Incident at 150NM SYDNEY / J. A. DOUGLAS McCURDY NS (CYQY)

Update TSB #A18A057: C-FMZR, an Embraer ERJ 190-100 aircraft operated by Air Canada, was conducting flight ACA687 from St. John?s Intl, NL (CYYT) to Toronto/Lester B. Pearson Intl, ON (CYYZ). After reaching cruise altitude, the number 2 engine oil pressure indication began decreasing. The flight crew followed the QRH and shut the engine (General Electric CF34-10E5A1) down. The flight crew declared a PAN PAN and requested a diversion to Halifax/Stanfield Intl, NS (CYHZ) where the aircraft landed without further incident. The operator?s maintenance is investigating.
An Air Canada Embraer ERJ 190-100 IGW (C-FMZR/ACA687) from St. John's, NL (CYYT) to Toronto, ON (CYYZ) declared a PAN PAN and advised that they were doing an engine shutdown. They requested diversion to Halifax, NS (CYHZ). The Area Control Centre (ACC) duty manager, Rescue Coordination Centre (RCC), National Operations Centre (NOC) and the Transportation Safety Board (TSB) were all notified via phone. Aircraft landed safely at CYHZ on Runway 23 at 0149Z. Runway inspection after landing caused all runways to be unavailable from 0148Z to 0203Z. Operational impact: A WestJet Encore Ltd. de Havilland DHC-8-402 (WEN3426) from Halifax, NS (CYHZ) to St. John's, NL (CYYT) and an Air Canada Boeing 737 MAX 8 from Toronto, ON (CYYZ) to Halifax, NS (CYHZ) ACA620 were delayed approximately 10 minutes.

Update 196609-V2: An Air Canada Embraer ERJ 190-100 IGW (C-FHJU / ACA623) from Halifax / Stanfield, NS (CYHZ) to Toronto / Lester B. Pearson, ON (CYYZ) reported possible problem shortly after departure. The aircraft levelled off at 7000' and then requested a return to CYHZ. The aircraft then declared Mayday and emergency and reported unknown smoke in the cockpit. The aircraft returned to CYHZ and landed safely at 0134Z. 70 souls on board (SOB) and 7 tons of fuel. Air Canada (ACA620) pulled from approach in order to give ACA623 priority. Joint Rescue and Coordination Centre (JRCC) Halifax advised. The aircraft remained on the Runway for inspection from fire services and then taxied off the Runway at 0154Z.
An Air Canada Embraer ERJ 190-100 IGW (C-FHJU / ACA623) from Halifax / Stanfield, NS (CYHZ) to Toronto / Lester B. Pearson, ON (CYYZ) shortly after departure (10NW) of CYHZ ACA623 declared an emergency (smoke). Aircraft returned to CYHZ and landed without further incident at 0134Z. Emergency response services (ERS) responded to the incident.
UPDATE: TSB#A16A0003: The Air Canada Embraer 190 aircraft, C-FHJU, was operating as flight ACA623 from Halifax/Stanfield Intl, NS (CYHZ) to Toronto/ LBPIA, ON (CYYZ). In the initial climb through 1 500 feet, the crew noted a strong smoke/electrical odor. The crew and passengers experienced eye and throat irritation. The crew donned their oxygen masks and declared a Mayday. The aircraft returned to Halifax and landed uneventfully. The operator?s maintenance found that the number 2 pack had internal damage and was unusable. The number 2 air cycle machine had inlet blade tip damage, and the heat exchanger was contaminated. The aircraft has been released to service on a temporary MEL.

Incident at HALIFAX / STANFIELD INTL NS (CYHZ) (Aerodrome, runway or taxiway shutdown)

After landing Runway 32, a Jazz Bombardier CL-600-2B19 Series 100 (C-GQJA / JZA8896) from Boston, MA (KBOS) to Halifax, NS (CYHZ) got stuck exiting the runway at Taxiway H, requiring a tow. An Air Canada Boeing 767 300 (ACA620) from Toronto, ON (CYYZ) to Halifax, NS (CYHZ) was on final for Runway 32 and was overshot by the Tower. JZA8896 was removed from the runway at 0234Z. As a result, ACA620 diverted to Montreal, QC (CYUL). One departure and several arrivals were delayed up to 45 minutes.
UPDATE: TSB Report#A15A0008: The Jazz CRJ 100-200 (C-GQJA), operating as Jazz 8896, was on a flight from Boston (KBOS) to Halifax (CYHZ). After landing on Runway 32, the aircraft taxied onto Taxiway Hotel and slid into a snowbank on the right side of the taxiway. Both the right hand main gear and the nose gear departed the taxiway. Several aircraft were affected by delays in arrivals and departures and one aircraft diverted while the aircraft was stuck. Maintenance cleared the snow and towed the aircraft to the gate. There were no injuries to passengers and crew, and upon inspection, the aircraft did not sustain any damage.

