Aviation Incident Report: A Moncton Flight College Piper PA-44-180 (C-FZKG) contacted universal communications (UNICOM) to advise they were doing a touch and go on Runway 28. A privately registered Cessna 120 was on the Apron preparing to depart and spoke to inbound traffic to advise them of their intentions to depart on Runway 28. The Cessna 120 then departed Runway 10.

A privately registered Mooney M20J (HH297) from Bathurst, NB (CZBF) to Montreal/St-Hubert, QC (CYHU) was on visual approach for Runway 06L and was instructed to maintain 2 500ft until final and turn on base past Pont Champlain. The aircraft was cleared to descend at its discretion. HH297 was seen intercepting the base approximately 1.5NM from the threshold. HH297 was instructed to join the downwind leg to avoid a Cessna 150 that was 2NM on final for Runway 06R.

TSB Report #A19Q0164: a Jazz Aviation LP Bombardier DHC-8-300 (C-GETA) was on regular instrument flight rules (IFR) flight JZA8745 from Bathurst, NB (CZBF) to Montreal/Pierre Elliott Trudeau, QC (CYUL) with 48 passengers and 3 crew members on board. While it was in descent approximately 60 NM east-northeast of CYUL, the aircraft encountered heavy turbulence and the cabin crew member received minor injuries. The crew declared an emergency. The inspection revealed minor damage in the cabin ceiling.

Pilatus PC-12/47 (KUK848) from Bathurst, NB (CZBF) to Quebec/Jean Lesage, QC (CYQB) reported that it had seen a skunk on the runway while it was landing. Firefighters inspected the runway and picked up the carcass of the said skunk.

Incident at 22NM NE OMBRE intersection (Aerodrome, runway or taxiway shutdown)

A Jazz de Havilland DHC-8-301 (C-GTAT/JZA8740) from Montreal/Pierre Elliott Trudeau, QC (CYUL) to Bathurst, NB (CZBF) declared an emergency due to smoke in the cabin. Aircraft Rescue and Fire Fighting (ARFF) services were deployed. The aircraft landed without incident on Runway 24R at 1824Z. Inspection completed at 1825Z. No impact on operations.
Update: TSB Report #A19Q0155: a Jazz Aviation DHC-8-300 (C-GTAT/JZA8740) was on climb from Montreal/Pierre Elliott Trudeau, QC (CYUL) to Bathurst, NB (CZBF) when the crew noticed that the electrical circuit in the cockpit had overheated. An emergency was declared. The pilots used their oxygen masks. The aircraft returned to CYUL where it landed without incident. There was no smoke or odour in the cabin.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. Captains ID802 (Advisory Display) started to blink and shortly after, a burning smell was noticed by the crew. There was no smoke. The aircraft returned to YUL, where maintenance replaced the LH ID802 display and function tested it before returning the aircraft to service.

Things were back to normal at 1200Z.

RAM208 was instructed to conduct a go-around.

Aviation Incident Report #0559: A Moncton Flight College Diamond DA 20-C1 (C-FXVZ) wanted to conduct a touch and go at the Bathurst, NB (CZBF) aerodrome. An Air Traffic Control (ATC) official asked the pilot if they had checked their NOTAMs before leaving, and they said yes. The ATC official notified them of the applicable active NOTAM requiring pilots to give 30 minutes of prior notice. The pilot said they must have misread the NOTAM. The aircraft was then allowed to land at CZBF.

The Bathurst remote communications outlet (RCO) frequency cut out during the clearance request of a Jazz de Havilland DHC-8-300 (JZA8745) from Bathurst, NB (CZBF) to Montreal/Pierre Elliott Trudeau, QC (CYUL). A NOTAM was issued until January 16, 2019 at 2359Z. The frequency worked without any problems for a Jazz de Havilland DHC-8-300 (JZA8739) from Bathurst, NB (CZBF) to Montreal/Pierre Elliott Trudeau, QC (CYUL) at 0950Z. The technical operations coordinator (TOC) was notified. The NOTAM was cancelled at the TOCs request.

At the time of the reporting, the separation between JZA8739 and WEN3449 was 5.5 NM and 1 300 ft. Impact: 8JZA8739 was vectored out of WEN3449's wake.

