Incident at 12NM MANIWAKI QC (YMW) NDB

A WestJet Boeing 737-7CT (C-FWSF/WJA214) from Calgary, AB (CYYC) to Montreal/Pierre Elliott Trudeau, QC (CYUL) was cleared to FL310 with a correct read back. WJA214 was informed of traffic at FL300. WJA214 reported having the traffic in sight. At FL314, with a descent rate of around 2900/minute, WJA214 advised air traffic control (ATC) of a traffic alert and collision avoidance system (TCAS) resolution advisory (RA) and climbed a few hundred feet. The other aircraft in the vicinity was a Japan Airlines Boeing 787-9 (JA865J/JAL7) from Boston, MA (KBOS) to Tokyo/Narita Intl, Japan (RJAA). After crossing traffic, WJA214 was observed descending below FL310. No impact.

At 145613Z, an Envoy Air Embraer E135 (ENY3989) from Montreal/Pierre Elliott Trudeau, QC (CYUL) to New York/La Guardia, NY (KLGA) was cleared to line up for Runway 24L. An Air Canada Airbus A320-200 (ACA319) from Montreal/Pierre Elliott Trudeau, QC (CYUL) to Calgary, AB (CYYC) read back the clearance and lined up for Runway 24L. Air Traffic Control (ATC) noticed the error at 145653Z and cleared ACA319 for takeoff. No impact on operations.

Incident at PENTICTON BC (CYYF) (Aerodrome, runway or taxiway shutdown)

The pilot of a Northern Air Charter (P.R.) Inc. Beech B200 (C-GNAG/NLI239) from Penticton, BC (CYYF) to Calgary, AB (CYYC), readying for departure, called while exiting the runway and cancelled their clearance due to smoke in the cabin. C-GNAG shutdown briefly on Taxiway Alpha and the crew exited the aircraft. The flight service station (FSS) sent a truck (ST45) out to assist. ST45 escorted C-GNAG back to the apron. The FSS was advised that the flap actuator had overheated. C-GNAG departed safely at 1818Z. While Taxiway Alpha was obstructed, 2 light aircraft entered the runway via Taxiway B. Minimal impact to operations.

Incident at KAMLOOPS BC (CYKA)

Aviation Incident Report #16650: The crew of a WestJet de Havilland DHC-8-400 (C-GEWO) from Calgary Intl, AB (CYYC) to Kamloops, BC (CYKA) became suspicious of the interaction between 2 passengers travelling together. Flight crew arranged for the Royal Canadian Mounted Police (RCMP) to meet the flight through the CYKA tower. RCMP met the flight and questioned the two individuals.

The Fort St. John, BC (CYXJ) multilateration system (MLAT) source was unserviceable (U/S) at 1904Z with a WestJet Encore Ltd. de Havilland DHC-8-402 (WEN3101) from Calgary, AB (CYYC) to Fort St. John, BC (CYXJ), 7 miles final for Runway 30. The area control centre (ACC) and Technical Operations Coordinator (TOC) were advised. MLAT was returned to service at 1906Z with the WEN3101 flight on its short final.

Aircraft had positive indication after 15 minutes of circling and landed on Runway 30 without issue. No impact to operations.

Airports advised that a WestJet Encore de Havilland DHC-8-402 (C-FUWE/WEN3153) on a flight from Calgary Intl, AB (CYYC) to Nanaimo, BC (CYCD) had a possible bird strike at 500 feet on approach while landing Runway 16. The flight landed at 0143Z. Damage to the #1 engine side of plane which grounded the aircraft.

