Incident at MONTRÉAL / ST-HUBERT QC (CYHU)

An American corporately owned Piper PA-46-310P (N46CR) from Montreal/Saint-Hubert, QC (CYHU) to Burlington, VT (KBTV) took off on IFR from Runway 24L with validation from the terminal. The terminal control unit (TCU) controller called on the direct line to cancel the validation, but the aircraft had already begun its take-off run. The TCU asked the tower to instruct the aircraft to reject its takeoff, which it did. No other impact.

Bell Textron called to report that a Bell Helicopter Textron Canada Limited Bell 412EP (C-FWQO) from Montreal, QC (Bell) (Heliport) (CSW5) to Burlington, VT (KBTV) had arrived at KBTV and that KBTV did not have its flight plan. The transactions were verified and everything seemed to be in order: KBTVs "Roger" message had been received. Bell Textron requested to send an arrival message to KBTV. The request was carried out.

Bombardier CL-600-2C10 (N713EV/ASQ5537) from Montreal/Pierre Elliott Trudeau International, QC (CYUL) to Atlanta International, GA (KATL). N1105V indicated having traffic in sight. N1105V was instructed to pass behind the traffic. Subsequently, N1105V was less than 500 feet underneath ASQ5537. No impact on operations.

A privately registered Cessna 310C from Montreal/Mirabel, QC (CYMX) to Burlington, VT (KBTV) taxied for departure on Runway 24. A vehicle notified the flight services station (FSS) that something was hanging off of the aircraft?s left wing. After a check, it turned out that the aircraft was dragging a concrete block anchor that was still attached to the left wing. Security was deployed. The aircraft came to a stop on Taxiway Bravo and airport personnel removed the concrete block. The aircraft cancelled its flight and returned to the fixed base operator (FBO). Taxiway Kilo and part of Bravo were closed for inspection. An Exact Air Inc. Cessna 310R (C-FTNF) was delayed at departure for 7 minutes because Taxiway Kilo was closed (from 1810Z to 1826Z).

Incident at 18NM QUÉBEC / JEAN LESAGE INTL QC (CYQB)

UPDATE TSB Report no. A18Q0017: Cessna 441 N191MG was IFR from Quebec City/Jean-Lesage International Airport, Quebec (CYQB) to New Bedford Regional Airport, Massachusetts, USA (KEWB). As the aircraft climbed to flight level 240, the pilot declared an emergency because of a vibration at the rear of the aircraft. Less than two minutes later, the pilot advised that he had stabilized the situation and was no longer in a state of emergency, but requested a diversion to Burlington Intl Airport, Vermont USA where the aircraft landed without incident.
While climbing to FL240, an American corporately owned Cessna 441 (N191MG) from Quebec/Jean Lesage, QC (CYQB) to New Bedford, MA (KEWB) declared an emergency due to vibrations at the back of the aircraft and requested to land immediately. Less than two minutes later, the aircraft was in control of the situation and informed the air traffic controller (ATC) that there was no longer an emergency and that it preferred to be diverted to Burlington, VT (KBTV). At 1947Z, the aircraft reported that it was going to continue its flight with the landing gear down, and it still did not declare an emergency. No impact on operations.

At 1912Z, the London flight information centre (FIC) received a VFR flight plan for a Government Of Canada, Department Of National Defence Bell 412 (CFC145) from Wheeler-Sack Army Air Field, NY (KGTB) to Kingston, ON (CYGK) with a proposed time of 2000Z. At 2024Z, the London flight information centre (FIC) received an arrival (ARR) message from the CYGK flight service station (FSS) on this aircraft without getting a departure (DEP) message from LEIDOS. The flight plan and flight alerting was never received into Canada. LEIDOS indicated that a departure (DEP) message was sent from the KGTB base operations to the KBTV flight service station (FSS) however, a departure (DEP) message or active flight plan was never addressed to the London International, ON (CYXU) flight information centre (FIC).

Incident at MONTRÉAL / ST-HUBERT QC (CYHU)

TSB A17Q0104 UPDATE: A privately operated Piper PA-31 was on an IFR flight from Montréal/St-Hubert (CYHU), QC, to Burlington Intl (KBTV), VT, with 1 pilot and 5 passengers on board. During the initial climb after takeoff from Runway 06R, the left engine (Avco Lycoming TIO-540-A2C) power was reduced with significant speed fluctuations. The pilot declared an emergency and returned to land without incident on Runway 24R. No one was injured and the aircraft was not damaged. The post-flight inspection revealed a problem with the injectors.
A privately registered Piper PA-31, from Montréal/St-Hubert (CYHU), QC, to Burlington (KBTV), VT, declared an emergency during the initial climb from Runway 06 and was authorized to land on Runway 24R. The aircraft landed without further incident and then cancelled the emergency. The pilot mentioned having a problem with one of the two engines.

N133WC landed without incident at 0014Z. No loss of separation. No impact on operations.
