A SkyWest Airlines Embraer ERJ 170-200LR (SKW4560) on a flight from Minneapolis/St. Paul Intl, MN (KMSP) was cleared to FL290 by the Winnipeg area control centre (ACC) controller and the clearance was read back correctly. The controller subsequently updated the electronic flight strip (EFS) with FL320 as that was the altitude the controller had intended to clear the aircraft to. SKW4560 was observed descending through FL308 and was queried by the controller. SKW4560 was re-cleared to FL300. No impact to operations.

TSB #A19Q0055: A Vista Jet Bombardier BD-700-1A11 Global 5000 (N501VJ) was on an instrument flight rules (IFR) flight from Minneapolis-St. Paul/Wold-Chamberlain Field, MN (KMSP) to Montreal/Pierre Elliott Trudeau, QC (CYUL) with 3 crew members on board. While the aircraft was approaching the Execaire hangar at CYUL to park after its landing, the crew was guided by 2 ground staff members: the first was in front and the second was at a 45° angle on the aircrafts left side. Before making the final turn, a 90° left turn, the crew performed a visual check on the right and followed the instructions from the ground staff. A few minutes later, while the crew was leaving the aircraft, one of the ground staff members indicated that the right wing had hit the gate during the final turn, but the crew was unaware. A visual inspection revealed that the winglet had sustained light damage.

Impact: the separation between a Mesa Airlines Bombardier CL-600-2C10 (N513MJ/ASH6138) from Washington/Dulles, DC (KIAD) and EDV5503 was reduced to 2.6 NM and 400 ft, where 3 NM and 1 000 ft are required. No other impact on operations.
UPDATE TSB #A19Q0136: N902XS, an Endeavor Air Canadair CRJ900 (CL-600-2D24) (EDV5503) was flying IFR from Minneapolis—St. Paul/Wold-Chamberlain, MN, to Montreal/Pierre Elliott Trudeau International (CYUL), QC. During its approach to CYUL, the aircraft did not turn towards TIDAS after AGNAG, as prescribed, and instead continued on its course. Separation with N513MJ, a Mesa Airlines Canadair CRJ700 (CL-600-2C10) (ASH6138) flying from Washington-Dulles Intl (KIAD), Washington, DC, to CYUL was reduced to 2.6 NM horizontally and 400 feet vertically, whereas the required separation was 3 NM horizontally or 1,000 feet vertically.

A Delta Air Lines Boeing 737-900 (N923DZ/DAL1084) on a flight from Minneapolis/St-Paul Intl, MN (KMSP) to Vancouver Intl, BC (CYVR) was inbound on the GRIZZ6 ARRIVAL, at 3,000, and was issued a base turn. The pilot requested the approach via PEMSU informing air traffic control (ATC) that they were too close. The aircraft was subsequently issued the approach via PEMSU and climbed back to 4,000 for traffic. The aircraft was instructed to maintain 4,000 until fully established. The aircraft went through the final approach course by over 1.5NM and descended to 3,000 before being established. The aircraft self corrected and continued for the approach.

The flight crew elected to shutdown the left hand engine as a precaution and diverted to Minneapolis-St. Paul/Wold-Chamberlain (KMSP), MN. An emergency was declared with ATC and flight landed in KMSP without further incident.

TSB #A19F0145: C-FHOS, an Embraer ERJ 190-100 aircraft operated by Air Canada, was conducting flight ACA1074 from Denver Intl (KDEN), CO to Montreal/Pierre Elliott Trudeau Intl (CYUL), QC with 4 crew members and 80 passengers on board. Less than one hour after the departure from KDEN in cruise flight, the flight crew received an ALT mis-compare flag on both altimeters, with a corresponding APPR 2 NOT AVAIL EICAS message. The flight crew consulted with the operator’s maintenance, and the decision to divert the flight to Minneapolis-St. Paul/Wold-Chamberlain Intl (KMSP), MN was elected. A MAYDAY was declared, and the aircraft landed at KMSP without further incident. The operator’s maintenance replaced the Guidance Panel in accordance with the Aircraft Maintenance Manual (AMM). Following an operational check, the aircraft was returned to service.
UPDATE Aviation Incident Report: Federal Aviation Authority (FAA) operations reported that ACA1074 declared an emergency due to an instrumentation issue. Aircraft was initially going to continue to destination of Montreal/Trudeau, QC (CYUL). Aircraft ended up diverting and landing in Minneapolis, MN (KMSP) without further incident.

Since the flight had taken off from CYYZ under Minimum Equipment List (MEL) provision with the number 2 engine generator inoperative, the flight crew consulted with the operator’s Operational and Maintenance Control Centres, and the decision to divert to KMSP was elected. A PAN PAN was declared, and the flight landed at KMSP without further incident. ARFF met the aircraft after the landing for an inspection, and the aircraft was authorized to taxi to a gate. The operator’s maintenance replaced the APU De-Oiling Solenoid Valve and the APU Bleed Control Valve as per the Aircraft Maintenance Manual (AMM). The aircraft was then returned to service.

The departures were a Jazz de Havilland DHC-8-402 (JZA8583) on a flight from Saskatoon/John G. Diefenbaker Intl, SK (CYXE) to Calgary Intl, AB (CYYC) and a Delta Air Lines Airbus A319 (DAL2862) on a flight from Saskatoon/John G. Diefenbaker Intl, SK (CYXE) to Minneapolis/St. Paul Intl, MN (KMSP).
