The Joint Rescue Coordination Centre (JRCC) Trenton called to confirm a 7700 squawk code for a Government of Canada, Department of National Defence Lockheed C-130 (TIGR337) from Trenton, ON (CYTR) to Trenton, ON (CYTR) in the vicinity of Sudbury, ON (CYSB). The aircraft was reached on 126.7 MHz by the specialist, and asked to confirm the squawk code. No emergency was present. The specialist then requested the cycling of the aircrafts equipment, which corrected the emergency transmission.

A Skycare Air Ambulance Israel 1124A (C-GIAW) from Sudbury, ON (CYSB) to Moosonee, ON (CYMO) was taxiing for departure from Runway 22. A vehicle (ST29) and another vehicle were instructed to vacate Runway 22 onto Taxiway Bravo. ST29 called to report that it was off of Runway 22 and proceeding onto the open portion of Runway 30, along with adjacent Taxiways A, C and D. The location where ST29 proceeded onto without permission was along the route C-GIAW was taking for its position for departure. ST29 was allowed to proceed, with instructions to give way. There was no risk of collision and no impact on operations due to ST29 exiting onto Taxiway C, opposite from the taxiway to be used by C-GIAW (Taxiway A), almost immediately after being informed that it had not been given permission to be where it was, and due to the aircraft not yet having entered onto Taxiway Alpha.

A Toronto Airways Inc. Beech 100 (TOR802) on a flight from Cochrane, ON (CYCN) requested a descent to 10,000ft and then stated that it had smoke in the cockpit. An emergency was not declared. TOR802 requested a diversion to Sudbury, ON (CYSB) and was given priority handling. TOR802 landed uneventfully at 2031Z.

A Thunder Airlines Mitsubishi MU-2B-60 (C-GYUA/THU890) from Sudbury, ON (CYSB) to Moosonee, ON (CYMO) was observed to be off of the authorized route and proceeding "direct OMVET". The route known to the air traffic service (ATS) unit was "direct Fort Albany (CYFA), ON". No impact on operations.

A Nolinor Aviation Boeing 737-200 (NRL956) from Montreal/Mirabel, QC (CYMX) to Val-dOr, QC (CYVO) performed a go-around and was re-routed to Rouyn-Noranda, QC (CYUY), as Runway 18 was closed due to an accident involving a MAG Aerospace Cessna 337G (C-FBRH) on a flight from Sudbury, ON (CYSB) to Val-dOr, QC (CYVO). (See CADORS 2020Q1183 for reference to accident)

A Propair Inc. Beech 1900D (C-FTQR/PRO4370) from Sudbury, ON (CYSB) to Rouyn-Noranda, QC (CYUY), advised that they had been targeted by a green laser while vertical over SALCN. Police were notified. No impact on operations.

Although the pilot of a Chrono Jet Inc. de Havilland DHC-8-106 (C-FGCP/MBK780) from Sudbury, ON (CYSB) to Montreal/St-Hubert, QC (CYHU) was advised that the preferred runway to arrive was Runway 24R, they continued for 06L. The wind was less than 05kt. No impact on the operation.

A Mag Aerospace Cessna 337G (C-FBRH) in Val-dOr, QC (CYVO) encountered a broken front wheel support when landing at 1525Z. Breakage and immobilization occurred on Runway 18. 1 person was on board, 0 injured. Impact on operations: Runway 18 was closed until 1630Z, so a Nolinor Aviation Boeing 737-200 (NRL956) from Montreal/Mirabel, QC (CYMX) to Val-dOr, QC (CYVO) performed a go-around and re-routed to Rouyn-Noranda, QC (CYUY).
Update: TSB Report #A20Q0044: C-FBRH, a Cessna 337G operated by MAG Aerospace Canada Corp., was on visual flight rules from Sudbury, ON (CYSB) to Val-dOr, QC (CYVO). When landing on Runway 18, a gust of wind helped bounce the aircraft onto the runway. Following the rebound, the aircraft struck the ground hard, causing the front landing gear to collapse, and causing the propeller to strike the ground. The aircraft came to a stop on the runway. The pilot, alone on board, was not injured. The aircraft sustained significant damage. The company temporarily suspended flights from its other Cessna 337G aircraft to conduct detailed inspections of the nose landing gears, and no anomalies were reported.

A Propair Beech 1900D (C-FTQR/PRO4370) from Rouyn-Noranda, QC (CYUY) to Sudbury, ON (CYSB) went back and landed due to an unspecified mechanical problem and did not declare an emergency. No impact on operations.

Aviation Incident Report #16835: A passenger submitted a complaint about a Bearskin Airlines Swearingen SW4 aircraft (BLS371) from North Bay, ON (CYYB) to Fort Francis, ON (CYAG), with en route stops at: Sudbury, ON (CYSB); Sault Ste. Marie, ON (CYAM); Thunder Bay, ON (CYQT); and Kenora, ON (CYQK). The individual reported that the aircraft was scheduled to depart CYYB at 1115Z. The aircraft taxied out to the runway, but rejected its takeoff, because it did not gain any thrust. The aircraft returned to the gate, where passengers exited the aircraft. Passengers departed CYYB on a different aircraft at 1218Z.

