A privately registered Cessna 150M on a flight from Saint John, NB (CYSJ) to Saint John, NB (CYSJ) departed Runway 23 at 0046Z for circuits. After departure all further transmissions from the aircraft were unreadable. The aircraft landed Runway 23 at 0052Z then taxied back to the fixed base operator (FBO). A short time later, the pilot phoned the flight service station (FSS) and reported an alternator failure and losing radio and transponder. No impact on operations.

Several departures from Runway 24L were therefore delayed. A duck was found during the inspection of the runway. JZA8791 landed on Runway 24L at 1107Z.

Incident at In the vicinity of: MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL)

A de Havilland DHC-8-402 operated by Jazz (C-GUJZ/JZA8792) from Montreal/Trudeau International (CYUL), QC, to Saint John (CYSJ), NB, declared an emergency because of a fire in the cockpit. The aircraft landed safely at 0129Z.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. During climb, the Cabin Crew observed abnormal smoke originating from the outboard coffee maker along with a strong smell. After notifying the Flight Crew, the associated circuit breakers were pulled and an orange color glow was observed behind the coffee maker. A portable fire extinguisher was discharged along with a 1.5 liter water bottle. The Flight Crew declared an emergency and executed an air turn back without further incident. The coffee maker and the portable fire extinguisher were replaced before returning the aircraft to service. A Service Difficulty Report (SDR) was submitted. Note: Sugar packets were found behind the coffee maker; however, there was no evidence that it contributed to the incident.
Update - TSB Report #A19Q0003: C-GUJZ, a Jazz Aviation LP Bombardier DHC-8-400, was on instrument flight rules (IFR) flight JZA8792 from Montreal/Pierre Elliott Trudeau Intl, QC (CYUL) to Saint John, NB (CYSJ) with 4 crew members and 66 passengers on board. Approximately 45 NM east of CYUL, when the aircraft was climbing past 16 000 ft ASL, the cabin crew noticed smoke as well as a reddish glow behind the coffee maker. With the use of an extinguisher, the fire was brought under control. The flight crew members put on their oxygen masks, declared an emergency, and requested a return to CYUL, where the aircraft landed without any problems. The aircraft taxied up to the walkway, from which passengers were able to exit the plane. No one was injured and the aircraft sustained no damage.

As a result, a GoJet Airlines Bombardier CL-600-2C10 (N642CA/GJS6101) from Detroit Metro/Wayne County, IL (KDTW) to Toronto/Lester B. Pearson, ON (CYYZ) and a WestJet Boeing 737-7CT (C-FMWJ/WJA1203) from New York-LaGuardia, NY (KLGA) to Toronto/Lester B. Pearson, ON (CYYZ) were instructed by air traffic services (ATS) to go around. Both landed at 1259Z and 1302Z respectfully, without further issue (WOFI). No Operational Impact.

En route to Saint John, NB (CYSJ), a Porter Airlines de Havilland DHC-8-402 (POE227) from Ottawa/Macdonald-Cartier, ON (CYOW) reported the complete loss of its hydraulic circuit and requested a return to CYOW. POE227 declared an emergency. Coordination carried out. The aircraft landed on Runway 32 at 1818Z. No impact. The National Operations Centre (NOC) and the Aviation Operations Centre (AVOPS) were notified.
Update: TSB Report #A19Q0144: A Porter Airlines Bombardier DHC-8-400 (C-GKQE), was conducting flight POE227 under instrument flight rules from Ottawa Macdonald–Cartier Intl (CYOW), ON, to Saint-John (CYSJ), NB, with 33 passengers and 4 crew members on board. While the aircraft was cruising at approximately 130 NM from CYOW, the No. 1 hydraulic isolation valve caution light illuminated, indicating a hydraulic system loss. The crew declared an emergency and elected to return to land at CYOW. The aircraft landed safely at 1818Z and taxied to the apron. There were no injuries.

Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ) (FOD (foreign object debris))

