A Delta Air Lines Airbus A330-300 (N815NW/DAL162) on a flight from Minneapolis/St. Paul Intl, MN (KMSP) to Amsterdam, Netherlands (EHAM) requested a diversion to Reykjavik, Iceland (BIKF) due to smoke in the cockpit. The aircraft was re-cleared as requested and exited the gander (CZQX) Flight Information Regional (FIR) without incident at 0719Z.
UPDATE TSB Report #A19A0098: N815NW, an Airbus A330 operated by Delta Airlines, was conducting flight DAL 162 from Minneapolis-Saint Paul Intl (KMSP), US to Amsterdam Airport Schiphol (EHAM), Netherlands. During the cruise portion of the flight, a flight attendant noticed smoke and sparks from a screw head on the aft galley heated floor and discharged a fire extinguisher. The flight crew were notifiedand they consulted the QRH and pulled the associated circuit breakers. After coordination with dispatch and maintenance, the crew declared a MAYDAY and diverted to Keflavik Intl (BIKF), Iceland. The flight landed without further incident. Maintenance found discoloration in portions of the galley floor replaced the floor panel. The aircraft was returned to service with the floor heat unserviceable in accordance with the Minimum Equipment List (MEL).

At 0529Z, a westbound United States Department of Defence U.S. Transportation Command Boeing 767-33A (N351AX/CMB525) on a flight from Frankfurt-Hahn, Germany (EDFH) to Hurlburt Field, FL (KHRT) in the vicinity of 58N035W requested to divert to Shannon Intl, Ireland (EINN) due to a mechanical issue. Flight was re-cleared as requested. Exited the Gander (CZQX) oceanic control area (OCA) at 0609Z.

At 0810Z and position 5934N 03526W, the pilot of an American Airlines Boeing 787-8 (N811AB/AAL42) from Chicago/OHare, IL (KORD) to Venice/Marco Polo, Italy (LIPZ) declared a medical emergency and requested to divert to Keflavik, Iceland (BIKF). Flight was re-cleared as requested. Exited the Gander (CZQX) oceanic control area (OCA) at 0830Z. No traffic.

At 1145Z and 150NM North of Goose Bay, NL (CYYR), a Nordwind Airlines Boeing 777-300 (VPBJO/NWS353) from Moscow/Sheremetyevo, Russia (UUEE) to Varadero, Cuba (MUVR) declared a medical emergency and requested to divert to CYYR. Flight was re-cleared as requested. Aircraft circled CYYR for approximately 1 hour while burning fuel, then requested to continue to its destination. Flight was again re-cleared as requested. Exited the Gander, NL (CZQX) Flight Information Region (FIR) at 1340Z.

At 0033Z, and position 5054N05408W a Federal Express Corporation McDonnell Douglas MD 11F (N616FE/FDX5277) from Paris, France (LFPG) to Memphis, TN (KMEM) advised engine #2 had failed and requested to descend to F290. The flight was re cleared F290 and the crew advised they would be continuing to destination. The flight was then re cleared F280 for direction of flight. No emergency. No traffic. The Moncton Control Centre (CZQM) advised. The aircraft exited the Gander, NL (CZQX) flight information region (FIR) at 0057Z.
Update TSB #A19A0046: N628FE, a McDonnell Douglas MD-11F aircraft operated by Federal Express, was conducting flight FDX5277 from Paris/Charles de Gaulle (LFPG), France to Memphis Intl (KMEM), TN with 4 persons on board. During cruise flight approximately 100 nm north of Gander Intl (CYQX), NL, the flight crew received a number 2 engine Oil Filter Bypass indication. Approximately 30 minutes later, the number 1 autopilot and auto throttle systems disengaged due to the thrust roll back, and the aircraft entered a climb. The flight crew reengaged the auto flight systems, corrected the altitude, then observed an engine number 2 RPM Low indication; additionally, the oil pressure was below the red line, and indicating zero. The flight crew shut the engine down as per the QRH, and diverted the aircraft to Boston/General Edward Lawrence Logan Intl (KBOS), MA. Once at the top of the descent, the flight crew declared an emergency, and the aircraft landed without further incident. The operator’s maintenance removed the engine for investigation.

