Incident at PENTICTON BC (CYYF)

A privately registered Beech M35 from Osoyoos, BC (CBB9) to Penticton, BC (CYYF) received an for landing Runway 34. On its short final, the aircraft became "no radio" (NORDO). The aircraft landed and taxied safely. No operational impact.

Incident at OKANAGAN (PENTICTON) BC (ON) NDB

A WestJet Encore de Havilland DHC-8-402 (C-FOEN/WEN3281) from Calgary, AB (CYYC) to Penticton, BC (CYYF) was observed making a left turn after the Okanagan (Penticton), BC (ON) NDB without coordination with air traffic control (ATC). This turn left the aircraft at an altitude lower than the Minimum Radar Vectoring Altitude (MRVA). When questioned, WEN3281 indicated they were completing a company hold. Later, the crew confirmed they did not receive a hold clearance. No impact on operations.

Incident at PENTICTON BC (CYYF) (Other operational incident, Aerodrome, runway or taxiway shutdown)

Due to a forest fire Northwest of Penticton, BC (CYYF). The Canadian Aviation Regulation (CARs) 601.15 implementation restricted Penticton, BC (CYYF) operations, with no arrivals or departures. Several known departures were delayed or cancelled. 3 known arrivals were delayed or diverted. During the review, the restriction was reduced from 5 nautical miles to 2 nautical miles, and operations resumed at Penticton with less restrictions.

Incident at KAMLOOPS BC (CYKA)

A privately registered, amateur-built Searey from Kamloops, BC (CYKA) to Penticton, BC (CYYF) departed Runway 27 and did not respond to contact attempts for traffic information from a Central Mountain Air Ltd. de Havilland DHC-8-102 (C-GYSJ/GLR9370) from Prince George, BC (CYXS) to Kamloops, BC (CYKA), and did not transmit on the mandatory frequency (MF) after departure.

The pilot of a privately registered, amphibious, amateur-built Searey on a flight to Penticton, BC (CYYF) called when 10NM South and received the initial aerodrome advisory (AA). On subsequent contact to provide updates, the Searey went "no radio" (NORDO). A BP Aviation Cessna 172N (C-GCAW) on a flight from Penticton, BC (CYYF) received a departure advisory about the NORDO traffic, but was not yet finished their run up when the Searey landed. No operational impact.

At 0316Z, while on the climb out from Runway 34, a North Fork Oilfield Services Piper PA-28-140 (C-GEGB) from Penticton, BC (CYYF) to Penticton, BC (CYYF) reported pitot tube blockage and advised returning to land. C-GEGB landed safely at 0321Z. No impact to operations.

The flight service station (FSS) provided navigational assistance to the pilot when it became apparent that the pilot had mis-navigated and was possibly near Osoyoos, BC (CBB9). The aircraft landed safely at CYYF at 1733Z. No operational impact.

A privately registered Cessna 182Q from Duncan, BC (CAM3) to Penticton, BC (CYYF) was inbound with very poor radio readability. Not on surveillance and the very high frequency (VHF) directional finder (DF) was unserviceable (U/S), making it difficult to discern the aircrafts position. The departure of a privately registered Piper PA-22-108 from Penticton, BC (CYYF) to Penticton, BC (CYYF) was slightly delayed. The Cessna 182Q arrived without incident. No operational impact.

A Pacific Flying Club Cessna 172P (C-GFVB) from Vancouver/Boundary Bay, BC (CZBB) to Penticton, BC (CYYF) received an airport advisory for a touch-and-go (T/G) at CYYF. The flight service station (FSS) identified that the aircraft was likely near Oliver, BC (CAU3) instead of CYYF. Assistance was provided and the aircraft subsequently arrived safely at CYYF for the T/G. No impact to operations.

Incident at KAMLOOPS FIC / FIC DE KAMLOOPS (ATM - inaccurate aeronautical information)

At 07022115Z, the Kamloops (YKA) flight information centre (FIC) received a fax to issue a NOTAM that the Penticton, BC (CYYF) precision approach path indicators (PAPIs) for Runway 16/34 were unserviceable (U/S) until (TIL) approximately (APRX) 2007031700Z. At 07031709Z, the YKA FIC received a call from the CYYF airport manager (APM) saying that the NOTAM could be cancelled; however, the NOTAM had been issued with a TIL time of 07031700Z and had already been removed from the aeronautical fixed telecommunications network (AFTN).

A BP Aviation Ltd. Cessna 172M (C-GHBM) from Penticton, BC (CYYF) to Penticton, BC (CYYF) departed at 1950Z. At 1959Z, C-GHBM reported returning to the airport due an alternator problem. One inbound VFR aircraft was asked to, and did, give priority to C-GHBM. C-GHBM landed safely and taxied in. No impact to operations.

Incident at PENTICTON BC (CYYF) (Aerodrome, runway or taxiway shutdown)

The pilot of a Northern Air Charter (P.R.) Inc. Beech B200 (C-GNAG/NLI239) from Penticton, BC (CYYF) to Calgary, AB (CYYC), readying for departure, called while exiting the runway and cancelled their clearance due to smoke in the cabin. C-GNAG shutdown briefly on Taxiway Alpha and the crew exited the aircraft. The flight service station (FSS) sent a truck (ST45) out to assist. ST45 escorted C-GNAG back to the apron. The FSS was advised that the flap actuator had overheated. C-GNAG departed safely at 1818Z. While Taxiway Alpha was obstructed, 2 light aircraft entered the runway via Taxiway B. Minimal impact to operations.

