Incident at YELLOWKNIFE NT (CYZF) (Aerodrome - visual aids)

A Buffalo Airways Beech 95-C55 (C-FULX) on a flight from Yellowknife, NT (CYZF) to Hay River/Merlyn Carter, NT (CYHY) departed Runway 16 at 1309Z with Runway Visual Range (RVR) 34 at 1200 and aerodrome routine meteorological report (METAR) 1/2SM. Reduced Visibility Operations Plan (RVOP) not in effect since Runway 16/34 edge lighting was unserviceable (U/S). Aircraft provided with current METAR visibility and RVR.
UPDATE: AOR-257472-V2: While investigating this further to assist one of the operators involved with their Safety Management System (SMS) follow-up, it was determined that the aircraft departed with prevailing visibility at 1/2 mile, and this AOR should not have been filed. There was some confusion at the time, owing to the runway light unserviceability, but the prevailing visibility was 1/2 mile, and RVOP was not in effect, nor was it required.

Informed via local flight services that Yellowknife Airport duty manager has instituted Reduced Visibility Operations Plan (RVOP). Runway edge lights were unserviecable due to a long term construction project.

At 0300Z, the airport maintenance conducted a lighting inspection and reported every second light on Runway 12/30 was unserviceable. Instrument landing system (ILS) CAT 2 and Reduced Visibility Operations Plan (RVOP) were suspended until 0900Z. A WestJet Boeing 737-7CT (C-GWSY/ WJA315) from Halifax, NS (CYHZ) to Hamilton, ON (CYHM) diverted to Toronto, ON (CYYZ) at 0300Z due to weather being below ILS Cat 1 limits. A Flair Airlines Boeing 737-4Q8 (C-FLHJ/ FLE002) from Winnipeg, MB (CYWG) to Hamilton, ON (CYHM) diverted to CYYZ.

Runway Visual Range (RVR) below 6000 feet Reduced Visibility Operations Plan (RVOP) transition commenced at 0536Z. Infrastructure inspection didn't commence until 0610Z. RVR steady at 2400 ft RVOP initiated at 0636Z. At 0659Z, a Jazz Aviation Bombardier CL-600-2D24 (JZA8891) diverted to Prince George, BC (CYXS). At 0802Z, an Air North Boeing 737-500 (ANT556) arrived Runway 32L. RVR above 2600 feet in transition at 0937Z, RVR below 6000 RVOP transition commenced at 1254Z. RVR 2200 feet, RVOP initiated at 1306Z. Normal operations resumed at 1450Z.

Incident at YELLOWKNIFE NT (CYZF) (ATM - inaccurate aeronautical information)

Visibility at Yellowknife, NT (CYZF) necessitated the Reduced Visibility Operations Plan (RVOP) in effect when the Tower opened at 1300Z. At 1400Z the visibility improved to 1/2 mile but numerous attempts to contact the duty manager were unsuccessful resulting in RVOP staying in effect unnecessarily until 1525Z. Visibility improved to 4 miles before attempts to contact the Airport were successful. This resulted in numerous delays to taxiing aircraft and vehicles of more than 45 minutes while they waited for the airport to end RVOP.

Visibility 1/4 mile at time of occurrence with Runway Visual Range (RVR) for Runway 29 at 800 ft, light setting 3. Reduced Visibility Operational Plan (RVOP) was not in place at Fort St. John, BC (CYXJ) airport due to runway edge light issues on Runway 11/29. Lighting issue had NOTAM in place and the pilot a Bid Air Piper PA-31T, from Prince George, BC (CYXS) to Fort St. John, BC (CYXJ), was informed that RVOP was not available. The aircraft landed Runway 29 with 1/4 mile visibility, no RVOP.

At 0355Z, while preparing for Reduced Visibility Operational Plan (RVOP), the Vancouver North Field generator failed. Vancouver airport authority (YVRAA) responded and, while investigating, all North Airport lighting failed, including Runway 08L. YVRAA restored lighting within 6 minutes; generator remained off-line. Alternate runway configuration adopted. Minimal operational impact. NOTAM issued at 0455Z.

Incident at FORT ST. JOHN BC (CYXJ) (Aerodrome, runway or taxiway shutdown)

The airport operator implemented Reduced Visibility Operational Plan (RVOP). A Regional 1 Airlines de Havilland DHC-8-311 (C-GXYA/ TSH341) from Fort St John, BC (CYXJ) to Edmonton, AB (CYEG) was advised of the RVOP. TSH341 received a clearance with a cancelled time and were unable to meet the time. When air traffic control (ATC) cancelled the clearance, TSH341 exited onto a closed manoeuvring surface due to the RVOP. No operational impact.

Update: According to information from the Commercial and Business Aviation Division, a memo was sent to all crews.
An Execaire Bombardier BD-700-1A11 (C-GDPG) from Montreal, QC (CYUL) to Winnipeg, MB (CYWG) landed runway 18 in Reduced Visibility Operational Plan (RVOP). The pilot was informed that clearance could not be issued due RVOP. The pilot was subsequently given landing information and landed runway 18 without incident. No impact on operations.

The Field Electrical Centre diesel generator was started for Reduced Visibility Operational Plan (RVOP)/CAT II operations. The entire field lighting then failed for 1-2 seconds. The apron lights and central de-icing facility (CDF) failed for longer but did eventually return. Weather at 1300Z vis 3/4 mile, RVR below 2600'. United Parcel Service (UPS496) and Air Canada (ACA258) were on the manoeuvring area at the time. No other impact on operations.

A Government of Newfoundland and Labrador Beech B300 Medevac (C-GNLF) from Deer Lake, NL (CYDF) to St John's, NL (CYYT) on roll out after landing given taxi instructions and then had to change them due to taxiway closures. A Jazz Bombardier CL-600-2D15 (C-FUJZ/ JZA8992) from Halifax, NS (CYHZ) to St John's, NL (CYYT) was given overshoot instructions due to C-GNLF on runway 11 approximately 2 miles final. Reduced Visibility Operational Plan (RVOPs) were in effect.

Incident at YELLOWKNIFE NT (CYZF) (ATM - NAVAIDS/radar)

A First Air Aerospatiale ATR 42-300 (C-GSRR/ FAB5108) from Gjoa Haven, NU (CYHK) to Yellowknife, NT (CYZF) landed and taxied in when the visibility was 1/4 SM . Runway Visual Range (RVR) was unserviceable and Reduced Visibility Operational Plan (RVOP) was not in effect. Visibility was a 1/2 SM when FAB5108 was on the approach 8 DME, then the visibility dropped to 1/2 SM when FAB5108 was 6 DME.
Update Aerodrome Safety: Yellowknife Airport management and NavCanada site manager have reviewed the event and have initiated an amendment to the procedures for the provision of ATS services as provided by NavCanada in support of the Yellowknife Airport RVOP. The changes to the activation procedures are intended to reduce the time required to activate the RVOP once visibility has dropped below established minima. Changes became effective on Nov 8, 2013.
Update Aerodrome Safety: Yellowknife Airport operates with an RVOP program that permits some daytime operations in conditions of reduced visibility. The program is clearly articulated, and is reviewed and if necessary, updated annually. The RVOP plan was presented to air carriers for review and discussion at the Airport Operations Committee meeting on October 9, 2013.
Update Aerodrome Safety: After a review of the incident Yellowknife airport determined that their RVOP processes were conducted as they were supposed to be.
