A Perimeter Aviation Swearingen SW4 aircraft (PAG12) from Abbotsford, BC (CYXX) to Medicine Hat, AB (CYXH) was assigned and accepted the XX9 departure by the Ground controller. PAG12 misread the standard instrument departure (SID) and flew the runway heading. The Victoria (YJ) Terminal corrected the pilot.

Incident at CALGARY INTL AB (CYYC)

A SkyWest Airlines Bombardier CL-600 2B19 (Series 200) (SKW5531) from Calgary, AB (CYYC) to San Francisco, CA (KSFO) was instructed to taxi on Taxiway J and hold short JS. Instruction was read back, however SKW5531 taxied in the opposite direction as instructed. A Pacific Coastal Airlines Saab 340 (PCO3011) from Medicine Hat, AB (CYXH) to Calgary, AB (CYYC) on J was instructed to stop. SKW5531 was able to turn around and continue. SKW5531 was instructed to taxi onto Taxiway G. Instruction read back, however SKW5531 taxied past Taxiway G and was re-cleared onto C.

Incident at MEDICINE HAT AB (CYXH)

A Pacific Coastal Airlines Saab 340B (C-GPCF/PCO3012) on a flight from Calgary Intl, AB (CYYC) to Medicine Hat, AB (CYXH) was inbound for the area navigation (RNAV) Runway 21 via ROLTI, and cancelled IFR on 10 mile final Runway 21. Two Cessna 172s were doing circuits on Runway 21, a Super T Management Cessna 172N (C-FBWG) on a flight from Medicine Hat, AB (CYXH) to Medicine Hat, AB (CYXH) and a Super T Management Cessna 172P (C-GGAD) on a flight from Medicine Hat, AB (CYXH) to Medicine Hat, AB (CYXH). While C-GGAD was on the downwind leg, the flight service station (FSS) provided a traffic update of PCO3012 on 10 mile final. C-GGAD turned base leg with PCO3012 on 7 mile final. Multiple traffic updates were provided to both aircraft. Company suggested overshoot for C-GGAD, and C-GGAD initiated an overshoot, PCO3012 went into the missed approach. PCO3012 and C-GGAD rejoined the circuit for Runway 21 and landed without issue.

A Pacific Coastal Airlines Limited Saab 340B (C-GOIA/PCO3011) from Medicine Hat, AB (CYXH) to Calgary, AB (CYYC) received clearance for departure from Runway 21. Upon taxiing to position on Runway 21, the aircraft advised the flight service station (FSS) of an icing indication issue and taxied back to the apron. Area Control Centre (ACC) advised, clearance cancelled. No impact to operations.

The pilot of a Super T Management Ltd. Cessna 172N (C-GPHW) on a flight from Medicine Hat, AB (CYXH) to Medicine Hat, AB (CYXH) called via cell phone to advise of total electrical failure. Aerodrome advisory service (AAS) was provided over telephone. The pilot was asked if they were declaring an emergency, and they indicated they were not. Communications established with aircraft entering the mandatory frequency (MF). The aircraft landed Runway 03, no impact on operations.

Incident at MEDICINE HAT AB (CYXH) (Aerodrome - visual aids)

At Medicine Hat, AB (CYXH), the flight service station (FSS) arrived on position and lights were on continuous rather than aircraft radio control of aerodrome lighting (ARCAL). NOTAM had been submitted the previous night to begin at 0735Z until 1800Z, declaring the ARCAL as unserviceable (U/S). FSS contacted the airport and they informed that the night that the Pacific Coast flight was unable to land was due to the ARCAL not working. Airport came in to turn the lights on continuous and issue the NOTAM. It was determined that the ARCAL were not turned on.

Cessna 172P (C-FDJI) on a flight from Medicine Hat, AB (CYXH) to Medicine Hat, AB (CYXH) too closely on final and was forced to initiate a missed approach. JZA7995 was able to rejoin the left downwind (LDW) and land Runway 21 without issue.

