An Alitalia Societa Aerea Italiana Airbus A330-200 (EIEJP/AZA69E) from Havana/Jose Marti Intl, Cuba (MUHA) to Rome, Italy (LIRF) reported at 4100N 05000W, and had not been prior coordinated. Flight was provided an oceanic clearance by Gander area control centre (ACC). No traffic. No impact to operations.

A Blue Panorama Airlines Boeing 767-323 (EIGEP/BPA1712) from Milan/Malpensa, Italy (LIMC) to Havana/Jose Marti Intl, Cuba (MUHA) climbed 300 feet above the assigned altitude of FL360. Controller noticed immediately and advised pilot. Aircraft descended initially to 200 feet above, then climbed again to 300 feet above briefly before correcting to 200 feet below FL360. After a number of corrections, aircraft levelled at FL360. Boston (BOS) air route traffic control center (ARTCC) was advised.

The Cleveland Air Route Traffic Control Center (ZOB) shift manager advised that an Air Canada Rouge Airbus A319-114 (C-FYJP/ROU1877) on a flight from Havana/José Marti Intl, Cuba (MUHA) to Toronto/Lester B. Pearson Intl, ON (CYYZ) had an unruly passenger on board. No emergency declared but they requested priority into CYYZ. North American Aerospace Defence Command (NORAD) advised and ROU1877 landed at 1503Z without further incident.

UPDATE ON TSB Report no A18Q0052: C-GCKR, an Airbus 320-200 operated by Air Transat, effected Flight TSC671 on Instrument Rules from Havana /José Martí Intl, Cuba (MUHA) to Montreal/Pierre Elliott Trudeau Intl, QC (CYUL). On the approach to Runway 06R at CYUL, ATC instructed the crew of TSC671 to climb, as they were catching up to CGTSO, an Airbus 330-300 operated by Air Transat, effecting Flight TSC561 from Cancún Intl, Mexico (MMUN) to CYUL. Horizontal separation had dropped to 4.8 nm, whereas the required separation is 5 nm.
An Air Transat Airbus A320 (TSC671) from Havana, Cuba (MUHA) to Montreal/Pierre Elliott Trudeau, QC (CYUL) was instructed to pull up because it was overtaking an Air Transat Airbus A330-300 (TSC561) from Cancun, Mexico (MMUN) to Montreal/Pierre Elliott Trudeau, QC (CYUL) in front of it. The separation was reduced to 4.8 NM while the aircraft was 6 NM away on final for Runway 06R.

Incident at LAX - Los Angeles International Airport

TSB Report #A18F0058: C-GSJB, an Airbus A319-112 aircraft operated by Air Canada rouge, was conducting flight ROU1875 from Havana/José Martí Intl., Cuba (MUHA) to Toronto/Lester B. Pearson Intl., ON (CYYZ) with 5 crew members and 136 passengers on board. During cruise, the flight crew identified an abnormal humming noise emanating from under the forward galley. The noise stopped abruptly, and an odour accompanied by a slight amount of smoke was detected in the flight deck. The flight crew diverted to Jacksonville Intl., FL (KJAX), declared a PAN PAN and completed the landing with ARFF on standby. The smoke and odour dissipated prior to landing, and the aircraft arrived at the gate without further incident.

Incident at HALIFAX / STANFIELD INTL NS (CYHZ)

UPDATE: TSB Report#A18A0021: D-ABUI, a Boeing 767-300ER aircraft operated by Condor Flugdienst, was conducting flight DE2185 from Havana/José Martí Intl., Cuba (MUHA) to Frankfurt/Rhein-Main, Germany (EDDF) with 10 crew members and 253 passengers on board. While in cruise flight, one of the flight crew members became incapacitated and the third pilot took over this member's duties. A mayday was declared and DE2185 continued on route while the flight crew member's condition was being assessed. Initially, a diversion to St. John's Intl airport, NL (CYYT) was requested, however due to the weather conditions at CYYT, DE2185 subsequently diverted to Halifax/Stanfield Intl Airport, NS (CYHZ). About 1 hour prior to landing, the mayday was cancelled and a Pan Pan was declared. DE2185 landed at CYHZ without further incident.
At 0805Z, the Halifax Tower was advised that a Condor Flugdienst GMBH Boeing 767 300 (DABUI/CFG185) from Havana, Cuba (MUHA) to Frankfurt, Germany (EDDF) was diverting to Halifax / Stanfield Intl., NS (CYHZ) due to an incapacitated pilot. The aircraft landed at 0940Z. No impact to operations.

