Incident at 6045N / 07269W

At 1915Z, a Cargojet Airways Ltd. Boeing 767-39H (C-FGSJ/CJT462) from Winnipeg/Richardson, MB (CYWG) to Iqaluit, NU (CYFB) was cleared for DCT YFB. At 1934Z, an automatic dependent surveillance – contract (ADS-C) report indicated that the flight was not following the cleared route even though the pilot confirmed that he was following DCT YFB. The flight was seen 2 NM off course and straying away from the cleared route. No impact on operations.

Incident at Kelowna

A Pacific Coast Jet Charter Inc. Cessna 525B (N525CR/PXT525) from Kelowna, BC (CYLW) to Seattle/Boeing Field, WA (KBFI) was given taxi instructions that included a hold short of Runway 16. PXT525 read back the hold short but, upon taxiing, crossed over the hold line and incurred on Runway 16 with a WestJet Encore Ltd. de Havilland DHC-8-402 (C-GWFE/WEN3181) from Winnipeg/Richardson, MB (CYWG) to Kelowna, BC (CYLW) on a 5 mile final. The arriving WEN3181 was instructed to reduce as safely able and continue approach as an aircraft had incurred and was clearing the runway. The arriving WEN3181 was given their landing clearance at 3NM final and landed safely. No other operational impact.

Aviation Incident Report #16554: The pilot of a WestJet flight on a flight from Winnipeg/James Armstrong Richardson Intl, MB (CYWG) to Toronto/Lester B. Pearson Intl, ON (CYYZ) called in via satellite phone to alert of two guests onboard that were acting strangely. Police were arranged for arrival to meet the aircraft, but were not as concerned as the passengers were escorted to their ride home by a Westjet employee.

A Flair Airlines Boeing 737-4Q8 (C-FLHJ/FLE134) from Winnipeg/James Armstrong Richardson, MB (CYWG) to Toronto/Lester B. Pearson, ON (CYYZ) declared a PAN PAN emergency when 4NM final for Runway 24R at CYYZ due to a hydraulic issue. They elected to continue the approach with no equipment required. The aircraft landed safely at 2037Z and taxied to the gate without further incident.
UPDATE TSB Report #A20O0038: Flair Airlines flight FLE134, a Boeing 737-400 (C-FLHJ) was on final approach to Toronto /LBPIA (CYYZ) Runway 24R, when a master caution illuminated and a number of indications were lost, including: hydraulic, engine, electrical, flight control, and pneumatic. The flight crew declared a PAN PAN and determined that it was only indication problems. The aircraft landed on Runway 24R without further incident, taxied clear of the runway, and had the aircraft inspected for hydraulic leaks.

Incident at BOXUM intersection

After departure, a Sky Regional Airlines Embraer ERJ 170-200 SU (C-FEJL/SKV7557) from Toronto/Lester B. Pearson, ON (CYYZ) to Winnipeg/James Armstrong Richardson, MB (CYWG) indicated they had an issue with their nose gear door, and would like to return to CYYZ, after burning off fuel to reduce landing weight. SKV7557 entered a hold at BOXUM to burn off fuel. No emergency was declared. SKV7557 landed on Runway 23 at 2108Z without further incident (WOFI). No operational impact.

TSB Report #A20C0029: C-FSKA, a Beech A100 aircraft operated by Kississing Lake Lodge, was on a local flight at Winnipeg/James Armstrong Richardson Intl (CYWG), MB, with a crew of 2 and 1 passenger on board. While on final approach to Runway 36 at CYWG, the crew extended the landing gear and received a nose gear unsafe indication. Following an emergency gear extension, the unsafe gear indication remained. The crew conducted a fly-by the tower to assess the gear position. The nose gear appeared to be down and an emergency was declared. Prior to touchdown, the crew shut down both engines and the aircraft then landed safely.

Accident at OAK HAMMOCK AIR PARK MB (CAV9)

The Winnipeg duty shift manager received a call regarding an aircraft accident at Oak Hammock Air Park, MB (CAV9) from the provincial 911 services. There was little information given. The National Operations Centre (NOC) was advised by the Aviation Operations Centre (AVOPS). A privately registered, amateur-built Pegazair-100 ADAC flipped on landing. There was 1 person on board (POB), no injuries, no impact to operations.
Update TSB Report #A20C0035: A privately registered Pegazair-100 ADAC aircraft, was conducting a local fight from Oak Hammock Air Park (CAV9), MB with only the pilot on board. On the downwind leg the pilot selected carburetor heat between mid and 3/4 travel. While turning from base leg onto a final approach for Runway 36, the engine power began to falter after reducing power from 2000 rpm to 1500 rpm. There was no throttle response and the engine (Teledyne Continental Motors O-200) stopped a few seconds later while the aircraft was roughly 200-300 feet above ground level. The pilot checked that the fuel tank was selected "on". Carburetor heat was "on". Magnetos were on "both". The pilot maintained control and conducted a forced landing into a field approximately 0.5 nautical miles from the runway. After the 3 point touchdown the aircraft decelerated rapidly due to the soft, muddy terrain. The tail rose and the aircraft overturned and came to rest inverted. The pilot was uninjured and was able to exit the aircraft. The pilot signaled to people at the airport and returned to the aircraft to shut of the fuel and electrical system. EMS responded to the scene and evaluated the pilot. The aircraft sustained damage to the wings, cowling, tail, and a wing strut. The aircraft was recovered back to the airport. At Winnipeg/James Armstrong Richardson Intl, MB (CYWG) the temperature and dew point were reported to be 15 C and 3 C respectively at 1900 UTC, shortly before the occurrence.

