Incident at LONDON ON (CYXU)

A privately registered, amateur-built Vans RV3-A on a flight to London, ON (CYXU) was observed taxiing on Taxiway Golf, after landing on Runway 09, without clearance. Aircraft was instructed to exit onto the Shell ramp to allow opposite direction traffic to continue taxiing. Subsequently, the aircraft was issued a taxi clearance, but was observed taxiing on the vehicle corridor, and then behind the lights on the blast pad.

A Diamond Flight Centre London Inc. Diamond DA 20-C1 (C-GDFB) from London, ON (CYXU) to London, ON (CYXU) was instructed to backtrack to Taxiway Golf and exit after landing. An Aeolus Flight Academy Inc. Cessna 152 (C-GBZB) from London, ON (CYXU) to London, ON (CYXU) was cleared for take-off from Runway 09. The tower subsequently noticed C-GDFB had stopped at the corner of Taxiway Golf and Runway 09 and had not vacated the runway as instructed.

A Diamond Flight Centre London Inc. Diamond DA 20-C1 (C-GEWT) completing a VFR flight in the circuit at London, ON (CYXU) was instructed to conduct an overshoot for Runway 15 due to traffic, an ITPS Ltd. flight (SA61F), on the runway and slow to depart. C-GEWT was observed not commencing an overshoot and continued low-level. The departing traffic had begun their takeoff roll and became airborne. C-GEWT was subsequently cleared to land on Runway 15.

Incident at LONDON ON (CYXU)

A Diamond Flight Centre London Inc. Diamond DA 20-C1 (C-FMKC) from London, ON (CYXU) to London, ON (CYXU) was instructed to overshoot and fly the runway heading. As C-FMKC was overflying the departure end of the runway, the tower passed traffic to C-FMKC to follow another aircraft joining the downwind. C-FMKC was observed turning crosswind early as traffic information was being given. C-FMKC spotted the traffic immediately and adjusted their course to follow.

Incident at 15NM NE LONDON ON (CYXU)

Toronto (YYZ) passed information to the London tower about a privately registered Piper PA-32-300 from Strathroy (Blue Yonder), ON (CPK2) to Peterborough, ON (CYPQ) that was 15 miles Northeast (NE) and inbound to London, ON (CYXU) with intermittent electrical issues. The aircraft contacted London and was given clearance for the left base. Firehall was called and set up on Alpha. The aircraft, at 8NM northeast (NE) and at 4500ft, then informed the tower they would just continue to CPK2 for landing.

An Aeolus Flight Academy Inc Cessna 152 (C-GBZB) was cleared for a touch-and-go on Runway 15. Subsequently, a Waterloo-Wellington Flying Club Cessna 172S (C-GYEE) from London, ON (CYXU) to Kitchener/Waterloo, ON (CYKF) requested take-off clearance for Runway 15 from Taxiway A intersection, and was instructed to hold short of Runway 15. C-GYEE then lined up in front of C-GBZB, who was touching down north of Taxiway D. The tower controller observed the incursion and instructed C-GBZB to stop on the runway. C-GBZB came to a stop roughly at Taxiway D intersection.

An Aeolus Flight Academy Inc Cessna 152 (C-GBZB) from London, ON (CYXU) to London, ON (CYXU) was cleared for a touch-and-go on Runway 15 and was instructed to fly runway heading on departure. On departure, C-GBZB turned into the left crosswind without clearance, conflicting with an American corporately registered Cessna P337H (N1ZX) who had been cleared for takeoff from Runway 09 east southeast (ESE) bound. Both aircraft were vectored to avoid each other.

A 2529714 Ontario Corporation Piper PA-24-260 (C-FELI) from London, ON (CYXU) to London, ON (CYXU) was instructed to taxi to Runway 15 via Taxiway G and cross Runway 09. C-FELI was then observed conducting a run-up on the north edge of Runway 09, but not past the hold short line. C-FELI was queried and advised they would move onto Taxiway G north of Runway 09. No impact to operations.

Incident at LONDON ON (CYXU)

An American, corporately registered Gulfstream GV-SP (N587G) on an IFR flight from Lima/Allen County, OH (KAOH) to London, ON (CYXU) landed on Runway 09 and was instructed to backtrack to exit on Taxiway Golf. A VFR Diamond DV20 aircraft was crossing Runway 09 at Golf, and N587G was instructed to give way to the DV20 twice. N587G was then observed turning onto Taxiway Golf in front of the traffic. No impact on operations.

