An Xojet Aviation LLC. Cessna 750 (XOJ794) from Bedford, MA (KBED) to Aspen/Pitkin, CO (KASE) reported that one of their cockpit screens burned out and there was an electrical smell in the cockpit. XOJ794 did not wish to declare an emergency but requested a diversion to Detroit Metro/Wayne County, MI (KDTW). The Cleveland Air Route Traffic Control Center (ZOB) watch commander was informed.

Although it received and read back clearance to descend to 3 000 ft, a Flexjet Bombardier CL-600-2B16 (LXJ589) from Bedford, MA (KBED) to Montreal/Pierre Elliott Trudeau, QC (CYUL) was seen descending to 2 400 ft before it intercepted the approach final for Runway 24L. The VFR traffic was passed, and there was no loss of separation. No impact on operations.

A Piedmont Airlines Embraer EMB-145LR (N640AE/PDT5011) from Montreal/Trudeau, QC (CYUL) to Philadelphia, PA (KPHL) took off from Runway 06R, followed by a Netjets Aviation Bombardier BD-100-1A10 (N762QS/EJA762) from Montreal/Trudeau, QC (CYUL) to Teterboro, NJ (KBED). Initial loss of separation.
UPDATE: TSB Report #A19Q0210: N640AE, a Piedmont Airlines Embraer ERJ-145LR (DT5011), was on an IFR flight from Montreal/Pierre Elliott Trudeau International, QC (CYUL) to Philadelphia Intl, PA (KPHL). N762QS, a Netjets Bombardier Challenger 350 (EJA762), was on an IFR flight from Montreal/Pierre Elliott Trudeau International, QC (CYUL) to Bedford/Laurence G Hanscom Field, MA (KBED). PDT5011 was cleared to take off from Runway 06R. The controller then instructed EJA762 to line up on Runway 06R before clearing it for takeoff. Once it had taken off, EJA762 had a ground speed higher than that of PDT5011, which reduced the lateral separation between the two aircraft to 2.2 NM when they were the closest together, even though a separation of 3 NM was required.

Aircraft was able to exit the runway on Taxiway Charlie. A CargoJet Airways Boeing 767-200 (CJT518) from Vancouver, BC (CYVR) to Hamilton, ON (CYHM), 5 miles in trail, executed a missed approach and was vectored for and landed on Runway 30 after a full runway inspection.

EJA508 landed Runway 24L at 2145Z without further incident (WOFI). No operational impact.

Pearson, ON (CYYZ) reported responding to a traffic alert and collision avoidance system (TCAS) resolution advisory (RA) while descending through 8300 ft to 8000 ft. Traffic was a departing Ocean Aviation Services (2018) Limited Bombardier BD-100-1A10 (CL35) (C-FOCI) from Toronto/Lester B. Pearson, ON (CYYZ) to Bedford/Hanscom Field, MA (KBED), climbing through 6300 ft to 7000 ft with a good rate of climb. JZA8872 was observed to climb to 8600 ft before continuing with descent as cleared. No impact on operations.

An American co-owned Cirrus SR22T (N741KF) had originally filed an IFR flight plan from Parry Sound, ON (CNK4) to Bedford, MA (KBED) proposed for 1545Z. The aircraft went overdue for departure and flight planning called airport to find out that the aircraft had departed VFR. The aircraft was found safe on the ground in Norwood, MA (KOWD) at 1718Z. No VFR flight was found in system.

An American, corporately registered Beech 58 (N858W) from Toronto/Billy Bishop, ON (CYTZ) to Bedford/Hanscom Field, MA (KBED), after departure from CYTZ, was observed to deviate from the MAVAN2 standard instrument departure (SID) as cleared. When queried, the pilot advised they were tracking direct to RIKEM. This track put the aircraft into potential conflict with arriving aircraft, a Jazz de Havilland DHC-8-402 (JZA7965) from Montreal/Trudeau, QC (CYUL) to Toronto/Billy Bishop, ON (CYTZ), however, separation was maintained. N858W should have remained on the SID heading of 090 degrees.

On landing, an Irving Air Service Inc. Gulfstream G280 (C-FRKI) from Laurence G. Hanscom, MA (KBED) to Fredericton, NB (CYFC) indicated that birds had hit the windscreen of the aircraft. Two aircraft elected to continue their approaches for landing after the reported bird strike. When the runway inspection was completed, 5 birds were found on the runway. No impact on operations.

UPDATE Aviation Incident Report #16249: At approximately 1116Z, aircraft (C-GJEI) struck a killdeer during takeoff flight and continued. Killdeer carcass was recovered.

