A downed aircraft was reported by a British Columbia Helicopters Ltd. Guimbal Cabri G2 (C-GUET). The aircraft had two occupants. The aircraft was later identified as a privately registered Cessna 140 from Kelowna, BC (CYLW) to Pitt Meadows, BC (CYPK).
Update TSB Report #A20P0071: A privately operated Cessna 140 was conducting a VFR flight from the abandoned airstrip at Stave Lake, BC, to Pitt Meadows (CYPK), BC, with 1 pilot and 1 passenger-pilot on board. During the takeoff roll, control of the aircraft was lost. The aircraft came to rest inverted and in the opposite direction of the attempted takeoff roll. The pilot sustained fatal injuries. The passenger pilot sustained minor injuries. There was no post-impact fire. TSB investigators deployed to the site.

Incident at In the vicinity of: Saltair, BC

Aviation Incident Report #16675: During a cross-country flight at night, a Pacific Flying Club Cessna 152 (C-GINK) on a round-robin flight from Vancouver/Boundary Bay, BC (CZBB) to Vancouver/Boundary Bay, BC (CZBB), including en route stops at Campbell River, BC (CYBL), Victoria, BC (CYYJ), and Abbotsford, BC (CYXX), was subject to a laser strike over Saltair, BC (North of Chemainus, BC). At the time of the occurrence, the pilot was en route to CYYJ at 5500 feet when they noticed the underside of their wings suddenly reflecting a flashing green light. The pilot turned a few degrees left and right to ensure the light did not come from conflicting traffic or potentially aviation-related pyrotechnic signals. They noticed that the green laser beam was originating from a residential area in Saltair, BC. The occurrence lasted for approximately 30 seconds, during which the laser emitted a constant green beam that was moved around rapidly while aiming at the aircraft. The movement resembled the way a person would hold a handheld laser while pointing it at an object in the distance. The pilots vision was not notably impaired and they tried not to look directly at the light. Immediately after the strike, they reported the occurrence to the CYYJ terminal on 120.8 MHz. After making a report to the air traffic controller (ATC), a helicopter in the area also reported a laser strike around Chemainus. Further, the pilot of another aircraft from the same flight school was flying through the area just a few minutes behind and advised they saw the laser as well. The laser strike occurred about 1/2NM to 1NM East of a lighted stretch of Highway 1. The pilot believes the laser beam originated somewhere within 1/4NM of 48°57’16.40”N, 123°45’58.08”W. The pilot experienced the same event in the same location on June 26, 2020 at 0658Z - see CADORS 2020P1164.

Aircraft was instructed to cross KEKPO intersection at 9000 feet, as per the published standard terminal arrival (STAR) procedure. The aircraft was observed crossing KEKPO intersection at 8000 feet. Subsequently, the aircraft was cleared to 6000 feet and was observed descending to 5000 feet at the VOGAD intersection. No operational impact.

The flight plan (FP) was originally received in the system at 1726Z and, subsequently, the Kamloops flight information centre (FIC) received a cancellation message at 2012Z. NAV CANADA was not holding alerting service during N8585Us flight.

Accident at PITT MEADOWS BC (CYPK) (FOD (foreign object debris))

TSB Report #A20P0062: C-GZSH, a private amphibious float-equipped Cessna A185F, utilized by Conair Aerial Firefighting, was conducting a flight from Pitt Meadows (CYPK), BC, to Abbotsford (CYXX), BC, with 1 pilot and 1 passenger onboard. Shortly after takeoff, the pilot reported a loud bang followed by significant vibrations and abnormal handling characterisitics in the pitch axis. The pilot requested an immediate landing, performed a 180 degree turn, and landed without incident. There was substantial damage found on the leading edge of the horizontal stabilizer. Upon further inspection, the right hand float access hatch was missing and was presumed to have struck the horizontal stabilizer. The aircraft had recently undergone maintenance on the hydraulic system for the amphibious landing gear. The access hatch was recovered from the runway.
