Incident at 8NM TORONTO ON VOR/DME (YYZ)

A WestJet Boeing 737 800 (WJA700) from Vancouver Int'l, BC (CYVR) to Toronto/Lester B. Pearson Int'l, ON (CYYZ) received a clearance to 8000 feet. WJA700 received and acknowledged clearance correctly. The aircraft was observed by arrival controller descending through 7800 feet. WJA700 was asked about the cleared altitude and replied they thought they were cleared to 7000 feet. WJA700's descent through 8000 feet brought them within close proximity of an Air Canada Airbus A320 (ACA167) from Toronto/Lester B. Pearson Int'l, ON (CYYZ) to Edmonton Int'l, AB (CYEG) on departure from CYYZ that was climbing to 7000 feet. The controller turned assigned 7500 feet to WJA700 and turned the aircraft eastbound away from ACA167 to avoid further conflict. The closest the two aircraft were was 600 feet and 2.6NM turning away from one another.

Incident at 5245N / 11039W

An Air Canada Airbus A320 (ACA167) from Toronto/Lester B. Pearson Int'l, ON (CYYZ) to Edmonton Int'l, AB (CYEG) called an emergency (PAN PAN) soon after entering the CYEG flight information region (FIR) from Winnipeg center. The pilot said that there was a hydraulic issue and the aircraft required a longer than normal final. The aircraft landed safely at 2100Z and no other aircraft were affected.
Update TSB Report #A18W0140 :C-FPDN, an Airbus 320-200 aircraft operated by Air Canada, was conducting flight ACA167 from Toronto/Lester B. Pearson Intl, ON (CYYZ) to Edmonton Intl, AB (CYEG). During cruise flight in the vicinity of Saskatoon/John G. Diefenbaker Intl, SK (CYXE), the flight crew received a HYD G ENG 1 PUMP LO PR ECAM warning indication for the number 1 engine hydraulic pump. The prescribed actions of the ECAM were carried out, and it was decided that the flight would continue to CYEG. Subsequently, the flight crew received a second ECAM message (HYD G RSVR LO LVL) for low fluid level in the same (green) system. A PAN PAN was declared, and the flight landed at CYEG without further incident. The operator?s maintenance replaced the green hydraulic system accumulator and released the aircraft back to service.

UPDATE: TSB Report#A18W0040: C-FMZB, an Embraer 190-100 aircraft operated by Air Canada was conducting flight ACA167 from Toronto/Lester B. Pearson Intl., ON (CYYZ) to Edmonton Intl., AB (CYEG) with 4 crew members and 66 passengers on board. During descent from cruising altitude inbound to CYEG, the flight crew observed several engine indicating and crew alerting system (EICAS) warnings. The flight crew declared a PAN PAN emergency, and continued to CYEG where the aircraft landed without further incident. The operator?s maintenance replaced the Actuator Input/Output Processor module.

After landing Runway 35R, an Air Canada Embraer E190 (ACA167) from Ottawa, ON (CYOW) to Calgary, AB (CYYC) reported foreign object debris (FOD) on the runway by D4. Several departures were held on the ground pending a runway inspection. The runway inspection found a piece of a tire. At approximately 0250Z, Jazz Dispatch called to advise that a Jazz Bombardier CL-600-2B19 (JZA8223) from Calgary, AB (CYYC) to Yellowknife, NT (CYZF) may have blown a tire while departing Runway 35R at 0024Z.

UPDATE: TSB Report #A14O0047: Air Canada flight AC167, an Embraer 190 aircraft, C-FMZW, was on approach to Ottawa MCIA, when the flight crew received numerous EICAS messages related to the spoilers. The flight crew carried out the appropriate checklists and declared a PAN PAN. The aircraft landed without further incident with ERS on standby. Maintenance replaced power supply unit per AMM 31-41-02, and replaced the NIC processor module 6 per 31-41-04. AMM task 45-45-00-970-805-A was carried out and faults cleared the aircraft was returned to service carried.
On approach to Ottawa, ON (CYOW), an Air Canada Embraer E190 (ACA167) from Halifax, NS (CYHZ) to Ottawa, ON (CYOW) declared a PAN PAN due to brake problems. Emergency vehicles were deployed at CYOW. In preparation for landing, ACA167 requested the longest runway, runway 32. ACA167 landed smoothly on runway 32 at 2313Z. Impact: Runway 14/32 was unavailable from 2313Z to 2317Z. National Operations Centre (NOC) and Civil Aviation Contingency Operations (CACO) was notified.

Incident at THURO

On initial call with Ottawa TCU, an Air Canada Embraer E190 (ACA167) from CYHZ to CYOW reported leaving 9,000 feet in descent for 6,000. Controller requested confirmation as descent to 8,000 feet by procedures is generally issued without coordination. Pilot then confirmed that they had changed the cleared altitude guide inadvertently and that they concur they had been cleared to 8,000 feet. Aircraft did not overshoot the assigned 8,000 feet clearance. No impact on operations.