ACA620, Boeing 767-300, enroute from Toronto (CYYZ) to Halifax (CYHZ) reported ACA624 in sight and was cleared for the visual approach to follow ACA624. Less than a minute later, ACA620 questioned the traffic to its one o'clock position. The controller determined that ACA620 was actually following the aircraft ahead of ACA624. The controller cancelled the visual approach clearance and took action to ensure that separation was maintained. No loss of separation occurred.

ACA620, a Boeing 767 enroute from Toronto (CYYZ) to Halifax (CYHZ) completed an overshoot on the Flight Management System (FMS) transition to Runway (RWY) 32 into a Visual flight Rules (VFR) right circuit under tower control. The pilot commented that the crosswind and being a little high on the approach were factors. The wind at the time was approximately 360 degrees at 20 knots gusting to 27 knots. There was no impact on operations.

ACA620 while taxiing for departure advised ground they may have a suspicious package on board and wanted to hold position. Airport Operations Control Centre (AOCC) was advised. Approximately 10 minutes later Air Canada advised that the package was "peanut butter" and that the crew should dispose of it. After a few minutes the crew elected to return to the gate. The Air Canada Boeing 767-300 series aircraft (operating as flight ACA620) was preparing to depart on a scheduled IFR flight from Toronto (LBPIA) (CYYZ) to Halifax (Stanfield) International Airport (CYHZ). The United Air Lines, Inc. Airbus A-320 aircraft (operating as flight UAL1108) was landing at the conclusion of a scheduled IFR flight from Chicago (O'Hare) International Airport, IL (U.S.A.) (KORD) to Toronto (LBPIA) (CYYZ). The flight crew of ACA620 had been cleared to line up and wait on runway 24R. UAL1108 was landing on runway 24L and was instructed to exit onto taxiway DELTA4 and cross runway 24R. UAL1108 crossed runway 24R and was clear of the runway as the ACA620 flight crew reported rolling runway 24R without a take-off clearance. Ops. impact -- unknown.

Incident at 50NM au sud-est de Montréal/Trudeau (CYUL)

Air Canada Boeing 767-300 ACA620 was IFR from Toronto (CYYZ) to Halifax (CYHZ). About 50 nautical miles southeast of Montreal/Trudeau (CYUL), the crew asked to be rerouted to CYUL because of a hydraulic problem. No emergency was declared. The aircraft landed without incident 0025Z. There was no impact on ATC operations.
Update #1: The TSB occurrence number and class of investigation were added. The aircraft?s registration was added. According to the TSB report #A08Q0157: The Boeing 767-300 operated by Air Canada was en route from Toronto (CYYZ) to Halifax (CYHZ) when the flight crew noticed a hydraulic leak on the right side. The flight crew carried out the appropriate checklist, declared an emergency (PAN PAN), and requested a diversion to Montréal/Trudeau (CYUL). The aircraft landed without incident on Runway 24R, and cleared the runway via Taxiway BRAVO 2. The hydraulic leak was coming from the left elevator?s central power control actuator (PCA), which is actioned by the right hydraulic system. The PCA was replaced in accordance with the aircraft maintenance manual (AMM) 27-31-05-401 procedures.

An ELT signal was reported by: WJA683 20 S YVV FL230 at 2245Z, ACA462 440639/0782013 FL250 at 2256Z. Signal lost 441130/0774918, ACA620 30 E YYZ FL165 at 2315Z. The signal was tracked to Peterborough Airport (CYPQ) and parked aircraft identified as a Piper PA28-140. ELT was switched off by RCC. The owner was contacted. The NARCO ELT 10 had been installed Sept 22 2008 after it was received back from it's annual inspection. The owner had flown the aircraft on Sept 25th without any activation. The ELT actived on the 26th for unknown reasons.

UPDATE:The preliminary investigation has determined that an operating irregularity did occur when C-GSAB passed in close proximity to ACA 620 resulting in the ACA pilot reacting to the TCAS RA.
ACA620 A320 complied with a TCAS RA involving C-GSAB S61 in the vicinity of Halifax airport. The occurrence continues to be investigated to determine whether an Operating Irregularity occurred. TSB Evaluating
TSB update A04A0064: The Air Canada Airbus 320, flight number ACA620, was cleared to conduct a tight visual left hand approach to Rwy 33. While approximately two miles final, the crew received a TCAS traffic advisory (TA) indicating traffic at 12 o'clock and 100 feet below. The A320 crew had spotted the traffic, a departing VFR Canadian Helicopters Sikorsky S61, and had initiated a climbing right turn as the TCAS resolution advisory (RA) was received. The crew reported that they had not been informed of the traffic. The crew of the S61 was informed of the A320, and had visual contact with the A320 throughout the encounter. Nav Canada is investigating the occurrence to determine if an operating irregularity had occurred.

Once there, the pilot requested permission to return to the gate. Twenty-five minutes after the beginning of the incident, the aircraft finally returned to its departure point.
