Incident at PORT HARDY BC (CYZT) (ATM - ILS irregularity)

At Port Hardy, BC (CYZT), the instrument landing system (ILS) localizer (LOC) and glide path (GP) monitor alarm sounded at 1835Z. The Technical Operations Coordinator (TOC) was notified. During the call to the TOC, the pilot of an American, corporately registered Beech E-90 (N111TC) from Seattle/Paine Field, WA (KPAE) to Port Hardy, BC (CYZT) confirmed that they were still picking up the signal for the LOC and GP.

Incident at 8NM W VANCOUVER INTL BC (CYVR)

An American corporately registered Cessna 414 (N414ES) from Paine Field, WA (KPAE) to Vancouver, BC (CYVR) was vectored for an approach to Runway 08R. The pilot deviated north of the final approach and the controller cancelled their approach and vectored them for a second approach. During the second approach, the pilot descended below the glide path. They continued the approach and landed.

Incident at NANAIMO AIRPORT

An American privately registered Cessna 185E from Everett, WA (KPAE) to Campbell River, BC (CAE3) entered Ladysmith Harbour and the Nanaimo, BC (CYCD) mandatory frequency (MF) at an altitude of 800ft. The aircraft transited the zone at 800ft, northwest bound and exited to the NW at 500ft. There was no contact with the aircraft at any time. CYCD flight service station (FSS) attempted contact. Impact to operations: A Vital Aviation Cessna 172K (C-GTXA) in the circuit for Runway 34 was requested to turn left after a touch and go.

Incident at BELLINGHAM WA (KBLI)

An American, corporately registered Beech E90 (N111TC) from Bellingham, WA (KBLI) to Snohomish County/Paine Field, WA (KPAE) departed KBLI cleared for the KIENO 6 departure. The aircraft did not follow the standard instrument departure (SID) route and then climbed through its cleared altitude. No operational impact. Reported by the Vancouver Area Control Centre.

An American corporately registered Cessna 340A (N340SB) from Paine Field, WA (KPAE) to Victoria Int'l, BC (CYYJ) was given 2000 feet as a final altitude to intercept the ILS Runway 27 approach. The aircraft deviated 500 feet below prior to intercepting final.

Incident at EDMONTON CENTRE / ACC D'EDMONTON

An Oman Air Boeing 787-9 (OMA026) from Paine Field, WA (KPAE) to Muscat Intl Airport, Oman (OOMS) approaching 80W was observed off course. The flight plan was showing next position to be 6630N80W but the aircraft was proceeding to 66N80W, approximately 30 nautical miles off course. When queried, the pilot confirmed that they were supposed to fly over 6630N80W and that it was their error with flight plan input to their flight management system (FMS).

Incident at CAMPBELL RIVER BC (CYBL)

An American privately-registered Mooney M20K from Everett, WA (KPAE) to Campbell River, BC (CYBL) was observed crossing overhead at 0114Z to join downwind right for Runway 30 with no contact and was not on radar. Comox, BC (CYQQ) air traffic control (ATC) was queried and was advised that this aircraft was reported by Victoria, BC (CYYJ) terminal to be having electrical problems. The Aircraft landed safely at 0117Z. No impact to operations.

A Thai Airways Boeing 787-9 (THA8909) from Everett, WA (KPAE) to Samut Prakan, Thailand (VTBS) returned to KPAE due to a mechanical issue. No impact on operations.

A Caribbean Airlines Boeing 737-800 (9YKIN/BWA600) on a flight from Port of Spain, Trinidad and Tobago (TTPP) to Toronto/Lester B. Pearson, ON (CYYZ) was instructed to overshoot the approach to Runway 05 due to a NetJets Aviation Bombardier CL-600-2B16 (N214QS/EJA214) on a flight from Everett, WA (KPAE) to Toronto/Lester B. Pearson, ON (CYZZ) slow to exit. BWA600 landed Runway 05 at 2306Z without incident. No impact to operations.

TSB #A16P0194: JA886A, a Boeing 787-9 aircraft operated by All Nippon Airways, was conducting flight ANA9397 from Everett/Paine Field-Snohomish County, WA (KPAE) to Tokyo/Haneda, Japan (RJTT). During the climb to cruising altitude after departure, the flight crew declared an emergency due to a vibration in the right engine while in the vicinity of Port Hardy, BC. The flight crew elected to return to KPAE.

Incident at CAMPBELL RIVER BC (CYBL)

UPDATE: TSB#A16P0070: N585PB, a privately registered Beech RC-45J (BE18) aircraft, was being operated from Everett/Snohomish County, WA (KPAE) to Campbell River, BC (CYBL). After landing on Runway 12, the aircraft veered off the runway and went onto the graded surface on the north side of the runway. ARFF vehicles responded. The aircraft was not damaged and subsequently, was taxied to the apron. There were no injuries.
An American corporately-registered Beech RC-45J (N585PB) from Snohomish, WA (KPAE) to Campbell River, BC (CYBL) landed on Runway 12 at 2021Z. During the landing roll, the aircraft veered off the Runway onto the grass on the North side. Emergency vehicles responded. The aircraft was able to taxi under its own power. There was no impact on operations.

An American privately registered Cessna U206G from Snohomish County, WA (KPAE) to Prince George, BC (CYXS) entered the Quesnel, BC (CYQZ) Mandatory Frequency (MF) zone at 1912Z without making contact on 122.2. The aircraft flew through the zone between 4500-4700 ASL (CYQZ zone extends up to 4800 ASL). The aircraft exited the zone at 1917Z. While there were other aircraft on frequency, there was no operational impact.

