Incident at 6NM NE MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL)

Air traffic services (ATS) instructed an Air Transat Airbus A330-200 (TSC455) from Marseille, France (LFML) to Montreal/Pierre Elliott Trudeau, QC (CYUL) to conduct a go-around because its separation with the preceding aircraft, a Swiss International Airlines Airbus A330-300 (SWR86) from Zurich, Switzerland (LSZH) to Montreal/Pierre Elliott Trudeau, QC (CYUL) was not going to be maintained. TSC455 landed on Runway 24R at 1919Z.

Incident at 30NM SEPT-ILES QC VOR/DME (YZV) (ATS operating irregularity)

A United Airlines Boeing 777-200 (UAL56) from Paris, France (LFPG) to Newark, NJ (KEWR) and an Air Canada Boeing 787-9 (ACA879) from Zurich, Switzerland (LSZH) to Toronto/Lester B. Pearson, ON (CYYZ) had less than required separation after air traffic control (ATC) issued a climb clearance to UAL56.
TSB #A19Q0054: N788UA, a United Airlines Boeing 777-200, was on instrument flight rules (IFR) flight UAL56 from Paris/Charles de Gaulle, France (LFPG) to Newark/Liberty Intl, NJ (KEWR). C-FRTU, an Air Canada Boeing 787-9, was on instrument flight rules (IFR) flight ACA879 from Zürich-Kloten, Switzerland (LSZH) to Toronto/Lester B. Pearson Intl, ON (CYYZ). UAL56 was level at FL360, while ACA879 was also level at FL380. The two aircraft followed headings that were similar, but divergent. Approximately 30 NM southwest of Sept-Îles, QC (CYZV), UAL56 was cleared to climb to FL380. There was a loss of separation when UAL56 established itself at FL380 and the horizontal distance between the 2 aircraft was reduced to 8.8 NM. The required separation was 10 NM horizontally, or 1 000 ft vertically.

Incident at 130NM MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL)

An Air Canada Airbus A330-343 (C-GHLM/ACA878) from Toronto/Pearson Intl, ON (CYYZ) to Zurich, Switzerland (LSZH) requested to land at Montreal/Trudeau Intl, QC (CYUL) due to a mechanical issue. At 2316Z, ACA878 advised that they would have to land due to being too heavy and requested that emergency vehicles meet the aircraft. The aircraft landed at 2334Z with no issue. No impact on operations.
Update: TSB Report #A19Q0028: C-GHLM, an Air Canada Airbus 330-300, was on IFR flight ACA878 from Toronto/Lester B. Pearson Intl, ON (CYYZ) to Zürich-Kloten, Switzerland (LSZH) with 10 crew members and 215 passengers on board. Shortly after departure from CYYZ, the crew received several alert messages. The crew then asked to divert to Montreal/Pierre Elliott Trudeau Intl, QC (CYUL) and requested the presence of emergency vehicles with the intention of conducting an overweight landing. The aircraft landed without incident, and its mass upon landing was within the prescribed limits. There were no injuries. Various parts were found to be faulty (HP Bleed Valve #1, GPWC, FMGEC #1 and FCC #1) and were replaced.

TSB #A19F0111: C-FVLU, a Boeing 787-9 aircraft operated by Air Canada, was conducting flight ACA879 from Zurich-Kloten (LSZH), Switzerland to Toronto/Lester B. Pearson Intl (CYYZ), ON with 11 crew members and 175 passengers on board. Shortly after the departure from LSZH, the flight crew received a C1 Hydraulic Pressure message, followed a Generator Control L1 EICAS message, which caused the associated generator to trip offline. The flight crew declared a PAN PAN and, after executing the checklist, the system successfully returned online. The flight crew cancelled the emergency, and continued to destination without further incident.

Incident at Zurich

TSB #A18F0274: C-GFAH, an Airbus 330-300 aircraft operated by Air Canada, was conducting flight ACA878 from Toronto/Lester B. Pearson Intl (CYYZ), ON to Zürich-Kloten (LSZH), Switzerland with 11 crew members and 275 passengers on board. During the landing on Runway 14 at LSZH, the flight crew experienced a Nose Wheel Steering Fault and was unable to steer the aircraft. The aircraft needed to be towed off the runway, all the way to the gate. The operator’s maintenance replaced the hydraulic block in accordance with the Aircraft Maintenance Manual. Functional tests and leak checks were carried out, and no further faults were found. The aircraft was returned to service.

When questioned, CCA879 thought the authorization was intended for them. No operational impact.

Incident at 7255N / 07252W

Regarding a Swiss International Air Lines Ltd. Boeing 777-300 Series (SWR40) from Zurich, Switzerland (LSZH) to Los Angeles International, CA (KLAX), an air traffic controller received a non-conforming automatic position report where the next fix was reported as 7300N08500W rather than 7000N08500W, as indicated by the flight plan. The controller contacted Gander Radio to confirm the route with the aircraft. The aircraft checked in, and confirmed they were proceeding towards 73/85, and confirmed air traffic control (ATC) clearance to 70/85. The controller re-cleared the aircraft to 70/85 and obtained a position report. The aircraft was 33 nautical miles (NM) off course. Separation was not jeopardized. No impact to operations.

