Incident at 24NM SE TORONTO / LESTER B. PEARSON INTL ON (CYYZ)

The Cleveland Air Route Traffic Control Center (ZOB) controller advised the Toronto Centre (CZY) controller that an American, corporately registered Learjet 35A (N354EF) from Toronto/Lester B. Pearson, ON (CYYZ) to Teterboro, NJ (KTEB) that had just been handed off was dumping fuel due to a fuel balance issue. ZOB subsequently advised at time 1830Z that fuel dumping was complete. No impact on operations and there were no other aircraft in the vicinity of N354EF during the period of dumping. The fuel dumping occurred between coordinates 43.28N079.03W and 43.16N078.29W.

The traffic, a Cargair Ltd. Cessna 152 (C-FVSG) from Montreal/St-Hubert, QC (CYHU) to Montreal/St-Hubert, QC (CYHU) at 1 900 ft, was not a real conflict. No impact on operations.

At departure from Runway 24L, a Flightpath Charter Airways Embraer EMB-505 (KNT103) from Montreal/Trudeau, QC (CYUL) to Teterboro, NJ (KTEB) turned on a heading of 270 instead of 237. The aircraft thought it was on a heading of 237. No impact on operations.

While entering Canadian airspace from the Boston air route traffic control centre (ARTCC), a Netjets Aviation Cessna 680 (EJA621) from White Plains, NY (KHPN) to Montreal/Pierre Elliott Trudeau, QC (CYUL) and a Flightpath Charter Airways Pilatus PC-12/47E (C-FBDT) from Teterboro, NJ (KTEB) to Montreal/Pierre Elliott Trudeau, QC (CYUL) reduced their separation to 3.6 NM and 500 ft, while the requirement is 4 NM or 1 000 ft.
Update – TSB #A19Q0087: A Flightpath Charter Airways Pilatus PC-12/47E (C-FBDT) was on an instrument flight rules (IFR) flight from Teterboro, NJ (KTEB) to Montreal/Pierre Elliott Trudeau, QC (CYUL). A Netjets Aviation Cessna 680A (EJA621) was on a flight from White Plains/Westchester County, NY (KHPN) to CYUL. Before the two aircraft entered Canadian airspace approximately 30 NM south of CYUL, the Boston control centre (KZBW) transferred them to the Montreal control centre (CZUL), even though the separation between the two aircraft was reduced to 3.6 NM and 500 ft. A loss of separation occurred when the aircraft entered Canadian airspace, where the standard is 4 NM or 1 000 ft.

A Harbour Air de Havilland DHC-3 (C-GHAG) from Teterboro, NJ (KTEB) to Montreal/Trudeau, QC (CYUL) was assigned Runway 06L at arrival and was cleared for an approach on Runway 06L. C-GHAG read back the clearance for an approach on Runway 06R. Air traffic services (ATS) corrected the pilot and reconfirmed clearance for Runway 06L. The aircraft had to reject its approach and was vectored for Runway 06L. Runway 06R was closed at the time due to snow removal. No impact on operations.
Correction To Narrative/Aircraft Information: The aircraft involved was a Skyservice Business Aviation Learjet 45 (C-FHAG) and not a Harbour Air de Havilland DHC-3 (C-GHAG).

Incident at EBDOT intersection

A Skyservice Business Aviation Learjet 45 (C-FHAG) from Teterboro, NJ (KTEB) to Montreal/Trudeau Intl, QC (CYUL) passed through EBDOT at 13,000 feet in descent when the restriction on the standardized terminal arrival (STAR) CARTR3 indicates 11000 feet or lower. No impact on spacing or operations.

A Skyservice Business Aviation Gulfstream G150 (C-GXNW/SYB264) on a flight from Teterboro, NJ (KTEB) to Toronto/Lester B. Pearson Intl, ON (CYYZ) declared a PAN PAN in the downwind for Runway 05 due to an icing issue. Aircraft Rescue and Fire Fighting (ARFF) were deployed and SYB264 landed Runway 05 at 0042Z without incident. Four aircraft on approach were taken off the approach due to inspection of the runway requirement. Aircraft were routed to Runway 06L.

