Incident at QUÉBEC / JEAN LESAGE INTL QC (CYQB) (Communication navigation surveillance/air traffic system)

The Air Traffic Services (ATS) tower cancelled the take-off clearance for a Chrono Aviation Pilatus PC-12/45 (C-FCVD/NDL534) from Quebec/Jean Lesage, QC (CYQB) to Wabush, NL (CYWK), because they had not received validation from the Capital sector. No impact on operations.

Incident at QUÉBEC / JEAN LESAGE INTL QC (CYQB)

Air traffic services (ATS) assigned 155knts to a Chrono Aviation Pilatus PC-12 (NDL543) from Quebec/Jean Lesage Intl, QC (CYQB) to Wabush, NL (CYWK) behind a Grondin Transport Beech A100 (C-FDOV) from Quebec/Jean Lesage Intl, QC (CYQB) to Baie-Comeau, QC (CYBC) that was assigned 160knts or higher. ATS observed NDL543 accelerate and catch up to C-FDOV. ATS attempted contact with NDL543 on 4 occasions without success. When NDL543 communicated, ATS had to give vectors reassigning their speed in order to maintain separation. ATS requested that NDL543 be vigilant in listening to the frequency.

Incident at 79NM NNW SEPT-ÎLES QC (CYZV)

A Pascan British Aerospace Jetstream 3212 (PSC736) from Wabush, NL (CYWK) to Quebec/Jean Lesage, QC (CYQB) asked to divert to Sept-Îles, QC (CYZV) due to a mechanical problem. Two minutes later, the aircraft squawked 7700 and declared an emergency. No impact on operations.
UPDATE TSB Report #20Q0032: C-GPSK, a Pascan Jetstream 32 (PSC736), was on a regular IFR flight from Wabush, NL (CYWK) to Quebec Jean Lesage, QC (CYQB). While the aircraft was 79 nautical miles north-northwest of Sept-Îles, QC, the crew received an indication of an anomaly in the right engine air bleed system. The Quick Reference Handbook (QRH) checklist was completed, the right engine was turned off as a precaution, and the crew declared an emergency. The aircraft diverted to Sept-Îles, QC (CYZV), where it landed without incident. There were no injuries.

A Government of Quebec Canadair CL600-2B16 (series 601-3R) (C-GURG/QUE10) from Mont-Joli, QC (CYYY) to Quebec/Jean Lesage, QC (CYQB) conducted a missed approach on Runway 24 due to an instrument problem. The aircraft received and correctly read back the instruction to turn left on a heading of 150 and to climb to 3 000 ft. Impact: separation was reduced to 2.7 NM and 800 ft with the aircraft that preceded it due to late execution. No other impact on operations.
UPDATE TSB #A20Q0019: C-GURG, a Service aérien gouvernemental Canadair CL-600-2B16 (QUE10), was on an instrument flight rules (IFR) flight from Mont-Joli, QC (CYYY) to Jean-Lesage, QC (CYQB). C-GDTL, a Panorama Fixed Wings Pilatus (NDL543), was on an instrument flight rules (IFR) flight from CYQB to Wabush, NL (CYWK). QUE10 initiated a missed approach RWY 24 due to an instrument problem. It received and correctly read back the instruction to turn left on a heading of 150 degrees and to climb to 3 000 feet. When it reached 2 900 feet in altitude, separation with NDL543, which had taken off before it, was lost. Separation was reduced to 2.7 nautical miles (NM) horizontally and 800 feet vertically, while 3 NM horizontally or 1 000 feet vertically was required between the 2 aircraft. No injuries or damage.

Incident at WABUSH NL (CYWK) (Aerodrome - power failure, ATM - weather observation systems)

Commercial power to Wabush, NL (CYWK) failed prior to the site opening. To conduct an emergency repair, the backup power was removed at approximately 1905Z. Most equipment remained on the uninterruptible power supply (UPS). The Observation Data Exchange (ODEX) failed and weather was unable to transmit. The Technical Operations Coordinator (TOC) was advised. Power was restored at approximately 2012Z. All equipment reset to normal. No impact on operations.

The Wabush, NL (CYWK) aerodrome experienced a commercial power failure due to a lightning strike at approximately 2206Z. All the equipment was on backup and serviceable, except the very high frequency directional finder (VDF). The Technical Operations Coordination Centre (TOCC) was advised. Commercial power was restored at approximately 2235Z, and the VDF was serviceable.

