Update TSB Report #A20O0062: WestJet Encore flight WEN3516, a Dehavilland Dash 8 Q-400 (C-GWJK) aircraft, on route from London, ON (CYXU) to Toronto/LBPIA (CYYZ), ON, declared a PAN PAN due to flap issues. The aircraft continued to its destination where it landed without further incident. Maintenance found the Flap lever trigger to be sticking. Flap lever selector trigger mechanism was lubricated and flap functional test carried out, checked serviceable. The aircraft was returned to service.

After departure, a Sky Regional Airlines Embraer ERJ 170-200 SU (C-FEJL/SKV7557) from Toronto/Lester B. A Thunder Airlines Mitsubishi MU-2B-60 (C-GYUA/THU890) from Thunder Bay, ON (CYQT) to Toronto/Billy Bishop, ON (CYTZ) declared a PAN PAN due to an engine issue. Aircraft landed without further incident (WOFI) at 1714Z.
UPDATE TSB Report #A20O0025: Thunder Airlines flight THU890, a Mitsubishi MU2B-60 (registration C-GYUA) aircraft was on route from Thunder Bay International Airport (CYQT) ON to Toronto BBTC (CYTZ) ON, when the flight crew noticed the power output of the right engine (GARRETT TPE331-10-511M) was lower than expected. The flight crew contacted maintenance and a decision was made to continue to destination. During descent into CYTZ, the engine power dropped by 30%, despite the right engine power lever being at full power. The flight crew declared a PAN PAN and advised ATC of the situation, and the possibility of an engine shutdown in the event the situation degraded. The aircraft landed without further incident with both engines running. The flight crew carried out performance engine power runs and fuel control test later in the day and the results were normal. Thunder Airlines maintenance was sent to CYTZ to further troubleshoot the problem and it was determined that the Delta P Transducer was causing false indications and reducing the fuel flow to the right engine. The Delta P Transducer was replaced and the aircraft was returned to service.

Update TSB Report #A20O0024: Sunwing flight SWG298, a Boeing 737-800 (C-GLZX) aircraft was on route from Thunder Bay (CYQT) ON to Varadero, Cuba (MUVR) and had just reached cruising altitude when a flight attendant became ill and was unable to continue their duties. The flight crew decided to divert to Toronto LBPIA (CYYZ) and declared a medical emergency for a crew member. The aircraft landed in CYYZ without further incident and the flight attendant was taken to the hospital. A replacement flight attendant was sent to the aircraft and the aircraft continued to MUVR.

Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ) (Aerodrome, runway or taxiway shutdown)

The Cleveland Air Route Traffic Control Center (ZOB) advised West Low that they had information about an Air Canada Airbus A319-211 (C-GAQL/ACA715) from New York/LaGuardia, NY (KLGA) to Toronto/Lester B. Pearson, ON (CYYZ) losing one wheel on departure from KLGA. ACA715 conducted a fly-by the CYYZ tower and one wheel was confirmed missing on the right-hand (R/H) main gear. ACA715 landed on Runway 23 at 2055Z without further incident (WOFI), and stopped on the runway. Passengers will be deplaned on the runway into buses for transport to the terminal. Once the passengers are off, a determination will be made as to whether the aircraft can taxi or be towed off the runway. Operational impact involved a "Ground Stop" issued at 2125Z due to ACA715 disabled on Runway 23.
Update TSB Report #A20O0020: C-GAQL an Airbus A319-100 aircraft AC715 operated by Air Canada was taking off from New York, LaGuardia (KLGA) NY to Toronto Lester B Pearson (CYYZ), ON, when the #4 wheel assembly separated from the landing gear. ATC informed the flight crew and the flight continued to CYYZ. On arrival at CYYZ the aircraft did a fly-by the tower which confirmed the #4 wheel was missing. The crew declared an emergency and the aircraft landed uneventfully with ARFF standing by. The aircraft stopped on the runway and damaged was assessed before the aircraft moved to the operators maintenance facility. All Passengers were deplaned on the runway and transported by bus to the terminal. Photos taken of the landing gear showed that the outer bearing appears to have failed allowing the wheel assembly to slip out of the axle. The axle nut remained secure and safetied to axle. Subsequent reports from the FAA and NTSB stated the wheel had been found and retrieved fully inflated. The operator was notified that the wheel was available for further examination.

UPDATE TSB Report #A20O0006: Sky Regional Airlines flight SKV7531, an Embraer E-170 (C-FEJD) aircraft, operating as Air Canada Express was descending into Toronto/Lester B. Pearson Intl (CYYZ) when the flight crew received parking brake not released EICAS message. The flight crew attempted was unable to clear the message. The flight crew declared a PAN PAN and landed safely on runway 15L at CYYZ. Upon landing the flight was met by emergency and inspection vehicles. After preliminary inspection the aircraft taxied to the gate under its own power. Maintenance found the Emergency/Parking Brake pressure switch to be at fault. The switch was replaced and the appropriate checks were completed as per the aircraft maintenance manual (AMM).

UPDATE TSB Report #A19O0172: C-GMWJ, a Boeing 737-7CT aircraft was conducting Westjet flight WJA556. En route from Calgary Intl. (CYYC), AB to Kitchener-Waterloo (CYKF), ON, flight crew declared a PAN PAN after conducting a missed approach due to a flap problem and requested a diversion to Toronto/LBPIA (CYYZ), ON for a longer runway. The aircraft landed without further incident on runway 23 at CYYZ with aircraft rescue and firefighting (ARFF) standing by.
UPDATE Airworthiness: A Civil Aviation Safety Inspector looked into this occurrence. Flight crew of WJA556 reported "Trailing edge flap disagree - flap lever extended to 15, flap indicator indicates 5–2 split, noticed at flap 5 indication." WestJet maintenance replaced flap position indicator in accordance with (IAW) Fault Isolation Manual (FIM) 27-51 Task 820. However, when cycling flaps the right hand flap indicator needle split more than permitted. Further investigation identified lack of lubrication on the flap components. Maintenance lubricated the trailing edge flap tracks, rollers, gimbals and jack screws IAW B737 aircraft maintenance manual (AMM) 12-22-51 and reported "flaps operated after lubrication and indicator needles operated smoothly." The aircraft was returned to service 11/16/2019. The aircraft has operated 5 flights with no record of recurrence of the defect.

An Ocean Aviation Services Bombardier BD-100-1A10 (C-GJEI) from Saint John, NB (CYSJ) to Toronto/Lester B. Pearson, ON (CYYZ) reported bird strike upon landing on Runway 24L.
