Incident at NOSIV intersection

A United States Armed Forces, Military Airlift Command Boeing KC-135R (RCH553) from Fairchild Air Force Base, WA (KSKA) to Royal Air Force Mildenhall, UK (EGUN) was 5NM off course and pilot was questioned. RCH553 had cancelled their original flight plan and filed a different one. Canadian Automated Air Traffic System (CAATS) showed the new flight plan but RCH553 was flying their original one. Air Traffic Control (ATC) cleared RCH553 via their original flight plan. No further operational impact.

At 1644Z, approximately 25NM NE of St. John's, NL (CYYT), a United States Air Force Lockheed C-130 (Hercules) (01798T / RCH412) from Mildenhall, England (EGUN) to CYYT declared an emergency due to a possible fuel leak. Aircraft was vectored for straight in instrument landing system (ILS) 11 approach and landed without incident at 1701Z. No other traffic affected.
UPDATE: JRCC SARSUM Halifax Report#[H2016-00913]: (473712N 0524514W - St John's Airport, NL). Nav Canada (NAVCAN) called to inform of a Lockheed C130 that was declaring an emergency for a fuel leak 25NM northeast of St John's, NL. Gander Low confirmed that the aircraft landed safely at 1702Z. No further action was required.

The automatic dependent surveillance (ADS) position over POTAT Intersection at 0216Z indicated RCH170 was level at FL350. The pilot confirmed level at FL350 with the Edmonton Centre controller. No other impact on operations.

RCH609 taxied into taxiway B/RWY02 intersection. A Government of Canada, Department of National Defence Lockheed C-130 (RCH341) from St. John's, NL (CYYT) to Suffolk (EGUN) was lining up on runway 29. RCH609 back taxied into 'B' but had some directional difficulty returning to the hold short line. RCH609 was instructed to hold position 'B' for the departure. RCH341 was delayed one minute. No other impact on operations.

Incident at In the Vicinity of Sioux Narrows VOR/DME (VBI)

UPDATE: TSB REPORT#A13C0122: Air Canada flight 1103 an Embraer 190 aircraft, registration C-FNAJ was enroute from Montreal to Winnipeg at an altitude of 38000 feet. Flight RCH456 a Boeing KC-135R Stratotanker was enroute from Riverside, California to Mildenhall, England at an altitude of 39000 feet. While over the vicinity of Sioux Narrows, Ontario, RCH456 began a descent and ACA1103 received a TCAS advisory. Winnipeg centre advised both aircraft of each other's position and that RCH456 did not have clearance to descend. RCH456 advised the Winnipeg control centre that they had a trim problem and requested a lower altitude. As the traffic approached ACA1103 could see that RCH456 would pass to the left side and would move behind ACA1103 position and no avoidance action was taken. RCH456 was cleared to a lower altitude after traffic had passed. RCH456 did not declare an emergency and advised that they had the trim problem under control. At the lowest point, both aircraft passed each other with a 200 feet vertical and 2.8 miles lateral separation.
A United States Department of Defence Boeing KC-135R (RCH456) from March Air Reserve Base, CA (KRIV) to Midenhall, UK (EGUN), at FL390, started descent without authorization due to aircraft equipment issues. Traffic in the vicinity was an Air Canada Embraer E190 (ACA1103) from Montreal, PQ (CYUL) to Winnipeg, MB (CYWG) going the opposite direction at FL380. Both aircraft had been passed traffic one minute prior to the occurrence. At the time of the altitude loss RCH456 was instructed to maintain altitude due to the traffic below him. The aircraft climbed back up, requested a lower altitude due trim issues and was subsequently given lower once passed the traffic. The aircraft passed with 2.0 miles lateral and 500 feet vertical. At the lowest point, RCH456 was at an altitude 200 feet above ACA1103 and the aircraft were separated by 2.8 miles in airspace where 5 miles and 1000 feet is required. The aircraft did not declare an emergency and later advised believing the problem was under control. Civil Aviation Contingency Operations (CACO) was notified.

At 03:42Z, a vertical deviation report was received for RCH545, KC-135R, enroute from Seymour Johnson Air Force Base (KGSB) to RAF Mildenhall (EGUN), indicating that the flight was climbing above 39,000 feet. The flight was not issued a clearance to climb but advised that the autopilot disengaged due to turbulence which caused the flight to climb. The aircraft was observed climbing to 40,000 feet before the flight began descent back to 39,000 feet.

UPDATE TSB: A11A0083: The Evergreen International Airlines B747-212B aircraft (registration N485EV) operating as flight EIA1576 was enroute from McGuire (KWRI) to Mildenhall (EGUN) airport. While in cruise flight at about 49:30N 046:00W the crew received an intermittent aft lower cargo fire warning. The crew executed the contingency procedure and requested clearance to St. John's (CYYT) airport. Flight EIA1576 landed at St. John's without further incident. Maintenance personnel reset the aft lower fire warning card and tested the associated smoke detectors. The aircraft was returned the aircraft to service.
