Incident at In the vicinity of: TORONTO / LESTER B. PEARSON INTL ON (CYYZ)

At approximately 1910Z, an Emirates Airbus A380-800 (A6EDO/UAE242) from Toronto/Lester B. Pearson International Airport, ON (CYYZ) to Dubai, United Arab Emirates (OMDB) declared a "PAN PAN" with a #2 engine shut down. UAE242 stopped the climb at FL250 and continued eastbound. At 1952Z, northeast of Sherbrooke, QC (CYSC), UAE242 requested a return to CYYZ and began dumping fuel. There were 481 souls on-board, no hazardous materials (HAZMAT), and 93 tonnes of fuel estimated at arrival at CYYZ. UAE242 requested emergency response services (ERS) to be standing by. The North American Aerospace Defence Command (NORAD), the National Operations Centre (NOC), the Greater Toronto Airports Authority (GTAA), and the CYYZ Tower, Terminal, and East High were all advised. UAE242 landed on Runway 33R at CYYZ at 2057Z, and taxied to the gate without further incident (WOFI). Operational impact included a WestJet Encore Ltd. de Havilland DHC-8-402 (WEN3473) from Ottawa/MacDonald-Cartier International Airport, ON (CYOW) to Toronto/Lester B. Pearson International Airport, ON (CYYZ) conducting a go around for Runway 23 as UAE242 rolled through the intersection.
Update TSB #A19O0095: A6-EDO, an Airbus A380-800 aircraft operated by Emirates Airlines was conducting flight UAE242 from Toronto Pearson International Airport (CYYZ), ON to Dubai International Airport (OMDB), United Arab Emirates. Approximately 20 minutes after departure, UAE242 declared a PAN PAN with the #2 engine shut down. UAE242 requested to return to CYYZ and began dumping fuel. The aircraft landed uneventfully on runway 33R at CYYZ with ARFF standing by.

A WestJet Encore de Havilland DHC-8-402 (WEN8987) from Moncton, NB (CYQM) to Toronto/Pearson, ON (CYYZ) declared an emergency due to an electrical failure and requested a second approach with a flapless landing. Aircraft landed without further incident (WOFI) at 1134Z.
Update TSB #A19O0082: C-GKWE, a de Havilland DHC-8-400 aircraft operated by WestJet Encore, was conducting flight WEN8987 from Greater Moncton/Romeo Leblanc Intl (CYQM), NB to Toronto/Lester B. Pearson Intl (CYYZ), ON. During the extension of the flaps on the approach to CYYZ, a Flap Power caution light illuminated and the flaps stopped at the 5° position. A PAN PAN was declared, and the aircraft landed uneventfully with ARFF standing by. The operator’s maintenance replaced the main, auxiliary and standby batteries. A test flight was conducted and, following ground checks, no further faults were found. The aircraft was returned to service.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. In cruise, L LWR CB N10 released, 20 minutes later J10 and K10 released. Upon flap selection L7 released (with associated Flap Power Caution). After shutdown RH N10 released. The previous day, prior to putting power on the aircraft following run, found numerous CBs released. Electrical Power Control Unit (EPCU) was replaced at that time but defect occurred on subsequent flight as described above. Standby Battery replaced, Main Battery replaced, LH N10 Circuit breaker replaced and operational test of all affected systems accomplished. Additional troubleshooting carried out in cooperation with DeHavilland tech support. Test flight satisfactory.

