An Air Canada Airbus A320-211 (C-FMSX/ACA780) from San Francisco, CA (KSFO) to Montreal/Pierre Elliott Trudeau, QC (CYUL) was instructed to conduct a pull-up on Runway 06R. An American corporately registered Eclipse Aviation Corp EA500 (N838SB) on a flight to Montreal/Pierre Elliott Trudeau, QC (CYUL) missed the exit for Taxiway L and had to taxi to the end of Runway 06R. No other impact on operations.

QXE2269 was cleared to 90 to pass beneath traffic, an Air Canada Airbus A319 (ACA561) from San Francisco, CA (KSFO) to Vancouver, BC (CYVR), that was level at 100. Traffic was passed to QXE2269. Separation was not compromised.

Incident at VANCOUVER INTL BC (CYVR)

Tower controller lined up a Morningstar Air Express Inc. Cessna 208B (MAL8074) from Vancouver, BC (CYVR) to Victoria, BC (CYYJ) for Runway 08R but an Air Canada Airbus A319 (ACA574) from Vancouver, BC (CYVR) to San Francisco, CA (KSFO) took the clearance and lined up, then MAL8074 also replied and lined up. Tower noticed that both aircraft were lined up and departed them in the correct order.

Incident at 110NM NW TORONTO / LESTER B. PEARSON INTL ON (CYYZ)

An Air Canada Airbus A330-343 (C-GEGC/ACA739) from Toronto/Lester B. Pearson, ON (CYYZ) to San Francisco, CA (KSFO), after entering Cleveland Air Route Traffic Control Center (ZOB) airspace, elected to return to CYYZ due to an issue with the flaps. On first contact with Toronto Centre (CZY), pilot declared a PAN PAN, and advised they would be making a flapless landing and requested that the Aircraft Rescue and Fire Fighting (ARFF) unit be standing by to check brakes. ACA739 landed at 0309Z without further incident. No impact on operations, Aviation Operations Centre (AVOPS) notified.
UPDATE TSB Report #A20O0003: Air Canada flight ACA739, an Airbus A330 (C-GEGC) aircraft, was on route from Toronto (CYYZ) to San Francisco (KSFO), when the flight notified Cleveland Centre of a flap and hydraulic issue and requested a return to CYYZ. After arriving in Canadian airspace, the crew declared a PAN PAN in order to coordinate a landing with zero flaps. The crew requested aircraft rescue and firefighting (ARFF) standing by in Toronto to inspect the brakes on arrival. The aircraft landed without further incident and taxied off the runway under its own power. Prior to departure aircraft had MEL “Flap Sys 2 INOP and Yellow HYD Pump Low Pressure Alert”. In cruise flight crew received yellow hydraulic low indication. The flight crew in consultation with MOC decided to return to YYZ. Maintenance found the pressure switch to be leaking. The pressure switch and o-ring were replaced and the aircraft was returned to service after the appropriate AMM service checks.

TSB Report #A20F0062: C-FNJZ a Jazz Aviation LP CRJ900 was conducting flight AC8839 from San Francisco Intl. (KSFO), CA to Vancouver Intl. (CYVR) BC. Just after take off and gear retraction, the crew received a forward and rear smoke indication from the lavatories followed by reports of smoke in the cabin. Pilot declared an emergency and returned to KSFO. The passengers were evacuated on taxiway and there has been an initial report of a minor injury to a passenger. A bus was dispatched to pick up the passengers.

TSB Report #A20F0055: C-GAQX an Airbus A319-114 flight AC574 operated by Air Canada from Vancouver Intl. Airport (CYVR) BC to San Francisco Intl. Airport (KSFO) CA, experienced a TCAS RA at waypoint CORKK, on approach for KSFO. The crew had a visual on traffic and climbed 1000 feet, and advised ATC. Continuing with the approach, the aircraft was high and fast, causing a missed approach to be carried out, resulting in a flap overspeed of 5-10 knots. The aircraft landed in SFO without further incident.