After landing on Runway 18, a Vanguard Beech 200 (C-FDEB/VN08M) from St. Theresa Point, MB (CYST) to Winnipeg/James Armstrong Richardson, MB (CYWG) reported frozen brakes. The Winnipeg Airport Authority (WAA) escorted the ambulance onto the runway to transport the patient from the aircraft. VN08M was able to free the brakes and taxi to the apron. Runway 18 was unusable for 44 minutes. No other aircraft impacted.

UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. On approach into Norway House, MB (CYNE), the nose gear of flight PAG117 failed to come down upon gear down selection. It remained black while the main gear did the red in transit and gear down and locked. On approach into Winnipeg/Richardson, MB (CYWG), the gear all came down three green and locked on the first attempt. Aircraft has not flown since the CADOR was generated and maintenance troubleshooting is ongoing.

TSB Report #A20C0019: C-GJVO, a Fairchild SA227-DC aircraft operated by Perimeter Aviation as Bearskin Airlines flight 312, departed the Winnipeg/James Armstrong Richardson Intl. (CYWG), MB. for Red Lake (CYRL), ON with a crew of 2 and 5 passengers on board. Shortly after takeoff, the crew received a fire warning indication on the right engine. The crew decided to shut down the engine and return to CYWG. An emergency was declared and the crew then completed a successful single engine landing. An inspection of the aircraft by the operators maintenance revealed a faulty fire probe in the right engine bay area.

UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. On C-GJYZs takeoff, the #2 engine hydraulic pump caution light illuminated for approximately 1 minute, and the #2 system lost 1 qt. On return to Winnipeg/Richardson, MB (CYWG), gear down selection system gained 0.5 qt fluid. Rectification involved the #2 hydraulic system being serviced and bled.

TSB Report #A20W0024: C-GXWJ, a WestJet Boeing 737-600 operating as WJA493, was conducting a flight from Winnipeg (CYWG), MB to Calgary (CYYC), AB. During descent, the flight experienced a cabin depressurization which resulted in the cabin masks deploying and the cabin altitude warning activating. The flight crew increased their rate of descent and advised ATC. No PAN or MAYDAY was declared. The aircraft landed safely a 1300 MDT. Update: Maintenance replaced the outflow valve and oxygen generators and the aircraft was returned to service.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. Nil emergency declared. Outflow valve and all depleted oxygen generators were replaced. The crew O2 bottle was replenished. Cabin pressure leak check accomplished, and a test flight was conducted. Aircraft was returned to service.

A Government of Canada Department of National Defence Lockheed C-130 Hercules (ATLS33) from Winnipeg/James Armstrong Richardson, MB (CYWG) to Winnipeg/James Armstrong Richardson, MB (CYWG) declared an emergency due to an engine out and requested vectors to CYWG. Aircraft was VFR and proceeded direct to the field and landed without further incident at 1646Z. No impact to operations.

Incident at CALGARY INTL AB (CYYC)

Aviation Incident Report #16567: A call was received from the Calgary, AB (CYYC) airport that local police were removing a passenger from a WestJet Boeing 737-7CT (C-GWBT/WJA202) on a flight from Calgary Intl, AB (CYYC) to Winnipeg/James Armstrong Richardson Intl, MB (CYWG). This passenger had been arrested and released in Edmonton, AB (CYEG) earlier that day after an incident on another airline aircraft and was therefore denied boarding in CYEG. After the arrest, the passenger then bought a ticket on WestJet for flight WJA202 and departed to CYYC before anyone realized it. WestJet Corporate Security notified CYYC staff that the passenger was to be denied boarding. Local police were called to assist as the passenger was already on the plane. Passenger was removed from the flight with no incident and minimal delay to the flight.

Incident at REGINA INTL SK (CYQR)

A WestJet Encore de Havilland DHC-8-402 (C-GWUE/WEN3273) from Winnipeg/James Armstrong Richardson, MB (CYWG) to Regina, SK (CYQR) conducted a missed approach on final for Runway 31, due to a programming error in the Flight Management System (FMS). WEN3273 returned to Winnipeg Centre to conduct another approach. WEN3273 landed safely at 1820Z with no impact on operations.

Incident at WINNIPEG / JAMES ARMSTRONG RICHARDSON INTL MB (CYWG)

Aviation Incident Report # 16572: A guest was witnessed by crew members of a WestJet flight leaving their seat and going into the aft lavatory. Upon exiting the lavatory, an odor of cigarette smoke was present which was confirmed by crew. Crew confirmed that they had a conversation with the guest who admitted to smoking. The Royal Canadian Mounted Police (RCMP) met the aircraft upon arrival at gate, boarded the aircraft and escorted the guest off.