The pilot of a Jazz de Havilland DHC-8-102 (C-FGRP/JZA8676) from North Bay, ON (CYYB) to Toronto/Pearson, ON (CYYZ) reported they had completed a precautionary shut down of engine #2. JZA8676 declared an emergency (PAN PAN), received priority handling, and landed on Runway 15R at 1435Z. JZA8676 taxied to the gate without further incident (WOFI). No operational impact.
Update TSB #A19O0070: C-FGRP, a de Havilland DHC-8-100 aircraft operated by Jazz Aviation LP, was conducting flight JZA8676 from North Bay (CYYB), ON to Toronto/Lester B. Pearson Intl (CYYZ), ON with 3 crew members and 26 passengers on board. During cruise flight, the flight crew observed an overspeed indication for the number 2 propeller, and proceeded to shut the engine down as a precaution. A PAN PAN was declared, and the flight continued to destination where the aircraft landed without further incident. Following a visual inspection by ARFF, the flight crew taxied the aircraft to the gate. A small puddle with the appearance of oil was observed below the number 2 engine. The operator’s maintenance replaced the Hydro Mechanical Unit (HMU), the Electronic Control Unit (ECU), and the fuel pump prior to returning the aircraft to service.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into the occurrence. It was reported that during cruise the Flight Crew reported a No. 2 Propeller Over Speed Indication and elected to shut down the engine. The Flight Crew declared an emergency and continued to destination where the aircraft landed without incident. Maintenance conducted wiring checks between the Hydro Mechanical Unit (HMU) and Electronic Control Unit (ECU). Maintenance replaced the HMU, and as a precaution the ECU and Fuel Pump. The Propeller was also replaced and the aircraft returned to service following a successful engine run. On the fourth flight (June 2, 2019) following repairs there was a potentially related event, as follows: After departure the No. 2 Engine torque rolled back twice with no ECU light. The Torque Indication returned to normal after a few seconds. There was a corresponding momentary drop in Propeller RPM on the No. 2 Engine. The Aircraft continued to destination with no further issues. Additional troubleshooting resulted in the replacement of the No. 2 ECU, Engine Cable Harness, and Torque Signal Conditioning Unit. The aircraft was returned to service with no recurrence. Service Difficulty Reported submitted.

Update TSB #A19O0050: C-GRIF, a Bombardier CL600-2B16 Challenger 601 aircraft operated by Chartright Air, was conducting flight HRT093 from Toronto/Lester B. Pearson Intl (CYYZ), ON to Kitchener/Waterloo (CYKF), ON with 2 crew members and 1 passenger on board. Shortly after the departure from CYYZ, the flight crew requested a return to CYYZ due to an unsafe gear indication, and requested the emergency equipment (ARFF) on standby. The flight crew carried out the QRH items, and lowered the landing gear. The aircraft returned for an uneventful landing on Runway 05 with ARFF standing by. The operator’s maintenance inspected the landing gear and discovered a faulty nose gear proximity sensor, which was replaced and tested before returning the aircraft to service.

Embraer ERJ 170-100 SE (SKV7501) from Baltimore/Washington, MD (KBWI) to Toronto/Pearson, ON (CYYZ) was instructed to overshoot Runway 24L, due to an American, corporately registered Gulfstream GV-SP (N512JT), origin and destination unknown, missing the Runway 24L approach hold line and infringed on the obstacle limitation surface (OLS). SKV7501 landed at 1204Z without further incident (WOFI).

Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ)

The pilot of an Air Canada Boeing 777-333ER (C-FIVM/ACA873) from Frankfurt, Germany (EDDF) to Toronto/Pearson, ON (CYYZ) advised they had a passenger related medical emergency on board, and requested priority handling for Runway 15L at CYYZ. Priority handling was granted. ACA873 touched down at 1723Z and taxied to the gate without further incident (WOFI). The Toronto (YYZ) Tower and the Greater Toronto Airports Authority (GTAA) manager of operations (MO) were advised. No operational impact.

An Air Canada Airbus A330-343 (C-GFUR/ACA825) from Amsterdam/Schiphol, Netherlands (EHAM) to Toronto/Pearson Intl, ON (CYYZ) overshot Runway 23 at 1734Z due to a windshear warning while short final. ACA825 landed at 1748Z and taxied to the gate without further incident (WOFI).

Incident at LONDON ON VOR/DME (YXU)

The pilot of a WestJet Boeing 737-8CT (C-FCSX/WJA1117) from Las Vegas Intl, NV (KLAS) to Toronto/Pearson Intl, ON (CYYZ) advised the Toronto (YYZ) Area Control Centre (ACC) that they had a passenger who required medical attention. WJA1117 was given priority handling and landed at 2335Z. WJA1117 taxied to the gate without further incident (WOFI), and emergency medical services (EMS) met the flight on arrival. The Greater Toronto Airport Authority (GTAA) Manager of Operations (MO) and the WestJet dispatch were advised. No operational impact.