Incident at PENTICTON BC (CYYF)

A privately registered, amateur built Vans RV-6A from Vernon, BC (CYVK) to Penticton, BC (CYYF) departed well before the estimated time of departure (ETD) from a site without air traffic service (ATS). The aircraft arrived at CYYF and the message was received before the flight plan (FP) was able to be activated.

Incident at PENTICTON BC (CYYF)

A remotely piloted aircraft system (RPAS) operator was scheduled to fly in the Penticton, BC (CYYF) control zone (CZ). No call was made to CYYF at the commencement of the flight. The Royal Canadian Mounted Police (RCMP) called the flight service station (FSS) to ask about RPAS activity, as they had received a complaint in the area. The FSS contacted the operator, who confirmed they had been flying, and were unaware that they had to call the FSS first. No impact to operations.

An American, privately registered Aviat A-1 on a VFR flight from Walla Walla, WA (KALW) to Penticton, BC (CYYF) had an estimated time of arrival (ETA) of 1507Z. At 1537Z, the aircraft went into Information Request to Departure Station (QALQ) status and appropriate procedures were followed. At 1607Z, the aircraft went into information request (INREQ) status. INREQ procedures were in progress when the aircraft landed and closed their flight plan at Kamloops, BC (CYKA).

An Eclipse Helicopters Ltd. Aerospatiale AS 350 BA (C-FPMW) from Penticton, BC (CYYF) to Penticton, BC (CYYF), upon arrival, was asked to have their VFR flight plan (FP) closed. There was no FP on file at the Flight Information Centre (FIC). Pilot was advised that there was no FP on file. Pilot indicated they expected one to have been on file.

Incident at OLIVER BC (CAU3)

Aviation Incident Report #16638: A BP Aviation Ltd. Cessna 172M (C-GHBM) was on a training flight out of Penticton, BC (CYYF) to Oliver, BC (CAU3), and were entering the Class F (A) practice area East of the CAU3. The aircrafts reported altitude was 4500 feet above sea level (ASL). The aircraft had made calls to CAU3 on 122.8 and 126.7 a few minutes prior. The pilot of C-GHBM looked below and saw a yellow aircraft 200 feet below them. They never replied to C-GHBMs radio calls. C-GHBM took evasive action by climbing. BP Aviation was able to determine the identity of the aircraft a short time later when it landed in Penticton. The aircraft was a privately registered Piper J3C-65.

Incident at PENTICTON BC (CYYF)

The initial call request of a Conair Group Inc. Aero Commander 690B (C-GIAA/FDG137) from Penticton, BC (CYYF) to Penticton, BC (CYYF) was wind/altimeter only, with no stated intentions. FGD137 then started to taxi on Taxiway C from tanker base. Advisory provided included Runway 34 and helicopter traffic, which was a Government of Canada, Department of National Defence Bell 412 (FREE72) from Penticton, BC (CYYF) to Penticton, BC (CYYF) on the downwind for autorotation to Runway 34. FGD137 did not acknowledge the traffic and took position on Runway 16 with FREE72 now on final. FREE72 was provided with information about the traffic on the runway, and the aircraft conducted a missed approach and left the area. Not until after becoming airborne did FGD137 state their intended direction of flight. Traffic passed both ways and there was no risk of collision or impact on operations.

Incident at PENTICTON BC (CYYF)

TSB Report #A20P0051: C-FAVT, a Bell 407 operated by Canadian Helicopters Limited, was conducting company water bucket training over the NW corner of Skaha Lake, BC, with 2 pilots onboard. During the first water load, the pilot flying (PF) in the left seat attempted a water release test without success. The PF asked the pilot not flying (PNF) in the right seat to verify functionality of the water release system and the PNF depressed the cargo release button. The water bucket was released from the cargo hook and landed on the lake. There was no damage to people or property from the release, and there was no damage to the equipment. Maintenance confirmed all the aircraft systems were operating correctly. The aircraft does not have a water release on the left seat cyclic controls due to aircraft modification for NVG training.

The aircraft was next observed on A with no call to another aircraft that was taxiing. When queried as to their intentions, the pilot advised doing checks. No impact on operations; other aircraft was able to taxi past.

Incident at PENTICTON BC (CYYF)

A WestJet Encore Ltd. de Havilland DHC-8-402 (C-FQWE/WEN3267) from Calgary, AB (CYYC) to Penticton, BC (CYYF) was on the approach for Runway 16. Pilot initiated a missed approach at 0412Z. No reason given by pilot; wind was 350 at 11G22. WEN3267 received a second approach clearance for Runway 16 and landed safely at 0432Z.
UPDATE from Flight Operations: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. On approach into Penticton, while on short final for Runway 16, winds exceed tailwind limitations. Flight crew conducted the go-around procedure and coordinated with the center to come around for a second attempt. On the next approach, the winds were within limits, WEN3267 landed with no issues.

Aviation incident report #16312: Passenger was very angry when they found out their ticket had been cancelled for a WestJet Encore Ltd. de Havilland DHC-8-402 (C-GWEQ/WEN3280) from Penticton, BC (CYYF) to Calgary, AB (CYYC), as they had not shown up for the original flight. They paid to book new ticket on flight. Walking away from the ticket counter, the passenger said they wished "we would all go boom, and the airport and WestJet would disappear". Customer Service agents escalated to airport security. The passenger got to the gate and continued making threats. Captain was advised and the Royal Canadian Mounted Police (RCMP) unit was called. Boarding was delayed while waiting for the RCMP. RCMP escorted passenger out of the airport.
