Incident at 30NM NE Fort Myers International Airport (KRSW)

TSB Report #A18F0072: C-GTVG, a Boeing 737-800 aircraft operated by Sunwing Airlines, was conducting flight SWG685 from Varadero/Juan Gualberto Gómez Intl, Cuba (MUVR) to Toronto/Lester B. Pearson Intl, ON (CYYZ) with 6 crew members and 181 passengers on board. During an enroute climb from FL350 to FL370 approximately 30 nautical miles North East of Fort Myers/Southwest Florida Intl, FL (KRSW), the flight crew received an engine vibration that exceeded limitations on the number 1 engine. A PAN PAN was declared and the aircraft was levelled off at FL361, to subsequently return to FL350. The flight continued to destination without further incident. The operator?s maintenance confirmed that the indication was false and was caused by a faulty Fan Frame Compressor Case Vibration Sensor. The aircraft was put on MEL until the part was replaced.

UPDATE: TSB#A17Q0204: C-FTJO, an Air Canada Airbus 320-211, was on instrument flight rules (IFR) flight ACA1269 from Fort Myers/Southwest Florida, FL (KRSW) to Montreal/Pierre Elliott Trudeau, QC (CYUL). The flight crew mentioned knowing about a slight loss of control of the front landing gear during the takeoff roll at KRSW and, as a precaution before arrival at CYUL, requested that aircraft rescue and firefighting (ARFF) services be deployed. The aircraft landed without incident on Runway 24R.
An Air Canada Airbus A320-211 (C-FTJO/ACA1269) from Fort Myers, FL (KRSW) to Montreal/Pierre Elliott Trudeau, QC (CYUL) declared a need for emergency vehicles for landing. The aircraft reported that it had had a slight loss of control of the front landing gear at takeoff and that it was afraid this problem would reoccur during landing. The aircraft landed without incident on Runway 24R at 2151Z. Impact on operations: Several fuel dumps on Runway 24L, but no major delays.

While inbound to Ottawa, ON (CYOW) a Porter Airlines de Havilland DHC-8-400 (POE233) from Toronto Billy Bishop, ON (CYTZ) to Ottawa, ON (CYOW), reported hydraulic issue and declared a PAN PAN. Emergency measures were deployed. The aircraft landed Runway 32 without incident at 2048Z. Impact: a WestJet Boeing 737-700 (WJA1263) from Fort Myers, FL (KRSW) to Ottawa, ON (CYOW) and an Air Canada Airbus A320 (ACA1561) from Cancun, Mexico (MMUN) to Ottawa, ON (CYOW) were delayed 5 minutes on arrival. National Operations Centre (NOC) and Aviation Operations advised.
UPDATE: TSB Report#A17O0026: C-GLQN, a de Havilland DHC-8-402 aircraft operated by Porter Airlines, was conducting flight POE233 from Toronto/Billy Bishop Toronto City, ON (CYTZ) to Ottawa/MacDonald-Cartier Intl, ON (CYOW). While descending through 10 000 feet during the initial approach at CYOW, the flight crew noticed that the number 1 hydraulic system quantity was at zero. Simultaneously, a cabin crew member noted fluid leaking from the bottom of the left engine. A short time later, the number 1 hydraulic pressure dropped to zero and illuminated the corresponding caution lights. The flight crew completed the QRH for the loss of the number 1 hydraulic system and declared a PAN PAN. The aircraft landed without further incident. The operator?s maintenance replaced the number 1 hydraulic priority valve and the aircraft was returned to service.

Incident at 6NM MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL)

An Air Canada Airbus A319-114 (C-GAQZ/ACA1269) from Fort Myers, FL (KRSW) to Montreal, QC (CYUL) advised the Montreal Tower of traffic alert and collision avoidance system (TCAS) traffic 3 miles at 1,000 feet while on approach to Runway 06L, inside the control zone. The Tower advised ACA1269 that they were not in contact with the aircraft in question, tagged as a College d'enseignement generale et professionnel de Chicoutimi (C-GUVN) from Montreal, QC (CSP6) to St-Hubert, QC (CYHU). C-GUVN had contacted the Terminal and entered the zone prior to being identified. By the time C-GUVN contacted the Tower, ACA1269 had requested and received overshoot instructions. ACA1269 was resequenced and landed Runway 06L at 2015Z. No other impact on operations.
Update (Commercial and Business Aviation): A civil aviation safety inspector (CASI) looked into this incident. The CASI communicated with the operator to obtain more information. There was a meeting with the crew following this incident and airspace procedures were reviewed. The CASI was satisfied with the information provided and indicated that no other follow-up by Transport Canada was required for the time being.

UPDATE: TSB Report#A15O0002: Air Canada flight 1233, an Airbus A320 aircraft (C-FKPT) was on approach to Toronto LBPIA when the green hydraulic system failed. The flight crew declared a PAN PAN and landed uneventfully with ARFF standing by. The aircraft stopped on the runway and had to be towed to the gate where passengers were deplaned normally. Maintenance found a burst hydraulic line.
At 1432Z, an Air Canada Airbus A320 (ACA1233), from Fort Myers, FL (KRSW) to Toronto, ON (CYYZ), called PAN PAN PAN and declared an emergency citing hydraulic issues. Aircraft Rescue and Fire Fighting (ARFF) were advised. ACA1233 landed Runway 23 at 1448Z without incident and was later towed off the runway. Runway 23 was inspected and returned to service at 1510Z. No operational impact.

A Netjets Aviation Cessna 560 (EJA617) from Toronto, ON (CYYZ) to Fort Myers, FL (KRSW) was cleared to climb to 7000 feet and read back correctly. The Departure Controller observed the aircraft climbing through the assigned altitude to 7700 feet and questioned the aircraft. EJA617 returned to 7000 feet. No operational impact.

A Hainan Airlines Airbus A340-600 (CHH7976) from Toronto, ON (CYYZ) to Beijing (ZBAA) departed runway 23 at 2145Z. An Air Canada Airbus A321-211 (ACA1234) from Toronto, ON (CYYZ) to Fort Myers, FL (KRSW) departed runway 23 at 2148Z and reported " a little bit of wake from previous departure at 1000ft". Wake cautionary had been provided. No operational impact.

Incident at Charlotte International Airport (KCLT)

FAA report sent February 16, 2014: An Air Canada Airbus A321 (ACA1237) from Fort Meyers, FL (KRSW) to Toronto, ON (CYYZ), diverted to Charlotte, NC (KCLT) due to a passenger medical emergency. The aircraft landed without incident.
