Boeing 757-2B7 (MAL8090) from Calgary, AB (CYYC) to Toronto/Lester B. Pearson, ON (CYYZ) slow to vacate runway. No further impact to operations.

Incident at ST. JOHN'S INTL NL (CYYT)

Delayed entry as a result of NAV CANADA safety and evaluation quality assurance (QA) review: A Lockheed C-130 (Hercules) on a flight from St. Johns, NL (CYYT) was lined up on Runway 34 but was not ready to depart due to a maintenance issue. A VFR aircraft on a flight to St. Johns, NL (CYYT) that was on approach had to conduct a go-around, and was delayed by 2-3 minutes. An unrelated communication issue (stuck microphone) was happening concurrently for the VFR aircraft, and the inbound pilot communicated by cell phone.

Accident at ST. JOHN'S INTL NL (CYYT) (Aerodrome, runway or taxiway shutdown)

The pilot of a privately registered Cessna A185F from St. Johns, NL (CYYT) to St. Johns, NL (CYYT) advised that they could not get their landing gear down. Air traffic control (ATC) confirmed visually that only one side of the landing gear was retracted. In coordination with ATC, the aircraft performed a controlled crash landing. The aircraft landed safely at 2151Z. Impact on operations was the temporary closure of Runway 02, and a closure of Runway 11/29 until 03/09/20 at 1630Z.

An Exploits Valley Air Services Ltd. Beech 1900D (C-GUPW) from Goose Bay, NL (CYYR) to St. Johns, NL (CYYT) reported sighting some small birds, with a possible bird strike, while landing on Runway 11. The aircraft landed without incident. The airport wildlife patrol confirmed a single bird strike.

Incident at 157NM NNW HALIFAX / STANFIELD INTL NS (CYHZ)

The pilot of a Jazz Bombardier CL-600-2D24 (JZA8011) from St. Johns, NL (CYYT) to Toronto/Lester B. Pearson, ON (CYYZ) requested a diversion to Halifax/Stanfield, NS (CYHZ) due to a slow but steady loss of fuel on the left side of aircraft. No emergency was declared or assistance required. JZA8011 landed at 0013Z without incident. No impact on the operation.
Update: TSB Report #A20A0031: C-FKJZ, a Bombardier CL-600-2D24 (CRJ900) aircraft operated by Jazz Aviation LP, was conducting flight JZA8011 from St. Johns Intl (CYYT), NL, to Toronto/Pearson Intl (CYYZ), ON. While in cruise flight, the crew received an AUTO XFLOW INHIB message, followed by a XFLOW PUMP caution message and actioned the Emergency Checklist (QRH) to use gravity cross flow to correct a 300 pound left-right fuel imbalance. The crew continued to monitor the fuel quantity, and observed that the left side quantity was indicating as though it were decreasing in quantity relative to expected engine fuel burn. The crew discussed the issue with their maintenance control department, and at about 157NM North Northwest of Halifax/Stanfield Intl (CYHZ), NS, they decided to divert to CYHZ. The aircraft landed at CYHZ without further incident. Maintenance personnel found a fuel quantity gauging computer fault, and a faulty left hand primary ejector crossfeed fuel line. The faults were corrected, their relative systems tested and the aircraft was returned to service.

A staff vehicle reported a small fire on the side of Runway 34 at approximately 1520Z. The runway was closed for approximately 15 minutes while fire crews responded. The cause of the fire was due to a flare used by wildlife patrol. A MEDEVAC aircraft had to change departure runways due to the closure, but there was no delay or impact on their departure.

A Provincial Aerospace Beech 200 (C-GTJZ/SPR04) from St. Johns, NL (CYYT) to St. Johns, NL (CYYT) encountered an electrical smell in the cockpit shortly after take-off, declared an emergency, and requested clearance back to its point of departure. C-GTJZ was cleared as requested, and landed safely at 1339Z. No traffic. Impact to operations: The runway was closed for 5-10 minutes to vector the aircraft.

A Government Of Newfoundland And Labrador Beech B300C (C-GNLO) from St. Johns, NL (CYYT) to St. Anthony, NL (CYAY) was on approach into CYAY and advised the Deer Lake, NL (CYDF) flight service station (FSS) that aircraft radio control of aerodrome lighting (ARCAL) lighting was unserviceable. C-GNLO diverted to Goose Bay, NL (CYYR) after multiple attempts to turn on the lights. CYAY Airport Security and airport maintenance were advised. No traffic. No further impact on operations.

Incident at 62NM NNW ST. JOHN'S INTL NL (CYYT)

TSB Report #A20A0023: C-GEFA, an Airbus A330 aircraft operated by Air Canada, was conducting a flight from Montreal/Pierre Elliott Trudeau Intl (CYUL), QC, to Zurich Intl (LSHZ), Switzerland. The aircraft was approximately two hours into the flight and prior to ETOPS entry when the crew observed a Y system hydraulic pressure low warning. The crew consulted with company maintenance and then elected to return to CYUL. They contacted the Gander Area Control Center, declared a PAN PAN, and received clearance to return to CYUL. The aircraft landed uneventfully at CYUL. Company maintenance installed a new hydraulic accumulator and bled the Y system of air; the aircraft was returned to service.