At 1236Z, at the position 58N030W, an Air Canada Boeing 787-9 (C-FGHZ/ACA869) from Heathrow, UK (EGLL) to Toronto/Pearson Intl, ON (CYYZ) declared a medical emergency and requested to deviate to Keflavik Intl, Iceland (BIKF). IFR clearance was not available due to traffic. The aircraft followed contingency procedures to FL300. IFR clearance direct BIKF was issued at FL300. The aircraft exited the Gander (CZQX) oceanic control area (OCA) at 1309Z.

A Delta Airlines Boeing 767 300 (N183DN/DAL14) from Atlanta, GA (KATL) to Frankfurt, Germany (EDDF) declared an emergency with Moncton (CZQM) High and requested to divert to Bangor, ME (KBGR) due pressurization issue. Flight was changed to Gander (CZQX) Low for descent and re clearance. Flight was recleared to KBGR but then requested to change destination to Stephenville, NL (CYJT). Flight was recleared to CYJT and landed without incident at 0253Z. No traffic.

A foreign registered Cirrus SR22 (A6CTR) and a foreign registered Cirrus SR22 (A6CTP) on flights from Narsarsuaq, Greenland (BGBW) to Reykjavík, Iceland (BIRK) entered controlled airspace at the same level, and not their cleared levels. Separation was re-established and the flights exited the Gander, NL (CZQX) oceanic control area (OCA) without further incident. Flights relayed that the altitude deviation was due to weather. Both flights were advised that a report would be filed.

At approximately 0245Z, a United Airlines Inc. Boeing 767 300 (UAL931) from Chicago OHare Intl Airport, IL (KORD) to Heathrow Airport, London (EGLL) declared an emergency with Moncton, NB (CZQM) with high priority and requested to divert to Goose Bay Airport, NL (CYYR) due to a cracked windscreen. The flight was recleared to CYYR and changed to Gander, NL (CZQX) with low priority. It landed without incident at 0322Z. No traffic.
Update TSB Report #A18A0081: N660UA, a Boeing 767-300 aircraft operated by United Airlines, was conducting flight UAL931 from Chicago/OHare Intl (KORD), IL to London/Heathrow (EGLL), UK with 11 crew members and 178 passengers on board. Approximately 100 nm southwest of Goose Bay (CYYR), NL, the flight crew declared an emergency and requested a diversion to CYYR due to a cracked windscreen. The aircraft landed without further incident at CYYR.
UPDATE: Aviation Incident Report #15761: Cracked windshield/pressurization issue for flight UAL931 led to emergency descent. Aircraft landed at 0322Z. Emergency secured at 0342Z. Passengers remained on aircraft and transferred to rescue aircraft for flight to destination.

Royal Dutch Airlines Boeing 777-200 (PHBQO/KLM17K) from Amsterdam Airport Schiphol, Netherlands (EHAM) to Washington-Dulles International, VA (KIAD) declared a medical emergency and requested to divert to Keflavík International, Iceland (BIKF). The aircraft was descended and cleared as requested. No traffic. The flight exited the Gander oceanic control area (CZQX OCA) without further incident.

At 1715Z, it was reported that an unidentified float plane was within the restricted airspace outlined in Gander, NL (CZQX) notice to airmen (NOTAM) 180166 during the activation time on August 29 from 1400Z to 1859Z.

At 0156Z, an American Airlines Inc. Airbus A330-300 (N272AY/AAL720) from Charlotte International, NC (KCLT) to Rome/Fiumicino International, Italy (LIRF) requested to turn back to KPHL due to a minor system issue. The flight was cleared as requested and exited the Gander, NL (CZQX) airspace at 0235Z. No emergency was declared.

A Lockheed P-3 (Aurora/Orion) (160586/VVYB586) from Portsmouth, NH (KPSM) to Rota, Spain (LERT) reported smoke in the cockpit and declared an emergency. The flight requested and was cleared to St. John's, NL (CYYT). At 2225Z, the flight requested to change its destination to Halifax/Stanfield Int'l, NS (CYHZ) and then shortly after to KPSM, estimated time of arrival (ETA) 0130Z. The flight was cleared as requested and exited Gander Area Control Centre (CZQX) airspace at 2335Z.

At 0354Z and position 4530N03100W, an American Airlines Boeing 767-323 (N397AN/ AAL206) from Miami, FL (KMIA) to Milan, Italy (LIMC) requested to divert to Lajes Field, Portugal (LPLA) due several electrical problems. The flight was recleared to LPLA and exited Gander Oceanic Airspace (CZQX OCA) at 0407Z. Landed without incident at LPLA at 0500Z. No traffic.