Accident at WINNIPEG / JAMES ARMSTRONG RICHARDSON INTL MB (CYWG) (Dangerous goods/hazardous materials, Aerodrome, runway or taxiway shutdown)

An American privately registered Cessna 550 and a Morningstar Partners Bombardier CL-600-2B16 (C-FXWT) collided on an uncontrolled apron at Winnipeg/James Armstrong Richardson Int'l, MB (CYWG). One of the aircraft began to leak fuel and Aircraft Rescue and Fire Fighting (ARFF) responded. Apron 2 was closed by the airport authority. No operational impact.
UPDATE TSB Report #A18C0018: An American privately registered Cessna C550 Citation Bravo aircraft, was conducting a flight from Winnipeg/James Armstrong Richardson Intl, MB (CYWG) to Bedford/Laurence G Hanscom Field, MA (KBED). After starting the engines on Apron II at CYWG while under the guidance of a ground marshaller, C550 began to taxi for the departure. During the initial turn from the parking area, aircraft control was lost and C550's right wing collided with the nose gear of C-FXWT, a Bombardier CL-600-2B16 (Challenger 605) aircraft, which was parked. The pilot of C550 subsequently shutdown the aircraft and exited with the passenger without injuries. The right wing of C550 was substantially damaged, causing a fuel leak on the apron. The left wing of C-FXWT was also damaged in the collision by the nose of C550. Local 911 was advised of the collision and fuel leak. ARFF was dispatched to the site, and the fuel leak was contained.

An American corporately registered Bombardier BD-700-1A10 (N829RA) called Halifax Terminal stating they had lost high frequency (HF) contact with New York Oceanic Area Control Centre (NYOAC) and need to turn back to Bedford, MA (KBED) or Boston, MA (KBOS). Halifax Terminal gave them Moncton High Level frequency to contact. Moncton High Level radar identified the aircraft and contacted NYOAC for control. No loss of separation and no service delays.

Pearson, ON (CYYZ) slowed below expected airspeed. ACA423 landed Runway 24L at 2332Z.

No flight plan was received from Leidos. The pilot said they filed the flight plan through Foreflight. A call to Leidos indicated the pilot had filed a flight plan from KBED to CYTZ but it had not been activated.

A Cobalt Air Pilatus PC-12/47E (N367AF/CNS762) was on a flight from Bedford, MA(KBED) to Frankfort, MI (KFKS). The aircraft reported an engine indicator and opted to land at Toronto/Lester B. Pearson, ON (CYYZ) as a precautionary. Emergency services on standby as precaution. The aircraft landed Runway 23 without incident at 1629Z. The aircraft taxied to SkyService under its own power.
UPDATE TSB Report #A17O0176: N367AF, a Pilatus PC-12/47E aircraft, was enroute from Bedford/Laurence G Hanscom Field, MA (KBED) to Frankfort/Dow Memorial Field, MI (KFKS) when the torque indication started to fluctuate. The flight crew elected to divert to Toronto/Lester B. Pearson Intl, ON (CYYZ) and the aircraft landed without further incident. ARFF were on standby for the landing.

Incident at PORT HAWKESBURY NS (CYPD)

An American corporately registered Cessna 650 (N119ES) was on a flight from Port Hawkesbury, NS (CYPD) to Bedford, MA (KBED). The target was observed climbing through 12500 feet on a 1200 code. The target levelled off at 16500 feet. When the target was 55 miles west of CYPD, CB controller received call from Halifax, NS (HZ) flight information centre (FIC) advising that N119ES was requesting clearance. FIC was advised to give the aircraft 3650 code and make contact on 135.3. Clearance was issued to the aircraft at 2046Z. A Jazz Aviation de Havilland DHC-8-300 (JZA8810) was levelled at 15000 feet for its original clearance of 17000 feet and turned 10 degrees due to the unidentified target. No other impact to operations.

At 1708Z, the automatic dependent surveillance (ADS) reports from an American corporately registered Gulfstream GIV-X (N228RE) on a flight from Shannon, Ireland (EINN) to Bedford, MA (KBED)indicated FL403 and the aircraft continued to climb to FL430. The climb was not expected and clearance was not issued. No other traffic affected.