An Alaska Airlines Boeing 737 800 (ASA646) from Bellingham, WA (KBLI) to Las Vegas, NV (KLAS) departing southbound was held at 2000 feet until clear of the Cessna 172M.

Both a privately registered Globe GC-1B and a privately registered Cessna 120, from Snohomish County (KPAE) to Boundary Bay, BC (CZBB) flew transborder without an active flight plan. The Globe GC-18 landed at 0119Z, and the Cessna 120 landed at 0125Z. Pilots filed VFR flight plans with Prescott AFSS but failed to activate them.

An Atlas Air Boeing 747 400 (GTI4512) from Snohomish County, WA (KPAE) to Nagoya (RJGG) advised they had mechanical difficulties and requested a fuel dump for 30-40 min then return to KPAE. The aircraft was vectored to vicinity of Tofino, BC (CYAZ) FL210 to complete fuel dump before returning to KPAE. No emergency declared and no operational impact.

Incident at ABBOTSFORD BC (CYXX)

An American corporately registered Grumman GA-7 (N6178C) from Snohomish County, WA (KPAE) to Abbotsford, BC (CYXX) was cleared to land runway 25. Tower observed that the landing gear was not down. The aircraft was pulled up. Only the nose gear was partially down. The aircraft cycled gear down manually and ARFF was put on standby. Aircraft landed safely at 2335Z.

An American Corporately registered Learjet 60 (N-889DW) on a IFR flight from Paine Field (KPAE) to Edmonton (CYEG) requested decent from FL410 to FL390. A decent to FL390 was issued to a London Air Services Learjet 45 (C-GLYS) on a IFR flight from Vancouver (CYVR) to Medicine Hat (CYXH) , both N-889DW and C-GLYS simultaneously read back the clearance and both descended. The controller did not recognize N-889DW had taken the clearance for C-GLYS. A loss of separation did not occur.

An American privately registered Piper PA23, IFR Bellingham (KBLI) to Everett (KPAE), departed off runway 16 at KBLI and was issued the KIENO4 departure. The Piper turned towards the HUH VOR (very high frequency omnidirectional radio range). Controller turned aircraft south westbound and subsequently direct to CVV. No operational impact.

Incident at 90 NM North West of Port Hardy (CYZT)

An Atlas Air Inc. Boeing 747-400F on initial delivery flight GTI 4512, IFR Paine Field, WA (KPAE) to Nagoya, Japan (RJGG), reported mutliple computer system warnings and decided to return to Paine Field. Aircraft descended to FL190 and commenced dumping fuel over the ocean. Aircraft dumped fuel for approximately one hour. Aircraft handed off to Seattle Centre at 0340. No impact to operations.

Incident at ABBOTSFORD BC (CYXX) (Conflict - loss of separation, ATS operating irregularity)

A private American-registered Cessna C182 was IFR at 5,000 feet Bellingham, WA (KBLI) to Everett, WA (KPAE). An American-registered Imco General Construction corporate Beech BE58 departed KBLI IFR to Olympia (KOLM), flight planned at 5,000 feet and initially cleared to 5,000 feet by KBLI Tower. On initial contact with the Victoria Terminal controller, N275V was cleared to 4,000 feet and acknowledged by the pilot by repeating 4,000 feet. The aircraft was observed climbing to 4,300 feet when the controller restated the 4,000 feet clearance. Both aircraft were 2 NM apart and vertically separated by 700 feet at the time that the 4,000 feet clearance was restated.

The pilot of C-GHCJ, a Cessna 172, received a transponder code from Seattle (KSEA) FSS but did not activate a VFR flight plan at that time. Consequently, when the aircraft arrived at Boundary Bay (CZBB) from Snohomish County (KPAE), at 0010z without an active flight plan, the pilot was in contravention of the CARs.

N7711D a Cessna 210 enroute from Snohomish, WA (KPAE) to Prince George (CYXS) on a VFR flight plan, landed receiver only (RONLY), at Williams Lake (CYWL) at 2035z. Faulty mike repaired on the ground. The aircraft departed for CYXS at 2052z.

Incident at NANAIMO AIRPORT

N184CH H46 arrived without a flight plan on file from KPAE. Pilot advised that he had filed a flight plan with Seattle FSS. SEA FSS queried by Vancouver ACC and advised that a flight plan had not been filed within the last seven days. Seattle Operations Manager, Seattle ARTC advised for followup. Note: Aircraft type HR47 not listed in CADORS.

A9C-HMK, a Bahrainian-registered B747, was eastbound on a flight from Seattle and near Fort McMurray when the crew reported that they had lost both HF transmitters. They elected to return to Paine Filed (KPAE) in Washington.

Incident at CAMPBELL RIVER BC (CYBL)

PC12 aircraft proposed 0900Z IFR departure from CYBL to KPAE. No departure clearance or advisory requested. INREQ procedures commenced at 0930Z. RCMP did ramp check with negative results. They did however, report a flickering red glow on a ridge south of the airport. No ELT reports. Airport Manager did second ramp check including some hangers. He did not have access to all hangers. RCC dispatched a Labrador. Seattle FSS searching for any flight plan information either in or out of Paine Field. Aircraft was found down and safe. There had been problems with the flight plan. Pilot filed IFR flight plan through DUATS system in the States for inbound flight to Cambell River and outbound to Paine Field. Pilot cancelled these flights prior to departure in the States. The American portion inbound to Cambell River was cancelled, but the IFR flight plan portion from CYBL to KPAI was never closed. This was due to the difference between our systems. Search procedures were started with RCC Victoria and helicopters were searching the local area before aircraft was located at Paine Field.