Update TSB Report #A19O0163: N506DJ, a Falcon 2000 airplane operated by Dumont Aviation as flight SDU506, deviated from KEPRA2 SID RWY 23 after departure from Toronto/Lester B. Pearson Intl. (CYYZ). This resulted in a loss of separation with Rouge flight ROU1764, an Airbus A319 (C-FYJE) aircraft, departing runway 24L. The separation was reduced to 1.8nm and 400 feet.

On departure from Runway 06L, an Air Georgian Limited Bombardier CL-600-2B19 (Series 100) (GGN7266) from Toronto/Pearson, ON (CYYZ) to Pittsburgh, PA (KPIT) was instructed to turn direct MOBEL. A FlexJet Bombardier BD-100-1A10 (CL30) (LXJ589) from Toronto/Pearson, ON (CYYZ) to Teterboro, NJ (KTEB), which departed behind GGN7266, began a turn towards MOBEL at the same time. The controller instructed LXJ589 to level at 4000 feet and ensured that they had GGN7266 visual. Nearest approach was 1.7NM laterally at the same altitude. The controller advised LXJ589 of the requirement to maintain the runway heading until being issued with the vector to MOBEL. No other operational impact.

On departure, GGN7266 was instructed to turn direct to the MOBEL fix. The controller instructed LXJ589 to level at 4000 feet and to ensure that they had GGN7266 visual. The controller advised LXJ589 of the requirement to maintain runway heading until they are instructed to turn to the MOBEL fix. During the departure, the separation was reduced to 1.7 nm horizontally and at the same altitude where 1000 feet or 3 nm separation is required.

A Worldwide Jet Charter Learjet 60 (WWI28) from Teterboro, NJ (KTEB) to Toronto/Pearson Intl, ON (CYYZ) was cleared to 8000 feet. The read back was correct. WWI28 was then observed descending through 7500 feet. An Endeavor Air Bombardier CL-600-2D24 (Series 900) (EDV5508) from Toronto/Pearson Intl, ON (CYYZ) to Detroit/Wayne County, MI (KDTW) was in the vicinity. No operational impact.
Update TSB Report #A19O0019: N1127M, a Learjet 60 aircraft operated by Worldwide Jets, was conducting flight WWI28 from Teterboro (KTEB), NJ to Toronto/Lester B. Pearson Intl (CYYZ), ON. N903XJ, a Bombardier CL-600-2D24 CRJ900 aircraft operated by Endeavor Air, was conducting flight EDV5508 from CYYZ to Detroit Metropolitan/Wayne County (KDTW), MI. ATC cleared WWI28 to descend to 8000 feet during the downwind leg of the LINNG9 arrival (landing east). The flight crew read the clearance back correctly, however the aircraft was observed on radar descending through 7500 feet. This resulted in a loss of separation between WWI28 and EDV5508, which was level at 7000 feet after its departure on the ANCOL3 Standard Instrument Departure (SID). The separation was reduced to 1.1 nm laterally and 500 feet vertically, in an area where 3 nm or 1000 feet is required.

An Air Canada Rouge Boeing 767-333 (C-FMWP/ROU1712) from Toronto/Lester B. Pearson Intl, ON (CYYZ) to Punta Cana Intl, Dominican Republic (MDPC) reported a possible bird strike after departing Runway 23. An inspection vehicle reported finding debris and indicated it appeared to be a bird of prey which was ingested in the engine. A Private Air Inc. Beech 200 (C-GMNM/FSY134) from Teterboro, NJ (KTEB) to Toronto/Lester B. Pearson Intl, ON (CYYZ) was instructed to initiate a missed approach. Afterwards, Air Canada (ACA) Dispatch reported that ROU1712 would be returning to CYYZ due to vibration in the engine. No emergency was declared and ROU1712 landed on Runway 23 at 1920Z without incident. No impact to operations.
Update TSB Report #A19O0014: C-FMWP, a Boeing 767-300 aircraft operated by Air Canada rouge, was conducting flight ROU1712 from Toronto/Lester B. Pearson Intl (CYYZ), ON to Punta Cana Intl (MDPC), Dominican Republic with 8 crew members and 280 passengers on board. During the takeoff roll on Runway 23 at CYYZ, the aircraft struck a bird. The flight crew reported observing a large bird, hearing a loud bang, and a slight adverse yaw was detected, however engine parameters were normal; therefore, the takeoff was continued. Following a brief level off at 5000 ASL to evaluate the situation, the decision was made to continue the flight in the absence of abnormal parameters. During cruise flight at FL330, the flight crew noticed increasing vibrations on the left engine, and elected to return to CYYZ as a precaution. The aircraft landed overweight without further incident. The operator’s maintenance inspected the left engine and, based on the extent of the damage, elected to replace it.

