A Jazz de Havilland DHC-8-402 (C-GGFJ/JZA7997) from Sydney, NS (CYQY) to Montreal/Trudeau, QC (CYUL) reported a bird strike (with a family of ducks on the runway) while landing on Runway 24R. A Jazz de Havilland DHC-8-301 (C-GMON/JZA8791) from Saint John, NB (CYSJ) to Montreal/Trudeau, QC (CYUL) decided to conduct a go-around. Several arrivals were redirected to Runway 24L. Several departures from Runway 24L were therefore delayed. A duck was found during the inspection of the runway. JZA8791 landed on Runway 24L at 1107Z.

An American corporately owned Socata TBM 700 (N317WC) from Montreal/Saint-Hubert, QC (CYHU) to Saint John, NB (CYSJ) turned towards VOBOK at departure from Runway 24D, instead of following the standard instrument departure (SID). Air traffic services (ATS) rectified the situation. No impact on operations and no loss of separation.

Incident at In the vicinity of: MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL)

A de Havilland DHC-8-402 operated by Jazz (C-GUJZ/JZA8792) from Montreal/Trudeau International (CYUL), QC, to Saint John (CYSJ), NB, declared an emergency because of a fire in the cockpit. The aircraft landed safely at 0129Z.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. During climb, the Cabin Crew observed abnormal smoke originating from the outboard coffee maker along with a strong smell. After notifying the Flight Crew, the associated circuit breakers were pulled and an orange color glow was observed behind the coffee maker. A portable fire extinguisher was discharged along with a 1.5 liter water bottle. The Flight Crew declared an emergency and executed an air turn back without further incident. The coffee maker and the portable fire extinguisher were replaced before returning the aircraft to service. A Service Difficulty Report (SDR) was submitted. Note: Sugar packets were found behind the coffee maker; however, there was no evidence that it contributed to the incident.
Update - TSB Report #A19Q0003: C-GUJZ, a Jazz Aviation LP Bombardier DHC-8-400, was on instrument flight rules (IFR) flight JZA8792 from Montreal/Pierre Elliott Trudeau Intl, QC (CYUL) to Saint John, NB (CYSJ) with 4 crew members and 66 passengers on board. Approximately 45 NM east of CYUL, when the aircraft was climbing past 16 000 ft ASL, the cabin crew noticed smoke as well as a reddish glow behind the coffee maker. With the use of an extinguisher, the fire was brought under control. The flight crew members put on their oxygen masks, declared an emergency, and requested a return to CYUL, where the aircraft landed without any problems. The aircraft taxied up to the walkway, from which passengers were able to exit the plane. No one was injured and the aircraft sustained no damage.

The Greater Toronto Airport Authority (GTAA) Airline Operations Control Centre (AOCC) was notified and reported the sighting to local police. Departures were instructed to turn left after departing. No impact to operations.

At 1045Z, Air Nunavut Ltd. Dassault Falcon 10 (C-GSXJ/BFF102) from St.John, NB (CYSJ) to Toronto/Oshawa, ON (CYOO) was cleared to 14,000 ft and was instructed to change frequency. BFF102 read back instructions. At 1047Z, BFF102 changed frequency and contacted air traffic control (ATC) stating they were through FL230 for 7000 ft. BFF102 was cleared again to 14,000 ft. No operational impact.

Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ) (FOD (foreign object debris))

