Incident at MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL)

A Swiss International Air Lines Airbus A330-300 (SWR86) from Zurich, Switzerland (LSZH) to Montreal/Pierre Elliott Trudeau Intl, QC (CYUL) was instructed to pull up on final for Runway 24L, since the aircraft that had landed before it was still on the runway.

Incident at 1.2NM S MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL)

Immediately after takeoff, a Swiss International Air Lines Airbus A330-300 (HBJHE/SWR87) from Montreal/Pierre Elliott Trudeau, QC (CYUL) to Zurich, Switzerland (LSZH) declared an emergency due to a landing gear problem. Emergency vehicles were deployed. The aircraft landed without incident at 2155Z. A runway inspection was conducted and nothing unusual was reported. No impact on operations.
Update: TSB Report #A19Q0176: HB-JHE, a Swiss Airbus A330-300 (SWR87) with 12 crew members and 206 passengers on board, was on an IFR flight from Montreal/Pierre Elliott Trudeau Intl, QC (CYUL) to Zürich-Kloten, Switzerland (LSZH). After takeoff, the flight crew saw the message L/G R LENGTHENING FAULT on the Electronic Centralized Aircraft Monitor (ECAM). A PAN PAN was declared and the aircraft went back and conducted an overweight landing without incident at CYUL. Maintenance staff noted that the hydraulic cylinder extension on the right main landing gear was damaged.

Air traffic services (ATS) instructed an Air Transat Airbus A330-200 (TSC455) from Marseille, France (LFML) to Montreal/Pierre Elliott Trudeau, QC (CYUL) to conduct a go-around because its separation with the preceding aircraft, a Swiss International Airlines Airbus A330-300 (SWR86) from Zurich, Switzerland (LSZH) to Montreal/Pierre Elliott Trudeau, QC (CYUL) was not going to be maintained. TSC455 landed on Runway 24R at 1919Z.

Incident at 30NM SEPT-ILES QC VOR/DME (YZV) (ATS operating irregularity)

A United Airlines Boeing 777-200 (UAL56) from Paris, France (LFPG) to Newark, NJ (KEWR) and an Air Canada Boeing 787-9 (ACA879) from Zurich, Switzerland (LSZH) to Toronto/Lester B. Pearson, ON (CYYZ) had less than required separation after air traffic control (ATC) issued a climb clearance to UAL56.
TSB #A19Q0054: N788UA, a United Airlines Boeing 777-200, was on instrument flight rules (IFR) flight UAL56 from Paris/Charles de Gaulle, France (LFPG) to Newark/Liberty Intl, NJ (KEWR). C-FRTU, an Air Canada Boeing 787-9, was on instrument flight rules (IFR) flight ACA879 from Zürich-Kloten, Switzerland (LSZH) to Toronto/Lester B. Pearson Intl, ON (CYYZ). UAL56 was level at FL360, while ACA879 was also level at FL380. The two aircraft followed headings that were similar, but divergent. Approximately 30 NM southwest of Sept-Îles, QC (CYZV), UAL56 was cleared to climb to FL380. There was a loss of separation when UAL56 established itself at FL380 and the horizontal distance between the 2 aircraft was reduced to 8.8 NM. The required separation was 10 NM horizontally, or 1 000 ft vertically.

An Air Canada Airbus A330-343 (C-GHLM/ACA878) from Toronto/Pearson Intl, ON (CYYZ) to Zurich, Switzerland (LSZH) requested to land at Montreal/Trudeau Intl, QC (CYUL) due to a mechanical issue. At 2316Z, ACA878 advised that they would have to land due to being too heavy and requested that emergency vehicles meet the aircraft. The aircraft landed at 2334Z with no issue. No impact on operations.
Update: TSB Report #A19Q0028: C-GHLM, an Air Canada Airbus 330-300, was on IFR flight ACA878 from Toronto/Lester B. Pearson Intl, ON (CYYZ) to Zürich-Kloten, Switzerland (LSZH) with 10 crew members and 215 passengers on board. Shortly after departure from CYYZ, the crew received several alert messages. The crew then asked to divert to Montreal/Pierre Elliott Trudeau Intl, QC (CYUL) and requested the presence of emergency vehicles with the intention of conducting an overweight landing. The aircraft landed without incident, and its mass upon landing was within the prescribed limits. There were no injuries. Various parts were found to be faulty (HP Bleed Valve #1, GPWC, FMGEC #1 and FCC #1) and were replaced.

