Incident at À la verticale du point de compte-rendu sur demande MAIRE

ACA655, an Embraer 190 operated by Air Canada, was on an IFR flight from Halifax (CYHZ) to Montréal/Trudeau (CYUL). While the aircraft was flying over the MAIRE on-request reporting point at an altitude of 6 000 ft, the crew reported having encountered wake turbulence from AFR346, an Airbus A-380 that was 9.3 NM ahead of it. The pilot reported roll movement of 20?25°.

Incident at MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL) (Aerodrome, runway or taxiway shutdown)

Update #1: The TSB number and class of investigation were added. The aircraft?s registration was added. According to TSB report #A11Q0210: An Embraer E190, operating as flight number ACA655 (C-FFYM), took off from Halifax (CYHZ) for Montréal/Trudeau (CYUL). While the aircraft was on approach, the crew reported a problem with the flaps and requested the presence of emergency vehicles. The aircraft landed without incident.
ACA655, an Embraer 190 operated by Air Canada, was on an IFR flight from Halifax (CYHZ) to Montréal/Trudeau (CYUL). At 2130Z, the crew declared an emergency due to a problem with the flaps and requested that emergency vehicles be on site upon arrival. The aircraft landed without incident at 2143Z. Impact: Runway 24R was closed from 2143Z until 2148Z. The Trenton Rescue Co-ordination Centre (TR RCC), the National Operations Centre (NOC) and Civil Aviation Contingency Operations (CACO) were notified.

Incident at ST. JOHN'S INTL NL (CYYT)

ACA655, Embraer ERJ 190, after being cleared for takeoff in St. John?s (CYYT), reported an indication and the takeoff clearance was cancelled. No assistance was required. The flight crew then reported the loss of nose wheel steering. The aircraft exited the runway. The flight subsequently departed for Halifax (CYHZ) at 19:55Z.

ACA655, an Embraer 190 departing St. John?s (CYYT) enroute to Halifax (CYHZ) reported a possible bird strike on departure from Runway 34. The remains of two Plovers were found. The pilot was advised. No impact on operations.

Incident at HALIFAX / STANFIELD INTL NS (CYHZ) (Diversion, Declared emergency/priority, Conflict - loss of separation, Alleged Canadian Aviation Regulations (CARs) infraction)

Trend 25-26, F18, enroute from Spangdahlem Air Base (ETAD) to Shaw Air Force Base (KSSC), declared an emergency and requested clearance to land in Halifax (CYHZ). Trend 25-26 was cleared to 10,000 ft. and issued vectors. ACA655, Embraer E190, enroute from Halifax to Montreal (CYUL) was departing from Runway 32 in Halifax with a clearance to climb to 9,000 ft. The aircraft was cleared direct to Montreal, away from the formation. Trend 25 was issued vectors to turn behind ACA655 but continued before starting the turn and continued descent to 9,600 ft. This resulted in less than the minimum required separation between the aircraft.

ACA655, Embraer E190, enroute from St. John?s (CYYT) to Halifax (CYHZ), after receiving wind shear warnings for Runway 23, diverted to Montreal (CYUL). The aircraft was later rerouted to St. John?s by the company dispatch.

WJA798, Boeing 737 7CT, enroute from Toronto (CYYZ) to Halifax (CYHZ), conducted a tower controller initiated overshoot on approach to Runway 14 due to insufficient spacing with ACA655 landing on Runway 23. Weather was VFR and both aircraft were under tower control. WJA798 landed at 23:09. Nil TSB.

ACA655, Embraer ERJ190, enroute from St. John?s (CYYT) to Halifax (CYHZ), was cleared to taxi for Runway 29 and to hold short of the runway. The aircraft crossed the hold short line with another aircraft on the final approach. The traffic was advised and continued the approach, landing without incident. Nil TSB
UPDATE TSB: TSB status updated to "Nil". There was no risk of collision as the runway is 800 ft away

Incident at HALIFAX / STANFIELD INTL NS (CYHZ) (Aerodrome - runway or taxiway surface condition)

ACA674, Embraer ERJ 190, enroute from Ottawa (CYOW) to Halifax (CYHZ) landed and taxied to the apron followed by ACA655, ERJ 190, enroute from St. John?s (CYYT) to Halifax. Both aircraft stopped to await gate assignments but could not resume movement due to icy conditions. As a result four subsequent arrivals and three departures were delayed (on the ground) no less than 60 minutes due to the gridlock. The delayed aircraft were ACA674, ACA655, ACA608, ACA612, ACA664, ACA256 and departures CME320, CJA9800, MAL7054.

The Montréal area control centre notified ACA655 that its transponder was transmitting intermittently.

Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ) (Conflict - potential)

ACA655 was cleared to descend to 8000 feet and was descending through 9000 feet when the pilot responded to a TCAS RA and climbed to 10000 feet. the traffic was CGMMA just levelling at 7000 feet after a rapid climb. No conflict and no impact on operations
