Incident at 12NM MANIWAKI QC (YMW) NDB

A WestJet Boeing 737-7CT (C-FWSF/WJA214) from Calgary, AB (CYYC) to Montreal/Pierre Elliott Trudeau, QC (CYUL) was cleared to FL310 with a correct read back. WJA214 was informed of traffic at FL300. WJA214 reported having the traffic in sight. At FL314, with a descent rate of around 2900/minute, WJA214 advised air traffic control (ATC) of a traffic alert and collision avoidance system (TCAS) resolution advisory (RA) and climbed a few hundred feet. The other aircraft in the vicinity was a Japan Airlines Boeing 787-9 (JA865J/JAL7) from Boston, MA (KBOS) to Tokyo/Narita Intl, Japan (RJAA). After crossing traffic, WJA214 was observed descending below FL310. No impact.

Air Traffic Control (ATC) noticed the error at 145653Z and cleared ACA319 for takeoff. No impact on operations.

Incident at 4NM SE PENTICTON BC (CYYF) (Other operational incident, Aerodrome - operations)

A forest fire was 4NM Southeast of Penticton, BC (CYYF). A NOTAM was issued restricting all aircraft except fire related aircraft. Upon review, the NOTAM was revised to allow other aircraft with authorization from the flight service station (FSS). The departure of a WestJet Encore Ltd. de Havilland DHC-8-402 (WEN3280) from Penticton, BC (CYYF) to Calgary, AB (CYYC) was delayed by approximately 1 hour, and several VFR aircraft were denied entry.

Incident at FORT ST. JOHN BC (CYXJ) (FOD (foreign object debris))

North Peace Airport Services (NPAS) advised that they had word that the inbound flight, a WestJet Encore de Havilland DHC-8-402 (C-FWEZ/WEN3101) from Calgary, AB (CYYC) to Fort St. John, BC (CYXJ) had struck a bird on arrival and requested to do a wildlife check on Runway 12/30. The remains of 2 small "Snipe" birds were found. Information passed to aircraft crew by NPAS.

Incident at OKANAGAN (PENTICTON) BC (ON) NDB

A WestJet Encore de Havilland DHC-8-402 (C-FOEN/WEN3281) from Calgary, AB (CYYC) to Penticton, BC (CYYF) was observed making a left turn after the Okanagan (Penticton), BC (ON) NDB without coordination with air traffic control (ATC). This turn left the aircraft at an altitude lower than the Minimum Radar Vectoring Altitude (MRVA). When questioned, WEN3281 indicated they were completing a company hold. Later, the crew confirmed they did not receive a hold clearance. No impact on operations.

Incident at NANAIMO AIRPORT

Upon landing Runway 16, a WestJet Encore de Havilland DHC-8-402 (C-GPWE/WEN3109) from Calgary, AB (CYYC) to Nanaimo, BC (CYCD) struck at least one small bird. Runway foreign object debris check completed.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. Evidence of a bird strike was found on the right hand inboard flap. The area was cleaned and a bird strike inspection was carried out in accordance with (IAW) the aircraft maintenance manual (AMM) 05-50-16-210-801, with no damage found. Serviceable.

Incident at FORT ST. JOHN BC (CYXJ)

A WestJet Encore Ltd. de Havilland DHC-8-400 (WEN3101) from Calgary, AB (CYYC) to Fort St. John, BC (CYXJ) was on a 15 mile final for Runway 30 and a privately registered Cessna A185E on a flight to Fort St. John, BC (CYXJ) was to the South of the approach for Runway 30. Traffic was passed between the two aircraft and the Cessna A185E stated that they were going to follow in behind WEN3101 and plan for number two. This was then relayed to WEN3101. As the aircrafts got closer, the Cessna A185E started to turn towards the approach of Runway 30. The Cessna A185E was then asked to remain at 033 feet and below the Cessna A185E acknowledged the instruction. This was then relayed to WEN3101. WEN3101 landed and stated that they had a resolution advisory (RA) from the aircraft.

