Incident at LINNG intersection

An American, corporately registered British Aerospace BAE 125-800 (N802WJ) on a flight from Princess Juliana Intl, Saint Maarten (TNCM) to Toronto/Lester B. Pearson Intl, ON (CYYZ) was given instructions, on several instances, by Terminal Arrival, and read back was correct. The aircraft then flew something completely different than cleared. No loss of separation. No operational impact.

Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ)

A WestJet Boeing 737-7CT (C-FWSO/WJA2652) on a flight from Toronto/Lester B. Pearson Intl, ON (CYYZ) to Princess Juliana Intl, St. Maarten (TNCM) rejected its take-off from Runway 24R at 1509Z, and again at 1521Z. The aircraft returned to the gate. No operational impact.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. Flight Crew of WJA2652 reported that the anti-ice master caution illuminated at about 70% N1, on the take-off roll. Maintenance carried out (FIM) 30-21 Task 809 with power engine runs, in accordance with (IAW) Aircraft Maintenance Manual (AMM) 71-00-00, above 70% N1. Maintenance could not duplicate fault during engine runs. Per FIM fault deemed intermittent. Aircraft returned to service. Aircraft has flown 82 cycles since the occurrence with no further related issues.

Incident at Princess Juliana Intl, St. Maarten (TNCM)

TSB Report #A20F0050: C-GOHI, an Airbus 321 operated by Air Transat was conducting flight TSC288 from Montréal/Pierre Elliott Trudeau Intl (CYUL), QC to St. Maarten/Princess Juliana Intl (TNCM), Netherlands Antilles. After being cleared to land on Runway 10 in TNCM, TSC288 had to perform a go-around at approximately 100 feet because of the presence of a small airplane on the runway threshold. The airplane climbed to 2600 feet as instructed by tower and returned for a normal landing. The Sint Maarten Civil Aviation Authority is conducting an investigation.

Aviation Incident Report #16564: The crew of a WestJet Boeing 737-8CT (C-GWSA/WJA2652) on a flight from Toronto/Lester B. Pearson Intl, ON (CYYZ) to Princess Juliana Intl, St. Maarten (TCNM) requested police to meet the aircraft on arrival due to an unruly passenger onboard. The flight notified via Aircraft Communications Addressing and Reporting System (ACARS) that they had a disruptive non-compliant passenger and a notice from the captain had been issued. Crew then advised that the situation was under control but the flight attendants were requesting for authorities to meet the aircraft. No threat to safety of guests or crew, just non compliance from guest. Police met the flight on arrival in St. Maarten.

A WestJet Boeing 737-700 (WJA2652) from Toronto/Pearson Intl, ON (CYYZ) to Princess Juliana Intl, Saint Martin(TNCM) strikes a bird on takeoff from CYYZ. Pilot elects to continue onto destination. No delay to operations.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into the occurrence. The pilot of WestJet (C-FIBW/WJA2652) recorded hitting multiple small birds at 1447Z on takeoff from Toronto. Following the bird strike, the aircraft did not continue to Princess Juliana Intl, Saint Martin (SXM) at that time and returned to Toronto Lester B. Pearson, ON (CYYZ).WestJet Technical Operations completed a maintenance release for a Bird Strike Inspection In Accordance With (IAW) the Boeing Aircraft Maintenance Manual (AMM) 05-51-18 on 06/09/2019 at 1615Z. Remains of a bird were found on the left-hand (LH) Krueger Flap. The remains of the bird were removed from the LH Krueger Flap. No damage was identified during the inspection. The aircraft departed Toronto / Lester B. Pearson, ON (CYYZ) to Princess Juliana Intl, Saint Martin (SXM) on 06/09/2019 at 1726Z.

Flight crew of a Sunwing Airlines Inc. Boeing 737-86N (C-GKVV/SWG763) from Princess Juliana Intl, Sint Maarten (TNCM) to Toronto/Pearson Intl, ON (CYYZ) advised the en-route controller that there was a medical situation on board. When queried, the crew stated that it was not an emergency and no priority was required, just no delay. The crew made arrangements through the company for the Aircraft Rescue and Fire Fighting (ARFF) unit to respond at the gate, which occurred at 0641Z.

