Incident at REVELSTOKE BC (CYRV) (Communication navigation surveillance/air traffic system, Flight plan – activation)

A Simm Enterprises Cessna 172L (C-GEJY) from Edmonton/Villeneuve, AB (CZVL) to Revelstoke, BC (CYRV) entered the Information Request to Departure Station (QALQ) phase at 2312Z. Kamloops flight information centre (FIC) officials called the pilots cell phone, and left a message. Officials had received an outgoing flight plan (FP) from the aircrafts destination, CYRV, from the Edmonton (YEG) FIC a few minutes prior to the QALQ phase. Officials called the YEG FIC at 2319Z and asked if the pilot had closed their FP before filing a new FP. YEG advised that the pilot did not close their FP. Attempted to contact the aircraft on the CYRV frequency, without success. The aircraft entered the information request (INREQ) stage at 2342Z. The search and rescue (SAR) contact was called at 2345Z and they advised that they would try and contact the pilot. Received a call from another pilot of the same aircraft advising that the SAR contact must have been wrong, as they were not flying. Contacted the pilot on Golden, BC (CYGE) frequency at 0000Z. The pilot advised that they had closed the flight plan with the YEG FIC and confirmed that they had landed safely in CYRV and departed on the next leg of the flight. FP was closed. Joint Rescue Coordination Centre (JRCC) Victoria advised. YEG FIC advised that the pilot did close the FP with them.

A Namao Flying Club Cessna 172M (C-GVFW) at 1 mile final was pulled up by air traffic control (ATC) as a result. C-GFWR subsequently departed after C-GVFW was clear of the departure path.

Incident at 51NM WHITECOURT AB (YZU) VOR/DME

An American, privately registered Cessna TP206E from Prince George, BC (CYXS) to Edmonton/Villeneuve, AB (CZVL) originally had Mode C readout errors and was instructed to turn off Mode C. About 30 minutes later, the transponder and frequency cut out. The pilot was able to re-establish contact with the centre on 121.5, and the transponder eventually came back after about 10 minutes. The aircraft landed at CZVL, no emergency declared.

Aviation Incident Report #16546: It was reported that the pilot of a Heliberty Helicopters Inc. (C-GKMJ) exited the helicopter shortly after startup to re-arrange luggage in the backseat area of the aircraft. Both the pilot & co-pilot seat where unoccupied during this time whilst the engine and rotor systems where engaged. The pilot re-entered the aircraft approximately 30-45 seconds later and resumed control of the aircraft.

Air traffic control (ATC) was unable to contact the aircraft. No other impact to the operation.

Pilot of aircraft called at 2006Z, advising of arrival at CFX4. CommSearch terminated, all appropriate units were advised.

Controller gave subsequent instructions to C-FNFD to avoid conflict with C-GBNB. No other impact to the operations.

The flight service station (FSS) observed a coyote on the north side of Runway 07/25 at Grande Prairie, AB (CYQU). The coyote remained near the runway edge lights. At the time, 3 aircraft were inbound for Runway 25. The FSS reported the animals position to a WestJet Encore Ltd. de Havilland DHC-8-402 (WEN3195) from Calgary, AB (CYYC) to Grande Prairie, AB (CYQU), a Surerus Construction & Development Ltd. Cessna A185F (C-GSPK) from Edmonton/Villeneuve, AB (CZVL) to Grande Prairie, AB (CYQU), and a Shock Trauma Air Rescue Service Messerschmitt MBB-BK117 A-4D (STAR5-T) from Grande Prairie, AB (CYQU) to Grande Prairie, AB (CYQU) while they continued their operations and landed. No operational impact. Airport maintenance advised.

A WestJet Boeing 737-6CT (WJA166) from Vancouver, BC (CYVR) to Edmonton, AB (CYEG) was cleared for an area navigation (RNAV) Y Runway 02 approach with a crossing restriction at APKIT of 8000. A 10770582 Canada Inc. Cessna 172N (C-GYPB) from Calgary/Springbank, AB (CYBW) to Edmonton/Villeneuve, AB (CZVL) was operating at 8500 VFR and passing through the approach path 4NM East of APKIT. WJA166 was passed traffic. At APKIT, WJA166 reported that they had a traffic advisory and wanted to be vectored around the traffic. WJA166 was vectored off of the RNAV Y approach and cleared for a visual approach. No impact on operations.

The Joint Rescue Coordination Centre (JRCC) Trenton advised that a Namao Flying Club Cessna 172R (C-FNFD) at Edmonton/Villeneuve, AB (CZVL) was the source of the ELT. The ELT was turned off at 1751Z.

When advised of visibility and pressed for intentions, the pilot indicated they were inbound and fuel critical. When asked if they were declaring an emergency the pilot responded they were not. C-GAEN landed without incident. No impact to operations.

Incident at 9.5NM WSW EDMONTON INTL AB (CYEG)

The pilot of a 10770582 Canada Inc. Piper PA-28-140 (C-FWFK) on a flight from Edmonton/Villeneuve, AB (CZVL) to Edmonton/Villeneuve, AB (CZVL) called indicating that they had a forced landing north of Calmar, AB. Emergency services already dispatched by area control centre (ACC). Joint Rescue Coordination Centre (JRCC) Trenton advised.
UPDATE TSB Report #A20W0004: The ClearGRID Ltd Piper PA28-140, C-FWFK, was operating on an aerial survey flight; departing and returning to Villeneuve (CZVL), AB. After approximately 2.5 hours of flying in central Alberta, the aircraft was on a northerly track returning to CZVL when the pilot observed the fuel pressure in the upper limits of the gauge. Attempts to control the pressure were unsuccessful and after a brief time the engine began surging between 1800 and 2400 rpm. The pilot declared an emergency with Edmonton ATS and shortly thereafter the engine (Avco Lycoming 0-320-D2B) rpm reduced to 1100 rpm where it remained. The pilot selected a field 1.6 nm NW of Calmar, AB and conducted a forced approach. During the touchdown and roll out the aircraft’s nose wheel failed. The propeller received bending damage. Within 15 minutes, emergency services were at the scene; the pilot, who was the sole occupant, was uninjured. The unregistered 406 ELT transmitted a good signal and a position was resolved within 15 minutes.

A Montair Aviation Cessna 172S (C-GAGV) on a flight from Red Deer Regional, AB (CYQF) to Red Deer Regional, AB (CYQF) and a Montair Aviation Cessna 172R (C-GGPQ) on a flight from Red Deer Regional, AB (CYQF) to Red Deer Regional, AB (CYQF) both entered control zone at the same time. Neither aircraft complied with air traffic control (ATC) instructions for circuit entry pattern. Neither were able to locate any aircraft traffic. Communication with both aircraft was poor due to inability of pilots to understand basic ATC instructions. Operations on the airport were delayed.

C-GYTX had to be rushed off of D.

Beech 200 (CWA921) on a flight from Slave Lake, AB (CYZH) to Edmonton Intl, AB (CYEG) on descent that was cleared to 6000 ASL. C-GILY was instructed to descend immediately to 5000.

A Montair Aviation Cessna 172R (C-FSOF) from Edmonton/Villeneuve, AB (CZVL) to Red Deer, AB (CYQF) reported an engine issue and requested a diversion to Edmonton, AB (CYEG). The aircraft rescue and fire fighting (ARFF) responded and the aircraft landed Runway 12 without issue. No impact to operations.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. Maintenance confirmed rough running engine and a high fuel flow indication resulting from a clogged fuel injector. Injectors cleaned, engine ground run satisfactorily and aircraft returned to service.
