Incident at ST. JOHN'S INTL NL (CYYT)

A United States Armed Forces Military Airlift Command Lockheed C-130 (Hercules) (RCH705) from St. Johns, NL (CYYT) to Westover, MA (KCEF) was taxiing for departure from Runway 29, and taxied across the hold short line on Taxiway Bravo. RCH705 was instructed to hold short and it was read back. An Exploits Valley Beech 1900D (C-GAAT/EV788) from Deer Lake, NL (CYDF) to St. Johns, NL (CYYT) on the CAT 1 approach to Runway 29 was advised, cancelled its IFR and landed on Runway 34.

An Air Transat Airbus A330-200 (TSC567) from Paris/Charles de Gaulle, France (LFPG) to Montreal/Pierre Elliott Trudeau, QC (CYUL) was instructed by the air traffic controller (ATC) to pull up on final for Runway 24R to maintain separation with the aircraft in front of it, an American, privately registered Cirrus SR22 from Westover Air Reserve Base, MA (KCEF) to Montreal/Pierre Elliott Trudeau, QC (CYUL). No impact on operations.

Pearson, ON (CYYZ) was instructed to overshoot Runway 06R due the previous arrival, an Airsprint Cessna 525A (C-FASR/ASP524) on a flight from Springfield/Chicopee, MA (KCEF) to Toronto/Lester B. Pearson, ON (CYYZ) still exiting the runway. No operational impact.

Incident at KALLU intersection

A United States Air Force Lockheed C-5 (RCH0448) on a flight from Anchorage, AK (PAED) to Westover Air Base, MA (KCEF) was observed on radar approximately 6 miles northeast off track at 1327Z. The controller queried the pilot who had the routing KALLU direct YRL. The pilot was 6 miles northeast of KALLU.

Incident at JEBBY intersection (Communication navigation surveillance/air traffic system)

New York Oceanic passed an estimate to the Moncton Low controller on a United States Air Force Lockheed C-130 (RCH39) on a flight from Westover Air Base, MA (KCEF) to St. John's, NL (CYYT). The controller accepted the estimate but had no data in the system. The data was not forwarded to the Air Traffic Operations Specialist (ATOS) for processing.

Incident at 4108N / 06129W

A United States Air Force Lockheed C-5 (RCH795) on a flight from Rota, Spain (LERT) to Westover Air Base, MA (KCEF) declared an emergency at 1650Z in New York oceanic area control centre (NYOAC). The aircraft shut down # 2 engine. 4:50 fuel at 1650Z. The aircraft requested direct routing to destination (KCEF). Estimated time of arrival (ETA) of 1756Z.

A Fox Flight Learjet 36A (C-FEMT) from Springfield, MA (KCEF) to Toronto, ON (CYYZ) while on approach to runway 15L advised the tower he had only 2 gear down indication. C-FEMT was instructed to overshoot and commenced a fly by. Tower observed 3 gear down. Information was passed to the pilot. C-FEMT landed runway 15L at 1637Z without incident. No operational impact.
Update: Follow-up information received from Airworthiness ? West [2014-05-26]: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. The CASI contacted the operator and was advised that the left main landing gear indicator lamp was burnt out. The flight crew performed appropriate checks and elected to have tower staff verify the position of the landing gear and that the landing gear light was operating. The CASI was satisfied with the operator?s additional information and subsequently advised that no further Transport Canada follow-up was required at this time.

RCH859, C5, enroute from Ramstein (ETAR) to Westover Air Force Base (KCEF), routing 46/50 TUSKY, declared an emergency at 02:07Z approximately 30 nautical miles prior to the Moncton Boundary (position 4529/5539) with one engine shut down. The aircraft requested and received clearance to descend from 36,000 ft. to 29,000 ft. The aircraft requested and was cleared direct to destination. Moncton was advised of the situation and aircraft control was transferred to Moncton.

Incident at Approximately 10 Nautical Miles South of Stephenville (CYJT)

RCH439, Lockheed C5, was enroute from Charleston Air Force Base (KCHS) to Rota (LERT). As the aircraft was crossing the boundary between Moncton and Gander ACC, the Moncton controller advised Gander that RCH439 was in rapid descent due to a pressurization problem. The aircraft leveled at 10,000 feet at 01:02Z. The crew later advised that the problem was with an aircraft door and at 01:18Z, requested to divert to Westover Air Reserve Base (KCEF) at 24,000 ft. No emergency was declared. Nil TSB.

RCH282, C-5 Galaxie, enroute from Rota (LERT) to Dover (KDOV), routing 44N 40W AT 0808Z TO 45N 50W, at 32,000 ft. At position 4430N 4530W #4 engine was shut down, emergency declared, and flight descended to 25,000 ft. Aircraft was still in New York airspace. Traffic at 29,000 ft routing eastbound 44N50W 0838Z to 49N 40W at 29000 ft. Traffic passed to RCH282 and coordinated with New York OAC. Pilot changed destination to Westover (KCEF). Aircraft departed Canadian airspace at 1115Z. Nil TSB.

At time 1840Z requested to proceed to KCEF (Westover AFB) at 28,000 ft. ATC cleared RCH6011 as requested. Nil TSB.

The United States Air Force Lockheed C-5 aircraft (operating as RCH459) was on an IFR flight from Sevilla (Moron) Airport, Spain (LEMO) to Fairfield (Travis) Air Force Base, CA (U.S.A.) (KSUU). While in the vicinity of Moosonee, the flight crew advised of an engine problem and requested a descent from FL350 to FL310. At 0218Z, the flight crew declared an emergency and requested to divert to Springfield-Chicope (Westover) Air Force Base Metropolitan Airport, MA (U.S.A.) (KCEF). The flight crew had shut down the aircraft's #4 engine. U.S.A.F. and J.R.C.C. Trenton were both advised.