Report from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. The following information was provided to Transport Canada Civil Aviation (TCCA) by the aircraft Operator following a full Safety Management System (SMS) investigation into this incident. A forward (FWD) Side Door Assembly (left hand (L/H) Engine – Outboard) was found on Runway 24R in CYYZ January 25, 2019. It was from a Jazz de Havilland DHC-8-402 (C-GGMU/JZA8946) from Toronto/Pearson Intl, ON (CYYZ) to Saint John, NB (CYSJ). The flight crew were unaware that the door had departed the aircraft; indications were normal and there was no vibration. The flight continued to its destination and landed uneventfully. After landing, the flight crew advised that the door impacted the L/H Inboard De-Ice Boot. Another aircraft was dispatched to operate the outbound flight. Company Maintenance Personnel and a replacement Door were on the aircraft. Prior to flight, a Service Check and Oil Check were carried out by CYYZ Maintenance, the aircraft overnighted in CYYZ. The Operator inspected the door and found all latches in the locked position with no obvious damage to any of them. The door support strut was missing. CYYZ was advised about the missing strut. They confirmed they had done a full sweep and runway inspection with no sign of the strut. TSB was notified. A Service Difficulty Report (SDR) was submitted. Maintenance issued a Maintenance Safety Alert to all Maintenance Personnel. The Aircraft was returned to service following replacement of the Door and L/H Inboard Leading Edge Assembly. This is the first event of this type for this Operator; however, there have been other similar events with other operators of this aircraft Type. Additionally, Bombardier released Service Bulletin (SB) which includes installing a placard to each cowl door describing (via symbols) the procedures for properly closing the door. All Jazz aircrafts will have this SB completed if not previously done.
Update TSB Report #A19O0005: C-GGMU, a Bombardier DHC-8-400 aircraft operated by Jazz Aviation LP, was conducting flight JZA8946 from Toronto/Lester B. Pearson Intl (CYYZ), ON to Saint John (CYSJ), NB. During the takeoff from CYYZ, the left hand engine cowl door separated from the nacelle, and struck the left wing leading edge, causing damage to the de-icing boot. The flight continued to CYSJ as the crew were not aware of the event, and there were no cockpit indications or engine vibrations. The flight proceeded uneventfully, and the missing door was only noticed after the aircraft was parked at the terminal. The door was found on Runway 24R at CYYZ. The operator’s maintenance had performed a preflight inspection before the departure from CYYZ, which included engine oil quantity checks in the area of the missing cowl. The operator is investigating.

The aircraft declared a "PAN PAN" with BOS ACC when 80NM North of the Boston Intl, MA (KBOS) aerodrome. The pilot requested emergency vehicles on standby at CYSJ. The aircraft landed at 2018Z. Audio and radar tapes were reviewed. No impact on other traffic.

Update TSB Report #A19A0015: C-GGBC, a Dassault Falcon 900EX aircraft operated by NovaJet, was conducting flight 58343 from Saint John (CYSJ), NB to Teterboro (KTEB), NJ with 2 crew members and 6 passengers on board. During cruise flight approximately 30 minutes after takeoff, the flight crew observed a number 1 engine low oil pressure indication, associated Master Caution light illumination, and subsequent Master Warning indication. The flight crew carried out checklist requirements, including the precautionary shut down of the number 1 engine. A return to CYSJ was requested, the flight crew declared a PAN PAN, and the aircraft landed at CYSJ without further incident. Maintenance personnel replaced the faulty number 1 engine oil filler cap o-ring, and returned the aircraft to service following cleaning, servicing and function tests.

Incident at 45NM SAINT JOHN NB VOR/DME (YSJ)

A WestJet Encore Ltd. de Havilland DHC-8-402 (WEN3485) at FL240 reported a strong emergency locator transmitter (ELT) signal west of Saint John, NB (CYSJ). A Moncton Flight College Beech C90A (C-GDHS) at 90 reported a weak ELT in the same area. The Boston (BOS) air route traffic control center (ARTCC) reported that the source was believed to be at Bangor, ME (KBGR). The Halifax Joint Rescue Coordination Centre (JRCC) was advised. No impact on operations.

Incident at 10NM W SAINT JOHN NB (CYSJ)

A privately operated Gulfstream GV (MUSIC) on a flight from Teterboro, NJ (KTEB) to Farnborough, UK (EGLF) declared PAN PAN. The aircraft advised of a cracked windshield and requested diversion to Boston Intl, MA (KBOS) and descent. No impact to operations or further assistance required.
UPDATE TSB Report #A19A0094: M-USIC, a privately operated Gulfstream G550, was on a flight from Teterboro (KTEB), NJ to London/Farnborough (EGLF), UK. During the cruise portion of the flight, while in Canadian airspace, the cockpit windshield cracked. The crew declared a PAN PAN and requested a diversion to Boston/General Edward Lawrence Logan Intl. (KBOS), MA . No other assistance was required.

Incident at SAINT JOHN NB (CYSJ) (FOD (foreign object debris))

Aviation Incident Report #16245: A dead bird (Killdeer) was found at Saint John, NB (CYSJ) on the centreline of Runway 05/23, at midway. The flight service station (FSS) was contacted to verify if there had been a reported bird strike. Station officials replied that no strike had been reported.
