After having been cleared to follow standard terminal arrival (STAR) on Runway 24L, an Aviation Actuelle Socata TBM 700 (series N) (C-GGKC) from Raleigh-Durham, NC (KRDU) to Montreal/Trudeau, QC (CYUL) was seen conducting a right turn at COMAU. Impact on operations: an American, corporately owned Gulfstream Aerospace GV-SP (N126HR) from Montreal/Trudeau, QC (CYUL) to Fort Lauderdale, FL (KFLL) was unable to climb higher than 7 000 ft while departing from Runway 24L.

TSB Report #A19F0039: C-GUBD, an Airbus 330-200 aircraft operated by Air Transat, was conducting flight TSC560 from Montreal/Pierre Elliott Trudeau Intl (CYUL), QC to Cancun Intl (MMUN), Mexico with 11 crew members and 334 passengers on board. Approximately 90 minutes into the flight, a cabin crew member became ill and was unable to continue duties. The affected cabin crew member’s duties were reassigned to the remainder of the cabin crew for the rest of the flight.

An Air Georgian Bombardier CL-600-2B19 (GGN7316) from Raleigh-Durham, NC (KRDU) to Toronto/ Lester B. Pearson, ON (CYYZ) landed Runway 24L at 2043Z and exited at D2. GGN7316 indicated they may have blown a tire. Greater Toronto Airport Authority (GTAA) inspected the area and found no debris. GTAA Manager operations advised aircraft was considered disabled and closed both Runway 24L and Runway 24R. Buses were dispatched to remove passengers. At 2131Z, GGN7316 taxied to the ramp. During the closure, operation was arrive and depart Runway 23.

TSB Report #A18F0091: C-FWRS, a Bombardier CL-600-2B19 (CRJ100) aircraft operated by Air Georgian, was conducting flight GGN7316 from Raleigh-Durham Intl, NC (KRDU) to Toronto/Lester B. Pearson Intl, ON (CYYZ) with 3 crew members and 50 passengers on board. As the aircraft was climbing through FL250 approximately 60 nautical miles South West of Washington/Dulles Intl, VA (KIAD), the flight crew received an R 14th DUCT OVHT message, accompanied by a Master Warning light. The flight crew declared a PAN PAN and requested a diversion to KIAD where the aircraft landed without further incident. The operator?s maintenance replaced the 14th stage bleed sensing element in accordance with Aircraft Maintenance Manual 36-21-04-00/400-807 and conducted an operational check. An engine run was carried out at high power (85% N2) with the 14th stage bleed ON and anti-ice ON. No further faults were found. The aircraft was returned to service.

Incident at 30NM TORONTO / LESTER B. PEARSON INTL ON (CYYZ)

An Air Georgian Bombardier CL-600-2B19 (GGN7318) on a flight from Raleigh/Durham, NC (KRDU) to Toronto/Lester B. Pearson, ON (CYYZ) communicated a flap indicator light issue. Emergency declared and Aircraft Rescue and Fire Fighting (ARFF) on standby. The aircraft landed Runway 23 at 0042Z without issue and taxied to gate under its own power.
UPDATE: TSB Report#A17O0218: C-GXJA, a Bombardier CL-600-2B19 (CRJ-200) aircraft operated by Air Georgian, was conducting flight GGN7318 from Raleigh-Durham Intl, NC (KRDU) to Toronto/Lester B. Pearson Intl, ON (CYYZ) with 3 crew members and 49 passengers on board. During the initial climb after the departure from KRDU, the flight crew observed a FLAP FAIL EICAS caution message when retracting flaps to the UP position. The flight crew levelled the aircraft off at 15 000 feet and confirmed that the flaps were in the up position by verifying on the secondary flight control page, in addition to a visual inspection from the cabin. The FLAP FAIL QRH checklist was executed and the flight proceeded to CYYZ at normal cruising altitude. Upon initial contact with Toronto ATC, the flight crew declared PAN PAN as a precaution, in case the flaps would not extend from the up position. When the flaps were selected for the approach, they remained in the up position. The flight crew completed the remainder of the FLAP FAIL QRH and configured the aircraft for a flapless landing. Runway 23, the longest runway available, was requested and the aircraft landed ithout further incident. The operator?s maintenance performed troubleshooting in accordance with FIM 27-50-00-01 and replaced the Flap Electronic Control Unit (FECU). Additionally, the Brake Position Sensor Units (BPSU) were rigged. After operational checks were completed, the aircraft was returned to service.

