Incident at LANGLEY REGIONAL BC (CYNJ)

A privately registered Mooney M20J from Langley, BC (CYNJ) to Vancouver/Boundary Bay, BC (CZBB) departed Runway 25 on the Cloverdale departure. There was no transponder on departure. When queried about altitude, the pilot responded that they were 1300ft. The pilot said they were not showing interrogation on their transponder (TS). No operational impact.

Incident at PITT MEADOWS BC (CYPK)

After landing on Runway 26L, a Pacific Flying Club Cessna 172P (C-GBLP) from Vancouver/Boundary Bay, BC (CZBB) to Pitt Meadows, BC (CYPK) was instructed to exit on Taxiway C after passing Taxiway B. Instead, the aircraft stopped and backtracked to exit on Taxiway B. As a result, a Pacific Rim Aviation Academy Diamond DA 20-C1 (C-GGUK) from Pitt Meadows, BC (CYPK) to Pitt Meadows, BC (CYPK) on final was instructed to pull up and go around. Additionally, C-GBLP attempted to taxi to Apron 1 without contacting Ground before being stopped by air traffic control (ATC).

Incident at PITT MEADOWS BC (CYPK)

A BPL Aviation Ltd. de Havilland DHC-2 MK. I (C-GBPL) from Vancouver/Boundary Bay, BC (CZBB) to Vancouver/Boundary Bay, BC (CZBB) entered the Pitt Meadows, BC (CYPK) control zone from the south at 1200ft above sea level (ASL) without calling the tower. C-GBPL then called the tower 1 mile inside the zone for a Northbound transit, and had to be instructed twice to fly an indirect routing and climb to avoid conflict with Runway 26R traffic.

Incident at 12NM W VANCOUVER INTL BC (CYVR)

A privately registered Beech C24R from Qualicum Beach, BC (CAT4) to Delta, BC (CAK3) was observed on radar entering the Vancouver, BC (CYVR) control zone 12 miles West of CYVR, at 2300 feet above sea level (ASL), heading East. The CYVR tower attempted to contact the aircraft multiple times, to no avail. At Coal Pile, at 2143Z, the aircraft contacted the tower and requested clearance through the control zone. The aircraft was instructed to contact the Vancouver/Boundary Bay, BC (CZBB) tower. No impact to operations.

Incident at LANGLEY REGIONAL BC (CYNJ)

A Pacific Flying Club Cessna 172P (C-GOSB) from Vancouver/Boundary Bay, BC (CZBB) to Vancouver/Boundary Bay, BC (CZBB) was overflying the Langley, BC (CYNJ) control zone. The aircraft was passed traffic when it was approximately 1 mile southwest of CYNJ. C-GOSB had traffic in sight, then proceeded to descend into the control zone without clearance as they proceeded southwest, and were not reachable on the radio. No impact on operations.

Incident at In the vicinity of: Saltair, BC

Aviation Incident Report #16675: During a cross-country flight at night, a Pacific Flying Club Cessna 152 (C-GINK) on a round-robin flight from Vancouver/Boundary Bay, BC (CZBB) to Vancouver/Boundary Bay, BC (CZBB), including en route stops at Campbell River, BC (CYBL), Victoria, BC (CYYJ), and Abbotsford, BC (CYXX), was subject to a laser strike over Saltair, BC (North of Chemainus, BC). At the time of the occurrence, the pilot was en route to CYYJ at 5500 feet when they noticed the underside of their wings suddenly reflecting a flashing green light. The pilot turned a few degrees left and right to ensure the light did not come from conflicting traffic or potentially aviation-related pyrotechnic signals. They noticed that the green laser beam was originating from a residential area in Saltair, BC. The occurrence lasted for approximately 30 seconds, during which the laser emitted a constant green beam that was moved around rapidly while aiming at the aircraft. The movement resembled the way a person would hold a handheld laser while pointing it at an object in the distance. The pilots vision was not notably impaired and they tried not to look directly at the light. Immediately after the strike, they reported the occurrence to the CYYJ terminal on 120.8 MHz. After making a report to the air traffic controller (ATC), a helicopter in the area also reported a laser strike around Chemainus. Further, the pilot of another aircraft from the same flight school was flying through the area just a few minutes behind and advised they saw the laser as well. The laser strike occurred about 1/2NM to 1NM East of a lighted stretch of Highway 1. The pilot believes the laser beam originated somewhere within 1/4NM of 48°57’16.40”N, 123°45’58.08”W. The pilot experienced the same event in the same location on June 26, 2020 at 0658Z - see CADORS 2020P1164.

