Incident at MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL) (Aerodrome, runway or taxiway shutdown)

After landing, Airbus A330-343 (C-GHKW/ACA669) from Halifax/Stanfield International, NS (CYHZ) to Montréal/Pierre-Elliott-Trudeau International, QC (CYUL) became immobile on Taxiway B2 and reported a loss of hydraulic fluid. 4 other aircraft were directed to Runway 24L as a result. Taxiway B2 was closed until 2330Z, resulting in a request for additional separation between arrivals for Runway 24R (5 nautical miles instead of 3 nautical miles).

Incident at MAIRE INTERSECTION

While inbound for CYUL on standard terminal arrival (STAR) OMBRE3, an Air Canada Embraer E190 (ACA669), from Halifax (CYHZ), NS, to Montréal (CYUL), did not comply with the altitude restriction at MAIRE. Instead of crossing MAIRE at 10 000 ft or lower, ACA699 crossed the intersection at 17 000 ft. Impact: ACA669 was instructed to fly circuits in order to lose altitude.

The pilot then declared PAN PAN and specified that no emergency vehicle was needed. The aircraft landed without incident at 2258Z.

Incident at HALIFAX / STANFIELD INTL NS (CYHZ) (FOD (foreign object debris))

An Air Canada Embraer ERJ 190-100 IGW (C-FHIU/ ACA669) from Halifax, NS (CYHZ) to Montreal, QC (CYUL), after being cleared for takeoff Runway 14, barely started to roll and then asked to backtrack to the threshold to depart again. Tower approved the request and asked if any assistance was required. ACA669 advised no assistance required, advised possible foreign object debris (FOD) was the reason for the rejected takeoff and departed without incident at 2216Z.

Incident at HALIFAX / STANFIELD INTL NS (CYHZ)

A Porter Airlines de Havilland DHC 8 400 (POE272) from Halifax, NS (CYHZ) to Ottawa, ON (CYOW) and an Air Canada Airbus A319 (ACA669) from Halifax, NS (CYHZ) to Montreal, QC (CYUL) both called ready for departure runway 32, both aircraft stepped on each other on the frequency. ACA669 instructed to line up runway 32, the aircraft passed the hold short line and became no radio (NORDO). An Air Georgian Beech 1900 (GGN747342) from Fredericton, NB to Halifax, NS (CYHZ) was approximately 4 NM final runway 32 and was re-cleared for runway 23. COMM was quickly re-established with ACA669 (approximately 1 minute) GGN7342 landed runway 23 at 2116Z. POE272 and ACA669 departed without further incident.

ACA669, an Embraer 190 operated by Air Canada, was on an IFR flight from Halifax (CYHZ) to Montréal/Trudeau (CYUL). At 2355Z, while the aircraft was on approach for Runway 24L, the crew was instructed to conduct a go-around because ACA1177, an Airbus A-320 operated by Air Canada, was on the runway threshold. ACA1177 could not take off due to the slow climb of CPZ5742, an Embraer 170 operated by Compass Airlines, which had taken off before it.

Incident at MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL)

According to TSB report #A12Q0097 : The Embraer 190, C-FLWH, operated by Air Canada as flight ACA669 was at the gate deplaning when a tailpipe fire was reported from the number 1 engine (CF34-10E5A1). The remaining 12 passengers were deplaned via the main entrance door. The engine was motored and the flames subsequently extinguished. ARFF was also called. The fire was caused by residual oil that had collected in the tail pipe. The engine has been replaced and the event remains under investigation by the company.

Incident at MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL)

Update #1: The TSB number and class of investigation were added. According to TSB report #A11Q0236: ACA669, an Embraer 175 operated by Air Canada, was on an IFR flight from Halifax/Stanfield Int'l (CYHZ), N.S., to Montréal Pierre Elliott Trudeau (CYUL), Que., with 62 passengers and 4 crew members on board. While the aircraft was on approach, the flaps were selected to the ?Flap 2? position, and the engine indications and crew alerting system (EICAS) issued the ?Slat Fail? message. The crew completed the checklist before landing the aircraft without incident with the flaps at 5° and the slats at 15°. No emergency was declared, and there were no injuries.
C-FEIX, an Embraer 170 operated by Air Canada as flight number ACA669, was on an IFR flight from Halifax (CYHZ) to Montréal/Trudeau (CYUL). At 2335Z, the crew reported a problem with the flaps and requested to abort the approach. No emergency was declared and no equipment was requested. The aircraft landed without incident at 2353Z. No impact on ATC operations.

ACA669, an Embraer E190 operated by Air Canada, was conducting an instrument flight rules (IFR) flight from Halifax (CYHZ) to Montréal/Trudeau (CYUL). The pilot reported that the aircraft was pulling up due to a mechanical problem. No emergency was declared and priority was not requested. The aircraft landed at 2313Z on runway 24R without further incident; however, it caused a delay of 2-3 minutes for three other aircraft. **** *** **** ACA669, un Embraer E190 exploité par Air Canada, effectuait un vol selon les règles de vol aux instruments (IFR) depuis Halifax (CYHZ) à destination de Montréal/Trudeau (CYUL). Incident at GREATER MONCTON ROMEO LEBLANC INTERNATIONAL AIRPORT

The aircraft was vectored for a right hand visual approach to runway 29. At around 5 miles NE of the airport at about 3,800 feet, the pilot reported the airport in sight, was cleared for the visual and changed to Moncton tower. The Airport Controller detected that the aircraft WAS NOT on final for runway 29 and, when queried, the pilot realized his error, corrected the alignment and landed safely. Recorded radar data indicated that the aircraft had continued on a course towards the VOR, located 6.2 miles E of Moncton and had then turned in towards the airport at 1.5 miles from the VOR. It appeared that the pilot had aligned the aircraft with the four lane highway leading from Shediac to Moncton, instead of runway 29. TSB evaluating.

Accident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ)

The aircraft arrived from Moncton and was being marshalled into gate 72. Two marshallers (positioned near the aircraft's nose and starboard wing) were guiding the aircraft in. The starboard wing marshaller attempted to signal the nose marshaller to have the aircraft stopped but the nose marshaller did not see this signal. The aircraft's starboard wing tip contacted an air conditioning truck, incurring minor damage. Company maintenance will replace the wing tip. Nothing further in TSB Initial Notification (#A97O0159)

An unidentified VFR target was observed by the north tower, three miles west direct of the the threshold of runway 06L. ONT337 was held in position on runway 24R and ACA669 was overshot. The target then went toward runway 06R and south tower had to stop a departure climbing out at 1,100 feet. Ops. impact -- aircraft was identified as NLU, and landed at Toronto (City Centre) Airport.

ACA669 ENROUTE FROM MONCTON TO TORONTO AT 21500' WAS ALSO DEVIATING DUE TO THUNDERSTORMS. THE CREWS REFUSED TURNS SUGGESTED BY THE SECTOR CONTROLLER & ACA669 PASSED WITH 500' VERTICAL SEPARATION & 2 MILES HORIZONTAL SEPARATION. BOTH CREWS HAD THE OTHER AIRCRAFT IN SIGHT. TIME - 2135Z.
