Impact: an Air Canada Airbus A320 (ACA326) from Calgary, AB (CYYC) to Montreal/Trudeau, QC (CYUL) was instructed to reduce its speed to 150 kt 12 NM on final to maintain the required separation.

En route from Québec/Jean Lesage, QC (CYQB) to Punta Cana, Dominican Republic (MDPC), an Air Transat Airbus A310 (TSC406) reported hydraulic problems to the Federal Aviation Administration (FAA) and asked to divert to Montreal/Pierre Elliott Trudeau, QC (CYUL) . No emergency declared; however, TSC406 had to be towed away to clear the runway. TSC406 landed on Runway 24R at 1803Z. At landing, TSC406 had a hydraulic fluid leak. Impact: Runway 06L/24R was unavailable from 1803Z to 1826Z. The National Operations Centre (NOC) was notified.

Reason for aborted take-off: LOW SPEED ABORT 75KTS R ENG Electronic Engine Control (EEC) engine-indicating and crew-alerting system (EICAS) message, and OVERHEAD FUEL GAGE ON OVERHEAD RT SIDE BLANKS ALONG WITH TOTALIZER. Maintenance action after aborted takeoff (T/O): Performed EEC test as per Aircraft Maintenance Manual (AMM) 73-21-07; performed EEC maintenance monitor unit bite per FIM 71-01 fault tree 804; performed MCDP A/T test as per AMM 22-00-02 - unable to reproduce fault, and removed and replaced fuel quantity indicating system processor unit as per AMM 289-41-08. Operational test normal, unable to reproduce fault as per AMM TASK28-41-00-715-001-001. Aircraft has not flown since event.

An Air Canada Rouge Boeing 767-333 (C-GHLT/ROU1713) from Punta Cana, Dominican Republic (MDPC) to Toronto/Pearson, ON (CYYZ) was instructed to initiate a missed approach to Runway 06L and landed on Runway 06L at 2325Z without incident. 3 other aircraft (ACA792, ASH6195, and ASH6196) were instructed to side step to Runway 06R. No birds were found, no impact to operations.

Update TSB Report #A19O0013: OO-SRO, a Boeing 737-800 aircraft operated by TUI Airlines Belgium on behalf of Sunwing Airlines, was conducting flight SWG425 from Punta Cana Intl (MDPC), Dominican Republic to Ottawa/MacDonald-Cartier Intl (CYOW), ON with 6 crew members and 177 passengers on board. Just prior to the top of descent into CYOW, the Hydraulic System B failed. The flight crew declared a PAN PAN when in range to the airport, and the aircraft landed without further incident on Runway 07. The operator’s maintenance replaced a hydraulic pump, and the aircraft was returned to service.
Update from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. Hydraulic failure of engine driven hydraulic pump (EDP) re-occurred. Maintenance action: Number 2 EDP replaced EDP (2nd time), EDP suction hose and filter replaced (due to contamination).

At the Ottawa/Macdonald-Cartier, ON (CYOW) aerodrome, during a controller change, the controller who was replaced did not inform their colleague that Taxiway Echo had been closed by NOTAM due to water that had pooled on it. The ground controller cleared an Air Canada Airbus A320-211 (C-GQCA/ACA1406) from Ottawa/Macdonald-Cartier, ON (CYOW) to Punta Cana, Dominican Republic (MDPC) to taxi on Echo. ACA1406, which indicated that it could cross the flooded zone, was escorted by a vehicle so that it could do so.

An Air Canada Rouge Boeing 767-333 (C-FMWP/ROU1712) from Toronto/Lester B. Pearson Intl, ON (CYYZ) to Punta Cana Intl, Dominican Republic (MDPC) reported a possible bird strike after departing Runway 23. An inspection vehicle reported finding debris and indicated it appeared to be a bird of prey which was ingested in the engine. A Private Air Inc. Beech 200 (C-GMNM/FSY134) from Teterboro, NJ (KTEB) to Toronto/Lester B. Pearson Intl, ON (CYYZ) was instructed to initiate a missed approach. Afterwards, Air Canada (ACA) Dispatch reported that ROU1712 would be returning to CYYZ due to vibration in the engine. No emergency was declared and ROU1712 landed on Runway 23 at 1920Z without incident. No impact to operations.
Update TSB Report #A19O0014: C-FMWP, a Boeing 767-300 aircraft operated by Air Canada rouge, was conducting flight ROU1712 from Toronto/Lester B. Pearson Intl (CYYZ), ON to Punta Cana Intl (MDPC), Dominican Republic with 8 crew members and 280 passengers on board. During the takeoff roll on Runway 23 at CYYZ, the aircraft struck a bird. The flight crew reported observing a large bird, hearing a loud bang, and a slight adverse yaw was detected, however engine parameters were normal; therefore, the takeoff was continued. Following a brief level off at 5000 ASL to evaluate the situation, the decision was made to continue the flight in the absence of abnormal parameters. During cruise flight at FL330, the flight crew noticed increasing vibrations on the left engine, and elected to return to CYYZ as a precaution. The aircraft landed overweight without further incident. The operator’s maintenance inspected the left engine and, based on the extent of the damage, elected to replace it.