Incident at KAMLOOPS BC (CYKA)

TSB Report #A20P0016: C-FTEN, a Bombardier DHC-8-400 aircraft operated by WestJet Encore, was conducting flight WEN3241 from Calgary Intl (CYYC), AB to Kamloops (CYKA), BC with 4 crew members and 77 passengers on board. During the descent into CYKA, the flight encountered severe turbulence and the crew initiated a missed approach; turbulence continued throughout the missed approach. The crew reported a momentary loss of control when the aircraft would not respond to their inputs. The crew decided to divert to their alternate destination (CYYC) due to the severity of the turbulence. The flight landed safely in CYYC with no injuries to crew or passengers. Maintenance completed a severe turbulence inspection with no faults found. De Havilland analyzed the flight data recorder information with no exceedances found. The aircraft has returned to service.

An American, corporately registered Cessna 525B (N525L) from Valemount, BC (CAH4) to Calgary, AB (CYYC) was cleared to FL230. Air traffic control (ATC) observed N525L descending to FL220. ATC subsequently instructed N525L to climb to FL280. No impact to the operation.

Incident at 62NM E Kelowna (CYLW)

TSB Report #A20P0018: C-GWSB, a Westjet 737-600, was operating as WS461 from Calgary Intl (CYYC), AB to Kelowna (CYLW), BC with 5 crewmembers and 99 passengers on board. When, at the top of descent for CYLW, the crew activated the #2 Engine Cowl Thermal Anti Ice (TAI), they got a warning indicating that the TAI was inoperative. The crew climbed out of icing conditions and ran through the appropriate checklists with no change. The flight crew made the decision to return to CYYC. With the TAI inoperative, the crew needed to avoid icing conditions and that was not possible if they complied with the normal descent profile. They declared a PANPAN to and received priority handling to avoid icing conditions. The aircraft landed without further incident. Maintenance replaced the #2 engine cowl TAI valve and the aircraft was returned to service.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. Flight crew on WestJet flight WSA461 from CYYC to CYLW recorded an issue with the #2 Engine Thermal Anti-Ice valve on log page AJ64252. The valve would not go to commanded position. Flight crew elected to return to CYYC shortly after take-off. WestJet Maintenance met the aircraft upon arrival and troubleshot the defect to a faulty #2 Thermal Anti-Ice valve itself. The valve was replaced and operationally checked in accordance with the Boeing Aircraft Maintenance Manual (AMM) 30-21-11. The aircraft departed and there have been no further instances of #2 engine thermal anti-ice issues in the ensuing time period indicating correct troubleshooting and rectification of the defect. 362 flight hours and 188 cycles have elapsed since defect rectification.

Accident at 15NM PITT MEADOWS BC (CYPK)

An Airsprint Cessna 525A (ASP510) from Calgary, AB (CYYC) to Vancouver, BC (CYVR) reported hearing a MAYDAY call on 121.5. The call was from a PG Aviation Services Robinson R44 II (C-FGAN) from Pitt Meadows, BC (CYPK) to Pitt Meadows, BC (CYPK) which had crashed at the Southwest end of Widgeon Lake. There were no injuries to souls on board. Joint Rescue Coordination Centre (JRCC) Victoria was advised and North Shore Search and Rescue responded.
Update TSB Report #A20P0028: C-FGAN, a privately operated Robinson R-44 II helicopter, was touching down on snow-covered ice at Widgeon Lake, BC when the pilot lost control and the helicopter rolled onto its side. The pilot and two passengers were not injured but the helicopter was substantially damaged. The 406 KHz ELT was manually activated and a Mayday call was made on 121.5 KHz to an overflying aircraft.

A Government Of Canada, Royal Canadian Mounted Police Pilatus PC-12 (C-GMPW) from Prince Albert (Glass Field), SK (CYPA) to Kelowna, BC (CYLW) was slowed and a Southern Interior Flight Centre Cessna 172S (C-GICG) from Kelowna, BC (CYLW) to Kelowna, BC (CYLW) was put in a hold for spacing to allow for a runway inspection. One bald eagle was found intact.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. The flight crew of WJA253 reported the bird strike on log page AK65711. Maintenance performed a bird strike inspection in accordance with (IAW) Aircraft Maintenance Manual (AMM) 05-51-18. No faults or damage found. Aircraft returned to service on log page AK65711.

