Incident at 5NM LONDON ON (CYXU) (ATM - operations)

An Air Bravo Corp. Pilatus PC-12/45 (C-FXAB/AB239) from Atikokan, ON (CYIB) to London, ON (CYXU) reported an issue with the localizer (LOC) for Runway 15 while 5NM final on the Instrument Landing System (ILS) approach for Runway 15. AB239 was observed to be below the glide path and was instructed to climb to 4000 ft. AB239 was subsequently cleared for the area navigation (RNAV) for Runway 27, and landed at 0357Z without further issues. The Technology Operations Coordinator (TOC) inspected the ILS for Runway 15 and did not observe any issues. The tower closed late, at 0400Z, due to the suspected equipment issue. No operational impact.

A Swoop Inc. Boeing 737-8CT (WSW221) from London, ON (CYXU) to Abbotsford, BC (CYXX) that was cleared for the area navigation (RNAV) required navigation performance (RNP) 25 at CYXX received a traffic alert and collision avoidance system (TCAS) resolution advisory (RA) at 5000 CYXX 037/14 in Class E airspace. Traffic information was passed. The other aircraft was a Langley Flying School Inc. Piper PA-28-140 (C-GCEP) from Langley, BC (CYNJ) to Langley, BC (CYNJ).

Incident at LONDON ON (CYXU)

Aviation Incident Report #16898: An ITPS (Canada) Ltd. Cirrus SR22 (C-FYWI/ST21) from London, ON (CYXU) to London, ON (CYXU) was conducting a normal approach for landing, with flaps at 100% and the landing reference speed (i.e. "V ref") at 85 knots. At the threshold (with everything looking normal), upon touchdown, the student did not hold the aircrafts nose firm, and the aircraft bounced. The student was instructed to "go-around", but no action was observed. Upon the second bounce, the aircraft was instructed again, the student pushed the throttle forward, and the instructor pulled the side-stick for a go-around. No specific noise or vibration of the propeller hitting the runway was heard or felt. After the aircraft returned to the hangar and the engine was stopped, the maintenance team observed damage to the propeller. No impact on operations. The aircraft has been removed from service until the repair is completed.

A Chrono Jet Inc. de Havilland DHC-8-106 (C-FGCP/MBK3031) from London, ON (CYXU) to Ottawa/Macdonald-Cartier, ON (CYOW) began to taxi via Taxiway G for departure from Runway 09. While taxiing, the aircraft reported a possible wildlife sighting. They lost sight of the wildlife (a fox) in the infield. MBK3031 continued to taxi slowly and departed safely. The Security Operations Centre (SOC) was advised of the wildlife presence and the direction in which it was last seen to be heading.

A Jet Aircraft Museum Canadair T33-AN MK3 (C-FUPP) from London, ON (CYXU) to London, ON (CYXU) departed Runway 15, and the tower observed the aircraft possibly experience a tail strike upon rotation. C-FUPP was notified and acknowledged, and additionally reported fuel venting out of a fuel cap. The pilot requested a return to land. No assistance was requested. An Aircraft Rescue and Fire Fighting (ARFF) vehicle inspected Runway 15 and found nothing. C-FUPP landed safely at 1923Z. No impact to operations.

Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ)

A WestJet Encore Ltd. de Havilland DHC-8-402 (C-GENK/WEN3516) from London, ON (CYXU) to Toronto/Lester B. Pearson, ON (CYYZ) reported sighting a drone while 7NM final for Runway 24R. WEN3516 was descending through 2800ft and advised that the drone was above them. The approach was changed to Runway 23 for 30 minutes, and no further sightings were reported. The Transportation Safety Board of Canada (TSB) and the Aviation Operations Centre (AVOPS) were advised.

A 7506406 Canada Inc. Agusta AW139 (C-GYNH), VFR, was conducting a local maintenance flight from London, ON (CYXU) to London, ON (CYXU) to the west of the CYXU control zone (CZ). C-GYNH was observed on the NAV CANADA Auxiliary Radar Display System (NARDS) passing approximately 6NM West Northwest (WNW) of the airport, at 1900 feet above sea level (ASL). Traffic was passed to a Cessna 140 on a VFR flight to London, ON (CYXU), inbound from the Northwest (NW). Traffic was passed to C-GYNH. C-GYNH was later observed turning into the CZ without having received a clearance. C-GYNH was given a vector to keep them clear of the Cessna 140 and an outbound Diamond DA 20-C1 on a VFR flight from London, ON (CYXU). Minimal impact on operations.