A fox was reported running across Runway 22 as a Biemann Crowle Aviation Inc. Cessna 172L (C-GBXP) from Sudbury, ON (CYSB) to Sault Ste. Marie, ON (CYAM) taxied for departure.

An Air Bravo Corp. Pilatus PC-12/45 (C-FTAB/AB229) from Thunder Bay, ON (CYQT) to Sudbury, ON (CYSB) reported a technical issue and returned to CYQT. No emergency was declared and no assistance was required.

An Air Bravo Corp. Pilatus PC-12/45 (C-FTAB/AB239) from Sudbury, ON (CYSB) to London, ON (CYXU) landed on Runway 27, and was instructed to exit via Taxiway Golf and contact ground control. The aircraft exited and continued to taxi on Golf. The aircraft checked in with ground control, abeam Apron II, while taxiing.

A Bearskin Airlines Swearingen SW4 (BLS809) from Sudbury, ON (CYSB) to Ottawa/Macdonald-Cartier, ON (CYOW) was observed taxiing across the main apron and entering Taxiway B prior to receiving the airport advisory or advising of its intentions. Two vehicles were operating on Taxiway B at the time.

The pilot of an ATE Capital Inc. Eurocopter EC120B (C-GTAZ) from Ottawa/Gatineau, QC (CYND) to Sudbury, ON (CYSB) made initial contact when 7NM from CYSB, entering the control zone (CZ). No operational impact.

A privately registered, amateur-built Vans RV-8 from Ottawa/Embrun, ON (CPR2) to Sudbury, ON (CYSB) made no mandatory call prior to entering the zone. The pilot called as they were entering the control zone (CZ), but did not respond to initial calls by the flight service station (FSS), and by the time they did, they were 2 statute miles (SM) within the zone. The Vans RV-8 flight path conflicted with circuit traffic for Runway 30 that was turning final. The Vans RV-8 conducted an immediate "straight-in" landing and a Mag Canada Cessna 172M (C-GWZV) from Sudbury, ON (CYSB) to Sudbury, ON (CYSB) extended their circuit, then completed a touch-and-go and on their next circuit, and landed safely with a full stop.

A privately registered Cessna 172L from Burlington, ON (CZBA) to Kitchener/Waterloo, ON (CYKF) was instructed to stay on the downwind. The aircraft turned base and cut off a Morningstar Partners Ltd. Embraer EMB-505 (C-FMPN) from Sudbury, ON (CYSB) to Kitchener/Waterloo, ON (CYKF) that was on final.

An Air Bravo Pilatus PC-12/45 (C-FKPI/AB1250) from Sudbury, ON (CYSB) to Wabush, NL (CYWK) reported an emergency locator transmitter (ELT) signal when climbing through A010, 10NM East of CYSB. The Joint Rescue Coordination Centre (JRCC) Trenton was advised. No operational impact.

Multiple aircraft reported a strong emergency locator transmitter (ELT) signal in the vicinity of Sudbury, ON (CYSB). All reports were from FL240-280. The reporting aircraft included a Jazz de Havilland DHC-8-402 (JZA8289) from Toronto/Lester B. Pearson, ON (CYYZ) to Timmins, ON (CYTS), an Ornge Global Air Inc. Pilatus PC-12/47E (PUL105) from Toronto/Billy Bishop, ON (CYTZ) to Thunder Bay, ON (CYQT), and an Air Bravo Pilatus PC-12/45 (AB1250). The Joint Rescue Coordination Centre (JRCC) Trenton was advised. No operational impact.

UPDATE: The source was subsequently determined to be a false alarm for a 2266837 Ontario Inc. Maule Air Inc. M-6-235 (C-FAUZ) that was in winter storage in the Grassy Lake, ON, area (46°20’44”N 081°23’34”W).

At Sudbury, ON (CYSB), at approximately 1600Z, a vehicle (ST21) was observed at the intersection of Runway 04/22 and Runway 12/30 without a clearance. No operational impact.

A privately registered Cessna 182G from Sudbury, ON (CYSB) to Gore Bay-Manitoulin, ON (CYZE) entered the information request (INREQ) stage. The Joint Rescue Coordination Centre (JRCC) Trenton was contacted by the flight information centre (FIC). JRCC Trenton located the pilot, safe and sound, at their destination.

A Jazz de Havilland DHC-8-300 (JZA8609) from Toronto/Lester B. Pearson, ON (CYYZ) to Sudbury, ON (CYSB) rejected its take-off and returned to the gate with an apparent mechanical issue. No operational impact.

