int'l, NL (CYYT) was on approach of Runway 29 with a Jazz Bombardier DHC-8-402 (JZA8958) from St. John's Int'l,NL (CYYT) to Halifax/Stanfield Int'l, NS (CYHZ) waiting on the threshold of Runway 29 for departure due to a Air Mobility Command Lockheed C-130 (Hercules) (71468T) from Portsmouth Int'l Airport , NH (KPSM) to St. John's Int'l, NL (CYYT) exiting the runway. Tower cleared JZA9958 for takeoff, then cleared ACA686 to land. ACA686 went into a missed approach due wind shear. ACA686 did not acknowledge the control tower's traffic update that JZA9958 had departed. When the ACA686 came back on frequency, traffic was passed. ACA686 came around for another approach and landed without incident.

Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ)

An Air Georgian Beech 1900D (GGN7254) from Toronto, ON (CYYZ) to Kingston, ON (CYGK) departed Runway 05 simultaneously with an Air Canada Embraer E190 (ACA686) from Toronto, ON (CYYZ) to St John's, NL (CYYT) off Runway 06L with each aircraft assigned their respective standard instrument departures (SID). GGN7254 appeared to be converging with ACA686. GGN7254 was queried as to their heading which they replied as "heading 065" as opposed to the 047 correct diverging heading. GGN7524 was corrected and commenced to diverge by 10 degrees as opposed to converging by 8 degrees. No further incident.

Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ) (ATM - NAVAIDS/radar)

An Air Canada Rouge Airbus A319-114 (C-FZUG/ ROU1865) from Orlando, FL (KMCO) to Toronto, ON (CYYZ), was instructed to initiate a missed approach to runway 06L as the controller was resolving an issue with an airport surface detection equipment (ASDE) radar return appearing on runway 06L and an Air Canada Embraer ERJ 190-100 IGW (C-FHNP/ ACA686) from Toronto, ON (CYYZ) to St John's, NL (CYYT) was in position to takeoff runway 06L. ROU1865 landed CYYZ runway 06L at 1916Z, without further incident.

At time 0430Z, KLM Royal Dutch Airlines (KLM644), an eastbound flight advised of an emergency locator transmitter (ELT) transmission beginning at 457103N 542812W until 480052N 540226W. The ELT was also reported by 4 other aircraft on the same track as the KLM644 and Air Canada (ACA686) inbound to St John's, NL (CYYT). Joint rescue co-ordination centre (JRCC) was advised.

An Air Canada Embraer ERJ 190-100 IGW (C-FHKE/ ACA686) from Montreal, QC (CYUL) to Halifax, NS (CYHZ) at 2359Z - landed at 0017Z.

Incident at MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL)

Peu de temps après le décollage de Dorval (CYUL) pour Washington (KIAD), le pilote du vol ACA686 a déclaré une urgence et a demandé le retour à (CYUL). Il soupçonnait une fuite de carburant et a demandé l`assistance des services d`urgence. A 1747Z, l`avion s`est posé sans autre problème sur la piste 24 droite. Shortly after taking off from Dorval (CYUL) for Washington (KIAD), the pilot of flight ACA686 declared an emergency and asked to return to (CYUL). He suspected a fuel leak and requested assistance from emergency services. The aircraft landed without further incident on rwy 24R at 1747Z.
Mise à jour #1 : Le numéro et la classe d`enquête du BST ont été ajoutés, ainsi que l`immatriculation de l`aéronef. Le BST nous rappporte que peu de temps après son départ, l`équipage a eu un avertissement de déséquilibre de l`essence. L`appareil semblait perdre de l`essence de l`aile gauche. L`équipage a déclaré une urgence et a demandé un retour immédiat à Dorval. La liste de vérification a été complétée et l`indication est momentanément disparue pour ré-apparaître à environ 12 MN de l`aéroport. L`équipement d`urgence a été requis. L`appareil a atterri sans autre problème. À la vérification le problème s`est révélé en être un d`indication; l`appareil n`ayant pas perdu d`essence. Le service d`entretien a remplacé les jauges dans les réservoirs #2 et #3 et une vérification de continuité de filage s`est avérée sans faute. Le module de contrôle de l`essence (FSCU-Fuel system computor unit) a aussi été remplaçé. L`appareil a été remis en service sans autre problème. Update #1: The TSB investigation number, class and aircraft registration have been added. The TSB reported that shortly after departure, the crew received a fuel imbalance warning. The aircraft appeared to be losing fuel from the left wing. The crew declared an emergency and requested clearance to return immediately to Dorval. The checklist was completed and the imbalance indicator turned off momentarily, but came on again close to 12 NM from the airport. Emergency equipment was required. The aircraft landed without further incident. Upon checking, it was revealed that there had just been a problem with the indicator: the aircraft had not lost any fuel. The maintenance service replaced the gauges in tanks #2 and #3 and upon checking the indicator, no defects were found. The FSCU was also replaced. The aircraft was returned to service without further incident.
