Incident at Vicinity of SCROD intersection

UPDATE: Aviation Incident Report#13439: A medical emergency was report for a passenger onboard a QATAR Airways flight and the passenger was transported to Goose Bay Hospital by ambulance.
A Qatar Airways Boeing 777 300 (A7BAF/ QTR721) from Doha (OTBD) to Chicago, IL (KORD) at 1600Z near intersection SCROD declared a medical emergency and requested a diversion to Goose Bay, NL (CYYR). Clearance was issued direct CYYR and the aircraft landed at 1645Z without incident. An ambulance and re-fuelling was requested.

Incident at Oceanic Airspace (ATM - other)

Westbound Qatar Airways Boeing 777 200 (QTR721 / A7BAI) from Doha (OTBD) to Chicago, IL (KORD) cleared 60N050W 59N060W LAKES. Fight progressed 50W reporting correct position at 60W but incorrect next position (IRBIM instead of LAKES). Ocean controller did not recognize incorrect position after 60W. At 60W the aircraft was observed proceeding to IRBIM by Gander High Radar. Corrective action was taken.

Incident at MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL)

A Jazz Aviation de Havilland DHC 8 311 (C-FSOU/JZA8757), from Montréal (CYUL) to Rouyn-Noranda (CYUY), initiated takeoff from Runway 24L but aborted it due to a faulty indication. Impact: a Qatar Airways Boeing 777 200 (A7BAN/QTR763), from Doha (OTBD) to Montréal (CYUL) and on final for Runway 24L, was instructed to pull up.

An Expressjet Airlines Bombardier CL600 2D24 (N538CA/ASQ5097), from Atlanta (KATL) to Montréal (CYUL) (1836Z); a Qatar Airways Boeing 777 200 (A7BAN/QTR763), from Doha (OTBD) to Montréal (CYUL), Runway 24L (1947Z); a WestJet Boeing 737-6CT (C-GWCY/WJA590) from Toronto (CYYZ) to Montréal (CYUL) (2118Z) pulled up on Runways 24R and 24L due to wind shear. No impact on operations.

Due to weather conditions, a Qatar Airways Boeing 777-300 (QTR928) took off from Runway 24L at Montréal (CYUL) for Doha (OTBD) and flew a heading of 215° until 2 200 ft. Impact: Noise abatement procedures were not followed.

Incident at VOR de Rivière-du-Loup (YRI)

QTR927, a Boeing 777 operated by Qatar Airways, was on an IFR flight from Doha (OTBD) to Montréal/Trudeau (CYUL). While the aircraft was at the Rivière-du-Loup (YRI) VHF omnidirectional range (VOR), the crew flew a route that was different from the one ATC had. Following a verbal verification, it turned out that QTR927 was flying YRI PENTU OMBRE OMBRE3 while the route that ATC had was YRI OMBRE OMBRE3. No impact on ATC operations.

Incident at VOR de Rivière-du-Loup (YRI)

QTR927, a Boeing 777 operated by Qatar Airways, was on an IFR flight from Doha (OTBD) to Montréal/Trudeau (CYUL). At 1754Z, the aircraft was seen turning left after the Rivière-du-Loup (YRI) VHF omnidirectional range (VOR), heading toward the ILERO intersection, even though the flight plan that ATC had indicated YRI OMBRE. ATC corrected the manoeuvre. There was no impact on ATC operations.

At 0400Z, the crew advised the tower controller that they had struck a bird during takeoff from rwy 24L. QAF002, Airbus A330-200, enroute from New York (KJFK) to Doha (OTBD), reported moderate to severe turbulence at 35,000 feet. The flight did not request a change in altitude. The flight was then was handed off to the Gander Area Control Centre (ACC).

Incident at MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL) (Overshoot/missed approach, Conflict - loss of separation)

QTR927, a Qatar Airways Boeing 767, was on an IFR flight from Doha (OTBD) to Montréal/Trudeau (CYUL). SKV7514, a de Havilland Dash-8-400 operated by Sky Regional Airlines, was on an IFR flight from Toronto/Billy Bishop (CYTZ) to Montréal/Trudeau (CYUL). QTR927 was pulled up due to insufficient separation with the previous aircraft, SKV7514. QTR927 landed without incident at 1952Z. No impact on ATC operations.