TSB A19F0125: C-GKVV, a Boeing 737-800 aircraft operated by Sunwing Airlines, was conducting flight SWG763 from St. Maarten/Princess Juliana Intl (TNCM), Sint Maarten to Toronto/Lester B. Pearson Intl (CYYZ), ON with 6 crew members and 165 passengers on board. Approximately one hour after the departure from TNCM, one of the cabin crew members experienced a medical issue, and was unable to continue duties. Incident at PRINCESS JULIANA INTL PHILIPSBURG, ST. MAARTEN (TNCM)

UPDATE According to the TSB Final Report A17F0052 authorized for release on 25 April 2018: On 07 March 2017, a WestJet Boeing 737-800 aircraft (registration C-GWSV, serial number 37158), operating as flight 2652 (WJA2652), was conducting a scheduled instrument flight rules flight from Toronto/Lester B. Pearson International Airport, Ontario, to Princess Juliana International Airport, Sint Maarten. During the approach to Runway 10, the aircraft deviated from the normal descent path. At 1534 Atlantic Standard Time (AST), the aircraft was 0.30 nautical miles from the runway threshold and had descended to an altitude of 40 feet above the water. The crew then initiated a missed approach. Given that visibility was below the limits for conducting a second approach, the flight was cleared to hold until conditions had improved. After visibility improved, the crew conducted a second approach and landed at 1618 AST without further incident.
TSB #A17F0052: C-GWSV, a Boeing 737-800 aircraft operated by WestJet, was conducting flight WJA2652, from Toronto/Lester B. Pearson Intl, ON (CYYZ), to Sint Maarten/Princess Juliana Intl, Netherlands Antilles (TNCM). The CASI contacted the operator involved for more information and was provided with additional details of this occurrence. On take-off, the crew experienced a stall warning. Airspeed indication red flagged due to frozen angle of attack probe. Return and landing was uneventful. The CASI was satisfied with the information provided and advised that no further follow-up action by Transport Canada was required at this time.

TSB REPORT#A16O0157: C-GNCH, a Boeing 737-800 aircraft operated by Sunwing Airlines, was conducting flight SWG762 from Toronto/Lester B. Pearson Intl, ON (CYYZ) to St. Maarten/Princess Juliana Intl, Netherlands Antilles (TNCM). During the rotation at takeoff from CYYZ, the aircraft experienced a tail strike. The flight crew notified ATC and entered a holding pattern in order to accomplish the necessary checklist items. The decision was made to return to CYYZ where the flight crew performed an overweight landing. The operator?s maintenance inspected the aircraft, replaced the tail skid crushable cartridge, repainted the skid shoe and returned the aircraft to service the following day.

A Westjet Boeing 737 800 (WJA2652) from Toronto, ON (CYYZ) to Saint Maarten (TNCM) taxied out from the ramp and it was later discovered that they had a fuel leak on their left side. This caused delays due to the taxi route being compromised to Runway 23. Upon inspection parts of the taxiway and ramp required cleanup. Numerous departures were delayed roughly 15 minutes as the area was inspected and taxi routes became available.

New York air traffic control center (ARTCC) reported to Moncton area control centre (ACC) that during a position report, an Executive Jet Management Inc. Dassault Falcon 2000 (EJM427) from Sint Maarten (TNCM) to Halifax, NS (CYHZ) advised CUDAS Intersection next rather than ELERI Intersection. New York ARTCC was unable to contact EJM427 to query CUDAS Intersection which is a reporting point that has been defunct for several months. ELERI Intersection is approximately 8 miles west of where CUDAS Intersection was. On contact with Moncton ACC, EJM427 confirmed proceeding over CUDAS Intersection. The Moncton ACC controller corrected the route. Minimal impact on air traffic services (ATS) operations, no conflicting traffic.

Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ) (Ground handling services, Aerodrome - other)

Update: Follow-up information received from Flight Operations ? West [2015-01-20]: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. The CASI contacted the operator for more information about this incident. The CASI was advised that this was not an accident, rather, it was an incident. The de-icing vehicle has safety proximity sensors (fibreglass wands) mounted on it and one safety proximity sensor activated when the spray arm came close to the aircraft?s vertical stabilizer. The safety proximity sensor shuts the de-icing vehicle down to ensure that contact does not occur. The flight crew is advised when this happens and the flight crew (in this instance) elected to return to the gate to have their Maintenance staff inspect the area. There was no visible damage and the aircraft subsequently returned to the de-icing pad for de-icing and then departed without further incident. The CASI was satisfied with the information provided and advised that no further Transport Canada (Flight Operations) follow-up action was required at this time.