Incident at VICTORIA INTL BC (CYYJ)

Delayed entry as a result of NAV CANADA safety and evaluation quality assurance (QA) review: A WestJet Boeing 737-600 (WJA227) from Calgary, AB (CYYC) to Victoria, BC (CYYJ) reported a possible bird strike while over the water. No operational impact.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. The aircraft that flew flight 227 on January 15, 2020 was identified to be tail 609 but there are no recorded bird strikes on that tail on that date in the CMTS. Research was accomplished for the aircraft operating flight 227 on January 14 and 16, 2020, and flight 227 operated by tail 601 on January 14, 2020 recorded a possible bird strike on log page AK54368. It is assumed that this is the CADORS related incident. Maintenance in CYYJ conducted a bird strike inspection in accordance with the Boeing aircraft maintenance manual (AMM) 05-51-18, with no evidence of a birdstrike found. No further maintenance action was required and the aircraft was returned to service.

Incident at 50NM PITT MEADOWS BC (CYPK)

A 2791455 Canada Inc. Lake LA-4-200 (C-FBOQ) from Pitt Meadows, BC (CYPK) to Calgary, AB (CYYC) departed VFR. C-FBOQ was not on a VFR flight plan. At 2310Z, when approximately 50NM East of CYPK, the aircraft ascended above 12,500 feet. When queried by the Kamloops controller, the pilot advised needing to climb to avoid a cloud. The controller advised that the aircraft could not climb above 12500 feet unless it was CVFR. When cleared CVFR to 15000ft, the pilot climbed to 15500ft, without clearance, to avoid a cloud. The controller subsequently provided clearance to 17000ft, however, the pilot started a descent without clearance to avoid a cloud. The controller vectored another aircraft to ensure separation. The CVFR flight plan generated an IFR Canadian Automated Air Traffic System (CAATS) flight plan for the aircrafts new destination of Lethbridge, AB (CYQL). An Edmonton Air Traffic Operations Specialist (ATOS) initiated a Communications Search when C-FBOQ became overdue at CYQL. C-FBOQ subsequently contacted the Edmonton Centre for flight following.

Incident at FORT ST. JOHN BC (CYXJ) (Aerodrome - runway or taxiway surface condition)

After departure A North Cariboo Flying Service Ltd. Beech 1900D (NCB2304) from Fort St. John, BC (CYXJ) to Calgary, AB (CYYC) requested a runway inspection after hearing a bump or slight noise during the departure phase of flight. CYXJ authority checked the runway for anything that could have caused the bump or noise. After the inspection they reported that there was nothing found on the runway, but there was a small crack on the south side that could have possibly been associated with it. NCB2034 was advised. No impact to operations.

Incident at PENTICTON BC (CYYF) (Aerodrome, runway or taxiway shutdown)

The pilot of a Northern Air Charter (P.R.) Inc. Beech B200 (C-GNAG/NLI239) from Penticton, BC (CYYF) to Calgary, AB (CYYC), readying for departure, called while exiting the runway and cancelled their clearance due to smoke in the cabin. C-GNAG shutdown briefly on Taxiway Alpha and the crew exited the aircraft. The flight service station (FSS) sent a truck (ST45) out to assist. ST45 escorted C-GNAG back to the apron. The FSS was advised that the flap actuator had overheated. C-GNAG departed safely at 1818Z. While Taxiway Alpha was obstructed, 2 light aircraft entered the runway via Taxiway B. Minimal impact to operations.
UPDATE: Aviation Incident Report: At approximately 1730Z, maintenance received a call from the Flight Service Station (FSS) asking for assistance with a Northern Air Charter (P.R.) Inc. Beech B200 (C-GNAG/NLI239) that taxied onto Taxiway Alpha after landing and did a full shut down with no radio contact with FSS. Upon arrival, with a vehicle (Staff 45) at the location on Taxiway Alpha, the Co-pilot indicated that they encountered smoke within the cab and figured it was electrical in nature. They asked to be escorted as they wanted to taxi to the main apron with all electrics off so that they could try and identify the cause. C-GNAG taxied to the apron under escort with no incident. After shut down, the Captain exited the aircraft and indicated that they thought the issue was with the flap actuator and motor. The Co-pilot was on the telephone with their maintenance department, where it was confirmed that the flap actuator had overheated. The Captain indicated that they were going to wait for the motor to cool down and then depart.
Update: TSB Report #A20P0065: C-GNAG, a Beechcraft King Air 200 operated by Northern Air Charter, was conducting a flight from Penticton (CYYF), BC, to Calgary Intl (CYYC), AB, with 2 crew members on board. While taxiing for departure in CYYF, smoke was noticed in the cabin. The aircraft stopped on Taxiway Alpha, the engines were shut down, and the captain investigated the cause of the smoke. The operators maintenance was consulted and the flap motor was determined to be responsible for the smoke. The system was isolated and the aircraft flew to Calgary without further incident.