At 0250Z, a Mexican commercially registered Gulfstream GV (XAAHM) from Newark, NJ (KEWR) to London, UK (EGSS) requested to return to KEWR due to a minor pressurization issue. The flight was cleared as requested. No emergency declared or further assistance required. Exited Gander (CZQX) airspace at 0322Z.

After being transferred from Gander (CZQX), an Air Century Aerospatiale ATR 72-202 (HI1000) from Goose Bay, NL (CYYR) to Bangor, ME (KBGR) entered Montreal CZUL airspace without establishing radio contact with the air traffic controller (ATC). The aircraft followed the route and maintained the altitude outlined in the flight plan. There was coordination with the centre at Moncton (CZQM). No impact on operations.

An Ethiopian Airlines Boeing 777 200 (ETH503) from Toronto/ Lester B. Pearson, ON (CYYZ) to Addis Ababa, Ethiopia (HAAB) was erroneously cleared for ocean clearance and was handed off to Gander Area Control Centre (CZQX).

At 0802Z, an Air France Airbus A380-800 (FHPJD/AFR179) on a flight from Mexico City, Mexico (MMMX) to Paris, France (LFPG) requested lower via controller-pilot data link communications (CPDLC) but air traffic control (ATC) was unable to give IFR clearance. The flight then requested lower via satellite communications (SATCOM) and was requested to make descent request through high frequency (HF) or CPDLC. The flight then reported PAN PAN via CPDLC due to severe turbulence and requested lower. A descent clearance was issued to the flight at 0808Z. The aircraft exited Gander, NL (CZQX) oceanic control area (OCA) at 0940Z.

At 1100Z, 60NM SW of Stephenville, NL (CYJT), a United States Air Force McDonnell Douglas F-15 Eagle (TABOR55) on a flight from Goldboro, NC (KGSB) to RAF Lakenheath, UK (EGUL) declared an emergency due to a hydraulic issue and departed military reservation Coronet East 003, accompanied by TABOR52. The flight exited Gander, NL (CZQX) airspace at 1110Z. The flight landed at Greenwood, NS (CYZX) at 1203Z.

A Porter Airlines Inc. de Havilland DHC-8-402 (C-GKQC/POE490) from St. John's, NL (CYYT) to Halifax, NS (CYHZ) reported to Gander Area Control Centre (CZQX) that they had a medical emergency on board and requested to return to CYYT. C-GKQC then reported that the patient was 'stable' and crew elected to continue to CYHZ. C-GKQC requested an ambulance be standing by and CYHZ Tower was advised. Aircraft arrived safely at 2212Z.

At 1730Z, in the vicinity of IRBIM, a United Airlines Boeing 777 200 (N77006/ UAL86) from Newark, NJ (KEWR) to Shanghai, China (ZSPD) advised of a passenger with a medical emergency and requested to return to KEWR. The flight was cleared from FL310 to FL300 and then advised they would need to dump fuel. The fuel dump commenced at 1802Z at 5448N6340W and ended at 1825Z at 5212N6554W. The flight exited Gander Area Control Centre (CZQX) airspace at 1828Z. No other traffic affected.

At 0316Z, approximately 80NM north Goose Bay, NL (CYYR), a United Airlines Boeing 767 323 (N659UA/ UAL972) from Chicago, IL (KORD) to Brussels, Belgium (EBBR) advised one of their on board systems was down which would prevent them from landing in Europe. The flight requested and was cleared to return to KORD. No other traffic affected. UAL972 exited Gander Area Control Centre (CZQX) airspace at 0359Z.

At 2120Z, in the vicinity of SUTKO, an Air India Boeing 777 300 series (VTALR/AIC102) from New York, NY (KJFK) to Delhi, India (VIDP) requested to hold due to a medical emergency. Shortly thereafter, the flight requested to return to KJFK. AIC102 exited Gander Area Control Centre (CZQX) airspace into Moncton Area Control Centre (CZQM) airspace at 2129Z. No other traffic was affected.