TSB Report #A20F0059: C-FIVS, a Boeing 777-300 operated by Air Canada, was conducting flight AC737 Toronto/Lester B. Pearson Intl. (CYYZ), ON to San Francisco Intl. (KSFO), CA. During the approach, flight crew levelled at 6000’ on vectors for FMS bridge visual to TRDOW. A TCAS TA (traffic advisory) for traffic at 6500 was received and ATC advised to descend. Flight was down through 5000 when they received TCAS RA (resolution advisory), flight 400 above. No action taken as traffic was already in descent and meeting what the RA was advising. Flight continued without further incident.

A Skywest Airlines Bombardier CL-600 2B19 (SKW5493) from San Francisco, CA (KSFO) to Calgary, AB (CYYC), while on arrival to CYYC, reported a flap issue. SKW5493 declared an emergency and was required to perform a no-flap landing. SKW5493 landed without further issues on Runway 35R. No operational impact.
Update TSB Report #A20W0027: N908EV, a Bombardier CRJ200 operated by Skywest Airlines, was conducting flight SKW5493 from San Fransisco Intl. (KSFO), CA, to Calgary Intl. (CYYC), AB, with 3 crew members and 29 passengers on board. During the descent into the greater CYYC area, the flight crew were configuring the aircraft for landing and selected "FLAP 8". They immediately received a "FLAP FAIL" caution message. The approach was discontinued and the captain declared an emergency with ATS. The flight crew completed the appropriate QRH procedures, briefed the flight attendant, made the passenger announcements, and proceeded to an uneventful flap 0 landing in CYYC. There were no injuries.

During taxi the right engine (CFM International CFM-56-5A1) flamed out. The aircraft returned to the gate. Subsequent maintenance actions, as per the operators maintenance manual, replaced the engine electronic control (EEC) unit. The aircraft was returned to service.

As a result of the MA, traffic in position on the South Runway was unable to depart or clear the runway. A KF Cargo Convair 340 (C-GKFS/KFA574) on a flight from Victoria Intl, BC (CYYJ) to Vancouver Intl, BC (CYVR) was given a VFR circuit and eventually landed Runway 26L at 0136Z. UAL1542 was vectored for a 2nd approach and landed Runway 26R without further issue at 0144Z.

A WestJet Boeing 737-7CT (C-FUWS/WJA1775) from San Francisco, CA (KSFO) to Vancouver, BC (CYVR) reported a bird strike landing Runway 26L. Upon inspection, one large bird was recovered. Runway 26L re-opened at 0222Z.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. The flight crew reported bird strike on landing gear. Maintenance completed bird strike inspection with no signs or evidence of impact or ingestion. Aircraft returned to service.

A Swiss International Air Lines Airbus A340-313 (HBJMF/SWR8010) from Zurich, Switzerland (LSZH) to Vancouver, BC (CYVR) was at Gate 64 and was struck by an Air Canada Boeing 737-8 (C-GHPU/ACA566) from Vancouver, BC (CYVR) to San Francisco, CA (KSFO) that was taxiing behind.
Update TSB #A19P0116: HB-JMF, an Airbus A340-300 aircraft operated by Edelweiss Air, conducted flight SWR8010 from Zurich (LSZH), Switzerland to Vancouver Intl (CYVR), BC. Upon arrival at gate 64, HB-JMF was marshalled into the gate area, signaled to stop and set the parking brake. The engines were shut down. The ground personnel connected a tug and were preparing to tow the aircraft to the final gate parking position. Meanwhile, C-GHPU, an Air Canada Boeing 787-800 aircraft operating flight AC566 from CYVR to San Francisco Intl (KSFO), CA was taxiing westbound on JA taxiway, abeam gate 64, when the left wing tip contacted the tail section of HB-JMF. There were no injuries. Both aircraft sustained substantial damage.
