Pearson, ON (CYYZ) was instructed to overshoot Runway 05 due to the previous arrival, a Jazz de Havilland DHC-8-400 (JZA8656) from London, ON (CYXU) to Toronto/Lester B. Pearson, ON (CYYZ) still exiting the runway. No operational impact.

An Air Canada Embraer ERJ 190-100 IGW (ACA623) on a flight from Halifax/Stanfield, NS (CYHZ) to Toronto/Lester B. Pearson, ON (CYYZ) was struck by a laser emanating from the Steeles and Bayview area. The flight landed Runway 24R at 0250Z without incident. No operational impact.

Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ) (Aerodrome, runway or taxiway shutdown)

An Air Canada Airbus A320 (ACA623) from Halifax, NS (CYHZ) to Toronto, ON (CYYZ) landed Runway 15R. The aircraft was observed on airport surface detection equipment (ASDE) to depart west side of runway. The aircraft reported having left the runway and then returned to surface. 1D was called out. An American Airlines Airbus A319 (AAL1383) and an Air Canada Embraer E190 (ACA1118) were overshot by tower. All other aircraft were pulled off 15R approach and moved to 15L. Runway was closed by NOTAM until 1700Z. Numerous aircraft were delayed for departure until obstacle protection surface (ops) change at 0532Z when weather permitted to land Runway 05 and depart Runway 06L.
UPDATE: TSB Report#A17O0025: C-FDRP, an Airbus 320-211 aircraft operated by Air Canada, was conducting flight ACA623 from Halifax/Stanfield Intl, NS (CYHZ) to Toronto/Lester B. Pearson Intl, ON (CYYZ). During the landing sequence on Runway 15R at CYYZ, the aircraft travelled through the grass on the western side of the runway before coming to a stop on the runway centerline. The aircraft sustained minor damage; however, there were no reported injuries.

An American Airlines Airbus A319 (AAL1383) from Miami, FL (KMIA) to Toronto, ON (CYYZ) was instructed go around Runway 15R due to an Air Canada Airbus A320 (ACA623) stopped on runway. The aircraft on approach Runway 15L elected to go around due to wind shear warning, the aircraft landed Runway 05 safely at 0536Z.

Incident at HALIFAX / STANFIELD INTL NS (CYHZ)

Update 196609-V2: An Air Canada Embraer ERJ 190-100 IGW (C-FHJU / ACA623) from Halifax / Stanfield, NS (CYHZ) to Toronto / Lester B. Pearson, ON (CYYZ) reported possible problem shortly after departure. The aircraft levelled off at 7000' and then requested a return to CYHZ. The aircraft then declared Mayday and emergency and reported unknown smoke in the cockpit. The aircraft returned to CYHZ and landed safely at 0134Z. 70 souls on board (SOB) and 7 tons of fuel. Air Canada (ACA620) pulled from approach in order to give ACA623 priority. Joint Rescue and Coordination Centre (JRCC) Halifax advised. The aircraft remained on the Runway for inspection from fire services and then taxied off the Runway at 0154Z.
An Air Canada Embraer ERJ 190-100 IGW (C-FHJU / ACA623) from Halifax / Stanfield, NS (CYHZ) to Toronto / Lester B. Pearson, ON (CYYZ) shortly after departure (10NW) of CYHZ ACA623 declared an emergency (smoke). Aircraft returned to CYHZ and landed without further incident at 0134Z. Emergency response services (ERS) responded to the incident.
UPDATE: TSB#A16A0003: The Air Canada Embraer 190 aircraft, C-FHJU, was operating as flight ACA623 from Halifax/Stanfield Intl, NS (CYHZ) to Toronto/ LBPIA, ON (CYYZ). In the initial climb through 1 500 feet, the crew noted a strong smoke/electrical odor. The crew and passengers experienced eye and throat irritation. The crew donned their oxygen masks and declared a Mayday. The aircraft returned to Halifax and landed uneventfully. The operator?s maintenance found that the number 2 pack had internal damage and was unusable. The number 2 air cycle machine had inlet blade tip damage, and the heat exchanger was contaminated. The aircraft has been released to service on a temporary MEL.

Incident at 60nm E EBONY INTERSECTION

UPDATE: TSB#A15A0091: The Air Canada Embraer E190 (C-FHLH) was operating as ACA623 on a flight from Halifax/Stanfield, NS (CYHZ) to Toronto/LBPIA, ON (CYYZ). While in cruise at FL380, a BLEED 2 LEAK EICAS caution message was generated. The QRH was completed, a PAN PAN was declared, and the flight descended to FL300. The flight continued as per QRH guidance and landed at CYYZ without incident. The TSB is expecting a report from the operator's maintenance.
An Air Canada Embraer E190 (ACA623) from Halifax, NS (CYHZ) to Toronto, ON (CYYZ), climbing to FL380, reported problem with "bleed air" system (cabin pressure) and requested descent to FL300. PAN PAN PAN declared, no emergency. Aircraft stabilized at FL300 and continued to CYYZ. No impact.

