An Air Tindi Beech B200GT (C-GYKI/TIN510) from Yellowknife, NT (CYZF) to Norman Wells, NT (CYVQ) was on approach and advised of birds on the runway. A "low and over" was conducted and the birds immediately returned. A second "low and over" was conducted with the same result, while waiting for vehicle assistance. TIN510 safely arrived with a 15-minute delay.

Incident at YELLOWKNIFE NT (CYZF)

A Canadian North Boeing 737-200 (MPE245) on a flight from Norman Wells, NT (CYVQ) to Yellowknife, NT (CYZF) received clearance for departure to CYZF at 2241Z. At approximately 2246Z, MPE245 rejected takeoff. The pilots indicated an alarm sounded in regards to flap configuration. MPE245 returned to the threshold of the runway, then taxied back to the apron due to incoming traffic. MPE245 departed without incident at 2338Z.

A Canadian Helicopters Bell 412EP (C-GZEY) from Norman Wells, NT (CYVQ) to destination unknown received an advisory, and planned to depart Runway 28. A Canadian North Boeing 737-300 (MPE245) from Norman Wells, NT (CYVQ) to Yellowknife, NT (CYZF) made initial contact from the apron for Runway 10. Advisory and traffic information was passed, and MPE245 proceeded onto the runway in use via Taxiway Alpha. The flight service station (FSS) passed traffic on the runway information to C-GZEY. Subsequently, they taxied off Runway 28 south and elected to depart Westbound (WB) parallel to Runway 28.

Incident at In the vicinity of: NORMAN WELLS NT (CYVQ)

Norman Wells, NT (CYVQ) flight service station (FSS) received multiple calls starting at 2320Z with just static. At 2325Z, Northwrights office called the FSS and informed that a North-Wright Airways Cessna 207 (C-GDBI) from Tulita, NT (CFZN) to Norman Wells, NT (CYVQ) was inbound with a failed radio. Relevant information was passed, and no radio (NORDO) procedures followed. No impact on operations.
UPDATE from Aircraft Maintenance & Manufacturing: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. The aircraft was enroute from Tulita, NT (CFZN) to Norman Wells, NT (CYVQ) when radio operation was lost. On approach to CYVQ, the pilot was able to use their cell phone to contact the Company which relayed instruction from the flight service station (FSS) to the crew, and the aircraft landed without further incident. Company maintenance inspected the aircraft and found that the "Push To Talk" switch had failed. The switch was replaced, tested and the aircraft was returned to service. There have been no further reported incidents.

A Canadian North Boeing 737-36N (C-FGCN/MPE244) on a flight from Norman Wells, NT (CYVQ) to Inuvik/Mike Zubko, NT (CYEV) commenced the missed approach for the area navigation (RNAV) 24 at 2119Z due to gusting crosswind component. They then cancelled the IFR and landed on Runway 24 without incident at 2132Z.

At 0216Z, Edmonton (YEG) center (north low) advised tower that a Canadian North (C-FGCN/MPE245) from Yellowknife, NT (CYZF) to Norman Wells, NT (CYVQ) would be diverting to Fort McMurray, AB (CYMM) for a medical emergency. Emergency list was followed while additionally calling firehall for added assistance.

Incident at TULITA (FORT NORMAN) NT (CZFN) (Communication error, Incursion - runway - vehicle)

A blower and grader were operating on the Tulita (Fort Norman), NT (CZFN) runway but did not advise the community aerodrome radio station (CARS) operator when the CARS station was broadcast open at 1546Z. A North-Wright Airways Mooney M20C (C-GNWH/NWL125) on a flight from Normal Wells, NT (CYVQ) to Tulita (Fort Norman), NT (CZFN)) contacted the CARS at 1623Z with an estimated time of arrival (ETA) of 1633Z. The CARS operator was not aware of the ground traffic so it was not passed. The blower operator heard the aircraft call to CARS. The graders radio was unserviceable. The blower went back to the Grader to tell them to leave the runway. The arrival of NWL125 was delayed 5 minutes. No other impact on operations.

Aircraft advised when airborne and returned to CYVQ. Arrived at 2130Z without incident and departed again at 2143Z. No impact to operations.

Incident at NORMAN WELLS NT (CYVQ)

Emergency Locator Transmitter (ELT) signal went off briefly, then again one minute later, then became continuous on 121.5. Operations Specialist (ATOS) was called and informed that local companies were being contacted (a recently landed aircraft was suspected to be the source). North Wright and Great Slave Helicopters were contacted, to confirm with both that they did not have any inbound aircraft and to have them check their machines. Signal source based off the very high frequency directional finder (VDF) suggested a local Beechcraft 1900 (B190). ATOS called back and confirmed it was a Kenn Borek Air Ltd. Beech 1900D (C-GHUE) sending the signal as anticipated (C-GHUE had landed at 2321Z). North Wright was informed, maintenance arrived, and ELT turned off at 0006Z.

