A Jazz Bombardier CL-600-2B19 (C-FIJA/JZA8030) on a flight from Montreal/Pierre Elliott Trudeau Intl, QC (CYUL) to Philadelphia Intl, PA (KPHL) requested a return to CYUL at 2002Z, and requested emergency vehicles meet the aircraft. The aircraft did not declare an emergency, and landed safely at 2106Z. No impact on operations.
Update: TSB Report #A19Q0194: C-FIJA, a Jazz Aviation LP CL-600-2B19 (CRJ2) (JZA30), was on a regular IFR flight from Montreal/Pierre Elliott Trudeau, QC (CYUL) to Philadelphia Intl., PA (KPHL) with 46 passengers and 3 crew members. After the wheels were retracted during the initial climb after takeoff, the message "Gear Disagree" appeared on the engine-indicating and crew-alerting system (EICAS). The crew placed the gear control in the extension position and confirmed that all wheels were down and locked. The crew then decided to go back to CYUL and land, and it conducted a holding pattern for approximately 45 minutes so that it could reduce the aircrafts weight to below the maximum landing limit. The aircraft landed without incident on Runway 24L with Aircraft Rescue and Fire Fighting (ARFF) services on alert. There were no injuries or damage to the aircraft.

UPDATE: TSB Report #A19Q0210: N640AE, a Piedmont Airlines Embraer ERJ-145LR (DT5011), was on an IFR flight from Montreal/Pierre Elliott Trudeau International, QC (CYUL) to Philadelphia Intl, PA (KPHL). N762QS, a Netjets Bombardier Challenger 350 (EJA762), was on an IFR flight from Montreal/Pierre Elliott Trudeau International, QC (CYUL) to Bedford/Laurence G Hanscom Field, MA (KBED). PDT5011 was cleared to take off from Runway 06R. The controller then instructed EJA762 to line up on Runway 06R before clearing it for takeoff. Once it had taken off, EJA762 had a ground speed higher than that of PDT5011, which reduced the lateral separation between the two aircraft to 2.2 NM when they were the closest together, even though a separation of 3 NM was required.

A Jazz Bombardier CL-600 2B19 (series 200) (JZA8033) from Philadelphia, PA (KPHL) to Montreal/Trudeau, QC (CYUL) declared an emergency due to an engine bleed air problem. JZA8033 landed without incident at 2130Z.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. The crew reported a 14th stage duct warning message appeared without any action of the crew. They shut off the bleed for 1 minute then reselected it but message remained so they throttled back to idle on that side and landed without incident. Maintenance carried out the FIM procedure but NFF. The Bleed Leak Control unit was then replaced and system tested serviceable.

Pearson Intl, ON (CYYZ) to Philadelphia Intl, PA (KPHL) and an Air Georgian Bombardier CL-600-2B19 (C-GKGC/GGN7312) on a flight from Indianapolis Intl, IN (KIND) to Toronto/Lester B. Pearson Intl, ON (CYYZ) reported seeing white smoke from WJA711s left wing tip. WJA711 was advised and returned to land at CYYZ at 2232Z. No emergency declared. No further incident or operational impact.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. On departure two aircraft reported seeing smoke off the left wing. The Flight Crew confirmed a mist coming off of the left wing railing edge. Aircraft returned to YYZ. Maintenance carried out high power run in accordance with Boeing 737 maintenance manual (MM) 71-00-00 NFF. Maintenance Control Centre (MCC) requested the replacement of #1 fuel tank fueling shutoff valve. Maintenance carried out request in accordance with B737 MM 28-21-51. Aircraft returned to service on October 14, 2019. Aircraft has flown 48 cycles, 148 hours since the occurrence with no further related issues.

Incident at KERPU intersection (ATS operating irregularity)

A loss of separation occurred between an Air Transat Airbus A330-200 (TSC249) on a flight from Barcelona, Spain (LEBL) to Toronto/Lester B. Pearson Intl, ON (CYYZ) and a Chartright Air Canadair CL-600-2B16 (HRT056) on a flight from Philadelphia Intl, PA (KPHL) to Toronto/Lester B. Pearson Intl, ON (CYYZ) while the terminal controller was sequencing HRT056 behind TSC249. The loss occurred as terminal instructed HRT056 to contact the tower. The tower instructed HRT056 to go around. HRT056 landed at 2127Z. There was no operational impact.
Update TSB Report #A19O0145: C-GTSI, an Airbus A330-200 aircraft operated by Air Transat, was conducting flight TSC249 from Barcelona/El Prat del Llobragat (LEBL), Spain to Toronto/Lester B. Pearson Intl. (CYYZ), ON. CGPAJ, a Canadair CL600-2A12 aircraft operated by Chartright was conducting flight HRT056 from Philadelphia Intl. (KPHL), PA to Toronto/Lester B. Pearson Intl. (CYYZ), ON. A loss of wake turbulence separation occurred on approach to CYYZ while the terminal controller was sequencing HRT056 behind TSC249. The loss of separation occurred as terminal controller instructed HRT056 to contact the tower controller. HRT056 was less than the required 5 nm behind TSC249, coming up to 4.4 nm before the tower instructed HRT056 to go around.

After passing position DUSOM, a Piedmont Airlines Embraer EMB-145LR (N691AE/PDT4901) from Toronto/Lester B. Pearson Intl, ON (CYYZ) to Philadelphia Intl, PA (KPHL) was observed to be tracking direct to PSB contrary to the standard instrument departure (SID) transition i.e. DUSOM-RIGUS-MIKKY-PSB. PDT4901 was issued vectors to correct tracking and subsequently cleared direct to PSB. No impact on operations.

Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ) (ATS operating irregularity)

South Ground Controller received flight data entry (FDE) for a Sky Regional Airlines Embraer ERJ 170-200 SU (SKV7548) from Toronto/Pearson, ON (CYYZ) to Atlanta/Hartsfield–Jackson, GA (KATL) from the Apron Management Unit (AMU). Another Sky Regional Airlines Embraer ERJ 170-200 SU (C-FJBO/SKV7538) from Toronto/Pearson, ON (CYYZ) to Philadelphia, PA (KPHL) called for taxi clearance, and the controller issued taxi instructions for SKV7548. Pilot readback taxi clearance and frequency change to tower as SKV7538. South Tower Controller issued take off clearance to SKV7548 as per FDE, and the pilot responded to the clearance as SKV7538. The Terminal Control Unit (TCU) informed the tower that the wrong aircraft had departed.