Incident at In the vicinity of: TORONTO / LESTER B. PEARSON INTL ON (CYYZ)

A Flightpath Charter Airways Inc. Bombardier CL-600-2B16 (Series 604) (C-GOSN) from Toronto/Pearson Intl, ON (CYYZ) to Grantley Adams Intl, Barbados (TBPB) declared an emergency with the Cleveland Air Route Traffic Control Centre (ZOB) at 2252Z, advising that the aircraft had a pressurization warning and requesting a return to CYYZ. C-GOSN advised the CYYZ tower that the aircraft did not require emergency equipment and landed at 2315Z. C-GOSN taxied to the north end without further incident (WOFI).
Update TSB Report #A19F0048: C-GOSN, a Bombardier Challenger 604 aircraft operated by Flightpath Charter Airways, was conducting a flight from Toronto/Lester B. Pearson Intl (CYYZ), ON to Bridgetown/Grantley Adams Intl (TBPB), Barbados with 2 crew members and 2 passengers on board. During the departure from CYYZ, the Auxiliary Power Unit (APU) was being used as the bleed air source for pressurization, which can be done to improve takeoff and climb performance. During the climb at approximately 9000 feet ASL, the APU was shut down, however the engine pressurization valves were not opened manually as necessary to maintain the pressurization. As the climb continued, the aircraft depressurized slowly, until the CABIN ALTITUDE amber caution message illuminated, as the aircraft climbed through 18 000 feet. Approximately 20 seconds later, the CABIN ALTITUDE red warning illuminated, accompanied by the automatic deployment of the passenger oxygen masks. The flight crew donned their oxygen masks, requested an immediate descent, as well as a return to CYYZ. Descending below 9000 feet, the flight crew recognized that the bleed source switch was in the OFF position, and elected to continue toward CYYZ where the aircraft landed without further incident. There were no injuries or damage to the aircraft reported.

Pearson Intl, ON (CYYZ) requested an overshoot of Runway 05 for unknown reasons. WEN3521 landed at 1540Z without further incident (WOFI). No operational impact.

Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ)

A Jazz de Havilland DHC-8-402 (C-GXJZ/JZA8655) from Toronto/Pearson International, ON (CYYZ) to London, ON (CYXU) was taxiing from CYYZ Taxiway AK to Runway 23 via A-H. A WestJet Boeing 737-7CT (C-GQWJ/WJA323) from Toronto/Pearson International, ON (CYYZ) to Saskatoon/John G. Diefenbaker International, SK (CYXE) called when at AJ for taxi instructions to Runway 23 and was issued a clearance to taxi from A-H. WJA323 was questioned by the ground controller if they were on the move and WJA323 answered yes. The ground controller re-issued taxi instructions to follow JZA8655 as it taxied on A-H. WJA323 immediately questioned their outbound control point, and in the ensuing conversation no readback was given on the following instruction or requested. As a result, WJA323 cut off JZA8655, who stopped short of WJA323 on Taxiway A at intersection AJ. Both flights continued without further impact (WOFI).

Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ)

UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence and Maintenance found one spring in the left hand (LH) main landing gear (MLG) uplock broken. The spring was replaced.
A WestJet Encore Ltd. de Havilland DHC-8-402 (C-FOEN/WEN3123) from Toronto/Lester B. Pearson Int'l, ON (CYYZ) to Thunder Bay, ON (CYQT) returned to CYYZ landing Runway 15L after declaring an airborne emergency at 1942Z due to unsecure/safe gear indication. The aircraft landed at 2002Z and stopped on the runway for gear inspection. The aircraft taxied to the gate at 2008Z WOFI (without further interference).

Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ)

A Charter Air Transportation Services Beech 390 (C-FJTN) from Toronto, ON (CYYZ) to Teterboro, NJ (KTEB), once airborne at 1643Z, advised air traffic control (ATC) that the aircraft was experiencing a cabin vent over heat issue and needed to return to CYYZ. No emergency declared. The aircraft landed at 1658Z and taxied to fixed base operator (FBO) WOFI.
Follow-up information received from Flight Operations ? West [2015-12-22]: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. The CASI contacted the operator involved for more information and was provided with additional details of this occurrence. The CASI was advised that the operator is managing this occurrence through their safety management system (SMS). Maintenance staff were unable to duplicate the fault on the ground and the aircraft was subsequently returned to service. There has been no recurrence of this issue since. The CASI advised that no further follow-up action by Transport Canada, Flight Operations was required at this time.