Incident at 13NM SE SAINT JOHN NB (CYSJ)

UPDATE: TSB REPORT#A17A0039: N990NB, a private Gulfstream GV SP aircraft, was conducting a flight from Bedford, MA (KBED) to Paris, France (LFPB). Approximately 13 nm southeast of Saint John, NB the crew received a RETRIM RIGHT WING DOWN crew alerting system message (CAS) followed by a RETRIM LEFT WING DOWN CAS message. The autopilot system was disconnected, and intermittent binding of the aileron control system was noted by the crew. The crew declared an emergency, requested a diversion to Bangor, ME (KBGR) and landed at KBGR uneventfully. Maintenance determined the fault was the #1 Aileron Autopilot Servo and it was replaced.
An American corporately registered Gulfstream GV-SP (N990NB) on a flight from Bedford, MA (KBED) declared an emergency with a flight control problem while on autopilot. The aircraft requested a diversion to Bangor, ME (KBGR). No assistance required.

Incident at 250NM NE HALIFAX / STANFIELD INTL NS (CYHZ)

An Isle of Man corporately registered Gulfstream IV (MWONE) from Bedford, MA(KBED) to Farnborough, England (EGLF) reported a hydraulic link indication and requested to divert to Halifax, NS (CYHZ). No emergency or assistance required. At 1612Z, the aircraft reported an emergency brake indication and requested emergency vehicles on standby for the landing. The aircraft landed without incident at 1642Z.
UPDATE TSB #A17A0016: M-WONE, a corporate owned Gulfstream IV-X aircraft, was conducting a flight from Bedford/Laurence G Hanscom Field, MA (KBED) to Farnborough, UK (EGLF) with 3 crew members and 4 passengers on board. While in cruise flight approximately 250 nautical miles North East of Halifax/Stanfield Intl, NS (CYHZ), the flight crew noted a CAS indication LEFT HYDRAULIC LOW PRESSURE and requested to divert to CYHZ. No emergency was declared, however the crew requested ARFF to be on standby as a precaution. The aircraft landed without incident. The left engine driven hydraulic pump was replaced and the aircraft was returned to service.
UPDATE: JRCC Halifax SARSUM Report#[S2017-00088]: (445285N 0633052W - CYHZ: Halifax Intl, NS). Moncton area control centre (ACC) called with an aircraft that has not declared an emergency; however they believed the aircraft to have a hydraulic leak. The M-WONE aircraft was a Gulfstream 4 with 7 persons on board diverting to Halifax International. Moncton Center later called back and confirmed that the aircraft had landed safely at Halifax Int'l Airport.

Three consecutive aircraft, a Bombardier Inc Bombardier BD-700-1A11 (C-FMYG/ BBA31) on a local flight from Montreal, QC (CYUL), an I.M.P. Group Bombardier CL-600-2B16 (C-GWLL) from Winnipeg, MB (CYWG) to Montreal, QC (CYUL), and a L.J. Associates Cessna 650 (N606KG/ LJY606) from Bedford, MA (KBED) to Montreal, QC (CYUL), flying the area navigation (RNAV) (global navigation satellite system (GNSS)) Runway 24L approach, reported global positioning system (GPS) signal loss at approximately 4NM final: Impact: for safety reasons the RNAV approach was NOTAMed unserviceable and the airport arrival rate (AAR) at CYUL was reduced to 26 requiring implementation of a ground delay program (GDP).

There was a loss of separation between a Ledair Inc. Raytheon Hawker 800XP (C-GKPP), from Bedford (KBED), MA, to Montréal (CYUL), QC, and a Jazz de Havilland DHC-8-402 (C-GGNY/JZA8731), from Québec (CYQB), QC, to Montréal (CYUL), QC, when C-GKPP was authorized to conduct a visual approach for Runway 06R.
UPDATE TSB#A16Q0056: A Hawker 800XP (C-GKPP) was on an IFR flight from Bedford/Laurence G. Hanscom Field (KBED), MA, to Montréal/Pierre Elliott Trudeau Intl (CYUL), QC. A Jazz Dash 8-400 (C-GGNY/ JZA8731) was on an IFR flight from Québec/Jean Lesage Intl (CYQB), QC, to Montréal/Pierre Elliott Trudeau (CYUL), QC. There was a loss of separation between the two aircraft while C-GKPP had been authorized to conduct a visual approach for Runway 06R. Separation was reduced to 700 ft vertically and 2 mi. horizontally, while the standard is 1000 ft vertical or 3 mi. horizontal.

Incident at MUSKOKA ON (CYQA)

A Netjets Aviation Cessna 560 Citation V (N846QS / EJA846) from Bedford, MA (KBED) to Muskoka, ON (CYQA) on a visual approach indicated they had a traffic alert and collision avoidance system (TCAS) warning and indicated they would fly overhead to join the circuit for Runway 36. EJA846 landed approximately 2 minutes later than their original estimate.