A Jazz de Havilland DHC-8-402 (C-GKUK/JZA8991) from St. Johns Intl, NL (CYYT) to Halifax/Stanfield Intl, NS (CYHZ) observed smoke in the cockpit at 30NM West of CYYT. The aircraft returned to CYYT without incident.
Update TSB report #A20A0022: C-GKUK, a de Havilland Dash 8-400 operated by Jazz Aviation LP, departed St. John’s Intl, NL (CYYT) on a scheduled flight to Halifax/Stanfield Intl, NS (CYHZ). The aircraft was in the climb phase at 30 NM west of CYYT when the flight crew detected smoke in the cockpit, simultaneous to a flight attendant report of haze in the cabin area. A short time after, the crew noted caution messages FLT COMP HOT and #2 BLEED HOT followed by a LAV smoke detector caution. The flight crew donned their oxygen masks and declared an emergency with air traffic control (ATC) at approximately 0745 NDT. They then received clearance from ATC to return to CYYT. The aircraft landed without incident at 0801 NDT while ARFF were standing by. ARFF confirmed there was no evidence of smoke or fire and the aircraft then taxied to the gate. The aircraft is currently undergoing maintenance.

Incident at 44NM HALIFAX / STANFIELD INTL NS (CYHZ)

A Jazz de Havilland DHC-8-402 (C-GJZG/JZA2271) from Halifax/Stanfield, NS (CYHZ) to St. Johns, NL (CYYT) had a pressurization issue. C-GJZG returned to CYHZ. No emergency. No operational impact.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. During the climb, the flight crew of JZA2271 were unable to control the pressurization in "In Auto" or "Manual". This was followed by "#1 and #2 Bleed Hot" cautions. The flight returned to CYHZ. The Pack Control Valve was replaced and the aircraft was returned to service. There has been no recurrence.

TSB Report #A20A0011: C-FDSN, an Airbus A320 operated by Air Canada was conducting flight ACA680 from Toronto Lester B Pearson International Airport, ON (CYYZ) to St John’s International Airport, NL (CYYT). While in cruise at FL350, the number 2 engine rolled back to approximately 54% N1, while the number 1 engine remained at 92% N1. The crew descended to FL290 with auto thrust commanded to 85% and both engines responded. The flight continued to destination without further incident. Maintenance is investigating.

Incident at 150NM E ST. JOHN'S INTL NL (CYYT)

At 1320Z, a Cougar Helicopters Inc. Sikorsky S-92A (C-GKNR/CHI51) from St. Johns, NL (CYYT) declared an emergency with a "PAN PAN" due to failure of anti-icing for engine number 1 and requested to return to CYYT. The flight was outside controlled airspace and was provided a clearance when in controlled airspace as requested. No assistance was required and pilot was returning as a precautionary measure. Prior to entering controlled airspace, the flight descended to remain clear of cloud and icing conditions. The flight cancelled its IFR clearance and returned to CYYT VFR. The fight landed at 1427Z. No impact to operations.
UPDATE TSB Report #A20A0013: C-GKNR, a Sikorski S92 operated by Cougar Helicopters was on an IFR flight from St. John’s International, NL (CYYT) to the Hebron Oil Platform (CHBR) at 7000 feet ASL. The aircraft was 150 NM East of CYYT when the INLET #1-A-ICE FAIL illuminated along with the Master Caution light. The crew referred to the Emergency Check List (ECL) and reset the INLET #1 A-ICE. The INLET #1-A-ICE remained in a failed state. Due to the light icing experienced in cloud and a PIREP from a company aircraft inbound from the Hebron area the crew elected to return to CYYT. The crew contacted Gander ACC, declared a PAN-PAN, and requested clearance to CYYT at 2400 feet ASL. The aircraft remained in cloud and icing so the crew cancelled IFR and descended to 900 feet ASL where they were clear of cloud and icing. The aircraft conducted a VFR approach and landed safely in CYYT. Maintenance determined that the #1 Engine Inlet required replacement.

A moose was reported behind the St. Johns, NL (CYYT) tower with a WestJet Encore Ltd. de Havilland DHC-8-402 (C-GDEN/WEN3422) from Halifax/Stanfield, NS (CYHZ) to St. Johns, NL (CYYT) inbound for Runway 34. Wildlife advised. Moose observed passing behind de-ice facility, across the Central De-ice Facility (CDF), and continuing towards Runway 34 with traffic approximately 3 miles final. Tower advised inbound traffic of moose and initiated a pull up and go around. Aircraft executed a wide left-hand VFR circuit while wildlife officer handled the moose situation. With the moose remains 30 feet off the runway, traffic landed safely and ground crew removed carcass from field.

At 1414Z, a Government Of Canada, Department Of National Defence Lockheed C-130 (Hercules) (C2001) from St. Johns, NL (CYYT) to St. Johns, NL (CYYT) reported being unable to change frequencies with the Gander (YQX) Area Control Centre (ACC). The aircraft declared an emergency. Clearances relayed through ACC. The aircraft landed safely at 1419Z and terminated emergency. No impact to operations.
