At approximately 1820Z, a United States Armed Forces Military Airlift Command Lockheed C-5 (Galaxy) (RCH708) from Rota, Spain (LERT) to Fairfield/Travis Air Force Base, CA (KSUU) stated they had a pressurization problem and requested descent to FL250. While on descent, the crew indicated they would like to climb back up to FL340. No impact on operations.

Incident at GANDER INTL NL (CYQX)

A United States Air Force Boeing E3 Sentry (SHUCK83) from Rota, Spain (LERT) to Gander, NL (CYQX) inbound on the very high frequency omnidirectional range (VOR)/distance measuring equipment (DME) for the Runway 21 approach, was suggested to go around by the air traffic control (ATC). The aircraft was less then 2NM final, did not have the airport in sight and did not appear to have a correct heading for the approach. The aircraft went around and landed on Runway 13 at 2117Z.

The flight requested and was cleared to St. John's, NL (CYYT). At 2225Z, the flight requested to change its destination to Halifax/Stanfield Int'l, NS (CYHZ) and then shortly after to KPSM, estimated time of arrival (ETA) 0130Z. The flight was cleared as requested and exited Gander Area Control Centre (CZQX) airspace at 2335Z.

The automatic dependent surveillance ? contract (ADS-C) report indicated that a Boeing CC-177 Globemaster III (RCH963), from Rota (LERT), Spain, to Tacoma (KTCM), WA, was not flying the flight planned route that had been communicated to air traffic control (ATC). After 61N80W, the aircraft headed to 59N100W even though the authorized route was 61N80W YRT150/124 59N100W. No impact on operations.

A United States Air Force Boeing CC-177 Globemaster III (RCH564) from Rota Naval Station, Spain (LERT) to Gander, NL (CYQX) advised that they were in the missed approach for Runway 03 at 0102Z, and landed at 0115Z without incident. The pilot reported a failure of the radar altimeter on short final as the reason for missed approach. The issue was resolved during vectors for the second approach.

NY Oceanic called informing that a US Air Force Boeing CC-177 Globemaster III (70046A / RCH256) from Norfolk, VA (KNGHU) to Rota, Spain (LERT) had a mechanical emergency and was routing direct to Bangor, MA (KBGR). The aircraft was switched to Moncton (YQM) frequency and radio and radar contact were established. The aircraft advised that they were not declaring an emergency and required no assistance. The nature of the problem was a faulty indicator on their auto throttle system. At 0010Z aircraft requested and was granted routing to McGuire Field, Burlington, NJ (KWRI). Aircraft YQM flight information region (FIR) at 0023Z. No impact on the operation.

SPUR22 checked in with the CH sector and advised him of the situation. No assistance required at this time. Moncton, NB (CYQM) LOW and Boston, MA (BOS) supervisor advised. SPUR22 left CYQM airspace at 1620Z without incident. No impact.

Incident at East JAROM intersection

A United States Air Force McDonnell Douglas C-17 Globemaster II (RCH924) from Bangor, ME (KBGR) to Rota (LERT) was on a confirmed route in part JAROM TALGO 44N050W...After passing JAROM at 0005Z flight was observed on radar turning 30 degrees to the left. When queried flight crew confirmed they were heading direct TALGO. Flight was still observed heading away from TALGO and when queried again reported heading to TALGO. Gander HL controller then issued RCH924 the coordinates for TALGO and RCH924 proceeded direct to that point. Maximum deviation was 25 miles off course. No traffic. RCH924 progressed overhead TALGO at 0019Z.

The flight was rerouted into Gander's airspace and upon radar identification, just south of TALGO Intersection, was cleared from 34,000 feet to 30,000 feet. No emergency was declared and the flight continued to destination.

Twenty miles south of KEVLU Intersection, CMB130, MD11, enroute from Norfolk Naval Station (KNGU) to Rota Naval Station (LERT) reported a hydraulic problem and requested clearance to return to the departure point as a precaution. No emergency was declared and no assistance was requested. The aircraft was rerouted to Norfolk.

Moncton (CYQM) Area Control Center (ACC) controller received an estimate on RCH249, a C5 enroute from Rota Naval Station (LERT) to Norfolk (KNGU) from the New York Oceanic Controller. The estimate was applied to the strip for a RCH468, a C17 enroute from LERT to Virginia Beach (KNTU) and read back to the NY Oceanic controller as RCH468. The error was not corrected by the NY Oceanic Controller.

No emergency was declared and the aircraft landed safely in CYQX at 2233Z without incident.

A C-17 enroute Rota (LERT) to Gander (CYQX) was approximately 13 nautical miles south of the airport on vectors for Runway 03 when the crew received a Ground Proximity warning. The aircraft was descending through 2800 feet at the time and climbed to 3700 feet in response. After discussion with ATC, the aircraft subsequently continued with the approach and landed safely at 0241z. The minimum Vectoring Altitude (MVA) in the airspace concerned is 2200 feet. Nil TSB

Approaching 50/30, the crew declared an emergency due to a pressurization problem and descended from 32,000 ft. to 24,000 ft. using Oceanic contingency offset procedures. The crew then requested to divert to Gander (CYQX) and was cleared as requested. No further assistance was required at that time. RCH439, Lockheed C5, was enroute from Charleston Air Force Base (KCHS) to Rota (LERT). As the aircraft was crossing the boundary between Moncton and Gander ACC, the Moncton controller advised Gander that RCH439 was in rapid descent due to a pressurization problem. The aircraft leveled at 10,000 feet at 01:02Z. The crew later advised that the problem was with an aircraft door and at 01:18Z, requested to divert to Westover Air Reserve Base (KCEF) at 24,000 ft. A07A0083: RCH714, an Evergreen Airlines Boeing 747-273C on a military charter, was en route from Rota Naval Station, Spain to Norfolk Naval Station. A temporary shut down of the New York oceanic airspace control facility resulted in the aircraft being diverted two degrees north of the cleared track. The change of track resulted in a potential fuel shortage for the aircraft; therefore, the flight crew requested and received a clearance to divert the aircraft to Gander. The aircraft landed safely at Gander without further incident.
At VODOR intersection the crew of RCH714, Boeing 747, enroute from Rota (LERT) to Norfolk (KNGU), requested to divert to Gander at company request due to fuel concerns. The aircraft had been diverted two degrees north of flight plan while in oceanic airspace due to the closure of New York Oceanic airspace as reported in CADOR 2007A0828. The aircraft landed at 10:51Z without incident. TSB Case Closed.