Report from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. The following information was provided to Transport Canada Civil Aviation (TCCA) by the aircraft Operator following a full Safety Management System (SMS) investigation into this incident. A forward (FWD) Side Door Assembly (left hand (L/H) Engine – Outboard) was found on Runway 24R in CYYZ January 25, 2019. It was from a Jazz de Havilland DHC-8-402 (C-GGMU/JZA8946) from Toronto/Pearson Intl, ON (CYYZ) to Saint John, NB (CYSJ). The flight crew were unaware that the door had departed the aircraft; indications were normal and there was no vibration. The flight continued to its destination and landed uneventfully. After landing, the flight crew advised that the door impacted the L/H Inboard De-Ice Boot. Another aircraft was dispatched to operate the outbound flight. Company Maintenance Personnel and a replacement Door were on the aircraft. Prior to flight, a Service Check and Oil Check were carried out by CYYZ Maintenance, the aircraft overnighted in CYYZ. The Operator inspected the door and found all latches in the locked position with no obvious damage to any of them. The door support strut was missing. CYYZ was advised about the missing strut. They confirmed they had done a full sweep and runway inspection with no sign of the strut. TSB was notified. A Service Difficulty Report (SDR) was submitted. Maintenance issued a Maintenance Safety Alert to all Maintenance Personnel. The Aircraft was returned to service following replacement of the Door and L/H Inboard Leading Edge Assembly. This is the first event of this type for this Operator; however, there have been other similar events with other operators of this aircraft Type. Additionally, Bombardier released Service Bulletin (SB) which includes installing a placard to each cowl door describing (via symbols) the procedures for properly closing the door. All Jazz aircrafts will have this SB completed if not previously done.
Update TSB Report #A19O0005: C-GGMU, a Bombardier DHC-8-400 aircraft operated by Jazz Aviation LP, was conducting flight JZA8946 from Toronto/Lester B. Pearson Intl (CYYZ), ON to Saint John (CYSJ), NB. During the takeoff from CYYZ, the left hand engine cowl door separated from the nacelle, and struck the left wing leading edge, causing damage to the de-icing boot. The flight continued to CYSJ as the crew were not aware of the event, and there were no cockpit indications or engine vibrations. The flight proceeded uneventfully, and the missing door was only noticed after the aircraft was parked at the terminal. The door was found on Runway 24R at CYYZ. The operator’s maintenance had performed a preflight inspection before the departure from CYYZ, which included engine oil quantity checks in the area of the missing cowl. The operator is investigating.

A Novajet Dassault Falcon 900EX (C-GGBC) from Saint John, NB (CYSJ) to Teterboro, NJ (KTEB) declared an engine failure with the Boston (BOS) area control centre (ACC) and advised returning to CYSJ. The aircraft declared a "PAN PAN" with BOS ACC when 80NM North of the Boston Intl, MA (KBOS) aerodrome. The pilot requested emergency vehicles on standby at CYSJ. The aircraft landed at 2018Z. Audio and radar tapes were reviewed. No impact on other traffic.
UPDATE: CADORS 2019A0318 (AOR 246151-V2) was a duplicate of this occurrence and was deleted.
Update TSB Report #A19A0015: C-GGBC, a Dassault Falcon 900EX aircraft operated by NovaJet, was conducting flight 58343 from Saint John (CYSJ), NB to Teterboro (KTEB), NJ with 2 crew members and 6 passengers on board. During cruise flight approximately 30 minutes after takeoff, the flight crew observed a number 1 engine low oil pressure indication, associated Master Caution light illumination, and subsequent Master Warning indication. The flight crew carried out checklist requirements, including the precautionary shut down of the number 1 engine. A return to CYSJ was requested, the flight crew declared a PAN PAN, and the aircraft landed at CYSJ without further incident. Maintenance personnel replaced the faulty number 1 engine oil filler cap o-ring, and returned the aircraft to service following cleaning, servicing and function tests.

Beech 1900D (EV770) from Saint John, NB (CYSJ) to Halifax/Stanfield, NS (CYHZ) overshot Runway 32 due to the previous arrival, a British Royal Air Force (Air Transport) Airbus A330-200 (RRR9201) from Keflavik, Iceland (BIKF) to Halifax/Stanfield, NS (CYHZ) rolling past Taxiway Hotel and requiring a backtrack to exit. EV770 returned to land at 1934Z.

Accident at SAINT JOHN NB (CYSJ)

On departure, a privately registered, amateur-built Vans RV-6A from Saint John, NB (CYSJ) to Saint John, NB (CYSJ) appeared to veer off the runway into the grass. The aircraft veered to left, and took 2 bumps into air. On third bump in long grass it flipped forward onto its back. One person onboard taken to hospital. All required parties informed of incident. Operational impact: Runway 05/23 closed. Three other aircraft (JZA8788, C-FRKI, and a privately registered, amateur-built Bowers Flybaby 1A) diverted to another destination.