After landing and before exiting Runway 25, a Pacific Flying Club Cessna 172R (C-FPAK) from Vancouver/Boundary Bay, BC (CZBB) to Vancouver/Boundary Bay, BC (CZBB) veered off and stopped in the grass area along Runway 25. There were no injuries, and Runway 25 was closed until the aircraft was removed.

Incident at VANCOUVER / BOUNDARY BAY BC (CZBB)

At 2315Z, the Kamloops flight information centre (FIC) received a flight plan, filed through the Internet, for a Cirrus Aviation Services Cirrus SR22 (C-GEHK) from Victoria, BC (CYYJ) to Vancouver/Boundary Bay, BC (CZBB). The departure time was 2115Z. The Flight Service Station Information Management System (FIMS) interpreted the flight plan for the next day. Alerting position called the pilots cell phone to confirm the time was correct and left a message. At 0100Z, the pilot called back to discuss the error, noting that they were on the ground at CZBB and had already completed the flight without alerting, and asked the flight plan be cancelled.

Incident at PITT MEADOWS BC (CYPK)

After landing on Runway 08L, a Pacific Flying Club Cessna 172R (C-FPAK) from Vancouver/Boundary Bay, BC (CZBB) to Vancouver/Boundary Bay, BC (CZBB) was instructed to taxi via Taxiway D and contact the tower while holding short of Runway 08R for a southbound departure. C-FPAK was observed crossing the hold short line at taxi speed and was instructed to stop. No impact on operations.

At Vancouver/Boundary Bay, BC (CZBB), a strong emergency locator transmitter (ELT) signal was heard for two minutes. Afterwards, the signal was heard intermittently. An aircraft in the circuit also reported hearing the signal. The Joint Rescue Coordination Centre (JRCC) Victoria and the area control centre (ACC) shift manager (SM) were notified. The pilot of an International Flight Centre Inc. Cessna 172M (C-FQSW) from Vancouver/Boundary Bay, BC (CZBB) to Vancouver/Boundary Bay, BC (CZBB) later called to advise of an accidental ELT activation.

At 0436Z, an arrival (ARR) message was received from the CZBB tower, but the flight plan had not been opened, resulting in a loss of alerting.

While departing Runway 19, a Pacific Professional Flight Centre Ltd. Cessna 172M (C-GRXV) from Langley, BC (CYNJ) to Vancouver/Boundary Bay, BC (CZBB) struck and killed a Canada Goose as a flock departed from Pad C, Westbound. When queried, the pilot was unaware of the impact and chose to fly to CZBB, as both runways were occupied with birds. The pilot reported the aircraft did not seem affected. No operational impact.

The gear of a privately registered Aero Commander 112TCA from Vancouver/Boundary Bay, BC (CZBB) to Vancouver/Boundary Bay, BC (CZBB) collapsed while landing on Runway 13. No injuries reported. The National Operations Centre (NOC) and the Transportation Safety Board of Canada (TSB) were advised.

Incident at ABBOTSFORD BC (CYXX)

A CZBB Aviation Cessna 172M (C-GEVV) from Vancouver/Boundary Bay, BC (CZBB) to Vancouver/Boundary Bay, BC (CZBB) flew into the Abbotsford, BC (CYXX) control zone twice without contacting the CYXX tower. The second incursion occurred at 0348Z. Both times were in the paradrop zone, however paradrops were not active at the time.

Incident at VANCOUVER INTL BC (CYVR)

A privately registered Cessna 172M from Kelowna (CYLW) to Vancouver/Boundary Bay, BC (CZBB) was observed on radar entering the Vancouver, BC (CYVR) Class C airspace at 6300ft without clearance. The aircraft made no contact while transiting the control zone, and landed at CZBB. Traffic had to be vectored around the aircraft to avoid conflict.

Incident at LANGLEY REGIONAL BC (CYNJ)

A Pacific Flying Club Cessna 172R (C-FPAK) from Vancouver/Boundary Bay, BC (CZBB) to Abbotsford, BC (CYXX) descended into the Langley, BC (CYNJ) control zone, conflicting with an overflying Pacific Flying Club Cessna 172P (C-GOSB). The tower was not able to make contact on 119.0.

At Vancouver/Boundary Bay, BC (CZBB), a vehicle without a beacon was reported maneuvering on Taxiway Echo by an aircraft in the run-up bay for Runway 25. The vehicle was observed by the tower controller crossing Runway 25 via Echo and Foxtrot for Apron 3. The vehicle was then observed leaving the airport property. No impact on operations.