Aircraft identified their ELT was unintentionally deployed and they would advise maintenance. Edmonton (CZEG) flight data was advised. No impact to operations.

The aircraft was vectored for noise abatement and then on-course. No impact to operations.

Accident at 72 NM W NORMAN WELLS NT (CYVQ)

TSB #A19W0096: C-GOOY, a Piper Aircraft Corporation PA-18-150, operated by Gana River Outfitters, was conducting a private flight in the Mackenzie Mountains approximately 72 nm West of Norman Wells, NT (CYVQ) with only the pilot onboard. While executing a landing at a possible future landing site, the wind direction changed during the landing roll. As there was insufficient distance remaining to initiate a go around, the pilot applied hard braking which resulted in the aircraft nosing over and coming to a rest upside down on its roof. The pilot exited the aircraft and was uninjured. The aircraft sustained substantial damage.

A Canadian Helicopters Aerospatiale AS 350 B-2 (C-GRBT), origin and destination unknown, reported inbound from the southeast to overfly Norman Wells, NT (CYVQ) northbound. When the estimated time of arrival (ETA) was reached, CYVQ radio attempted to make contact and was unsuccessful. Alerting action was taken 5 minutes later. Aircraft reported safe approximately 40 minutes later. No significant impact to operations.

At 1340Z, a Gana River Outfitters Ltd. Piper PA-18-150 from origin unknown to Norman Wells, NT (CYVQ) was heard broadcasting on 126.7 inbound to CYVQ with an estimated time of arrival (ETA) of 10 minutes. Aircraft did not contact the CYVQ flight service station (FSS) on 122.2 prior to entering the mandatory frequency (MF) area. The FSS contacted the aircraft on 126.7 when the pilot was heard broadcasting that they were on base for Runway 10 and switched them to 122.2. Aircraft landed safely at 1349Z. No impact on operations.

A NorthWestern Air Lease Cessna U206E (C-GWQW/PLR407) from Norman Wells, NT (CYVQ) to Norman Wells, NT (CYVQ) departed at 1524Z and at 1534Z advised they were returning to CYVQ due to high oil temperature, no assistance required. The aircraft landed safely at 1546Z and departed safely at 1600Z. No impact on operations.
UPDATE from Maintenance: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. After the aircraft departed from Norman Wells, NT, the pilot noted the engine oil temperature was increasing past its normal range, and the aircraft returned and landed without further incident. The pilot realized that the outside air temperature had warmed from the day before and that the engine winter fronts were still installed. The winter fronts were removed and the flight continued with no further issues.

A North-Wright Airways Ltd. Cessna U206G (C-GZVX) from Norman Wells, NT (CYVQ) to Norman Wells, NT (CYVQ) contacted the CYVQ flight service station (FSS) via phone indicating they had lost communications and were returning to CYVQ. C-GZVX arrived safely. No impact to operations.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. The aircraft was on a local (Norman Wells area) Company training flight when it experienced being unable to transmit radio communication. The crew contacted the Flight Service Station via cell phone and the aircraft returned and landed without further incident. Company maintenance inspected the aircraft and found that the push to talk switch was faulty. The switch was replaced with new unit and checked serviceable. The aircraft was returned to service with no further reported incidents.

At 1708Z, a North-Wright Airways Beech 1900 (NWL141) from Norman Wells, NT (CYVQ) to Fort Good Hope, NT (CYGH) contacted the CYGH airport radio on 121.1, reporting inbound at 17NM Southeast, estimating to land at 1715Z on Runway 07. The aircraft was given the airport advisory and weather conditions. Vehicles were removed from the runway. No further communications could be obtained from the aircraft on the mandatory frequency (MF) or on 121.5 until 1725Z, 10 minutes past the stated estimated time of arrival (ETA). Communication resumed when the aircraft called at 3 miles final, at 1725Z, and landed safely at 1726Z.

A Great Slave Helicopters Ltd. Bell 205B (C-FNTR) on a flight from Norman Wells, NT (CYVQ) lifted off with a sling load. After it crossed the runway, it advised it was returning. The specialist noticed an item fall from the sling load near the runway. The helicopter was advised. The helicopter re-secured the sling load and departed a few minutes later. The airport authority was advised and a foreign object debris (FOD) check was completed.