Incident at Kelowna

A Carson Air Swearingen SA227-AT (C-GTTM/CA8081) on a flight from Calgary Intl, AB (CYYC) to Kelowna, BC (CYLW) reported engine issues. The engine was subsequently shut down and the pilot indicated they would do a single engine approach. The pilot indicated they were in VFR conditions with the field in sight. No emergency was declared and the aircraft landed safely at 1525Z. No operational impact.
UPDATE TSB Report #A20P0012: C-GTTM, a Swearingen SA227-AT operated by Carson Air Ltd. was conducting flight CA8081 from Calgary Intl (CYYC), AB to Kelowna (CYLW), BC, with 2 flight crew members on board. During cruise flight the pilot flying determined the right engine (Garret TPE 331-11U) was no longer responding to cockpit power lever movements. The flight crew actioned the emergency checklist for no response to power lever movement. With no change to the condition, the crew actioned the engine shutdown checklist. The flight crew notified ATC of the event, but did not declare a PANPAN or MAYDAY. The flight landed without further incident. Subsequent maintenance actions determined the propeller pitch control bracket became detached during flight. The hardware including the bolt and washer were located within the engine cowling, but the lock wire was not. The aircraft has since been returned to service.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. Internal investigations performed by the company found that a maintenance error led to the secondary retention screw on the Propeller Pitch Control (PPC) input arm not being locked and the arm detaching from the engine at cruise power. Safety Management System (SMS) investigation and Maintenance Quality Assurance (QA) investigations were carried out, and short term and long term corrective actions were put in place.

A WestJet Boeing 737-8CT (C-GJWS/WJA209) from Calgary, AB (CYYC) to Victoria, BC (CYYJ) reported a bird strike when on short final for Runway 09. WJA209 executed a missed approach and diverted to Vancouver, BC (CYVR). Aircraft Rescue and Fire Fighting (ARFF) found remains in the approach area (10+ geese). No impact on operations.
UPDATE TSB Report #A20P0006: C-GJWS, a WestJet Boeing 737-800, was operating as flight WJA209 from Calgary Intl (CYYC), AB to Victoria (CYYJ), BC. During short final at CYYJ the aircraft encountered a flock of geese and struck approximately 10 of them. The crew executed a missed approach and diverted to Vancouver Intl (CYVR), BC. With ARFF on standby, the aircraft was given priority clearance and it landed without further incident. The aircraft nose cone had been punctured by the impact. There were no injuries.

UPDATE TSB Report #A20P0001: C-GAPY, an Airbus A319-114 aircraft operated by Air Canada was conducting flight ACA229 from Calgary Intl (CYYC), AB to Vancouver Intl (CYVR), BC with 5 crewmembers and 112 passengers on board. At 30 000 feet ASL, near Kelowna, BC, the #2 engine (CFM Intl CFM56-5A5) began to stall with some visible sparking. The crew reduced power to idle on the affected engine and declared a PAN PAN with ATC. The aircraft landed safely at CYVR with ARFF standing by. Maintenance conducted an initial borescope that revealed wear to High Pressure Compressor (HPC) stages 6 and 7 that was beyond limits. The company stated that it was a high time engine with only 1100 cycles remaining and that an engine change was conducted to return it to service.

A Jazz de Havilland DHC-8-402 (C-GUJZ/JZA8427) on a flight from Calgary Intl, AB (CYYC) to Kamloops, BC (CYKA) advised they had a passenger in medical distress.

Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ) (FOD (foreign object debris))

An Air Canada Boeing 787-9 (ACA7245) from Toronto/Lester B. Pearson, ON (CYYZ) to Calgary, AB (CYYC) reported hitting a bird on its departure. The runway was inspected and one bird was removed. The aircraft continued on to its destination. No impact to operations.