Pearson Intl, ON (CYYZ) with 11 crew members and 240 passengers on board. During cruise flight, one of the cabin crew members became ill and was unable to continue duties. Positions were reassigned among the other cabin crew members for the remainder of the flight.

An Air Canada Airbus A330-300 (ACA878) on a flight from Toronto/Lester B. Pearson, ON (CYYZ) to Zurich, Switzerland (LSZH) departed Runway 24R. On departure, N229BW started a shallow turn towards ACA878. The controller in departure position assigned N229BW a heading away from ACA878. N229BW's initial turn was in the opposite direction of the standard instrument departure (SID). The pilot stated flight management system (FMS) issues at the time of occurrence.

UPDATE TSB Report #A17O0184: N728YX, an Embraer ERJ 170-200 LR aircraft operated by Republic Airline, was conducting flight RPA3553 from Newark/Liberty Intl, NJ (KEWR) to Toronto/Lester B. Pearson Intl, ON (CYYZ). After the landing on Runway 24L at CYYZ, the flight crew exited on Taxiway D6 and was instructed to hold short of Runway 24R. The tower controller observed the aircraft approaching the hold short line at approximately 20 knots of groundspeed and instructed the aircraft to stop. The aircraft came to a stop across the hold short line. ACA878, a Boeing 787 aircraft operated by Air Canada from CYYZ to Zürich-Kloten, Switzerland (LSZH), was departing off Runway 24R and passed overhead RPA3553.
A Republic Airlines Embraer ERJ 170-200SU (RPA3553) on a flight from Newark, NJ (KEWR) to Toronto/Lester B. Pearson, ON (CYYZ) was instructed to hold short of Runway 24R. The read back was correct. Tower controller observed the aircraft not appearing to stop and instructed the aircraft to stop. An Air Canada Boeing 787-9 (ACA878) on a flight from Toronto/Lester B. Pearson, ON (CYYZ) to Zurich, Switzerland (LSZH) was on take off roll when RPA3553 stopped with the nose of the aircraft across the hold short line.

A Swiss International Air Lines Boeing 777 300 (HBJND/ SWR40) from Zurich, Switzerland (LSZH) to Los Angeles, CA (KLAX) (diverted to Iqaluit, NU, see CADORS# 2017C0405) got stuck in the turn around bay at the Threshold of 16. The runway was NOTAMed closed until the aircraft was towed clear at 2110Z. The closure resulted in a First Air Bae Systems RJ85 (C-FLRJ/ FAB955) from Yellowknife, NT (CYZF) to Iqaluit, NU (CYFB) holding until able to land at 2121Z and a Canadian North Boeing 737-36N (C-GZCN/ MPE436) from Ottawa, ON (CYOW) to Iqaluit, NU (CYFB) diverting to Kuujjuaq, QC (CYVP).

Incident at 6830N 07000W

UPDATE: JRCC Halifax SARSUM Report#[H2017-00084]: (660972N 0735097W - Baffin Island, NU). Montreal area control centre (ACC) called to report that a Swiss International Air Lines Boeing 777-300 (SWR40) had an engine failure and was diverting to Iqaluit, NU (CYFB). The aircraft landed in Iqaluit without further incident.
A Swiss International Air Lines Boeing 777 300 (SWR40) from Zurich, Switzerland (LSZH) to Los Angeles, CA (KLAX) declared a PAN PAN due to engine failure at 1825Z. The aircraft descended to FL220 and diverted to Iqaluit, NU on single engine. SWR40 requested emergency services at destination. The aircraft landed safely without incident at 2003Z. No impact to operations.
UPDATE: TSB Report#A17Q0012: HB-JND, a Boeing 777-300 aircraft operated by Swiss International Air Lines, was conducting flight SWR40 from Zürich-Kloten, Switzerland (LSZH) to Los Angeles Intl, CA (KLAX). During cruise flight approximately 288 nautical miles North West of Iqaluit, NU (CYFB), the flight crew declared a Pan Pan due to the left engine (GE90-115B) shut down and requested a diversion to CYFB where it landed without further event at 2003Z with ARFF on standby. After landing, the aircraft got trapped in the turn around bay of the threshold of Runway 16. The runway was closed by NOTAM until the aircraft was towed at 2110Z.

Update 218730-V3: The aircraft landed CYYT at 1746Z.
At 1546Z, a Swiss International Air Boeing 777-300 (HBJND/SWR8T) on a flight from Zurich, Switzerland (LSZH) to Chicago/O'Hare, IL (KORD) declared a medical emergency. At 1602Z, the flight requested to divert direct to St. John's, NL (CYYT). The flight was advised it was unable to get IFR clearance due to traffic and advised negative traffic at FL300. SWR8T descended, using contingency procedures, to FL300 and was issued IFR clearance direct CYYT. Traffic was a Lufthansa German Airlines Airbus 340-300 (DLH426) on a flight from Frankfurt, Germany (EDDF) to Philadelphia, PA (KPHL) on same track at FL330, 2 minutes behind.

A Swiss International Air Lines Airbus A330-300 (SWR14E) on a flight from Zurich, Switzerland (LSZH) to New York/John F. Kennedy, NY (KJFK) declared a medical emergency with NY at 1805Z. The aircraft requested diversion to Halifax, NS (CYHZ) estimating CYHZ 1851Z. At 1822Z, the aircraft requested and was recleared to original destination KJFK. No assistance required. No impact.
