An Air Canada Rouge Airbus A319-114 (C-GBHZ/ROU1651) from Miami Intl, FL (KMIA) to Montreal/Pierre Elliott Trudeau Intl, QC (CYUL) declared PAN PAN. There was significant vibration in the control column. The aircraft landed without any problems. No impact.
UPDATE TSB report #A20Q0036: C-GBHZ, an Air Canada Rouge LP Airbus A319-114 (ROU1651), was on an instrument flight rules (IFR) flight from Miami, FL (KMIA) to Montreal Pierre Elliott Trudeau, QC (CYUL). While the aircraft was on final approach at 3 000 ft, the crew felt vibrations and heard loud bangs coming from the engine. The crew declared an emergency and landed without incident. No apparent damage was reported by the Aircraft Rescue and Fire Fighting (ARFF) crew during the ground inspection conducted once the aircraft had come to a stop on the runway. The aircraft then moved, without any other problems , to the gate for deplaning passengers.

C-FMAO was cleared to take off with a right turn on a heading of 320. An American Airlines Boeing 737-823 (N861NN/AAL2702) from Miami Intl, FL (KMIA) to Montreal/Pierre Elliott Trudeau, QC (CYUL) was cleared to land. C-FMAO taxied slowly, and the controller decided to pull AAL2702 up on a heading of 205.

Update TSB #A19F0094: C-FZUG, an Airbus 319-100 aircraft operated by Air Canada rouge, was conducting flight ROU1653 from Miami Intl (KMIA), FL to Montreal/Pierre Elliott Trudeau Intl (CYUL), QC with 5 crew members and 130 passengers on board. Approximately 10 minutes prior to the Top Of Descent (TOD) in the vicinity of New York City, NY, the number 1 engine (CFM 56-5A5) started to exhibit symptoms of a compressor stall with vibrations, N1 roll back, airspeed decrease, and high Exhaust Gas Temperature (EGT). The flight crew executed the QRH, and declared a PAN PAN. During the descent to CYUL, the engine parameters returned to normal values. The aircraft proceeded to land without further incident. The operator’s maintenance performed a borescope inspection on the engine, and no faults were found. The Fuel Pump/Hydro Mechanical Unit (HMU) package was removed, and replaced in accordance with (IAW) the Aircraft Maintenance Manual (AMM). Serviceability tests were carried out satisfactorily, and the aircraft was returned to service. A Service Difficulty Report (SDR) was also raised.

Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ) (ATS operating irregularity)

TSB #A19O0053: AAL2569, an Airbus 319 aircraft operated by American Airlines, was conducting a flight from Miami Intl (KMIA), FL to Toronto/Lester B. Pearson Intl (CYYZ), ON. During the intermediate approach, ATC cleared AAL2569 for an ILS or Visual Approach to Runway 06L, with instructions to contact Toronto Tower upon crossing VEPNA (4.6 DME to the runway). The flight crew acknowledged, however they contacted Toronto Tower at approximately 2.1 miles on final, and stated that they were on short final for Runway 06L. 15 seconds later and after ATC passed instructions in the event of a missed approach, the flight crew acknowledged and stated that they were on short final for Runway 06R. The controller did not realize the error, and AAL2569 was now proceeding to manoeuver for the landing on Runway 06R. The controller then cleared AAL2569 to land on Runway 06L; however, the flight crew decided and executed a go around just before the threshold of Runway 06R.

An American Airlines Airbus A319 (N9017P/AAL2589) from Toronto/Lester B. Pearson, ON (CYYZ) to Miami, FL (KMIA) was cleared to taxi for Runway 06L and mistakenly crossed Runway 06L and held short of Runway 06R. Traffic that was short final, an Air Canada Rouge Airbus A319-114 (C-FYIY/ROU1671) from Orlando, FL (KMCO) to Toronto/Lester B. Pearson, ON (CYYZ), was issued go around instructions by the tower.
Update TSB #A19O0067: N9017P, an Airbus 319-100 aircraft operated by American Airlines, was conducting flight AAL2589 from Toronto/Lester B. Pearson Intl (CYYZ), ON to Miami Intl (KMIA), FL. During the taxi out for the departure from CYYZ, Ground Control authorized AAL2589 to taxi for a departure on Runway 06L, which was read back correctly by the flight crew. After switching to the Tower frequency, AAL2589 reported that they were taxiing to Runway 06R. The incorrect runway read back was not noticed by the Tower controller, who continued on to state that they had one more aircraft to land prior to allowing AAL2589 to depart. AAL2589 proceeded to taxi across 06L and held short of the 06R ILS hold line, which resulted in the aircraft occupying Runway 06L. No clearance was given to AAL2589 to cross a runway, and the only south complex runway in use at the time was 06L. The Tower controller directed C-FYIY, an Airbus 319-100 aircraft operated by Air Canada rouge, conducting flight ROU1671 from Orlando Intl (KMCO), FL to CYYZ, to pull up and go around as there was traffic on the runway. When queried on the radio, the flight crew of AAL2589 stated that they believed they were cleared to taxi to Runway 06R, and were holding short of the ILS hold line as the weather conditions warranted.