Despite having been assigned and reading back 160 kts to waypoint BIRPO, a de Havilland DHC-8-102 (JZA8745) flying from Bathurst (CZBF), NB, to Montreal/Pierre Elliott Trudeau Int'l (CYUL), QC, was observed at a ground speed of 90 kts at 7 NM on final 06L, leading to a loss of separation with an Airbus A330-300 operated by Air Canada (ACA416) flying from Toronto/Lester B. Pearson Int'l (CYYZ), ON, to Montreal/Pierre Elliott Trudeau Int'l (CYUL), QC, on final 06R. Terminal requested that the tower pull up JZA8745 to vector it for a second approach. JZA8745 landed on 06L at 2200Z.
UPDATE ? TSB #A18Q0054: JZA8745, a Bombardier DHC-8-300 operated by Jazz Aviation LP, was IFR from Bathurst, NB (CZBF), to Montreal/Pierre Elliott Trudeau Intl, QC (CYUL). While on final approach with a speed restriction of 160 kts toward waypoint BIRPO, the aircraft was observed at a speed of 90 kts. This deviation resulted in a loss of separation with ACA416, an Airbus 330-300 aircraft operated by Air Canada that was IFR from Toronto/Lester B. Pearson Intl, ON (CYYZ), to CYUL. Separation was reduced to 2.5 nm horizontally, whereas the required separation is 3 NM. JZA8745 was instructed to pull up and was then vectored for a second approach.

Incident at 10NM BATHURST NB (CZBF)

Aviation Incident Report #15385: At approximately 10NM South of Bathurst, NB, the left side of the pilot door of a Forest Projection Limited Air Tractor AT-802 (C-GJJX) from Bathurst, NB (CZBF) to Miramichi, NB (CYCH) was struck by a bird. The bird hit the hinge rod on the emergency release handle, popping the front rod out and the window flew away. The flight continued as normal to CYCH. On arrival, bird remains were noticed on the leading edge of the vertical stabilizer. No damage noted to the aircraft.

The aircraft was struck three times and each strike lasted 5 to 10 s. The pilots think that the laser came from Beloeil. The laser did not affect the pilots.

A Jazz de Havilland DHC-8-102 (C-FJMG/JZA8740) from Montreal, QC (CYUL) to Bathurst, NB (CZBF) declared a PAN PAN due to an engine problem. One engine was shut down. The crew requested to return to CYUL but required some time to go through checklist. Aircraft Rescue and Fire Fighting (ARFF) was deployed. The aircraft landed Runway 24L at 1932Z and taxied to the gate under its own power. No impact on operations. National Operations Centre (NOC), Aviation Operations Centre, Joint Rescue Coordination Centre (JRCC) Trenton, and the Manager, Area Control Centre Operations (MACCO) were advised.
TSB Update A16Q0167: A Jazz Aviation de Havilland DHC-8-100 (C-FJMG/ JZA8740) was on an IFR flight from Montréal/Pierre Elliott Trudeau Intl (CYUL), QC, to Bathurst (CZBF), NB, with 36 passengers and 3 crew members on board. During the initial climb after takeoff, a left engine defect (Pratt & Whitney Canada PW120A) occurred and the crew decided to stop the engine. The crew then declared an emergency (PAN PAN) and conducted the checklist. Aircraft rescue and firefighting (ARFF) services were deployed and the aircraft landed on Runway 24L without incident. No one was injured and there was no damage to the aircraft. After analysis of the over-torque conducted by Pratt & Whitney Canada, the engine and the left propeller (Hamilton Standard 14SF-11) were changed.

At the same time, a Jazz de Havilland DHC-8-311 (JZA8740), from Montréal (CYUL), QC, to Bathurst (CZBF), NB, was authorized to take off from Runway 06R and a privately registered Beech V35B, on a local flight from CYUL, was about 1 NM on final for Runway 06R. JZA8740 was instructed to abort takeoff and Beech V35B was instructed to pull up.

A Jazz de Havilland DHC-8-102 (JZA8742), from Montréal (CYUL), QC, to Bathurst (CZBF), NB, indicated that it had been targeted by a green laser on takeoff from CYUL and on the south shore of Montréal between the Champlain and Mercier bridges. Police were notified.

A Jazz de Havilland DHC-8-102 (C-GONY/JZA8740), from Montréal/Pierre Elliott Trudeau (CYUL), QC, to Bathurst (CZBF), NB, reported to the tower that there was possibly a dead bird on Runway 24L. The airport authority was notified. A dead animal was found during inspection. Impact: A Jazz de Havilland DHC-8-101 (JZA8459), from Montréal /Pierre Elliott Trudeau (CYUL), QC, to Boston (KBOS), MA, had its takeoff delayed by 15 minutes.

Incident at 40NM W BATHURST NB (CZBF)

A Jazz de Havilland DHC-8-311 (C-FJVV/JZA8745) from Bathurst, NB (CZBF) to Montreal, QC (CYUL) reported a pressurization problem but no emergency was declared. They were handed off to Boston Center. After the hand off, JZA8745 descended to 11000' followed by a climb to filed altitude. The Boston Controller advised that everything was back to normal with the aircraft. JZA8745 left Moncton, NB airspace at 2300Z.