UPDATE: TSB Report#A17O0036: GGN7315, a Bombardier CL-600-2B19 (CRJ200) aircraft operated by Air Georgian, was conducting a flight from Toronto/Lester B. Pearson Intl, ON (CYYZ) to Raleigh-Durham Intl, NC (KRDU). ASQ5457, a Bombardier CL-600-2D24 (CRJ900) aircraft operated by ExpressJet Airlines, was conducting a flight from Detroit Metropolitan Wayne County, MI (KDTW) to CYYZ. As GGN7315 was climbing to cruising altitude after takeoff from Runway 33R in a Southwesterly direction, the flight crew read back and complied with an instruction that was intended for another aircraft. GGN7315 and ASQ5457 were at the same altitude when the separation reduced to 2.6 nautical miles laterally, where 3 nautical miles or 1000 feet is required.

Incident at 123NM NE Pittsburgh International Airport (KPIT)

TSB Report#A17F0246: C-FWJF, a Bombardier CL-600-2B19 (CRJ-100) aircraft operated by Air Georgian, was conducting flight GGN7314 from Raleigh-Durham Intl, NC (KRDU) to Toronto/Lester B. Pearson Intl, ON (CYYZ) with 3 crew members and 25 passengers on-board. During cruise flight at FL330, the flight crew received a "L Engine Oil Press" warning message on the EICAS. The oil temperature had dropped 20 degrees in 2 minutes and was fluctuating. The QRH was actioned; the number 1 engine was shut down as per procedures, and the crew elected to divert to Pittsburgh Intl, PA (KPIT). ATC was informed and cleared the flight direct to KPIT, where the aircraft landed without further incident. The post-flight walk-around revealed traces of oil from the drain mast; however, no indication of damage was found. The aircraft was grounded pending maintenance.

Both a Virgin Atlantic Airways Airbus A330-300 (VIR104L) from Atlanta, GA (KATL) to London, UK (EGLL) at FL370 and an American Airlines Boeing 767 300 (AAL174) from Raleigh-Durham, NC (KRDU) to London, UK (EGLL) at FL350 targets were co-located on radar. AAL174 request climb from FL350 to FL360. Controller verified altitudes of both and then issued climb to AAL174 to FL360. The controller passed traffic to both aircraft and advised AAL174 if he wanted to offset due wake turbulence it was approved and advise. Due to the rate of climb of AAL174, VIR104L reported a resolution advisory but did not change altitude.

The aircraft requested to divert to Boston, MA (KBOS). The aircraft requested the long runway and for emergency equipment waiting for them in KBOS. The aircraft exited Canada at 0317Z. No impact on the operation.
UPDATE: TSB Report #A15A0055: The American Airlines Boeing 777-200, registration N762AN, operating as AAL174, was on a flight from Raleigh-Durham, NC (KRDU) to London Heathrow, UK (EGLL). Approximately two hours into the flight, the crew received an hydraulic quantity indication. The crew confirmed the loss of fluid, declared an emergency and diverted to Boston, MA (KBOS). Maintenance discovered a faulty actuator and upon replacement, the aircraft was returned to service.

An Air Georgian Bombardier CL-600-2B19 (GGN7316) from Raleigh/ Durham, NC (KRDU) to Toronto, ON (CYYZ) was on an instrument landing system (ILS) approach to Runway 24R when they received a traffic alert and collision avoidance system (TCAS) resolution advisory (RA) from a Bombardier de Havilland DHC-8-400 (BBA24) on a local flight from Downsview, ON (CYZD) departing YZD, VFR. BBA24 was restricted to not above 1500' and GGN7316 was at 3000' on the glide path (GP). Both aircraft continued without further incident. Traffic was not passed to GGN7316.

