While inbound for Quebec/Jean Lesage Intl, QC (CYQB), a Sunwing Boeing 737-800 (SWG386) from Cancun, Mexico (MMUN) to Quebec/Jean Lesage Intl, QC (CYQB) reported a fuel leak and declared an emergency. SWG386 requested to be diverted to CYUL. Coordination carried out. SWG386 landed on Runway 24R at 2142Z. Impact: Runway 06L/24R was unavailable from 2142Z to 2154Z due to the fuel spill. The National Operations Centre (NOC) and Aviation Operations (AVOPS) were notified.
TSB Update A19Q0199: C-GOWG, a Sunwing Airlines Inc. Boeing 737-800 (SWG386), was on an instrument flight rules (IFR) flight from Cancún (MMUN) international airport in Mexico to Quebec Jean Lesage, QC (CYQB) with 185 passengers and 6 crew members on board. While the aircraft was in descent 85 NM south of CYQB, the crew notified the air traffic control (ATC) centre that it wanted to be diverted to Montreal/Pierre Elliott Trudeau, QC (CYUL) due to a fuel leak. Afterwards, the crew declared a state of emergency and the aircraft landed on Runway 24R at CYUL without incident with Aircraft Rescue and Fire Fighting (ARFF) services on standby. No one was injured and there was no damage. The post-landing runway inspection revealed fuel spillage.

There was a loss of separation between an Air Canada Rouge Airbus A319-112 (ROU1519) from Quebec/Jean Lesage, QC (CYQB) to Montreal/Pierre Elliott Trudeau, QC (CYUL) on final for Runway 24L, and an Air Canada Rouge Airbus A321-211 (C-GHQI/ROU1883) from Cancun, Mexico (MMUN) to Montreal/Pierre Elliott Trudeau, QC (CYUL). The separation was reduced to 2.0 NM and 800 ft, where it should be 3 NM or 1 000 ft.
TSB Update #A19Q0141: C-GHQI, an Air Canada Rouge Airbus A321, was on flight ROU1883 from Cancún Intl, MX (MMUN) to Montreal/Pierre Elliott Trudeau, QC (CYUL). While ROU1883 was on approach for CYUL, the controller made it turn too sharply on base, which resulted in a loss of separation with C-FYIY, an Airbus A319 also operated by Air Canada Rouge, which was on final approach for Runway 24L. The aircraft came within 2 NM of each other laterally and 800 ft vertically, though the required separation was 3 NM or 1 000 ft.

An ABC Aerolineas Airbus A320 (AIJ2821) from Montreal/Trudeau, QC (CYUL) to Cancun, Mexico (MMUN) called in initial contact in its climb to 5 000 ft. AIJ2821 received and read back the instruction to turn right on a heading of 150 (H150) to 3 000 ft, but was seen turning to 2 100 ft. It stopped climbing at 3 000 ft instead of continuing to 5 000 ft. Impact: AIJ2821 did not obey the noise abatement measures.

An Air Transat Boeing 737-8HX (C-FTVF/TSC731) from Cancun Int., Mexico (MMUN) to Montreal/Trudeau Int., QC (CYUL), which was cleared to follow H210 to intercept the instrument landing system (ILS) for Runway 24R, mentioned that it had a faulty indication. Air traffic services (ATS) cleared TSC731 to follow H270 so that it could intercept the approach from south of the localizer beacon (LOC). TSC731 conducted a left turn instead of turning right, towards H270, and mentioned an autopilot error. No impact on operations.

An Air Transat Boeing 737-8K2 (C-GZEH/TSC437) from Cancún, Mexico (MMUN) to Montreal/Trudeau, QC (CYUL) overshot Runway 24R, because it had a check to complete. At 0644Z, TSC437 declared an emergency due to a flap problem. The aircraft landed without incident. At 0715Z, TSC437 was towed to the gate. No other impact on operations.
Update - TSB Report #A19Q0014: an Air Transat Boeing 737-800 (C-GZEH/TSC437), with 6 crew members and 173 passengers on board, was on an instrument flight rules (IFR) flight from Cancún, Mexico (MMUN) to Montreal/Pierre Elliott Trudeau, QC (CYUL). While C-GZEH was on approach for Runway 24R, its flaps were stuck in position 2, with slightly asymmetric flaps. The aircraft rejected its approach and reviewed the checklist in the Quick Reference Handbook (QRH). The crew declared an emergency, and the intervention of aircraft rescue and firefighting services (ARFF) was required. C-GZEH landed without incident, then was towed to the terminal after inspection. No one was injured.

Incident at VICTORIA INTL BC (CYYJ)

Aviation Incident Report #15946: A passenger aboard a WestJet Boeing 737-76N (C-FJWS/WJA2049) from Cancun, Mexico (MMUN) to Victoria, BC (CYYJ) was told by another passenger to silence their child from making noise. After some words, the passenger with the child says they were struck in the head by the other passengers elbow. There was a witness for the incident. Passenger who was elbowed wanted to press charges. Royal Canadian Mounted Police (RCMP) met the flight on arrival in CYYJ. Once on the ground in CYYJ, witness confirmed that the altercation seemed accidental. RCMP does not believe charges will be pressed. RCMP took statements from all 3 individuals.

UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. Review of Technical Records from September 13th to 17th indicate no report of bird strike.

An Air Transat Boeing 737-8Q8 (C-GTQJ/TSC838) on a flight from Toronto/Lester B. Pearson Intl, ON (CYYZ) to Cancun, Mexico (MMUN) aborted takeoff at CYYZ Runway 23 due to an indication and returned to the gate. TSC838 lined up again at 1153Z, aborted takeoff and returned to the gate without incident.

Possible traffic was an Air Canada Boeing 787-9 (C-FRTG/ACA857) on a flight from London/Heathrow Intl, UK (EGLL) to Toronto/Lester B. Pearson Intl, ON (CYYZ) on a straight in approach Runway 24R 5NM ahead. No impact on operations.

Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ) (Aerodrome, runway or taxiway shutdown)

A Sunwing Airlines Boeing 737-400 (SWG272) on a flight from Cancun, Mexico (MMUN) to Toronto/Lester B. Pearson Intl, ON (CYYZ) reported a possible blown tire on rollout. An Air Canada Boeing 777-300 (ACA847) on a flight from Munich, Germany (EDDM) to Toronto/Lester B. Pearson Intl, ON (CYYZ) was overshot. Several arrivals were diverted to Runway 06R. A few departures were delayed. Greater Toronto Airports Authority (GTAA) inspected the runway and no debris was found. The aircraft began leaking fluid in the holding bay Runway 24R. Subsequently the GTAA closed Runway06L due to the tail of the disabled aircraft being in the obstacle limitation surface (OLS).

Pearson, ON (CYYZ) was slow to vacate. SKV7669 landed at 0207Z without further incident. No operational impact.

Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ)

A WestJet Boeing 737-800 (WJA2685) from Cancun, Mexico (MMUN) to Toronto/Lester B. Pearson, ON (CYYZ) reported flying through a flock of birds over the threshold. Nothing was found.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. The pilot entered a defect into the journey log reporting a bird strike on final approach. Maintenance verified the defect, cleaned and inspected the area in accordance with Boeing Aircraft Maintenance Manual (AMM) 05-51-18. No faults were found. Aircraft was returned to service.
