Incident at 5000N / 05400W

A British Airways Boeing 747 400 (GVIIB/BAW40F) from New York/John F. Kennedy, NY (KJFK) to London/Heathrow, United Kingdom declared a PAN PAN due to smoke in the cockpit at 0105Z, approximate position was 50N054W and requested to divert to Gander, NL (CYQX). The flight was re-cleared as requested. The aircraft dumped fuel from 0113Z to 0129Z. Landed without incident at 0147Z.
UPDATE: CADORS 2019A0769 (AOR 250972-V1) was a duplicate of this occurrence and was deleted.
TSB #A19A0044: G-VIIB, a Boeing 777-200 aircraft operated by British Airways, was conducting flight BAW40F from New York/John F Kennedy Intl (KJFK), NY to London/Heathrow (EGLL), UK with 15 crew members and 227 passengers on board. During cruise flight approximately 50 nm north of Gander Intl (CYQX), NL, the cabin crew reported an odour of smoke in the vicinity of the 2R door. The flight crew completed the checklist, and the odour abated after the APU bleed and recirculation fans were turned off. Approximately 45 minutes later, the odour was again noted in the same area, but spreading to the 2L and 2R doors and accompanied by high heat near the overhead lockers. A mist was also observed by the cabin crew. The checklist was run through without any change in conditions. The flight crew declared a PAN PAN, requested a diversion to CYQX, and proceeded to jettison 3.5 tons of fuel in order to prevent an overweight landing. The aircraft landed without further incident. The aircraft was later ferried to Cardiff Intl (EGFF), UK, while the operator sent a relief aircraft to take the passengers to EGLL.

N388CA, Gulfstream G IV, enroute from Bedford (KBED) to Cardiff (EGFF), advised of a pressurization problem and requested clearance to descend to 35,000 ft. No emergency was declared and the pilot reported no further issues.

Incident at Radiophare de Charlevoix (ML NDB)

C-GWPB, a Gulfstream 200 operated by Partner Jet Inc., was conducting an instrument flight rules (IFR) flight from Toronto (CYYZ) to Cardiff (EGFF). At 1449Z, the crew reported a minor technical problem and requested to return to Toronto. No emergency was declared. The aircraft landed without incident on Runway 06L in Toronto at 1632Z.

Incident at Aux environs du VOR de Mirabel ((YMX)

On an instrument flight rules (IFR) flight from Toronto (CYYZ) to Cardiff (EGFF), flight OOM710 did not follow the route that air traffic control (ATC) had on record in the Mirabel (YMX) VHF omnidirectional range (VOR) sector. The incident did not cause any loss of separation or impact on operations. ** ** *** ** ** De Toronto (CYYZ) à Cardiff (EGFF) aux instruments (IFR) le vol OOM710 n'a pas suivi la route enregistrée par le contrôle de la circulation aérienne (ATC) dans le secteur du VOR de Mirabel (YMX). Incident at EGFF Cardiff, South Wales, United Kingdom

From TSB Occurrence Summary A06F0128: The Sunwing Airlines Boeing 737-800 (Flight #XLA1521, registration C-FTAH) aircraft experienced smoke emanating from an aft galley oven. An extinguisher was discharged into the oven and the smoke source was eliminated. No emergency was declared as the flight progressed to Cardiff, South Wales, UK. The aircraft landed without further incident. Maintenance removed the oven and the circuit breakers for all remaining ovens have been pulled and collared. Maintenance deferred the item under the MEL.

Incident at 50NM North of Charlo NB, Moncton FIR

ZOOM Airlines (OOM)111 Boeing 767-300 enroute from Cardiff (EGFF) to Toronto (CYYZ) reported a passenger heart attack, and requested diversion to Moncton (CYQM) with an ambulance on standby. Four minutes later, the aircraft reported that there was a doctor on board, the patient was stabilized, and the flight would continue to Toronto. Montreal supervisor advised. Incident at 150 NM Ouest de Mirabel (CYMX)

The Air Transat Boeing B757 (TSC) was en route IFR from Toronto (CYYZ) to Cardiff (EGFF). The TSB (A00Q0117) reports that, when the aircraft was 150 miles west of Mirabel, a burning odour was smelled coming from the ventilation system. The crew diverted the aircraft to Mirabel (CYMX). A fan motor was defective. The motor was disconnected, and the aircraft was returned to service. Le Boeing B757 d'AirTransat (TSC) était en route aux instruments (IFR) de Toronto (CYYZ) à Cardiff (EGFF). Le B.S.T. (A00Q0117) rapporte qu'alors que l'aéronef était à 150 milles à l'ouest de Mirabel, une odeur de brûlé provenant du système de ventilation s'est fait sentir. L'équipage a dérouté l'appareil à Mirabel (CYMX). Le moteur d'un ventilateur était défectueux. Le moteur a été débranché et l'appareil a été remis en service.