Airport advised to remove bird.

Numerous attempts were made to contact the pilot, none were successful. A few radio squelch breaks an interference heard. The aircraft performed a stop and backtrack on Runway 21 then departed south west (SW).

The pilot advised that a group of crows on east side of the threshold of Runway 21 became airborne while on short final. Pilot elected to do a overshoot. No collision with crows.

TSB #A19W0076: a privately operated Cessna 180 aircraft, was conducting a local flight from the Medicine Hat (CYXH), AB with the pilot and a passenger on board. Upon touching down in a crosswind landing at CYXH, the left side of the pilots yoke broke, resulting in a momentary loss of control. The aircraft ground looped, before coming to rest on the surface of the runway. There were no injuries to the 2 occupants, and the aircraft received minor damage to the left wing tip and the left elevator tip.

Incident at In the vicinity of: MEDICINE HAT AB (CYXH)

TSB #A19W064: C-GTQC, a Boeing 737-800 aircraft operated by Air Transat, was conducting flight TSC873 from Calgary Intl (CYYC), AB to Toronto/Lester B. Pearson Intl (CYYZ), ON with 6 crew members and 186 passengers on board. Approximately 30 minutes after the departure from CYYC during cruise flight, the cabin crew observed smoke coming from a galley oven that was heating meals. The cabin crew pulled the associated circuit breaker and, several minutes later, the oven door was opened. No sign of fire was observed, and the smoke began to dissipate. The flight continued to CYYZ without further incident. The operator’s maintenance found the oven contaminated with food spills.

Accident at HANNA AB (CEL4) (Aerodrome, runway or taxiway shutdown)

Pilot of a Super T Aviation Cessna 172P (C-FEUO) from Calgary/Springbank, AB (CYBW) to Hannna, AB (CEL4) called to report that they had crash landed the aircraft at CEL4. The pilot was the only person on board and they indicated that they were OK. The aircraft was 20 feet off the runway and on fire. Emergency services were notified. The Transportation Safety Bureau (TSB) was contacted by the aircraft owner. The Airport Manager (APM) at CEL4 was notified and the runway was closed via NOTAM.
Update TSB #A19W0101: C-FEUO, a Cessna 172 aircraft operated by Super T Management Ltd. was on a night crosscountry flight from Medicine Hat (CYXH), AB to Calgary Springbank (CYBW), AB to Hanna (CEL4), AB and then return to CYXH. On the approach into CEL4, the private pilot decided to commence a go-around and try another approach. On the go-around, control of the aircraft was lost. The aircraft crashed on airport property and a small fire erupted. The pilot was able to exit the aircraft with minor injuries and called for help; police, fire, and EMS attended the scene. The 406 ELT activated and was detected by overflying aircraft, as well as the Joint Rescue Coordination Center (JRCC). The aircraft was substantially damaged.

Accident at BROOKS REGIONAL AB (CYBP)

TSB #A19W0041: C-GQGF, a Piper PA-28-181 aircraft operated by Excel Flight Training, was conducting a studentpilot solo cross-country training flight from Medicine Hat (CYXH), AB to Brooks Regional (CYBP), AB. During the landing on Runway 30 at CYBP, the pilot landed long and decided to abort the landing after touchdown. The pilot applied full throttle, however the aircraft departed the end of the runway and struck the airport perimeter fence. The aircraft came to rest approximately 200 feet beyond the paved surface of the displaced threshold of the runway. There were no injuries, and the ELT did not activate. The aircraft received damage to the propeller, and the wing leading edges received substantial damage from contact with the airport perimeter fence posts.

The Automated Weather Observation System (AWOS) was reset by Calgary Technical Operations. AWOS wind sensors appeared unserviceable (U/S). NOTAM issued. Operational impact involved wind data being unavailable from 0345Z to 1245Z, when the station is closed.