Incident at Miami (KMIA)

TSB #A19F0129: C-GKVV, a Boeing 737-800 aircraft operated by Sunwing Airlines, was conducting flight SWG033 from Orlando Intl (KMCO), FL to Miami Intl (KMIA), FL with 6 crew members and 175 passengers on board. When Flaps 30 were selected during the approach at KMIA, a temporary airspeed increase caused the activation of the Flap Load relief system. As the flaps returned to 30° after the airspeed decreased again, the LE FLAPS TRANSIT light illuminated steadily. The flight crew executed a missed approach, and ran the LE FLAPS TRANSIT checklist. A PAN PAN was declared, and the flight crew requested the approach and landing to Runway 9. After the landing, the aircraft stopped on the runway for inspection of wings and flaps for damage, as well as the condition of the brakes. No apparent damage was noted, and brake temperatures were within the normal range. The aircraft was authorized to taxi to the terminal, followed behind by ARFF as a precaution.

On initial contact, the controller-pilot data link communications (CPDLC) UM137, seeking to confirm the assigned route for a Cargolux Airlines Boeing 747-800 (LX-VCB/CLX45B) from Miami, FL (KMIA) to Findel, Luxembourg (ELLX), indicated that the aircraft was proceeding KOGAD BIMGO after 20W.

Incident at Palm Beach Intl - West Palm Beach (KPBI)

TSB Report #A18F0280: C-GTQC, a Boeing 737-800 aircraft operated by Air Transat, was conducting flight TSC934 from Montreal/Pierre Elliott Trudeau Intl (CYUL), QC to Fort Lauderdale/Hollywood Intl (KFLL), FL with 6 crew members and 192 passengers on board. During the descent to destination, TSC934 was instructed to hold at waypoint FATHR at FL330 due to heavy thunderstorms (+TS) at KFLL. The filed alternate airport, Miami Intl (KMIA), FL, was also experiencing +TS with an estimated holding time of 25 to 30 minutes. Since weather conditions were not improving at both the destination and the alternate after 38 minutes of holding, the flight crew declared a fuel emergency for priority handling at KFLL. ATC offered West Palm Beach Intl (KPBI), FL as an option, which the flight crew accepted. TSC934 landed at KPBI uneventfully with emergency vehicles in attendance, and with 1600 kgs of fuel on board, which was above the minimum reserve fuel required.

Incident at 80NM WINNIPEG MB VORTAC (YWG)

A Flair Airlines Ltd. Boeing 737-800 (FLE809) from Winnipeg/Richardson Intl, MB (CYWG) to Miami Intl, FL (KMIA) declared an emergency (PAN PAN) after shutting down an engine and requesting to return to CYWG. FLE809 landed on Runway 36 without further incident at 1628Z.
Update TSB Report #A19F0044: OK-TSS, a Boeing 737-800 aircraft operated by Smartwings on behalf of Flair Airlines, was conducting flight FLE809 from Winnipeg/James Armstrong Richardson Intl (CYWG), MB to Miami Intl (KMIA), FL with 6 crew members and 63 passengers on board. During the climb after the departure from CYWG, the flight crew noticed a low oil pressure indication on the number 1 engine (CFM56-7B27). The Non-Normal Checklist (NNC) was executed as per the Quick Reference Handbook (QRH) and, after passing FL270 in the climb, the flight crew shut the number 1 engine down and declared a PAN PAN. The aircraft returned to CYWG and landed without further incident. Maintenance personnel carried out an inspection and found an internal oil leak within the engine. The engine was replaced, and the aircraft was returned to service.

New York (NY) advised that a British Airways Boeing 747-400 (GCIVU/BAW32N) on a flight from Miami Intl, FL (KMIA) to London/Heathrow Intl, UK (EGLL) had declared a medical emergency and requested and was cleared to divert to St. Johns Intl, NL (CYYT). No traffic. No impact on operations.

Incident at 4520N / 04240W

On controller-pilot data link communications (CPDLC) DM40 message and subsequent 40W waypoint position report (WPR), a Martinair Holland Boeing 747-400 (PHCKA/MPH7342) on a flight from Miami Intl, FL (KMIA) to Amsterdam, Netherlands (EHAM) indicated incorrect route after 30W. The aircraft was re-cleared on air traffic control (ATC) protected route. No impact on operations.

Incident at GANDER CENTRE / ACC DE GANDER

Due to a medical emergency, a Virgin Atlantic Airways Airbus A330-300 (GVRAY/VIR6J) on a flight from Miami Intl, FL (KMIA) to London/Heathrow Intl, UK (EGLL) requested to divert to St. Johns Intl, NL (CYYT). Clearance was provided as requested, no traffic affected and no impact to operations. Flight landed at 0805Z.

Incident at JEBBY intersection (Communication navigation surveillance/air traffic system)

An Alitalia Societa Aerea Italiana Airbus A330-200 (AZA63C) on a flight from Miami Intl, FL (KMIA) to Rome, Italy (LIRF) deviated from planned route due to weather and entered the Moncton area control centre (CZQM) airspace without coordination from New York (NY) oceanic. Workload increased due to lack of flight plan information and procedural environment. No separation issues. NY oceanic advised. Audio and radar reviewed.
