Incident at STONY RAPIDS SK (CYSF)

At 2039Z, a secondary surveillance radar (SSR) target was observed entering the Stony Rapids, SK (CYSF) mandatory frequency (MF) area from the Northwest (NW) at 033 above sea level (ASL) (unverified) without establishing radio communication with the flight service station (FSS). The FSS made a “blind call” on the MF for the aircraft to identify itself and the pilot responded as a privately registered Cessna A185E “at three point five” en route to La Ronge (Barber Field), SK (CYVC).

Incident at PRINCE ALBERT (GLASS FIELD) SK (CYPA) (Aerodrome - power failure, ATM - weather observation systems, ATM - inaccurate aeronautical information)

At Prince Albert (Glass Field), SK (CYPA), a commercial power bump occurred at 0154Z. A loss of the Human Weather Observation System (HWOS), wind, temperature, barometer, etc., lasted for roughly 5 minutes. There was a loss of the mandatory frequency (MF) 122.3 transmission-receiver (TX/RX) circuit on the National Voice Communication System (NVCS). A loss of communication occurred with a Transwest Air Ltd. Saab 340B (C-FJVW/TW103) from Stony Rapids, SK (CYSF) to Prince Albert (Glass Field), SK (CYPA) nearing its final approach. A backup hand-held radio (NICAN) was tuned to 122.3 and appeared to be working. The backup radio was used for further communications. The Technical Operations Coordinator (TOC) was advised.

Incident at STONY RAPIDS SK (CYSF)

The pilot of a privately registered Cessna A185E on a flight to Stony Rapids, SK (CYSF) reported when on final for Runway 24 at 2326Z, however, no report was made when the aircraft was off of the runway. After arranging a ramp check with the Black Lake Royal Canadian Mounted Police (RCMP) unit, the flight service station (FSS) was able to confirm the aircrafts safe arrival with a local at 2341Z.

The pilot of a privately registered Cessna A185E on a flight to Stony Rapids, SK (CYSF) reported they were on final for Runway 06 at CYSF at 1628Z. No call was made once the aircraft was off of the runway. The Royal Canadian Mounted Police (RCMP) unit of Black Lake was contacted to confirm that the aircraft had landed safely, but before they were able to verify, the pilot of the Cessna A185E called to depart for La Ronge, SK (CYVC) at 1650Z. RCMP reported that C-FVZA landed safely at 1802Z. (Duplicate AOR 264640 deleted after information merged)

A Can-West Corporate Air Charters Ltd. Cessna 337G (C-FVAS) from Fond-Du-Lac, SK (CZFD) to Stony Rapids, SK (CYSF) reported a gear indication issue at 2053Z while turning onto final for Runway 06 at CYSF. The aircraft moved off and manually lowered the landing gear. The aircraft landed without further incident at 2057Z.

During cruise, in the vicinity of Stony Rapids (CYSF), SK, a cabin crewmember was injured and was unable to continue their duties. The duties were assumed by the rest of the cabin crew.

An American, corporately registered Bombardier CL-600-2B16 (N951TP) from Stony Rapids, SK (CYSF) to Minneapolis-St. Paul, MN (KMSP) received a departure advisory via the remote aerodrome advisory service (RAAS) at CYSF. The aircraft was next observed on radar airborne. The flight service station (FSS) called the aircraft but received no reply. The pilot did not make the required and requested call when airborne nor did they report clear of the mandatory frequency area (MFA). FSS confirmed with La Ronge Sector that the aircraft was in contact with them.

A North Cariboo Flying Service Ltd. Beech 200 (NCB210) from Stony Rapids, SK (CYSF) to Edmonton, AB (CYEG) requested priority due to an equipment issue. Aircraft landed safely at 1818Z. No impact on operations.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. The Operator was contacted and provided additional information as follows: On descent into Edmonton Intl, AB (CYEG) the aircraft Master Warning and Caution Annunciator came on for the Cabin Door. The crew requested priority handling from Air Traffic Control (ATC) and ran through the emergency checklist. The aircraft landed without incident. Prior to passenger deplaning the cabin door was found to be fully locked. The company’s maintenance department has advised that the aircraft is currently undergoing heavy maintenance and will provide an update when the information becomes available.

While on the take-off roll Runway 24, a Saskatchewan Air Ambulance Beech B200 (C-GSAU/SLG3-M) from Stony Rapids, SK (CYSF) to Saskatoon, SK (CYXE) struck a raven on the starboard side wing. Aircraft aborted take-off and taxied back to apron. Foreign Object Debris (FOD) check completed and produced remains of one raven. Aircraft appeared to sustain damage to starboard side wing. No other impact on operations.

On final for Runway 06, a Transwest Air Limited Beech 200 (C-GWWN) from Fond-du-Lac, SK (CZFD) to Stony Rapids, SK (CYSF) executed a go-around when another aircraft, then holding short, reported a fox encroaching on the runway protected area (RPA). While C-GWWN rejoined the downwind for Runway 06, a West Wind Aviation Beech 1900D (C-FBPK/WEW615) from Stony Rapids, SK (CYSF) to Fond-du-Lac, SK (CZFD) then taxied out, scaring away the animal, and departed Runway 06 at 2129Z. C-GWWN landed without further incident at 2131Z.

When taxiing for departure the main gear brakes of a West Wind Aviation Partnership Beech 1900D (C-FBPK/WEW615) from Stony Rapids, SK (CYSF) to Fond-du-Lac, SK (CZFD) froze up. The crew was able to work them free and taxied off the runway at 2113Z. Operational Impact: CYSF Runway 06/24 was closed by NOTAM (notice to airmen) from 2109Z to 2115Z. No other aircraft delayed.

Cessna 180H (C-FKTT) from Buffalo Narrows, SK (CYVT) to Stony Rapids, SK (CYSF). At 2035Z, C-FKTT departed CYSF, again without establishing communication with FSS. No impact on operations.

A Transwest Air Saab 340B (C-FJVW/TW280) from Fond-du-Lac, SK (CZFD) to Stony Rapids, SK (CYSF) inbound for Runway 06 at CYSF from the west was followed by a Transwest Air de Havilland DHC-6 Series 200 (C-FGLF) landing at CYSF. At 2357Z, C-FGLF advised TW280 that the company had just called to inform them of 2 snowmobiles by the south side of runway. TW280 decided to break off the approach to fly overhead the airfield to confirm. At 2359Z, TW280 confirmed 2 snowmobiles beside the runway and that the company would be notifying the Royal Canadian Mounted Police (RCMP). At 0003Z, TW280 was advised by the company that the snowmobiles had left the area. C-FGLF arrived at 0004Z with no snowmobiles in sight. TW280 arrived at 0010Z. No other impact on operations.