A Diamond Flight Centre London Inc. Diamond DA 20-C1 (C-GXHC) from London, ON (CYXU) to London, ON (CYXU) landed on Runway 09 and was instructed to backtrack to and exit onto Taxiway G. C-GXHC passed Taxiway G and subsequently asked if they should exit via Taxiway C. A Diamond Flight Centre London Inc. Diamond DA 20-C1 (C-GDFP), on a flight to London, ON (CYXU) and approximately 1/2 mile final, initiated an overshoot before instructions to conduct a go-around could be given by the tower controller. Note: time is estimated as the incident could not be located in the Tower Aircraft Movement (TAM) system.

The pilot of a Big Box Mobile Inc. Robinson R44 (C-GCOD) from Carlisle, PA (KN94) to London, ON (CYXU) called the London flight information centre (FIC) to close their transborder VFR flight plan. The London FIC had a proposed flight plan for the aircraft, however it did not receive a proper departure from the United States (US). The pilot indicated that they did not specifically request an assumed departure in the US.

Incident at LONDON ON (CYXU)

After finishing the run-up on Apron 2, an ITPS Ltd. Long EZ (C-GMJH/SA25) from London, ON (CYXU) to London, ON (CYXU) requested taxi clearance multiple times despite being told to standby due to traffic in the opposite direction. Immediately after taxi clearance was given to taxi Taxiway G and to hold short (H/S) for Runway 27, the aircraft could no longer be reached by the ground to give secondary surveillance radar (SSR) code. They switched to the tower very early, the tower controller got a hold of the aircraft and instructed them to contact the ground as they were approaching Taxiway B.

Incident at LONDON FIC - NW ONT / FIC DE LONDON - NW ONT (Communication navigation surveillance/air traffic system, Flight plan – activation)

The pilot of a Goldcorp Canada Ltd. de Havilland DHC-8-314 (C-GMWT) from Thunder Bay, ON (CYQT) to Opapimiskan Lake, ON (CKM8) was requesting taxi and clearance at 0925Z, but there was no flight plan (FP) in the system. Pilot said they filed 45 minutes earlier. The Winnipeg Area Control Centre (CZWG) Air Traffic Operations Specialist (ATOS) was called for the FP and they had nothing in the system. The pilot re-filed with the London, ON (CYXU) flight information centre (FIC) over the radio but still nothing was received by the CZWG ATOS FP. Flight plans were incorrectly routed again. Pilot called CZWG ATOS directly to file for a third time. C-GMWT was delayed approximately 15 minutes for departure. No other operational impact.

C-GLTZ was observed taxiing southbound on the closed portion of Taxiway Golf, outside the lights, about to pass Apron 2. C-GLTZ was told to stop and turn onto Apron 2. No impact on operations.

A Government of Canada, Department of National Defence McDonnell Douglas CF-18 Hornet (HRNT01) on an IFR flight from Toronto/Lester B. Pearson, ON (CYYZ) to London, ON (CYXU) declared an emergency en route. HRNT01 landed safely and taxied to parking under its own power. Minimal impact on operations. Runway 33 was unusable for approximately 10 minutes for MAAS crew to remove arrester cable.

A Chartright Air Inc. Beech 1900D (C-GORA/HRT326) from Hamilton, ON (CYHM) to Atlanta, GA (KATL) declared an emergency due to a trim issue and diverted to London, ON (CYXU). Requested emergency equipment. The aircraft landed safely at 1509Z. One aircraft was delayed in a hold for 5 minutes.
UPDATE: TSB Report #A22F0254: C-GORA, a Chartright Air B1900D (BE02) was conducting flight HRT326 from John C. Munro/Hamilton International Airport (CYHM), ON, to Hartsfield-Jackson Atlanta International Airport (KATL), GA, when approximately 20 minutes after departure the aircraft levelled off at FL240. The electric trim failed and the crew cut out the system and reverted to manual trim. When they attempted to trim manually the trim wheel would not move. The aircraft was still trimmed nose up and the PF had to apply moderate control force to hold the aircraft level. The crew declared an emergency and diverted to London International Airport (CYXU), ON. The flight landed at CYXU without further incident. Control force in the flare, given the position that the trim was stuck in, was close to normal and there were no issues on landing.