UPDATE: TSB#A16A0011: Two aircraft transiting on the North Atlantic Track (NAT) system under reduced lateral separation minima (RLatSM) had a controller-pilot data link connections (CPDLC) connection loss with Gander ATC. This caused a conflict alert with ATC as several aircraft were 30 nautical miles in separation and now no longer met the RLatSM requirements. The data link issue was not resolved but the two aircraft that lost their data link were cleared descent or climb respectively to resolve the conflicts.
At 0300Z a Delta Airlines, Inc Airbus A330-300 (N811NW/DAL72) on reduced lateral separation minima (RLatSM) Track W, lost it's controller-pilot data link communications (CPDLC) connection with Gander Area Control Centre (CZQX) and was showing a conflict alert with a United Airlines, Inc. Boeing 777-222 (N778UA/UAL999). After several unsuccessful attempts by DAL72 to re-log on to CPDLC, UAL999 was descended to FL350, subsequently DAL72 was descended to FL360 to resolve the conflict. At 0311Z an United Airlines, Inc Boeing 787-8 (N38955/UAL20), on RLatSM Track U, also lost it's CPDLC connection with CZQX, showing them in conflict with a Virgin Atlantic Airways Limited Airbus A330-300 (G-VGEM/VIR112) and another Virgin Atlantic Airways Limited Airbus A330-300 (G-VWAG/VIR104L). UAL20 was climbed to FL400 to resolve these conflicts.

At 1152Z Gander, NL (CYQX) international flight service station (IFSS) received a MUSVA position report from a Viking Air DHC-6 series 400 (C-GVOT) from Iqaluit, NU (CYFB) to Keflavík, Iceland (BIKF) indicating FL150. The flight was queried about flight level and was advised that Gander Area Control Centre (CZQX) showed the cleared level to be FL130 which was confirmed by Montreal Area Control Centre (CZUL). At 1204Z, the flight was cleared to FL150 and at 1206Z advised they were in ice conditions and had made a request through Arctic Radio for higher to avoid icing. No other traffic.

When queried by the controller the pilot responded they were headed to IKEXE. The Controller asked if Moncton, NB (CZQM) or Gander, NL (CZQX) had cleared them to that point; the pilot responded no and that they were going to the airport. The Controller recleared N711NK direct IKEXE. No loss of separation. No aircraft in the vicinity.

At 0150Z, upon entering Gander, NL (CZQX) airspace, a British Airways Boeing 747 400 (GCIVK/ BAW32N) from Miami, FL (KMIA) to London, UK (EGLL) requested with international flight service station (IFSS) to confirm their oceanic clearance. Flight was issued clearance via SATPHONE due to poor High Frequency (HF) conditions at 0158Z.

At 0140Z, an American Airlines Boeing 767 323 (N381AN/ AAL64) from New York, NY (KJFK) to Zurich, Switzerland (LSZH) advised their airspeed indicator was inoperative and requested to return to KJFK. Flight was cleared as requested, and departed Gander, NL (CZQX) airspace at 0205Z. There was no traffic.

The Deer Lake, NL (CYDF) remote communication outlet (RCO) 122.3, Halifax, NS (CYHZ) RCO 123.275, and the Gander area control centre (CZQX) 133.0 were unserviceable at 1130Z.

A Cayman Island registered Airbus A319 from Newark, NJ (KEWR) to London/ Stansted (EGSS) was 100 nm east of Sydney, NS (CYQY) being handed off to Gander, NL (YQX) when Moncton, NB controller realized aircraft was not on the Canadian Automated Air Traffic System (CAATS) flight plan routing. The aircraft was 100 nm south of the flight plan routing. Gander, NL (CZQX) was informed and the investigation determined that more than one flight plan had been filed by company. The aircraft was flying the initial routing YQY DCT JOOPY rather than the revised YQY J580 YJT DCT ALLRY.

Airpsace moving reservation (German Air Force Deployment) A German Air Foce Airbus 310 (GAF598) and two Dassault Mirage aircraft (FAF9171 and FAF9172) were handed off at PEPRA from Moncton (CZQM) to Gander (CZQX). Moncton advised that both mirages had declared emergency and the entire cell would be diverting to St. John's, NL (CYYT). GAF598 checked in and advised that both mirages were experiencing icing problems, which was causing control issues and they had lost all instruments. They were following the A310 visually. Because of a low overcast layer at CYYT they diverted to Gander, NL (CYQX). FAF9172 on at 1655Z, FAF9171 on at 1656Z and GAF598 on at 1710Z.

A Delta Airlines Boeing 767-300 (DAL210) from New York, NY (KJFK) to Prague, Czech Republic (LKPR) advised Gander ACC, NL (CZQX) of a hydraulic issue and returning to KJFK. No assistance requested. No impact on operations.
