Incident at 6NM NE EPSAT Intersection

A US Army, Priority Air Transport Beech 200 (PAT738) from Toronto, ON (CYYZ) to Goose Bay, NL (CYYR) was observed diverting from EPSAT towards AGLUK rather than EPSAT VERTI AGLUK ANCER ALKOB YYR, the route known by Air Traffic Control (ATC). When questioned about the situation, PAT738 confirmed the next point after EPSAT was AGLUK. PAT738 had received and readback Direct EPSAT enroute previously. Impact on operations: Air Canada (ACA409) had to stay at FL260 while directing PAT738 to AGLUK.

Aviation Incident Report: During an absence of apron control exercise, an Air Canada Airbus A321-211 (C-GITU/ACA409), from Montréal (CYUL), QC, to Toronto (CYYZ), ON, pushed back from Gate 48 and taxied in the direction opposite to what is stipulated in the CAP and the CFS. Communications occurred in French only, which also contravenes the Plan of operations in the absence of apron control published in the CFS. Once it was moving in the opposite direction of traffic and in order to avoid waiting for a Porter flight, which was pushing back from Gate 9, ACA409 left the guidance line and taxied in a vehicle corridor located at the far east end of the aeroquay by gates 30 and 28.

The Jazz Aviation Limited Partnership de Havilland DHC-8-102 aircraft (operating as Air Canada Express flight JZA7930) was on a scheduled IFR flight from Toronto (LBPIA) (CYYZ) to Baltimore/Washington (KBWI). The Air Canada Airbus A320 aircraft (operating as flight ACA409) was on a scheduled IFR flight from Montreal (CYUL) to Toronto (LBPIA) (CYYZ). Nav Canada staff at the Toronto ACC reported that the flight crew of JZA7930 reported a TCAS RA at 1640z, 8 miles southeast of CYYZ. JZA7930 was climbing to 7,000ft. Traffic was ACA409 descending to 8,000ft. Targets merged. Traffic had been passed. Separation was not jeopardized. No op impact.

The Air Canada Airbus A-320 aircraft (operating as flight ACA409) was concluding a scheduled IFR flight from Montréal (PETIA) (CYUL) to Toronto (LBPIA) (CYYZ). NAV CANADA staff at Toronto (LBPIA) control tower reported that ACA409 was instructed to overshoot their approach to runway 05 as the landing traffic ahead (TSO517) missed the planned exits off at taxiway HOTEL3 and runway 15L. The aircraft subsequently exited at taxiway BRAVO. Ops. impact -- unknown.

La tour de Dorval (CYUL) a autorisé JZA8721, un Dash-8 d`Air Canada Jazz à atterrir sur la piste 06L alors qu`ACA409, un Airbus 319 d`Air Canada était en position sur la même piste. Lorsque le pilote du Dash-8 a questionné la validité de l`autorisation, le contrôleur s`est apercu de la présence de l`Airbus sur la piste et a initié la remontée de JZA8721. The tower at Dorval (CYUL) cleared Air Canada Dash-8 JZA8721 for landing on rwy 06L, even though Air Canada Airbus 319 ACA409 was in position for the same rwy. When the pilot of the Dash-8 questioned whether it should be given clearance, the controller noticed that the Airbus was on the rwy and instructed JZA8721 to pull up.

Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ)

TSB Occurrence Summary A04O0334: Air Canada flight 409, an Airbus A321 aircraft, was on descent into Toronto LBPIA when the flight crew noticed the oil pressure on the right engine (CFM-56-5B3) was dropping. The flight crew called PAN/PAN/PAN and requested priority for landing. The aircraft landed on runway 33R with the right engine at idle. The engine was shut down immediately after landing. EMS met the flight for the landing. The cause of the oil pressure problem is being investigated by company manitenance.
Air Canada Airbus A321, flight ACA409 declared a PAN emergency inbound to Toronto from Montreal due to low oil pressure on the right engine. Aircraft received priority and landed 33R, few, if any delays.

Narrative provided by the TSB of Canada Daily Notification Log 13-04-2004: A04O0090: During the approach to Toronto/LBPIA, Ontario, the Air Canada Airbus A320 (Flight #ACA409) aircraft flight attendant injured a finger while stowing the galley carts and was unable to perform duties. Upon arrival, the flight attendant was taken to the doctor's office for examination.

Air Canada Boeing 737 ACA500 requested clearance to descend to FL250 due to light turbulence. The aircraft was cleared to FL290 to maintain separation with Air Canada Airbus 320 ACA409, cleared to climb to FL280. When the aircraft were 2 NM apart, ACA500 received a TCAS alert and advised ATC it was climbing. No loss of separation. ACA500, un Boeing 737 d'Air Canada, a demandé de descendre à 25000 pieds (FL250) à cause de la turbulence légère. L'appareil a été autorisé au FL290 pour espacement avec ACA409, un Airbus 320 d'Air Canada, autorisé à monter au FL280. Toronto/LBPAI (CYYZ) - Air Canada flight 3309 (ACA3309) hit a bird in the flare for runway 24R. GGN995 and ACA409 overshot as a result of the runway being closed. Inspection vehicle found bird body parts on the runway and info relayed to aircraft. Three aircraft moved to runway 23 by arrival. Note from System Safety: bird made contact below Captain's side window - no damage.

Air Canada B767 ACA409 was on a flight from Dorval (CYUL) to Toronto (CYYZ). On take-off at Dorval (CYUL) the pilot reported bird activity on his trajectory. On arrival at Toronto (CYYZ) the pilot noted engine damage. ACA409 un B767 appartenant à la cie. Air Canada effectuait un vol entre Dorval (CYUL) et Toronto (CYYZ). Lors du décollage à Dorval (CYUL) le pilote a rapporté de l'activité aviaire sur sa trajectoire. À son arrivée à Toronto (CYYZ) le pilote a constaté des dommages au moteur.

2 aircraft involved: RJ28, Bombardier Regional Jet CRJ2, on a local IFR flight at Dorval (CYUL). ACA409, Air Canada Boeing B762, IFR departure from Dorval for Toronto (CYYZ). Less separation than the required radar turbulence separation occurred between ACA409 taking off from Rwy 28 and RJ28 pulling up from Rwy 28. The observed separation was 3 to 4 miles whereas the required separation is 5 miles. 2 aéronefs impliqués: RJ28, Regional Jet CRJ2 de Bombardier, en vol local aux instruments (IFR) à Dorval (CYUL). ACA409, Boeing B762 d'Air Canada, départ IFR de Dorval à destination de Toronto (CYYZ). Un espacement moindre que l'espacement radar de turbulence requis s'est produit entre ACA409 qui décollait de la piste 28 et RJ28 qui effectuait une remontée piste 28. L'espacement observé a été de 3 à 4 milles alors que l'espacement requis est de 5 milles.