Incident at 10NM NE WABUSH NL (CYWK)

The pilot of a Canadian Helicopters Aerospatiale AS 355 N (C-FKNQ) on a flight to Wabush, NL (CYWK) advised the CYWK flight service station (FSS) that one of their engines had failed while inbound to CYWK and that they were not declaring an emergency. Subsequently, the pilot advised that the engine had been restarted. C-FKNQ landed without incident. No impact on operations.
Update: TSB Report #A20A0026: C-FKNQ, a Eurocopter AS 355 N helicopter operated by Canadian Helicopters Ltd, was on approach for landing at a worksite about 10NM Northeast of Wabush airport (CYWK), NL. The right engine chip light illuminated during the approach and the pilot conducted an overshoot, reduced power and then shut down the right engine (Turbomeca Arrius 1A) as per checklist. The pilot diverted to CYWK, and restarted the right engine for the landing. The helicopter landed without further incident. Company maintenance carried out the right hand engine inspection for chip light illumination as per the manufacturer instructions, and operated the engine with no recurrence of the engine chip light.

Incident at WABUSH NL (CYWK) (Aerodrome - operations, ATM - operations)

At approximately 2110Z, fire detectors alarms at Wabush, NL (CYWK) started going off. Airport Manager advised that everyone must evacuate the building, including the flight service station (FSS). At approximately 2121Z, the Montreal (ZUL) Area Control Centre (ACC) was notified, evacuation procedures were followed, and the FSS evacuation site was established a few minutes after. At approximately 2133Z, the FSS returned to normal operations, and all were advised. No known traffic, no known impact to aircraft operations.

Incident at 10NM SEPT-ÎLES QC (CYZV)

TSB Report #A19Q0165: A PAL Provincial Airlines DHC-8 was on flight PAL1901 from Wabush, TN (CYWK) to Sept-Îles, QC (CYZV). While in descent for its approach for CYZV, the flight crew had to perform an evasive manoeuvre after it had received a traffic alert and collision avoidance system resolution advisory (TCAS RA) concerning an unidentified target. The two aircraft passed within approximately 200 ft of each other. PAL1901 continued on its route and landed at CYZV without any problems. The other aircraft was a helicopter with a transport sling approximately 10 NM north of CYZV. The helicopter was not in radio communication with Sept-Îles.

Accident at 56.6NM N SEPT-ÎLES QC (CYZV)

An American, corporately operated Cirrus SR22 (N699RS) from Wabush, NL (CYWK) to Quebec/Jean Lesage, QC (CYQB) declared an emergency and requested to divert to Sept-Îles, QC (CYZV). En route to CYZV, the aircraft made an emergency landing using its parachute: 510072N/0670097W. The emergency locator transmitter (ELT) signal went off. Joint Rescue Coordination Centre (JRCC) Trenton was notified. Thanks to the help of aircraft in the area, the crash site was located. There was one person on board, who was safe and sound.
Update TSB #A19Q0119: N699RS, a privately operated SR22, was conducting an IFR flight from Wabush Airport, NL, to Quebec City/Jean-Lesage Intl, QC, at an altitude of 8,000 feet ASL. The pilot, who was alone on board, noted a drop in engine oil pressure and high oil temperature. The pilot declared an emergency and requested a diversion to Sept-Îles Airport (CYZV), QC. Approximately 60 NM north of CYZV, the engine (TELEDYNE CONTINENTAL MOTORS IO-550) stopped. The pilot glided the aircraft down to approximately 4,000 feet before deploying the aircraft’s parachute system. The aircraft crashed into trees about 56 miles north of CYZV and sustained significant damage. The pilot was not injured. The pilot activated the SOS mode of his Garmin InReach hand-held GPS. He was rescued about five hours after the crash by Canadian Armed Forces Search and Rescue.

At departure from CYYY, a Provincial Airlines Beech 1900D (C-GPBG/PVL903) from Mont-Joli, QC (CYYY) to Wabush, NL (CYWK) was seen on the radar approximately 10 NM northeast of CYYY, flying direct NORUS, even though it had received clearance for CYYY ME NORUS NANRO. PVL903 entered protected airspace for the approach of a Max Aviation Beech B100 (C-GJPL/MAX140) from Quebec, QC (CYQB) to Baie-Comeau, QC (CYBC). No other impact on operations.
UPDATE from operator: Pilot reported that the clearance "YYY NORUS NANRO YWK CYWK" was copied down and read back to YYY radio. The pilot stated that it was only after they were airborne and switched over to Montreal that they were told to go to waypoint ME. Pilot reported that there was no traffic alert and collision avoidance system (TCAS) alert or any aircraft showing for 20 miles plus on TCAS. Pilot stated that there were absolutely no near mid-air collisions.
