A VFR aircraft, which was not in communication with air traffic control (ATC), flew between 2500ft and 4000ft in the final approach zone for Runways 06L and 06R at Montreal/Pierre Elliott Trudeau, QC (CYUL). Delayed entry as a result of NAV CANADA safety and evaluation quality assurance (QA) review: A Government of Canada, Department of National Defence McDonnell Douglas F-18 (VIMY11) from Bagotville, QC (CYBG) to Greenwood, NS (CYZX) lost communication with the Canadian Air Defence Sector (CADS) while following the "scramble, intercept and recovery" (SIR) procedure. A Turkish Airlines Boeing 777-300 (THY7) from Istanbul, Turkey (LTFM) to Washington-Dulles, VA (KIAD) was vectored to maintain separation. Separation was not lost. No impact to operations.

The New York Oceanic Area Control Centre (NYOAC) advised that a Royal Air Maroc Boeing 787-9 (RAM218W) from Casablanca, Morocco (GMMN) to Washington/Dulles, VA (KIAD) was re-routing to Boston, MA (KBOS) due to a medical emergency. After 5 minutes of identifying and talking with the aircraft, the medical emergency was cancelled and the aircraft continued to KIAD. No loss in service and no other issues.

Communication was achieved through the controller-pilot data link communications (CPDLC), and then later "on guard", which worked. Safety and separation were assured, no impact on operations.

Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ)

Aviation Incident Report #16525: A Commutair/United Express Embraer EMB-145XR (N12195/UCA4949) from Toronto/Lester B. Pearson, ON (CYYZ) to Washington Dulles, VA (KIAD) was deiced at the Central Deicing Facility (CDF) on Pad 3N. After deicing was complete, Iceman provided the post deice information, and advised the Flight Crew to contact Pad Control for taxi (exit) instructions. Pad Control observed the aircraft start to taxi with a taxi clearance and electronic message boards displaying "HOLD 131.175" with two RED lights. Pad Control was unable to contact the flight on the very high frequency (VHF) airway, and then contacted South Ground via hotline to see if they were speaking to the flight. South Ground was in contact with the flight and was asked to stop the aircraft and have the flight crew switch back to Pad Control frequency. Flight crew contacted South Ground and requested taxi instructions, rather than Pad Control as they were instructed to.

At time 2202Z and just west of 50N050W, a British Airways Boeing 787-9 (GZBKD/BAW293) on a flight from Heathrow/London Intl, UK (EGLL) to Washington/Dulles, VA (KIAD) declared a PAN PAN due to a pressurization issue and requested descent from FL410 to FL350. The flight was recleared to FL340. BAW293 reported the situation good at FL340 and PAN PAN cancelled at 2215Z. Flight continued to its destination and exited the Gander Intl, NL (CYQX) flight information region (FIR) at 2248Z.
UPDATE TSB Report #A20A0006: G-ZBKD, an Boeing 787-900 operated by British Airways, was conducting flight BAW293 from London Heathrow Intl. (EGLL), UK to Washington Dulles Intl. (KIAD), VA. The aircraft dispatched with the right air conditioning pack inoperative in accordance with the MEL. During the cruise portion while at FL410, the left air conditioning pack failed, resulting in a PACK ALTITUDE LIMIT warning. The checklist required a descent to FL340. At 195 nm NE of St John’s International Airport, NL (CYYT), the crew requested FL340, however it was denied by ATC. The crew then declared a PAN-PAN due to the pressurization issue and a descent to FL340 was made. Once the aircraft reached FL340, the crew reported the situation was normal and cancelled the PAN-PAN and continued to destination without incident.

Impact: the separation between a Mesa Airlines Bombardier CL-600-2C10 (N513MJ/ASH6138) from Washington/Dulles, DC (KIAD) and EDV5503 was reduced to 2.6 NM and 400 ft, where 3 NM and 1 000 ft are required. No other impact on operations.
UPDATE TSB #A19Q0136: N902XS, an Endeavor Air Canadair CRJ900 (CL-600-2D24) (EDV5503) was flying IFR from Minneapolis—St. Paul/Wold-Chamberlain, MN, to Montreal/Pierre Elliott Trudeau International (CYUL), QC. During its approach to CYUL, the aircraft did not turn towards TIDAS after AGNAG, as prescribed, and instead continued on its course. Separation with N513MJ, a Mesa Airlines Canadair CRJ700 (CL-600-2C10) (ASH6138) flying from Washington-Dulles Intl (KIAD), Washington, DC, to CYUL was reduced to 2.6 NM horizontally and 400 feet vertically, whereas the required separation was 3 NM horizontally or 1,000 feet vertically.

A Mesa Airlines Bombardier CL-600-2C10 (series 700) (ASH6259) from Washington-Dulles, VA (KIAD) to Montreal/Trudeau, QC (CYUL) was seen flying at a speed that was incompatible with traffic on approach for Runway 24R and that did not match the instruction given out by air traffic services (ATS) and read back by the aircraft. ATS instructed ASH6259 to conduct a go-around to avoid reducing separation to an inadequate level. No impact on operations.