Aviation Incident Report #16650: The crew of a WestJet de Havilland DHC-8-400 (C-GEWO) from Calgary Intl, AB (CYYC) to Kamloops, BC (CYKA) became suspicious of the interaction between 2 passengers travelling together. Flight crew arranged for the Royal Canadian Mounted Police (RCMP) to meet the flight through the CYKA tower. RCMP met the flight and questioned the two individuals.

The area control centre (ACC) and Technical Operations Coordinator (TOC) were advised. MLAT was returned to service at 1906Z with the WEN3101 flight on its short final.

Incident at KAMLOOPS BC (CYKA)

TSB Report #A20P0016: C-FTEN, a Bombardier DHC-8-400 aircraft operated by WestJet Encore, was conducting flight WEN3241 from Calgary Intl (CYYC), AB to Kamloops (CYKA), BC with 4 crew members and 77 passengers on board. During the descent into CYKA, the flight encountered severe turbulence and the crew initiated a missed approach; turbulence continued throughout the missed approach. The crew reported a momentary loss of control when the aircraft would not respond to their inputs. The crew decided to divert to their alternate destination (CYYC) due to the severity of the turbulence. The flight landed safely in CYYC with no injuries to crew or passengers. Maintenance completed a severe turbulence inspection with no faults found. De Havilland analyzed the flight data recorder information with no exceedances found. The aircraft has returned to service.

Incident at 5307N / 11913W

An American, corporately registered Cessna 525B (N525L) from Valemount, BC (CAH4) to Calgary, AB (CYYC) was cleared to FL230. Air traffic control (ATC) observed N525L descending to FL220. ATC subsequently instructed N525L to climb to FL280. No impact to the operation.

Incident at 62NM E Kelowna (CYLW)

TSB Report #A20P0018: C-GWSB, a Westjet 737-600, was operating as WS461 from Calgary Intl (CYYC), AB to Kelowna (CYLW), BC with 5 crewmembers and 99 passengers on board. When, at the top of descent for CYLW, the crew activated the #2 Engine Cowl Thermal Anti Ice (TAI), they got a warning indicating that the TAI was inoperative. The crew climbed out of icing conditions and ran through the appropriate checklists with no change. The flight crew made the decision to return to CYYC. With the TAI inoperative, the crew needed to avoid icing conditions and that was not possible if they complied with the normal descent profile. They declared a PANPAN to and received priority handling to avoid icing conditions. The aircraft landed without further incident. Maintenance replaced the #2 engine cowl TAI valve and the aircraft was returned to service.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. Flight crew on WestJet flight WSA461 from CYYC to CYLW recorded an issue with the #2 Engine Thermal Anti-Ice valve on log page AJ64252. The valve would not go to commanded position. Flight crew elected to return to CYYC shortly after take-off. WestJet Maintenance met the aircraft upon arrival and troubleshot the defect to a faulty #2 Thermal Anti-Ice valve itself. The valve was replaced and operationally checked in accordance with the Boeing Aircraft Maintenance Manual (AMM) 30-21-11.