At 0105Z, a loader (LDR105) became stuck on the edge of Runway 32 at Halifax, NS (CYHZ) and was there until 0131Z. Impact: an Air Canada Boeing 767 300 (ACA620) from Toronto, ON (CYYZ) to Halifax, NS (CYHZ) was instructed to initiate an overshoot of Runway 32 and delayed approximately 26 minutes. An Air Canada Airbus A319 (ACA623) from Halifax, NS (CYHZ) to Toronto, ON (CYYZ) was delayed approximately 5 minutes.

The Air Canada Embraer ERJ-170-200-SU aircraft (operating as flight ACA623) was nearing the conclusion of a scheduled IFR flight from Halifax (CIA) (CYHZ) to Toronto (LBPIA) (CYYZ). NAV CANADA staff at Toronto ACC reported that a laser strike occurred when the aircraft was 10 miles final for runway 06L, descending through an altitude of 3,000 feet. Peel Regional Police were advised. Ops. impact -- none.

The Air Canada Airbus A-321 aircraft (operating as flight ACA725) was on a scheduled IFR flight from La Guardia Airport New York, New York, USA (KGLA) to Toronto (LBPIA) (CYYZ). The Air Canada Embraer E190 aircraft (operating as flight ACA623) was on a scheduled IFR flight from Halifax (CYHZ) to Toronto (LBPIA) (CYYZ). The flight crew of ACA623 were on the localizer at 4,000 feet cleared for the approach to runway 24R. The flight crew of ACA725 were at 4,000 feet and were instructed to fly heading 030. The heading was read back correct. ACA725 was observed to turn through heading 030 and were queried regarding their heading and relied they were turning to 330. ACA725 was then instructed to make an immediate left turn to heading 240.

Update #1: The TSB investigation class and number were added. The event location was specified. According to TSB report #A11Q0124: The Embraer E190, registration C-FHNX, operating as flight ACA623 from Halifax/Stanfield Int'l (CYHZ) to Toronto/LBPI (CYYZ) was diverted to Montreal/Pierre Elliott Trudeau Int'l due to a sick passenger. An emergency was declared while south of the Beauce VOR (VLV).
Air Canada Embraer 190 C-FHNX, under flight number ACA623, was IFR from Halifax (CYHZ) to Toronto (CYYZ). At 0110Z, the pilot declared a medical emergency and asked to be diverted to Montreal/Trudeau (CYUL) due to the passenger's state of health. The aircraft landed without incident on runway 24L at 0133Z.

JZA896 landed on Runway 23 in Halifax (CYHZ) and reported a possible small animal near touchdown. The runway was inspected. ACA623 departure and ACA616 arrival chose to wait. Four other IFR aircraft were delayed between 10 and 15 minutes each.

A bear encroached on Runway 23 in Halifax (CYHZ). Aircraft were advised to use Runway 14. JZA887 and ACA623 were delayed. Several other aircraft experienced minor delays as well.
UPDATE Aerodrome Safety: A live trap has been set and a safety bulletin issued.

Incident at HALIFAX / STANFIELD INTL NS (CYHZ) (ATM - ILS irregularity)

ASQ5615, RJ700, enroute from Detroit (KDTW) to Halifax (CYHZ), on an ILS14 approach reported no signal on the Glide Path. There were no indications on the ILS monitor but departing ACA623 confirmed the loss of signal. As ASQ5615 overshot, the flight crew reported that the glide path seemed to have returned. The flight completed an ILS 23 approach and landed without further incident.

FLG3853, RJ100, enroute from New York (KJFK) to Halifax (CYHZ), conducted a missed approach to Runway 14, as instructed by the tower, due to insufficient spacing from ACA623, Boeing 767-300 that was backtracking slowly on the runway for a departure to Toronto (CYYZ). FLG3853 landed with out further incident at 02:30Z.

Incident at ST. JOHN'S INTL NL (CYYT) (Overshoot/missed approach, Incursion - runway - aircraft, Conflict - loss of separation, ATS operating irregularity)

ACA623 A320 departing St. John's for Halifax was given clearance to taxi onto Runway 29 with a departure restriction not to depart until CME174 had landed. Inbound was CME174 a BE1900 from Gander was on a 5nm final for Runway 29 and cleared for an ILS approach Runway 29. IFR weather conditions existed at the time. The FSS advised CME174 of traffic on the Runway and as a result CME174 excuted a missed approach. ACA623 departed at 0831 and CME174 landed at 0838z. Appropriate separation minimum not insured resulting in an operating irregularity.
TSB Update July 31, 2001 A01A0086: The Air Canada Airbus 320, callsign ACA623, was issued a clearance for an IFR flight to Halifax. The clearance included a departure restriction from Runway 29 valid after a Prince Edward Air Beech 1900, callsign CME174, landed on Runway 29. The Beech 1900, inbound from Gander, was on a 5 mile final and cleared for an ILS approach when the FSS advised the crew that the Airbus 320 was on the runway. The crew of the Beech 1900 executed an overshoot and the Airbus 320 then departed at 06:01 NDT. The Beach 1900 landed safely on Runway 29 at 06:08NDT. IFR weather conditions existed at the time and required separation minimum was not assured. Nav Canada is conducting a fact finding board.