The operating hours of the London, ON (CYXU) tower were reduced for the period of October 13th to 16th, from 1215Z to 2000Z, due to COVID staffing restrictions. NOTAM issued. The London International Airport Authority (LIA) and the airport user were notified via e-mail and phone calls. NOTAM break times will be dealt with in the same manner as a tower nightly closure. The "picture" needs to be handed off to the Sudbury (YSB) flight service station (FSS) and, before reopening, the transfer of responsibility needs to be completed again.

An ITPS (Canada) Ltd. Aero Vodochody L39C (ST42) from London, ON (CYXU) to London, ON (CYXU), inbound for an overhead break at CYXU, came in close proximity of an IFR aircraft on final, a Waterloo-Wellington Flying Club Cessna 172RG (C-GPIQ) on a flight to London, ON (CYXU), with a high rate of closure. Traffic was passed to C-GPIQ and multiple times to ST42. C-GPIQ reported traffic in sight. After completing an overhead break, and during the briefing to the CYXU tower to reopen, ST42 started to turn base abeam VFR traffic, an Aeolus Flight Academy Cessna 172M (C-GPAS) from London, ON (CYXU) to London, ON (CYXU), that was within close proximity, but was stopped by the tower controller.

Subsequently, an Air Bravo flight (AB276) on an IFR flight to London, ON (CYXU), on a visual approach to Runway 09, confirmed that the PAPI was out of alignment, showing "white-red-white". Airport maintenance notified. Field electrician will inspect on Oct 13th. PAPI turned off.

UPDATE: Aviation Incident Report #16825: While executing a normal landing in VFR conditions, an ITPS (Canada) Ltd. Aero Vodochody L39C (C-GFNO/ST42) from London, ON (CYXU) to London, ON (CYXU) experienced a failure of the normal braking system. Emergency braking was used to bring the aircraft to a full stop. The aircraft was shut down and maintenance (MTC) crews were called to tow the aircraft back to the hangar. Maintenance is inspecting the brake system.

Intermittent two-way radio contact was established when the aircraft was 2 miles East. The aircraft was cleared to land on Runway 15. Impact to operations: two aircraft, a Diamond Aircraft Industries Inc. Diamond DA 20-C1 (C-GEVD) from London, ON (CYXU) to London, ON (CYXU), and a Diamond Flight Centre London Inc. A St. Catharines Flying Club Piper PA-28-161 (C-GNOP) on a VFR flight from St. Catharines/Niagara, ON (CYSN) to London, ON (CYXU) requested a simulated instrument landing system (ILS) approach to Runway 15. After two attempts, it was suspected that the pilot lacked the required knowledge or proficiency for an ILS approach. When queried, the pilot replied "it was simulation". The tower instructed the pilot to abandon the approach and continue visually for a touch-and-go on Runway 15, which was completed without incident. No impact on operations.

A Diamond Flight Centre London Inc. Diamond DA 20-C1 (C-GDFP) was conducting a VFR flight in the circuit pattern for Runway 33 at London, ON (CYXU). At 1339Z, C-GDFP was issued a landing clearance. C-GDFP landed on Runway 33, then was observed briefly stopping on Runway 33. Shortly thereafter, the aircraft began a takeoff roll and became airborne. No operational impact.

Incident at LONDON ON (CYXU) (Communication navigation surveillance/air traffic system, Flight plan – activation)

The pilot of an American, corporately registered Diamond DA-42 (N460TS) on a transborder VFR flight from St Clair County Intl, MI (KPHN) to London, ON (CYXU) called to close their flight plan. There was a proposed flight plan in the system, but it had not been activated by the U.S. flight service station (FSS). The pilot stated they had asked for an "Automatic Open."

At 1530Z, the London, ON (CYXU) Flight Information Centre (FIC) received a proposed flight plan (FP) from Leidos for a Great Lakes Helicopter Airbus EC130 T2 (C-GXYZ) on a transborder flight from Indiana County, PA (KIDI) to Hamilton, ON (CYHM). The CYXU FIC did not receive a departure (DEP) message or any other communication that the pilot had departed. At 1706Z, the CYHM tower called CYXU to report the arrival of the aircraft. When queried, Leidos told the CYXU FIC that the pilot had filed the FP at 1330Z, but did not activate it.