While passing 1500 feet during the initial climb after the departure off Runway 08 at MDPC, the number 1 engine (General Electric CF6-80C2A2) experienced compressor stalls. The flight crew executed the QRH procedure, and requested a return to MDPC. Initially, the number 1 engine was brought to idle, then back to 50% N1 for the landing. A MAYDAY was declared, and the aircraft landed on Runway 08 without further incident. ARFF inspected the aircraft after the landing, then cleared the flight crew to taxi to the ramp. The operator’s maintenance performed a borescope of the booster and High Pressure Compressor (HPC) STG1 and STG12 in accordance with (IAW) Aircraft Maintenance Manual (AMM) 72-31-00. Minor blade nicks and dents were observed, all within AMM limits. The High Pressure Turbine (HPT), Variable Stator Vane (VSV) and the Oil Magnetic Chip Detector (MCD) were also inspected IAW the AMM, and nothing was found. The Intermediate Pressure (IP) check valve and Compressor Inlet Temperature (CIT) sensor were replaced IAW the AMM, and following engine power runs, the aircraft was returned to service. The aircraft returned to CYUL without further incident.

An Air Canada Rouge Airbus A319-114 (C-FYIY/ROU1987) from Punta Cana, Dominican Republic (MDPC) to Montreal, QC (CYUL) requested clearance to conduct a descent at the Boston Centre due to depressurization. The aircraft landed at 2323Z without incident. No impact on operations.

Incident at MDPC - Punta Cana Intl - Punta Cana

TSB Report #A19F0074: C-GTSN, an Airbus 330-200 aircraft operated by Air Transat, was conducting flight TSC141 from Punta Cana Intl (MDPC), Dominican Republic to Toronto/Lester B. Pearson Intl (CYYZ), ON with 11 crew members and 332 passengers on board. During the turn around time in MDPC, a cabin crew member became ill and was unable to perform duties for the return flight. Duties were reassigned to other cabin crew members, the aircraft departed and completed its flight to destination without further incident.

TSB Report #A19F0065: C-FZUR, a Boeing 737-800 aircraft operated by Sunwing Airlines, was conducting flight SWG122 from Montreal/Pierre Elliott Trudeau Intl (CYUL), QC to Punta Cana Intl (MDPC), Dominican Republic with 6 crew members and 175 passengers on board. During cruise flight, a flight crew member started to feel sick, requiring repeated absences from the cockpit. Subsequently, the affected flight crew member was able to resume flying duties. The flight continued to destination without further incident.

Incident at PUNTA CANA INTL - PUNTA CANA, DOMINICAN REPUBLIC (MDPC)

TSB Report #A19F0003: C-FYJD, a Boeing 737-800 aircraft operated by Sunwing Airlines, was conducting flight SWG326 from Regina Intl (CYQR), SK to Punta Cana Intl (MDPC), Dominican Republic with 6 crew members and 187 passengers on board. Passing 2500 feet ASL as the aircraft was on final approach for Runway 08 at MDPC, the flight crew received a TCAS RA to LEVEL OFF, followed immediately by an RA to CLIMB. The conflicting traffic was a helicopter that was tracking towards SWG326, outbound from the field and approximately on the runway track. The flight crew executed the command, executed a missed approach, and returned for an uneventful landing.

Incident at SASKATOON / JOHN G. DIEFENBAKER INTL SK (CYXE)

A Sunwing Airlines Boeing 737-8HX (C-FLSW/SWG360) from Saskatoon/Diefenbaker International, SK (CYXE) to Punta Cana International, Dominican Republic (MDPC) rejected its takeoff.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. The pilot rejected take off at approximately 65 Knots Indicated Airspeed (KIAS) due to a "Cowl Anti-Ice" master caution. Subsequent take-off was normal.

A Porter Airlines de Havilland DHC-8-402 (C-GLQN/POE241) from Ottawa/MacDonald-Cartier, ON (CYOW) to Greater Moncton Romeo LeBlanc, NB (CYQM) missed the approach due to excess speed. The aircraft landed on second approach at 2028Z. A Sunwing Airlines Boeing 737-86N (C-GKVV/SWG430) from Greater Moncton Romeo LeBlanc, NB (CYQM) to Punta Cana, Dominican Republic (MDPC) on departure, may have been slightly delayed. No impact to operation.

Incident at 40NM YARMOUTH NS VOR/DME (YQI)

The pilot of a Sunwing Airlines Inc. Boeing 737-8K5 (C-FTUA/SWG455) from Punta Cana, Dominican Republic (MDPC) to Halifax/Stanfield, NS (CYHZ) reported that a flight attendant was not feeling well and requested an ambulance upon arrival. No emergency declared. The aircraft landed at 0531Z. No impact on operations.
Update TSB Report #A19A0018: C-FTUA, a Boeing 737-800 aircraft operated by Sunwing Airlines, was conducting flight SWG455 from Punta Cana Intl (MDPC), Dominican Republic to Halifax/Stanfield Intl (CYHZ), NS with 6 crew members and 162 passengers on board. During cruise flight, one of the cabin crew members became ill and was unable to continue duties. Duties were distributed to other cabin crew members and, prior to the descent into CYHZ, the ill cabin crew member started to feel worse. Paramedics were requested to meet the aircraft upon arrival, and the aircraft landed without further incident. The ill crew member received medical attention at the gate.
