On approach to Montreal/Pierre Elliott Trudeau Intl, QC (CYUL) and at 3 000 feet, an Air Inuit Beech B350 (AIE560) from Montreal/Pierre Elliott Trudeau Intl, QC (CYUL) to Montreal/Pierre Elliott Trudeau Intl, QC (CYUL) had a traffic alert and collision avoidance system (TCAS) resolution advisory (RA) generated by a Montreal Flying Club Cessna 172S (C-GRDB) from Montreal/St-Hubert, QC (CYHU) to Montreal/St-Hubert, QC (CYHU) that was co-located at 2500 feet. In reaction to the TCAS RA, AIE560 climbed 400 feet. Traffic had been passed and AIE560 had C-GRDB in sight. No impact.

Incident at 12NM MANIWAKI QC (YMW) NDB

A WestJet Boeing 737-7CT (C-FWSF/WJA214) from Calgary, AB (CYYC) to Montreal/Pierre Elliott Trudeau, QC (CYUL) was cleared to FL310 with a correct read back. WJA214 was informed of traffic at FL300. WJA214 reported having the traffic in sight. At FL314, with a descent rate of around 2900/minute, WJA214 advised air traffic control (ATC) of a traffic alert and collision avoidance system (TCAS) resolution advisory (RA) and climbed a few hundred feet. The other aircraft in the vicinity was a Japan Airlines Boeing 787-9 (JA865J/JAL7) from Boston, MA (KBOS) to Tokyo/Narita Intl, Japan (RJAA). After crossing traffic, WJA214 was observed descending below FL310. No impact.

Air traffic services (ATS) passed the traffic information along to the two aircraft. Despite this, PSC582 received a traffic alert and collision avoidance system resolution advisory (TCAS RA) and decided to pull up. No loss of separation and no other impact on operations.

The pilot of a WestJet Encore Ltd. de Havilland DHC-8-402 (C-FENY/WEN3354) from Victoria, BC (CYYJ) to Vancouver, BC (CYVR) reported a traffic alert and collision avoidance system (TCAS) resolution advisory (RA) with a privately registered Cirrus SR22T on a VFR flight from Qualicum Beach, BC (CAT4) to Vancouver/Boundary Bay, BC (CZBB). The SR22 and WEN3354 passed abeam of each other 1.4NM away and 600 feet. WEN3354 had been cleared to 3000 feet and NAV CANADA Auxiliary Radar Display System (NARDS) showed it level at 2900 feet. The SR22 traffic was level at 2500 feet when tower provided the WEN3354 traffic, and initiated descent to 2300 feet when passing WEN3354. WEN3354 did not deviate from heading or altitude. No operational impact.

Incident at FORT ST. JOHN BC (CYXJ)

A WestJet Encore Ltd. de Havilland DHC-8-400 (WEN3101) from Calgary, AB (CYYC) to Fort St. John, BC (CYXJ) was on a 15 mile final for Runway 30 and a privately registered Cessna A185E on a flight to Fort St. John, BC (CYXJ) was to the South of the approach for Runway 30. Traffic was passed between the two aircraft and the Cessna A185E stated that they were going to follow in behind WEN3101 and plan for number two. This was then relayed to WEN3101. As the aircrafts got closer, the Cessna A185E started to turn towards the approach of Runway 30. The Cessna A185E was then asked to remain at 033 feet and below the Cessna A185E acknowledged the instruction. This was then relayed to WEN3101. WEN3101 landed and stated that they had a resolution advisory (RA) from the aircraft.

Incident at ROCKTON ON (CPT3)

Aviation Incident Report #16645: A close encounter occurred between two aircraft over Rockton, ON (CPT3). The pilot of a privately registered Pezetel SZD-48-1 glider noted that as they were returning to CPT3 from a flight to the south, they made a radio call at 5NM out on the CPT3 frequency of 122.725 advising they were returning and would join a left downwind for Runway 21. They called again as they entered the downwind leg, at which point their FLARM (a portable collision avoidance system (PCAS) system used by glider pilots) started beeping a collision warning. They noted that a single-engine green and white low-wing Piper aircraft flew under them (plus or minus 300 feet) just as they were crossing Highway 8 on their downwind leg. It appeared that the Piper aircraft was on a right-hand base for Runway 36. The Piper aircraft then conducted a missed approach for Runway 36 before leaving the area. The glider pilot tried contacting the Piper aircraft on CPT3 frequency 122.725, with no success. The glider pilot noted that at no time as they were approaching the CPT3, or while flying the circuit, did they hear any radio calls on the airport frequency from the Piper aircraft. After landing, witnesses on the ground informed the glider pilot that the Piper aircraft was a Spectrum Airways Piper PA-28-161 (C-GGGH) from Burlington, ON (CZBA) to Burlington, ON (CZBA). The glider pilot also checked their radio and confirmed that it was set to the correct frequency. The pilot of C-GGGH failed to follow proper radio procedures at an uncontrolled airport, resulting in a near miss in the circuit and the potential for another near miss while using an intersecting runway. The glider pilot was on the CPT3 frequency for at least 10 minutes before the incident. CPT3 is a private airport with prior permission required (PPR) noted in the Flight supplement.

A Jazz de Havilland DHC-8-301 (C-GTAQ/JZA8646) from London, ON (CYXU) to Toronto/Lester B. Pearson, ON (CYYZ) was cleared for take-off from Runway 33, and was instructed to contact the Toronto Centre once airborne. While C-GTAQ was rolling, a 7506406 Canada Inc. Agusta AW139 (LF3) on a MEDEVAC flight from London, ON (CYXU) requested departure off of Apron II, northbound. LF3 was instructed to take-off northwest-bound, with a restriction of "no further East than Golf" and a wake turbulence cautionary for JZA8646. LF3 departed Apron II, and was instructed to proceed on course once JZA8646 was in sight. LF3 reported that JZA8646 was in sight and passed behind and below JZA8646, northbound. A Private Air Cessna 650 (FSY705) from Toronto/Oshawa, ON (CYOO) to Toronto/Oshawa, ON (CYOO) was in the left downwind for Runway 12. A 2652662 Ontario Limited Diamond DA 40NG (C-GDBE) from Toronto/Oshawa, ON (CYOO) to Barrie-Orillia/Lake Simcoe, ON (CYLS) was departing from Runway 12, Northbound. Mutual traffic information was passed. FSY705 reported traffic on the traffic collision and avoidance system (TCAS). The DA40 reported traffic in sight and was instructed to pass behind. FSY705 climbed in response to a resolution advisory (RA) at approximately the same time that the DA40 was passing behind them.

Incident at 7NM NW OTTAWA / CARP ON (CYRP)

Descending towards Ottawa/Carp, ON (CYRP), a Helicopter Transport Services Inc. Beech B300C (C-GPNC) from Val-dOr, QC (CYVO) to Ottawa/Carp, ON (CYRP) experienced a conflict with traffic that was not in contact with Air Traffic Control (ATC). Traffic exchanged, C-GPNC announced that they received a traffic alert and collision avoidance system (TCAS) resolution advisory (RA) alert, and was gaining altitude slightly. C-GPNC then had traffic in sight, and continued its descent. No other impact.

Traffic was a 7506406 Canada Inc. Agusta AW139 (LF1) from Toronto/Billy Bishop, ON (CYTZ) to Owen Sound/Billy Bishop, ON (CYOS) that was south of ACA857 and passing behind. Mutual traffic information had been passed and both aircraft had the other in sight. No impact on operations.