Incident at Aéroport Princess Juliana, Saint-Martin (TNCM)

Update: Follow-up information received from Airworthiness ? National Maintenance Operations [2014/08/19]: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. The CASI contacted the company for additional details pertaining to this event. There was an unexpected fuel transfer of 2,000 Kg inside of 400 NM from tank No. 1 to tank No. 2. Maintenance staff replaced an engine fuel crossfeed valve actuator and the aircraft was returned to service following a satisfactory functional test of the crossfeed valve. The CASI advised that no further Transport Canada follow-up was required at this time.
According to TSB report #A12F0167: The B737-800, C-FGVK, operated by Sunwing Airlines Inc. as flight number SWG764, was on an IFR flight from the Montréal Pierre Elliott Trudeau Airport (CYUL), QC to the Princess Juliana Int'l Airport (TNCM) on the Dutch island of Saint Martin (Sint Maarten) with 139 passengers and 7 crew members on board. While the aircraft was approximately 400 NM from its destination, the crew noticed a fuel loss indication of approximately 2 000 kg over the last 400 NM travelled. The crew completed the checklist and declared an emergency to ATC. The aircraft landed on Runway 10 at its destination without incident.

Canadair CL-600-2B16 (601-3A) Challenger Jet aircraft (C-GZUM) had just landed after an IFR flight from Saint Maarten (Princess Juliana) International Airport, St. Martin (TNCM) to Toronto (LBPIA) (CYYZ). NAV CANADA staff at Toronto Tower reported that the SWG570 flight crew was instructed to take their aircraft in an overshoot of its approach to runway 24L due to the previous arrival (C-GZUM) still on the runway. The WestJet Airlines Ltd. Boeing 737-700 series aircraft (operating as flight WJA2653) was nearing the conclusion of a scheduled IFR flight from Saint Maarten (Princess Juliana) International Airport, Saint Maarten (TNCM) to Toronto (LBPIA) (CYYZ). NAV CANADA staff at Toronto ACC advised that the flight crew reported that a green laser strike had occurred when the aircraft was on the south downwind for runway 24L while at an altitude of 4,000 feet (43º24'16"N 079º22'40"W). Peel Regional Police were advised. Ops. impact -- unknown.

The Westjet Boeing 737 7CT aircraft (operating as WJA2653) was on a scheduled IFR flight from St. Maarten (AW) (TNCM) to Toronto (CYYZ). During the landing, the flight crew reported that they may have struck a bird. Airport ground staff investigated but found no remains.

The Sunwing Airlines Inc. Boeing 737-800 series aircraft (operating as flight SWG761) was concluding an IFR flight from Saint Maarten (Princess Juliana) International Airport, Sint Maarten (St. Martin) (Netherlands Antilles) (TNCM) to Toronto (LBPIA) (CYYZ). The Comair Inc. Bombardier CL-600-2B19 (RJ100) Regional Jet aircraft (operating as Delta Connection flight COM3105) was about to depart on a scheduled IFR flight from Toronto (LBPIA) (CYYZ) to Detroit Metropolitan (Wayne County) International Airport, MI (U.S.A.) (KDTW). NAV CANADA staff advised that the SWG761 flight crew was instructed to overshoot their aircraft's approach to runway 06L due to COM3105 still in the take-off position. COM3105 had been cleared for take-off but the flight crew advised the Tower Controller that they needed more time to sort out an [unspecified] issue. Ops. impact -- no other.

Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ)

The Westjet Airlines Ltd. Boeing 737-7CT aircraft (C-FUWS, operating as WJA2653) was concluding a scheduled IFR flight from Princess Juliana International Airport, St. Maarten (TNCM) to Toronto (LBPIA) (CYYZ). During the aircraft's descent into Toronto, a male passenger was observed by the cabin crew consuming his own alcohol. The alcohol was subsequently confiscated from the passenger and dumped. According to the cabin crew report, the passenger was irate but not abusive. At the request of the crew, police met the passenger at the gate upon arrival into Toronto. Police spoke with the passenger, but no charges were laid. Ops. impact - unknown.

The Air Canada Boeing 767-300 series aircraft (operating as flight ACA1244) had departed on a scheduled IFR flight from Toronto (LBPIA) (CYYZ) to St. Maarten (Princess Juliana) International Airport, Netherlands Antilles (TNCM). The aircraft was approximately 50NM south of Toronto when the flight crew flap problems and returned to Toronto (LBPIA). The aircraft landed without incident. Ops. impact -- none.