At 2217Z, an Air Georgian Canadair CL-600-2B19 (GGN7318) from Raleigh/ Durham, NC (KRDU) to Toronto, ON (CYYZ) initiated overshoot Runway 24L due to wind shear. Aircraft landed at 2239Z, without further incident.
Follow-up information received from Flight Operations ? West [2015-11-19]: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. The CASI contacted the operator involved for more information and was advised that the operator is tracking this occurrence through their safety management system (SMS). The CASI was provided with additional information on the occurrence and was satisfied with it. The CASI advised that no further follow-up action by Transport Canada, Flight Operations was required at this time.

Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ)

An Air Georgian Bombardier CL-600-2B19 (GGN7314) from Raleigh/ Durham, NC (KRDU) to Toronto, ON (CYYZ) was at 3000' South of the field, cleared for a visual 24R, not to go through the centreline due traffic on 23 at 4000'. GGN7314 passed across the centreline, into the no transgression zone, requiring an Air Canada Airbus A320 (ACA441) from Ottawa, ON (CYOW) to Toronto, ON (CYYZ) to be pulled from the Runway 23 approach. GGN7314 corrected to 24R and landed no incident.

Separation was not assured with a Republic Airlines Embraer EMB-170 (RPA4464) from Toronto, ON (CYYZ) to Charlotte, NC (KCLT) when an Air Georgian Bombardier CL-600-2B19 (GGN7314) from Raleigh, NC (KRDU) to Toronto, ON (CYYZ) was cleared to 8000 feet but read back 6000 feet and the Controller did not correct it immediately.

Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ)

Update: Follow-up information received from Flight Operations ? West [2015-07-16]: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. The CASI contacted the operator for more information on this incident and was advised that this occurrence was being managed through the operator?s safety management system (SMS). Wind shear was not forecast or observed at Toronto (LBPIA) for the planned arrival time. The flight crew reacted appropriately and conducted a missed approach, informed air traffic control (ATC) and were vectored back for another approach, landing without further incident. The CASI was satisfied with the information the operator provided and advised that no further Transport Canada follow-up action was required at this time.

Incident at Johnstown, PA (KJST)

TSB Report#A15F0037: Air Georgian flight 7326, a Canadair CL-600-2B19 (CRJ-100) aircraft, was en route from Raleigh-Durham (KRDU) to Toronto LBPIA (CYYZ) at FL320 when the flight attendant advised the flight crew of smoke coming from an overhead panel in the cabin. The flight crew carried out the QRH procedures and declared an emergency with ATC. The aircraft was diverted to the closest suitable airport, Johnstown (KJST) and landed without further incident. Air Georgian maintenance inspected the aircraft and found moisture from condensation had caused an electrical short in the overhead wiring near row 7 on the right side of the aircraft.
Update: Follow-up information received from Flight Operations ? West [2015-05-11]: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. The CASI contacted the operator for more information and was provided with a detailed information about this occurrence. The operator is handling this occurrence through their safety management system (SMS). Maintenance staff determined that an electrical short had occurred due to condensation penetrating the electrical wiring bundle leading to the overhead passenger light ballast assembly at station 7D. There was no flame associated with this event. Maintenance staff replaced the affected wiring bundle, checked that the system was serviceable and returned the aircraft to service. The CASI was satisfied with the information provided, the maintenance actions taken, the operator?s planned fleet-wide type increased inspection plans, planned consultations with the aircraft manufacturer and advised that no further Transport Canada follow-up action was required at this time.

Incident at MONTRÉAL INTL (MIRABEL) QC (CYMX)

After taking off from Runway 29 at 1357Z, a Dassault Falcon 20 (AJI192) operated by Ameristar Jet Charter from Mirabel, Que. (CYMX), to Durham, NC (KRDU), requested to return to CYMX due to a landing gear indication. Emergency services were deployed. After contacting the terminal controller and levelling off at 2 000 ft, the pilot informed the controller that the problem had been solved and he was ready to continue his route. No impact on operations.