A Novajet Dassault Falcon 900EX (C-GGBC) from Saint John, NB (CYSJ) to Teterboro, NJ (KTEB) declared an engine failure with the Boston (BOS) area control centre (ACC) and advised returning to CYSJ. The aircraft declared a "PAN PAN" with BOS ACC when 80NM North of the Boston Intl, MA (KBOS) aerodrome. The pilot requested emergency vehicles on standby at CYSJ. The aircraft landed at 2018Z. Audio and radar tapes were reviewed. No impact on other traffic.
UPDATE: CADORS 2019A0318 (AOR 246151-V2) was a duplicate of this occurrence and was deleted.
Update TSB Report #A19A0015: C-GGBC, a Dassault Falcon 900EX aircraft operated by NovaJet, was conducting flight 58343 from Saint John (CYSJ), NB to Teterboro (KTEB), NJ with 2 crew members and 6 passengers on board. During cruise flight approximately 30 minutes after takeoff, the flight crew observed a number 1 engine low oil pressure indication, associated Master Caution light illumination, and subsequent Master Warning indication. The flight crew carried out checklist requirements, including the precautionary shut down of the number 1 engine. A return to CYSJ was requested, the flight crew declared a PAN PAN, and the aircraft landed at CYSJ without further incident. Maintenance personnel replaced the faulty number 1 engine oil filler cap o-ring, and returned the aircraft to service following cleaning, servicing and function tests.

Incident at TETERBORO AIRPORT - TETERBORO, NJ (KTEB)

TSB Report #A19F0015: C-GRPF, a Bombardier CL600-2B16 Challenger 601-3R aircraft operated by Skyservice Business Aviation, was conducting a flight from Iquitos/Coronel FAP Francisco Secada Vignetta Intl (SPQT), Perú to Teterboro (KTEB), NJ with 3 crew members and 2 passengers on board. When they were closing in to destination, the flight crew inquired to ATC about possible delays at KTEB; ATC replied that they were not aware of any delays. As they were subsequently handed over to another controller, the flight was issued a 45 minute hold, with possible longer delays. Since the flight did not have the fuel to hold this long, the flight crew declared Minimum Fuel. The flight crew requested a diversion to Newark/Liberty Intl (KEWR), NJ, which was granted by ATC. UPDATE TSB Report #A19A0094: M-USIC, a privately operated Gulfstream G550, was on a flight from Teterboro (KTEB), NJ to London/Farnborough (EGLF), UK. During the cruise portion of the flight, while in Canadian airspace, the cockpit windshield cracked. The crew declared a PAN PAN and requested a diversion to Boston/General Edward Lawrence Logan Intl. (KBOS), MA . No other assistance was required.

An American, corporately registered Dassault Falcon 200EX (N12MW) from Teterboro, NJ (KTEB) to Ibiza, Spain (LEIB) was flight planned NICSO 48/50 48/40 46/30 43/20 EKROL BANAL. At 0112Z, the aircraft was rerouted and readback the Oceanic clearance (CLX) of NICSO 48/50 49/40 47/30 44/20 ARMED. At 0235Z, the aircraft reported 40W and confirmed 47/30 and then 43/20. Aircraft was questioned and confirmed 43/20. After being advised that an air traffic control (ATC) intervention to prevent a gross navigational error (GNE) would be filled, the aircraft advised that a mistake had been made.

A foreign, corporately registered Hawker Beechcraft Corp. Hawker 4000 (TCNRN) from Keflavik, Iceland (BIKF) to Teterboro, NJ (KTEB) was cleared 63/30 61/40 57/50 HOIST by Reykjavik (RK). The pilot reported correct routing with the 30W position report. The aircraft then showed up at 60/40, not 61/40 as expected. The pilot indicated that they had input the wrong coordinates in their flight management system (FMS). There was no traffic. The aircraft was cleared M082 but indicated it was cruising M080. The aircraft was re-cleared at M080.