A Swiss International Air Lines Airbus A340-313 (HBJMF/SWR8010) from Zurich, Switzerland (LSZH) to Vancouver, BC (CYVR) was at Gate 64 and was struck by an Air Canada Boeing 737-8 (C-GHPU/ACA566) from Vancouver, BC (CYVR) to San Francisco, CA (KSFO) that was taxiing behind.
Update TSB #A19P0116: HB-JMF, an Airbus A340-300 aircraft operated by Edelweiss Air, conducted flight SWR8010 from Zurich (LSZH), Switzerland to Vancouver Intl (CYVR), BC. Upon arrival at gate 64, HB-JMF was marshalled into the gate area, signaled to stop and set the parking brake. The engines were shut down. The ground personnel connected a tug and were preparing to tow the aircraft to the final gate parking position. Meanwhile, C-GHPU, an Air Canada Boeing 787-800 aircraft operating flight AC566 from CYVR to San Francisco Intl (KSFO), CA was taxiing westbound on JA taxiway, abeam gate 64, when the left wing tip contacted the tail section of HB-JMF. There were no injuries. Both aircraft sustained substantial damage.

TSB #A19F0111: C-FVLU, a Boeing 787-9 aircraft operated by Air Canada, was conducting flight ACA879 from Zurich-Kloten (LSZH), Switzerland to Toronto/Lester B. Pearson Intl (CYYZ), ON with 11 crew members and 175 passengers on board. Shortly after the departure from LSZH, the flight crew received a C1 Hydraulic Pressure message, followed a Generator Control L1 EICAS message, which caused the associated generator to trip offline. The flight crew declared a PAN PAN and, after executing the checklist, the system successfully returned online. The flight crew cancelled the emergency, and continued to destination without further incident.

Incident at Zurich

TSB #A18F0274: C-GFAH, an Airbus 330-300 aircraft operated by Air Canada, was conducting flight ACA878 from Toronto/Lester B. Pearson Intl (CYYZ), ON to Zürich-Kloten (LSZH), Switzerland with 11 crew members and 275 passengers on board. During the landing on Runway 14 at LSZH, the flight crew experienced a Nose Wheel Steering Fault and was unable to steer the aircraft. The aircraft needed to be towed off the runway, all the way to the gate. The operator’s maintenance replaced the hydraulic block in accordance with the Aircraft Maintenance Manual. Functional tests and leak checks were carried out, and no further faults were found. The aircraft was returned to service.

A few moments later, an Air China Ltd. Boeing 787-9 (B1466/CCA879) from Beijing Capital International, China (ZBAA) to Montreal/Pierre Elliott Trudeau International, QC (CYUL) was observed descending through FL350 for FL340. When questioned, CCA879 thought the authorization was intended for them. No operational impact.

Aviation Incident Report: The Federal Aviation Administration (FAA) reported that an Air Canada Airbus A330 (ACA664) enroute from Montreal/Pierre Elliott Trudeau Intl, QC (CYUL) to Halifax/Stanfield Intl, NS (CYHZ) reported a medical emergency after a flight attendant sustained an injury during turbulence. The aircraft encountered wake turbulence from a Swiss International Boeing 777 (SWR8T) enroute from Zurich, Switzerland (LSZH) to Chicago/OHare, IL (KORD), 56 miles west-northwest of Bangor, ME at flight level FL350. The flight continued to CYHZ.

Incident at 7255N / 07252W

Regarding a Swiss International Air Lines Ltd. Boeing 777-300 Series (SWR40) from Zurich, Switzerland (LSZH) to Los Angeles International, CA (KLAX), an air traffic controller received a non-conforming automatic position report where the next fix was reported as 7300N08500W rather than 7000N08500W, as indicated by the flight plan. The controller contacted Gander Radio to confirm the route with the aircraft. The aircraft checked in, and confirmed they were proceeding towards 73/85, and confirmed air traffic control (ATC) clearance to 70/85. The controller re-cleared the aircraft to 70/85 and obtained a position report. The aircraft was 33 nautical miles (NM) off course. Separation was not jeopardized. No impact to operations.

Incident at 36NM GRINDSTONE (ILES-DE-LA-MADELEINE) QC (YGR) VOR/DME

An Edelweiss Air Airbus A340-300 (HBJMG/EDW25) on a flight from Cancun, Mexico (MMUN) to Zurich, Switzerland (LSZH) declared a medical emergency with a request to land at Halifax/Stanfield, NS (CYHZ). The aircraft conducted a fuel dump and landed safely at 0411Z with minimal impact on the operation.

Incident at 96NM E QUÉBEC / JEAN LESAGE INTL QC (CYQB)

UPDATE: TSB Report#A17F0235: C-GFAH, an Airbus 330-300 aircraft operated by Air Canada, was conducting flight ACA879 from Zürich-Kloten, Switzerland (LSZH) to Toronto/Lester B. Pearson Intl, ON (CYYZ) with 11 crew members and 240 passengers on board. During cruise flight, one of the cabin crew members became ill and was unable to continue duties. Positions were reassigned among the other cabin crew members for the remainder of the flight.
An Air Canada Airbus A330-300 (ACA870) on a flight from Zurich, Switzerland (LSZH) to Toronto/Lester B. Pearson, ON (CYYZ) declared a medical emergency onboard. Emergency services arranged by Air Canada. The aircraft landed Runway 15L without incident at 1619Z. No operational impact.
