Incident at ÎLES-DE-LA-MADELEINE QC (CYGR)

A Jazz de Havilland DHC-8-402 (C-GGND/JZA8734) from Fredericton, NB (CYFC) to Iles-de-la-Madeleine, QC (CYGR) declared PAN PAN and reported flaps stuck at 15 degrees. The pilot requested diversion to Halifax/Stanfield, NS (CYHZ). The aircraft landed at 1758Z, No impact on operations.
UPDATE AOR-266121-V1: The Moncton (ZQM) air traffic control (ATC) advised the Charlottetown, PE (CYYG) flight service station (FSS) that a Jazz de Havilland DHC-8-402 (C-GGND/JZA8734) on an IFR flight from Fredericton, NB (CYFC) to Îles-de-la-Madeleine, QC (CYGR) declared "Pan Pan" on approach into CYGR. JZA8734 was now diverting to CYYG and the pilot requested the CYYG Aircraft Rescue and Fire Fighting (ARFF) unit be alerted and put into position. The aircrafts flap operation was abnormal. At 1718Z ZQM ATC advised that JZA8734 was now diverting to Halifax/Stanfield, NS (CYHZ) vice CYYG. At 1721Z ZQM ATC confirmed that the CYYG ARFF unit could "stand down". Update: TSB Report #A20A0033: C-GGND, a de Havilland DHC-8-402 aircraft operated by Jazz Aviation LP, was conducting flight JZA8734 from Fredericton Intl (CYFC), NB, to Îles-de-la-Madeleine (CYGR), QC. On approach to CYGR, the crew received a FLAP POWER caution indication and the flaps did not extend beyond 17 degrees. The crew declared PAN PAN with air traffic services (ATS) and requested a diversion to Halifax/Stanfield, (CYHZ), NS. The aircraft landed at CYHZ without further incident.

At takeoff, a Jazz Bombardier CL-600-2B19 (C-FIJA/JZA8549) from Montreal/Trudeau, QC (CYUL) to Charlottetown, PE (CYYG) reported that it was going back to CYUL and landing. The nature of the issue was not indicated. No emergency was declared and no assistance was requested. The aircraft landed without incident at 0306Z. Impact: a GoJet Airlines Bombardier CL-600-2D24 (GJS6238) from Detroit Metro/Wayne County, MI (KDTW) to Montreal/Pierre Elliott Trudeau Intl, QC (CYUL), an Air Canada Airbus A320 (ACA426) from Toronto/Lester B. Pearson Intl, ON (CYYZ) to Montreal/Pierre Elliott Trudeau Intl, QC (CYUL), and another Air Canada flight (ACA4235) were delayed at landing, and a few flights were delayed at takeoff. No other impact on operations.

While on backtrack for departure from Runway 28, a To The Tabs Aviation Inc. Piper PA-28-161 (C-FDME) on a flight from Charlottetown, PE (CYYG) to Charlottetown, PE (CYYG) taxied off the Runway 28 threshold onto soft ground. The aircraft requested assistance, and the Canadian Airport Authority (CAA) responded. Runway 10/28 was closed via NOTAM for 59 minutes via voice advisory. Moncton area control centre (CZQM) was advised. The aircraft was moved back onto the runway surface and subsequently taxied back to Apron 2. Runway 28 was re-opened by the CAA. CZQM was advised.

UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. Technical records were reviewed for C-GWSI, with no bird strike related defects found recorded.

Incident at HALIFAX / STANFIELD INTL NS (CYHZ) (Aerodrome, runway or taxiway shutdown)

A Smartwings Boeing 737-8FN (OKTVM) operating as an Air Transat flight (TSC2651) from Holguin/Frank Pais, Cuba (MUHG) to Halifax/Stanfield, NS (CYHZ) conducted a missed approach (MA) for Runway 23 due to a flap issue. No emergency declared but Aircraft Rescue and Fire Fighting (ARFF) unit was requested as precaution. The aircraft landed at 2013Z. Runway 23 was closed for 15 minutes. 2 aircraft on approach for Runway 23 were required to circle.
UPDATE Duplicate AOR-262154-V1: TSC2651 pilot initiated overshoot 4NM final for Runway 23 due to flap issue. Landed safely at 2012Z. When given exit instructions, pilot asked to remain on the runway for a visual inspection by the ARFF vehicles. An Exploit Valley Air Services Ltd. Beech 1900D (EV753) from Charlottetown, PE (CYYG) to Halifax/Stanfield, NS (CYHZ) conducted a tower-initiated overshoot due to this, and landed on Runway 32 safely. One other departure, a Jazz aircraft (JZA812), was delayed slightly as well. A Moncton Flight College Piper PA-44-180 (C-FZKG) that was on a training flight discontinued its approach. (Duplicate AOR-262154-V1 deleted)
UPDATE TSB Report #A20A0009: OK-TVM, a Boeing 737-800 aircraft owned by Smartwings and contracted to Air Transat, was conducting flight TSC 2651 from Holguin/Frank País Intl. (MUHG), Cuba to Halifax/Robert L. Stanfield Intl.(CYHZ), NS with 162 passengers and 6 crew. On approach to CYHZ, the crew selected flaps 5 and they became lodged at flap 2. The flight crew overshot the approach and troubleshot the issue until deciding to land with flap 2, as they would not move. The crew requested ARFF to meet the aircraft when it stopped on the runway. The aircraft landed without further incident. Once the aircraft stopped on the runway the ARFF inspected the brakes and flaps, including taking temperatures. Once the inspection was complete, the crew taxied to the gate and the passengers were deplaned. Maintenance cleared the fault by resetting the flap computer and the aircraft was released to service.

