A Jazz Bombardier CL-600 2B19 (series 200) (JZA8033) from Philadelphia, PA (KPHL) to Montreal/Trudeau, QC (CYUL) declared an emergency due to an engine bleed air problem. JZA8033 landed without incident at 2130Z.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. The crew reported a 14th stage duct warning message appeared without any action of the crew. They shut off the bleed for 1 minute then reselected it but message remained so they throttled back to idle on that side and landed without incident. Maintenance carried out the FIM procedure but NFF. The Bleed Leak Control unit was then replaced and system tested serviceable.

After having been vectored for an approach to Runway 06L, a Piedmont Airlines Embraer EMB-145LR (PDT4845) from Philadelphia, PA (KPHL) to Montreal/Pierre Elliott Trudeau, QC (CYUL) crossed the final approach area line. Air traffic services (ATS) told the aircraft to reject its approach, and it did so. No impact on operations. At 2040Z, PDT4845 crossed the line a second time, but was able to land without incident.

South Ground Controller received flight data entry (FDE) for a Sky Regional Airlines Embraer ERJ 170-200 SU (SKV7548) from Toronto/Pearson, ON (CYYZ) to Atlanta/Hartsfield–Jackson, GA (KATL) from the Apron Management Unit (AMU). Another Sky Regional Airlines Embraer ERJ 170-200 SU (C-FJBO/SKV7538) from Toronto/Pearson, ON (CYYZ) to Philadelphia, PA (KPHL) called for taxi clearance, and the controller issued taxi instructions for SKV7548. Pilot readback taxi clearance and frequency change to tower as SKV7538. South Tower Controller issued take off clearance to SKV7548 as per FDE, and the pilot responded to the clearance as SKV7538. The Terminal Control Unit (TCU) informed the tower that the wrong aircraft had departed.

TSB #A19F0137: C-GTAG, a de Havilland DHC-8-300 aircraft operated by Jazz Aviation LP, was conducting flight JZA8470 from Montreal/Pierre Elliott Trudeau Intl (CYUL), QC to Baltimore/Washington Intl Thurgood Marshall (KBWI), MD with 3 crew members and 32 passengers on board. After holding for 40 minutes because of thunderstorms east of KBWI, the flight crew elected to divert to Philadelphia Intl (KPHL), PA. While enroute to KPHL, the flight crew received a pressurization warning, and declared a PAN PAN. The indication subsequently went away, so the flight crew cancelled the emergency and proceeded to KPHL where the aircraft landed without further incident.

Incident at 175NM NE GOOSE BAY NL (CYYR)

TSB Report #A19A0009: N305UP, a Boeing 767-300 aircraft operated by United Parcel Service (UPS), was conducting flight UPS218 from Paris/Charles de Gaulle (LFPG), France to Philadelphia Intl (KPHL), PA. During cruise flight approximately 175 nm north east of Goose Bay (CYYR), NL, the flight crew received several EICAS messages (IAS DISAGREE, RUDDER RATIO, AILERON LOCKOUT), in addition to several stall warning activations. A few minutes later, the EICAS message L/R EEC (Electronic Engine Control) was received. The flight crew declared an emergency and requested a diversion to CYYR, where the aircraft landed without further incident. Maintenance personnel conducted a pitot static leak check, and inspected the pitot drains. No faults were found. The right Air Data Computer (ADC) was replaced, and the aircraft was released to service.

A Republic Airlines Embraer ERJ 170-200 LR (N114HQ/RPA4389) from Montreal/Trudeau, QC (CYUL) to Philadelphia, PA (KPHL) rejected its takeoff from Runway 24R when the tower gave landing clearance to a Republic Airlines Embraer ERJ 170-200 LR (N111HQ/RPA4387) from Charlotte, NC (KCLT) to Montreal/Trudeau, QC (CYUL), which had had the wrong registration. The tower then asked RPA4387 to conduct a go-around on final for Runway 24R.

While inbound for Montreal/Pierre Elliott Trudeau, QC (CYUL), the separation between a WestJet Encore Boeing 737-7CT (C-GWJE/WEN3530) from Toronto/Lester B. Pearson, ON (CYYZ) to Montreal/Pierre Elliott Trudeau, QC (CYUL) and a Jazz Bombardier CL-600-2B19 (series 100) (C-FZJA/JZA8033) from Philadelphia, PA (KPHL) to Montreal/Pierre Elliott Trudeau, QC (CYUL) was reduced to 2.3 NM and 600 ft. No other impact on operations.

Incident at MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL)

A Bombardier CL-600-2B19 (100 series) operated by Jazz (C-GJZZ/JZA8033), flying from Philadelphia (KPHL), PA, to Montreal/Pierre-Elliott-Trudeau (CYUL), QC, declared an emergency. It reported mechanical issues with the right engine and requested emergency vehicle response. The aircraft landed without incident at 0003Z. No operational impact.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. Flight Crew conducted an in-flight shut down during descent due to a low oil pressure on the right engine. During maintenance troubleshooting, it was noted that the preformed packing for the air turbine starter (ATS) carbon seal assembly (assy) outer body was missing. A packing was installed, an oil leak check performed, and the aircraft released to service. An Safety Management System (SMS) report was submitted and the event investigated by the Company.
Update TSB #A18F0268: C-GJZZ, a Bombardier CL-600-2B19 (CRJ200) aircraft operated by Jazz Aviation LP, was conducting flight JZA8033 from Philadelphia Intl (KPHL), PA to Montreal/Pierre Elliott Trudeau Intl (CYUL), QC with 3 crew members and 41 passengers on board. Approximately 25 minutes after the departure during cruise flight at FL290, the flight crew received a fluctuating oil pressure indication on the number 2 engine. Passing through 15 000 feet during the descent to CYUL, the flight crew elected to shut the number 2 engine (General Electric CF34-3B1) down since the oil pressure was decreasing and the oil pressure warning activated. The flight crew declared a PAN PAN, briefed the passengers, and completed a successful landing at CYUL without further incident. The operator’s maintenance found that the O-ring behind the number 2 starter seal was missing. A new seal with the O-ring was installed as per GE’s maintenance manual 72-61-00. The number 2 engine oil tank was serviced, engine runs were performed, and no leaks were found. The aircraft was returned to service.

A Jazz Canadair RJ 200 Regional Jet (JZA8031) from Philadelphia, PA (KPHL) to Montreal, QC (CYUL) received and read back the instruction to maintain a speed of 160 kt up to the final approach fix (FAF). At 7 NM on final for Runway 06R, JZA8031 reduced its speed to 145 kt. No impact on operations.