An Air Canada Rouge Airbus A319 (ROU1753) from Havana, Cuba (MUHA) to Toronto, ON (CYYZ) conducted a missed approach Runway 24R indicating a flap issue. At 1505Z the aircraft declared PAN PAN PAN. The aircraft landed Runway 15L safely at 1526Z. No operational impact.
UPDATE: TSB Report#A17O0030: C-GBHR, an Airbus 319-114 aircraft operated by Air Canada rouge, was conducting flight ROU1753 from Havana/José Marti Intl, Cuba (MUHA) to Toronto/Lester B. Pearson Intl, ON (CYYZ). During the final approach to Runway 24R at CYYZ, the flight crew received a Flap Fault message and performed a go-around. A PAN PAN was declared and the aircraft returned for a landing on Runway 15L with ARFF standing by. The aircraft was not damaged and there were no injuries.

TSB Report#A17F0329: C-GBIN, an Airbus 319-100 aircraft operated by Air Canada rouge, was conducting flight ROU1877 from Havana/José Martí Intl, Cuba (MUHA) to Toronto/Lester B. Pearson Intl, ON (CYYZ) with 5 crew members and 36 passengers on board. A little more than two hours after the departure while in cruise flight, a cabin crew member became incapacitated due to illness and was subsequently removed from further duties. Positions were reassigned among the other cabin crew members for the remainder of the flight. The aircraft landed in CYYZ without further incident.

TSB REPORT#17F0097: C-GBIN, an Airbus 319-100 aircraft operated by Air Canada rouge, was conducting flight ROU1987 from Havana/José Martí Intl, Cuba (MUHA) to Toronto/Lester B. Pearson Intl, ON (CYYZ). During cruise flight, the flight crew received an ECAM spoiler fault message related to the LH Spoiler Number 3. The fault indicated that the spoiler was not fully retracted. The flight crew declared a PAN PAN and requested a diversion to Miami Intl, FL (KMIA). Subsequently, the flight continued to CYYZ without further incident.

TSB REPORT#A17F0104: C-GTQF, a Boeing 737-800 aircraft operated by Air Transat, was conducting flight TSC949 from Montego Bay/Sangster Intl, Jamaica (MKJS) to Havana/José Martí Intl, Cuba (MUHA). While the aircraft was being marshalled to Gate 13 at MUHA, the left winglet came in contact with the wing of a parked Airbus A330 at Gate 12. The operator?s maintenance removed the left winglet and the aircraft was flown back to Canada on a ferry permit. The winglet was replaced and the aircraft was returned to service.

After having received a point-out approval from Montreal VA controller, on an Air China Boeing 777-300 (CCA879) from Montreal, QC (CYUL) to La Habana, Cuba (MUHA) with a condition to respect a Jazz de Havilland DHC-8D (JZA8940) from Toronto, ON (CYYZ) to Moncton, NB (CYQM), separation was not assured by the PLB controller. Lateral separation was 4.4NM and vertical separation was 600 feet. Traffic was given to JZA8940 by the Montreal air traffic control (ATC). No other impact on operations.
Update TSB#A16Q0016: A Jazz Dash 8-400 (JZA940) was on an IFR flight from Toronto/Lester B. Pearson Intl Airport (CYYZ), ON to Moncton/Greater Moncton Intl (CYQM), NB. After receiving a pointout from the Montréal control centre about an Air China Boeing 777-300 (CCA879) on an IFR flight from Montréal/Pierre Elliott Trudeau Intl (CYUL), QC, to Havana/José Marti Intl (MUHA), Cuba, separation was not ensured by the Boston Centre controller. Separation between the two aircraft was reduced to 600 ft vertically and 4.4 mi. horizontally while required separation is 1000 ft (vertical) and 5 mi. (horizontal). JZA940 was notified of traffic by Montréal centre.

When New York Oceanic attempted initial coordination with Gander for an Airbus A319 (P4MIS) from Havana, Cuba (MUHA) to Zurich, Switzerland (LSZH) at FL360 on Track Z, Gander was not willing to accept the flight as information showed that the flight was not data link mandate (DLM)-capable. New York advised that the flight was capable but Gander was not able to modify the flight plan information. Since there was turbulence at lower non-DLM levels, Gander Oceanic agreed to accept the flight at FL 360 even though they were not DLM-certified.

An American corporately registered Boeing 737 700 (N788DP) from Havana, Cuba (MUHA) to London, UK (EGSS), FL370, was routing TUSKY direct ELSIR. When the flight passed TUSKY, it made a sharp turn to the right. The controller queried the flight about the route and the pilot said they were routing to SUPRY as a result of a requested change in the Oceanic Entry Point (OEP). Traffic at FL370, 30 NM south and converging. N788DP was given a new clearance to SUPRY and given climb to FL390. No impact to Air Traffic Service (ATS) operations.

Incident at Approx. 200NM au sud-ouest de Montréal/Trudeau (CYUL)

CUB171, an Airbus A-320 operated by Cubana De Aviacion S.A., was on an IFR flight from Montréal/Trudeau (CYUL) to Habana/Jose Marti (MUHA). At 0020Z, while the aircraft was approximately 200 NM northwest of Montréal/Trudeau, the crew declared a medical emergency because a passenger was ill. The crew requested to return to Montréal/Trudeau.