Incident at MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL)

QTR928, a Boeing 777 operated by Qatar Airways, was on an IFR flight from Montréal/Trudeau (CYUL) to Doha (OTBD). At takeoff, the Montréal area control centre (ACC) noticed that the aircraft was flying at a speed greater than 250 kt while flying under 10 000 ft. The crew was informed of Canadian regulation but the crew reported that for operational reasons, it could not respect the regulation.

Incident at Région de Doha (DOH) Qatar (OTBD)

The collective lever (P/N 412-010-408-101 S/NA-770) failed on A7-HBC, a Qatar Gulf Helicopters Bell 412EP, while in level flight for Qatar. The aircraft had a total of 8034.3 flying hours. Non destructive testing (NDT) had been conducted 612.5 hours earlier. The seven passengers and two crew members were uninjured.

Incident at Près de Brisay

Qatar Airways Boeing 777-200 QTR077 was IFR from Doha International (OTBD) to Houston (KIAH). At 1800Z, the pilot declared a medical emergency due to a passenger?s state of health and asked to be diverted to Montreal/Trudeau (CYUL). Arrangements were made for their arrival. The crew dumped fuel between coordinates 5104N/07154W and 4920N07228W, at FL 300. The aircraft landed without incident on runway 24L at 1918Z. In order to give priority to the medial emergency, 10-15 minute delays were experienced by ACA871, JZA715, JZA962, ACA667 and ACA831.

Between reporting point KLANT and coordinates 60N/050W, the crew flew a different route (REZIN 5330N/070W LOMTA MOATT) from the one that air traffic control had in its possession (46N/080W YMT 5230N070W YDP PRAWN). No impact on ATC operations.

Incident at HECKK Intersection

QTR084 (Qatar Airways), Boeing 777-300, enroute from New York (KJFK) to Amman (OTBD) at 33,000 ft. with an oceanic clearance at 33,000 ft. climbed to 34,000 ft. without clearance at the ocean entry fix HECKK. The aircraft was in radar contact in domestic airspace and was re-cleared to 33,000 ft. When queried, the pilot reported that he had received an Aircraft Communications Addressing and Reporting System (ACARS) clearance at 34,000 ft. There is only one record of a clearance and that was at 33,000 ft.

QTR051 (Qatar Airways) Boeing 777-300, enroute from Doha (OTBD) to Washington (KIAD), routing 63N040W at 15:22Z, 34,000 ft., estimating 62N050W at 15:54Z then rerouting 60N060W LOPVI at 15:43Z, advised that it was descending from 34,000 ft. to 32,000 ft. due to severe turbulence. The aircraft reported at 32,000 ft. at 15:45Z. At 15:49Z, Gander Oceanic issued clearance to immediately decent to 31,000 ft. due to traffic. Nil TSB.

Incident at At approximately 5527N6010W (Communication navigation surveillance/air traffic system, ATS operating irregularity)

Prior to QTR078 (Qatar Airways), Boeing 777-200, enroute from Houston (KIAH) to Doha (OTBD), entering oceanic airspace, a modified route clearance was planned, however the updated clearance was not relayed to the aircraft. The aircraft was subsequently re-cleared on the coordinated route after entering oceanic airspace. There was no route deviation or traffic. Nil TSB.

An operating irregularity occurred when QTR077 (Qatar Airways), Boeing 777-300, enroute from Doha (OTBD) to Houston (KIAH), flew a different routing than what was on file with Air Traffic Control (ATC). QTR077 flew TAPLU VBS J579 YUL SYR J29 PXV J131 RUSLR J131 LFK HUB TXMEX1 KIAH while ATC had in his possession TAPLU LOPVI COPUR ROUND TVC OBK J35 SPI J101 READS J180 SWB TXMEX1 KIAH. There was no impact on operations.

Incident at KAMLOOPS BC (CYKA)

Update from M&M - PNR: The snag entered on journey log (part 2) 14131 ""Roll Splr IDBD & OTBD Gnd caution lights came on on takeoff roll approximately 35% TQ. Maintenance YVR did a ground run carried out at take-off power, they were unable to duplicate snag on ground serviceable.
Aircraft rejected take off due to a caution light warning. Problem was fixed and flight departed routinely at 0157z.
Update from the TSB: During power-up for take-off from Kamloops, the outboard roll spoiler caution lights illuminated on the Air Canada Regional DHC-8-102 (C-GGOM, operating as Air Nova ARN646). The crew rejected the take-off and cleared the runway. The problem was rectified and the flight departed without further incident.