A Pacific Flying Club Cessna 172P (C-GFVB) from Vancouver/Boundary Bay, BC (CZBB) to Penticton, BC (CYYF) received an airport advisory for a touch-and-go (T/G) at CYYF. The flight service station (FSS) identified that the aircraft was likely near Oliver, BC (CAU3) instead of CYYF. Assistance was provided and the aircraft subsequently arrived safely at CYYF for the T/G. No impact to operations.

A Pacific Flying Club Cessna 172P (C-GPTF), en route to Vancouver/Boundary Bay, BC (CZBB), declared PAN PAN PAN while 5500ft over Nanaimo Harbour, BC, due to an alternator failure. C-GPTF diverted to Nanaimo, BC (CYCD), approximately 7 miles away, and landed safely at 2100Z. The departure time of a Jazz de Havilland DHC-8-402 (JZA2265) was delayed by 5 minutes.

The pilot of a privately registered, amateur-built Mustang II from Vancouver/Boundary Bay, BC (CZBB) to Vancouver/Boundary Bay, BC (CZBB) was instructed to taxi via "E" and contact the tower on 119.0, holding short. The Mustang II crossed the hold short line onto Runway 01, where the pilot was then instructed to backtrack Runway 01 to line up. The aircraft proceeded to commence a backtrack for Runway 19. The tower informed the pilot that they were backtracking for the wrong runway, at which point they were instructed to turn around, eventually departing Runway 01. A Pacific Professional Flight Centre Cessna 172P (C-GHFH) in the circuit (CCT) for Runway 01 was extended to accommodate.

Aviation Incident Report #16695: A Pacific Professional Flight Centre Ltd. Cessna 172M (C-GWCG) from Vancouver/Boundary Bay, BC (CZBB) to Vancouver/Boundary Bay, BC (CZBB) experienced a laser attack while in-flight. The approximate location of the laser strike was the Northeast (NE) quadrant of Chilliwack city, about 1NM NE of the city centre, just after the aircraft passed Chilliwack mountain to the East and setting up for a mid downwind over Chilliwack, BC (CYCW) at 2500ft. At exactly 0700Z, the aircraft spotted a green laser activity attempting to aim at the aircraft. There were two attempts. Laser activity ended before the aircraft turned midfield. It did not cause any reduction in vision or disorientation.

The Cessna 172H was observed climbing through 1300ft and conflicting with a privately registered Champion 7ECA on a flight to Vancouver/Boundary Bay, BC (CZBB) that was flying in the opposite direction at 1500ft. After several attempts to establish communication, the pilot of the Cessna 172H was eventually instructed to descend back down to 1000ft. No operational impact.

Head-to-head conflict occurred with a Harts Aviation BC Zenair Zodiac CH 601 HD ultralight (C-IABP) from Langley, BC (CYNJ) to Langley, BC (CYNJ), that was on the left base. C-FJCD was given an immediate Northbound turn to avoid traffic.

Pilot of a Kisik Aerial Survey Inc. Piper PA-31-350 (C-GXHK) from Vancouver/Boundary Bay, BC (CZBB) to Vancouver/Boundary Bay, BC (CZBB) advised that an aborted takeoff occurred prior to the tower opening at CZBB at 181331Z on June 18th. The takeoff was aborted to avoid contact with 2 loitering bald eagles on Runway 13. The aircraft successfully avoided colliding with the birds, and backtracked on Runway 13 to accomplish a successful takeoff on the second attempt. No injuries or damages occurred. No operational impact.

Incident at ABBOTSFORD BC (CYXX)

The pilot of a Pacific Professional Flight Centre Cessna 152 (C-GBXJ) from Vancouver/Boundary Bay, BC (CZBB) to Abbotsford, BC (CYXX) entered the control zone without clearance, did not follow the route clearance, and was confused while taxiing (i.e. they appeared confused about right versus left). On their departure, the pilot started their take-off roll before the clearance was given, then did not turn when instructed.

A privately registered Cessna 172M from Langley, BC (CYNJ) to Vancouver/Boundary Bay, BC (CZBB) was cleared to depart Runway 19 via the noise avoidance procedure (left turn on departure) until South of the power lines, before proceeding on course. The aircraft turned right on departure, conflicting with the departure on Runway 25. The flight was informed of the course deviation and was instructed to turn away from the departure path of Runway 25. A Langley Flying School Cessna 172N (C-GBPT) was delayed on the ground until the Cessna 172M was confirmed clear of the departure path for Runway 25.
