Incident at GANDER INTL NL (CYQX)

An Airmed International Raytheon Hawker 800XP (N823AM) from Gander, NL (CYQX) to Charlotte/Douglas, NC (KCLT) was issued a takeoff clearance for Runway 31. The aircraft began its roll, but aborted the takeoff due to an indication. The aircraft taxied back to the main apron and requested taxi instructions to depart again, stating that the issue was resolved. The aircraft was instructed to backtrack and line up, and subsequently was issued another takeoff clearance. Moments after the takeoff clearance was given, the aircraft advised that it needed to taxi back to the main apron due to another indication. No impact on operations.

The separation between a PSA Airlines Bombardier CL-600-2D24 (JIA5698) from Charlotte, NC (KCLT) to Toronto/Lester B. Pearson, ON (CYYZ) and an Air Canada Embraer ERJ 190-100 IGW (ACA447) from Ottawa/Macdonald-Cartier, ON (CYOW) to Toronto/Lester B. Pearson, ON (CYYZ) dropped to 2.3NM while both flights were on base for Runway 23. The controller re-established separation and there was no further operational impact. Both flights landed uneventfully.

The tower instructed an Envoy Air Embraer E135 (N826AE/ENY3368) from New York/LaGuardia, NY (KLGA) to Montreal/Trudeau, QC (CYUL) to overshoot Runway 06R, while the aircraft was 1 1/2 NM on final, because a Republic Airlines Embraer ERJ 170-200 LR,(N106HQ/RPA4700) from Charlotte, NC (KCLT) to Montreal/Trudeau, QC (CYUL) was still on the runway because its surface was slippery. An Air Canada Airbus A320-214 (C-FXCD/ACA316) from Calgary, AB (CYYC) to Montreal/Trudeau, QC (CYUL) was instructed to reject its approach to allow ENY3368, whose situation was deemed a priority by air traffic services (ATS), to land with priority. ENY3368 landed on Runway 06R at 1747Z.
TSB Update #A19Q0045: N826AE, an Envoy Air Embraer EMB-135KL, was on instrument flight rules (IFR) flight ENY3368 from New York/LaGuardia, NY (KLGA) to Montreal/Pierre Elliott Trudeau, QC (CYUL) with 3 crew members and 41 passengers on board. During the final approach for Runway 06R at CYUL, ATC instructed the flight crew to conduct a pull-up because of the presence of an aircraft still on the runway after landing due to slippery conditions. ENY3368 declared an emergency and requested priority landing due to low fuel and to rapidly worsening weather conditions. The aircraft returned and landed without incident on Runway 06R. No one was injured.

A privately registered Beech A36TC from Charlotte, PE (KCLT) to St. Catharines/Niagara, ON (CYSN) departed KCLT, filing a flight plan (FP) at 1226Z. Flight plan was never activated. Pilot contacted London, ON (CYXU) flight information centre (FIC) at 1633Z to close the flight plan, informing the operator that they had flight following the entire route from KCLT. The operator advised the aircraft that the flight plan would be closed, but that no flight plan was ever received from Leidos. A subsequent call to Leidos confirmed that a flight plan had been filed, but never activated.

Pearson Intl - Toronto (CYYZ) and climbing through FL240 when the crew received a FLAPS FAIL message. The crew declared a PAN PAN and continued to CYYZ. The aircraft performed an uneventful flapless landing at CYYZ with ARFF standing by. Maintenance found a fault with the left Brake/Position Sensor Unit. The unit was replaced and the aircraft was returned to service.

Pearson Int'l, ON (CYYZ) in landing flare was instructed to execute a missed approach. Subsequent arrivals moved to Runway 24L for landing. JIA5079 landed Runway 24L at 1903Z.