The Air Canada Embraer ERJ-190-100-IGW aircraft (operating as flight ACA167) was nearing the conclusion of a scheduled IFR flight from Toronto (LBPIA) (CYYZ) to Ottawa (M-CIA) (CYOW). NAV CANADA staff at Montréal ACC reported that the aircraft was flown in an overshoot from the runway 07 ILS approach after the flight crew was unable to capture the glidepath after receiving radar vectors on final very close to the final approach fix (FAF) with a tailwind component of 25 knots. The aircraft subsequently landed at 2317Z. The Air Canada Embraer ERJ-190-100-IGW aircraft (operating as flight ACA197) was on scheduled IFR flight from Ottawa (CYOW) to Toronto (LBPIA) (CYYZ). Nav Canada staff at Toronto ACC advised that the flight crew of ACA197 reported a dead bird on runway 24L. They did not hit the bird, but saw it there. The was no Rapid Response Vehicle available so the Airport Authority Communications Centre dispatched a vehicle from the garage. There were five departure delays off 24L and three arrivals moved to runway 23. Runway 24L was returned at 1302z

UPDATE from TSB Daily Notification Log 05-05-2010 Occurrence Summary A10O0064: After take-off at Ottawa/MCIA on a flight to Calgary, ACA167, an Embraer E190 airplane registration C-FNAX, got a SLAT FAIL message on the EICAS when flaps were retracted. The flight diverted to Toronto/LBPIA. The airplane landed with slat 0 and flap 2 with ARFF on standby. Maintenance found the right-hand slat 2 outboard indicator tripped. The slat actuator was replaced and the airplane returned to service.

Incident at OTTAWA / MACDONALD-CARTIER INTL ON (CYOW)

UPDATE from TSB Daily Notification Log Occurrence Summary A09O0246: The Air Canada ERJ 190 (registration C-FGMF, flight number 167) was on the approach into Ottawa/MCIA when the flight crew received a gear disagree and brake control fault message. A missed approach was performed and a pan-pan was declared as a precaution. The flight crew cycled the gear and returned for landing without further incident. After landing the landing gear system was inspected and lubricated, and air was bled form the up-lock actuator. The system was tested and an emergency extension system test was carried out as per SNL 190 -32-0025.
The Air Canada Embraer EMB190 operating as ACA167 was on a flight from Halifax International Airport, Halifax, NS (CYHZ) to MacDonald - Cartier International Airport, Ottawa, ON (CYOW) when it overshot Runway 07 due to landing gear problems. The flight crew declared PAN PAN PAN and requested the Airport Rescue and Fire Fighting services (ARFF). ACA167 landed safely on Runway 32. ACA838 was delayed briefly.

Incident at vicinity overhead WATTO intersection (Communication navigation surveillance/air traffic system)

The Air Canada Embraer ERJ-190-100-IGW aircraft (C-FNAI. operating as flight ACA167) was on a scheduled IFR flight from Halifax (S.I.A.) (CYHM) to Ottawa (M-CIA) (CYOW). NAV CANADA staff at Montréal ACC reported that on arrival, the flight crew did not follow an instruction issued by ATC. Passing over WATTO intersection, instead of turning left to a heading of 120º as instructed, the aircraft was observed to continue on course on a heading of 240º. Corrective instructions were issued to the flight crew. Ops. impact -- unknown. __________ À l'arrivée, l'équipage n'a pas respecté une instruction émise par ATC. À la verticale de WATTO, au lieu de virer à gauche sur un cap de 120 tel qu'instruit, l'appareil a été observé poursuivant sa course sur un cap de 240. Incident at about 50 miles east of Red Lake

The Air Canada Airbus A-319 aircraft (operating as ACA167) was on a scheduled IFR flight from Ottawa (M-CIA) (CYOW) to Calgary International Airport (CYYC). The aircraft was proceeding westbound at FL360 when the flight crew advised Winnipeg ACC of a possible medical emergency. At 0134Z, the flight crew declared a medical emergency and elected to divert to Winnipeg International Airport (CYWG). Arrangements were made through the company and ACA167 landed on runway 36 at 0205Z. Ops. impact -- no other aircraft were affected.

Incident at OTTAWA / MACDONALD-CARTIER INTL ON (CYOW)

The Skyservice Aviation Inc. British Aerospace Jetstream 3112 aircraft (C-GEOC) was on an IFR flight from C.F.B. Trenton (CYTR) to Ottawa (M-CIA) (CYOW). While on final approach, the flight crew elected to perform an overshoot with the aircraft due to a possible landing gear problem. After flying around the airport for approximately 15 minutes (while working on the landing gear problem), the aircraft returned to land without incident on runway 32 at 2313Z (sic). ARFF and the Airport Manager were advised. Ops. impact -- one (1) aircraft (ACA167) was delayed approximately eight (8) minutes.

The Air Canada Airbus A-320 aircraft (operating as ACA167) was departing on a scheduled IFR flight from Ottawa (M-CIA) (CYOW) to Calgary International Airport (CYYC). The flight crew reported a bird strike occurred during the departure from runway 14. The runway was closed for an inspection and the remains of a small blackbird were recovered. Ops. impact -- one (1) departing aircraft (JZA7660) had a five (5) minute delay on departure.

The Air Canada Airbus A-320 aircraft (operating as ACA167) was departing on a scheduled IFR flight from Toronto (LBPIA) (CYYZ) to Edmonton International Airport (CYEG). The flight crew reported that the aircraft had struck a small bird during the departure from runway 06L. The runway was closed for an inspection and one (1) arriving aircraft was overshot. Nothing was found and the runway was returned to service at 1834Z.