During taxi, a WestJet Boeing 737-6CT (C-GWSB/WJA331) from Charlottetown, PE (CYYG) to Toronto/Lester B. Pearson, ON (CYYZ) reported an animal, possibly a coyote, on CYYG Taxiway A. The pilot of WJA331 advised they would hold short of the runway for an inspection by the Canadian Airport Authority (CAA). Animal was observed proceeding towards the south of Taxiway A. WJA331 departed with a 5 minute delay. Moncton/Gander Flight Information Region (CZQM) advised.

At takeoff from Runway 24R, the separation between an Air Canada Rouge Airbus A321-211 (C-GHPD/ROU1686) from Montreal/Pierre Elliott Trudeau, QC (CYUL) to Charlottetown, PE (CYYG) and an Envoy Air Embraer E135 (N812AE/ENY3903) from Montreal/Pierre Elliott Trudeau, QC (CYUL) to New York/La Guardia, NY (KLGA) was reduced to 2.5 NM, where it should normally be 3 NM. Impact: ENY3903 deviated to the south and north of the runway centreline due to wake turbulence produced by ROU1686.
Update: TSB Report #A19Q0142: An Air Canada Rouge Airbus A321 (C-GHQI) was conducting flight ROU1686 from Montreal/Pierre-Elliott Trudeau Intl (CYUL), QC, to Charlottetown (CYYG), PEI. During takeoff, separation between ROU1686 and Envoy Air Embraer EMB-135 (ENY3903) flying to New York/LaGuardia (KLGA), NY, was reduced to 2.5 NM, whereas the required separation was 3 NM. However, vertical separation was maintained. As a result, ENY3903 deviated north and south of the runway centreline because of the wake turbulence produced by ROU1686.

Approximately 63 nautical miles southeast of Quebec/Jean Lesage, QC (CYQB), the crew was informed of the smell of smoke coming from the back section of the cabin. No smoke was visible; however, the crew decided to divert to CYQB. No emergency was declared, and the aircraft landed without incident. No one was injured.
Aviation Incident Report: The Federal Aviation Administration (FAA) reported that a Sky Regional Airlines Embraer ERJ 170-200SU (C-FRQW/SKV7519) reported the smell of smoke while in the Boston Air Route Traffic Control Center (ZBW) airspace. The aircraft diverted to Quebec/Jean Lesage Intl, QC (CYQB). Source of smoke unknown.

Incident at MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL) (Aerodrome - runway or taxiway surface condition, Aerodrome, runway or taxiway shutdown)

Although it received and read back the instruction to clear Runway 06L via Taxiway B1 after landing, a Jazz Bombardier CL-600-2B19 (series 100) (C-GUJA/JZA8512) from Charlottetown, PE (CYYG) to Montreal/Pierre Elliott Trudeau, QC (CYUL) did so by exiting via Taxiway E; it took a high-speed exit. Impact on operations: JZA8501 was instructed to reject its approach on Runway 06L and was reassigned to Runway 06R for its landing. Runway 06L was closed from 1113Z to 1242Z. Due to poor weather conditions and to the state of the runways surface, Runway 06R was closed from 1236Z to 1243Z for snow removal. The following departures were delayed: TSC954, ROU1986, and ROU1842. The National Operations Centre (NOC) and the Transportation Safety Board of Canada (TSB) were notified.
Update - TSB Report #A19Q0001: C-GUJA, a Jazz Aviationi LP Bombardier CL-600-2B19 CRJ200, was on instrument flight rules (IFR) flight JZA8512 from Charlottetown, PE (CYYG) to Montreal/Pierre Elliott Trudeau, QC (CYUL) with 3 crew members and 49 passengers on board. At landing on Runway 06L at CYUL, air traffic control (ATC) instructed the flight crew to exit on Taxiway B1. Although the instruction was read back correctly, the pilot-in-command steered the aircraft towards Taxiway Echo, which was closed by NOTAM. When he noticed this error, the pilot-in-command applied the brakes with the intention of bringing the aircraft onto Runway 06L. However, a layer of ice covered the ground within the protected area on Taxiway Echo, which prevented the aircraft from braking effectively and turning. The aircraft ended its run on the lawn. Runway 06L was closed temporarily to allow for the towing. The aircraft did not sustain any damage on the landing gear, and no one was injured.

An Air Canada Rouge Boeing 767 300 (ROU1644) from Toronto/Lester. B Pearson, ON (CYYZ) to Miami, FL (KMIA) was vectored within close proximity of an Air Canada Rouge Airbus A321-211 (ROU1706) from Toronto/Lester. B Pearson, ON (CYYZ) to Charlottetown, PEI (CYYG) resulting in a loss of separation.
Update TSB Report #A19O0158: C-FYJH, an Airbus A319 aircraft operating as Air Canada Rouge flight ROU1640 was vectored within close proximity of Air Canada Rouge flight ROU1706, an Airbus A321 (C-GHQI) aircraft, resulting in loss of separation. Separation was reduced to 2.5nm where 3nm was required.