A Jazz de Havilland DHC-8-300 (JZA8603) from Toronto/Lester B. Pearson, ON (CYYZ) to Sudbury, ON (CYSB) began its take-off roll without take-off clearance. No operational impact.
UPDATE: TSB Report #A20O0100: C-GABO, a de Havilland DHC-8-311 operated by Jazz, was conducting flight JZA8603 from Toronto/Lester B. Pearson, ON (CYYZ) to Sudbury (Greater Sudbury), ON (CYSB). While taxiing for departure at CYYZ, the control tower instructed JZA8603 to "line up Runway 05, airborne, left turn heading 360". JZA8603 read back the instructions to the tower. A minute later, JZA8603 called the tower to announce it was on the takeoff roll. Once airborne and climbing through 2000 feet, JZA8603 called the tower to confirm if it was cleared for takeoff. The tower indicated that JZA8603 never received an official takeoff clearance and mentioned they did not want to disrupt the roll, as everything was clear. There was no impact on operations, and no risk of collision.

Delayed entry as a result of NAV CANADA safety and evaluation quality assurance (QA) review: A Morningstar Air Express Cessna 208B (MAL8056) from Sudbury, ON (CYSB) to Timmins/Victor M. Power, ON (CYTS) advised the CYTS Flight Service Station (FSS) of a bird strike on approach for Runway 03. Three buntings were removed from the runway. No operational impact.

A privately registered Piper PA-28-180 from Sudbury, ON (CYSB) to Montreal/Les Cèdres, QC (CSS3) departed Runway 30 at 1629Z and immediately declared an emergency and loss of power. The aircraft went into a sharp left bank and landed on Runway 04 safely, with no impact to operations. The aircraft taxied off the runway under its own power.

A Bearskin Airlines Fairchild SA227-CC (C-GYTL/BLS384) on a flight from Thunder Bay, ON (CYQT) diverted to Sault Ste. Marie, ON (CYAM) due to smoke in the cockpit. The aircraft landed without further incident.
UPDATE: TSB Report #A20O0098: A Bearskin Airlines Fairchild SA227-CC (C-GYTL), flight BLS384, was on route from Thunder Bay, ON (CYQT) to Sudbury, ON (CYSB) and approximately 40NM North of Sault Ste. Marie, ON (CYAM) when the crew noticed smoke in the flight deck. The flight crew carried out the checklist, declared a "PAN PAN" and diverted to CYAM. The aircraft landed without further incident. Maintenance inspected the aircraft and found that a center pedestal back lighting circuit panel had failed.

On departure from Runway 22, the pilot of a Jazz de Havilland DHC-8-311 (C-FACF/JZA8606) from Sudbury, ON (CYSB) to Toronto/Lester B. Pearson, ON (CYYZ) reported a possible bird strike. A small bird carcass was found during the runway check. No operational impact.

The pilot of a Province of Ontario, Ministry of Natural Resources Eurocopter EC-130 B4 (C-FONM) from Sudbury, ON (CYSB) to Sudbury, ON (CYSB) failed to call 5 minutes prior to entering the zone. The first call from C-FONM was when the aircraft was 9 miles north, inbound at 2000 feet and below surveillance coverage, for continuous circuits. Although a Waterloo-Wellington Flying Club Diamond DA40 (C-GIXL) from Sudbury, ON (CYSB) to Timmins/Victor M. Power, ON (CYTS) was at 3500 feet at the time of the first call, they were 12 oclock by 2 miles in the opposite direction.

A Kississing Lake Lodge Ltd. Beech 100 (C-GTLS/WN16) from Geraldton, ON (CYGQ) to Sudbury, ON (CYSB) aborted its take-off due to bird activity.

After touching down, the pilot of a Thunder Airlines Limited Mitsubishi MU-2B-60 (C-GZNS/THU870) from Timmins/Victor M. Power, ON (CYTS) to Sudbury, ON (CYSB) said they may have hit a bird. Two snowbird carcasses were recovered after a runway inspection. Maintenance personnel advised that they would inform the flight crew.

At approximately 1858Z, an Air Bravo Corp. Pilatus PC-12 (AB237) from Sudbury, ON (CYSB) to Chapleau, ON (CYLD) called while taxiing for Runway 30. AB237 was requested to hold short of Taxiway A until a Jazz de Havilland DHC-8-402 (JZA8611) from Toronto/Lester B. Pearson, ON (CYYZ) to Sudbury, ON (CYSB) cleared. AB237 was observed approaching the entrance of Taxiway A. The pilot of AB237 was reminded that they said theyd hold short of Taxiway A. AB237 had to enter Taxiway A to turn around and let JZA8611 taxi in.

A Bearskin Airlines Fairchild SA227-DC (C-GJVC/BLS377) from Sudbury, ON (CYSB) to Sault Ste. Marie, ON (CYAM) reported a possible bird strike. The aircraft backtracked on the runway, but no carcass was seen.

At approximately 1650Z, a Province Of Ontario, Ministry Of Natural Resources Bombardier CL-215-6B11 (Series CL-415) (C-GOGX/TKR276) from Sudbury, ON (CYSB) to Sudbury, ON (CYSB) rejected its take-off from Runway 04 and returned to the apron. The pilot later reported via telephone that the aircraft had a technical issue.

Diamond DA40 (C-FTRM) from Timmins/Victor M. Power, ON (CYTS) to Killarney, ON (CPT2); a privately registered, amateur-built Weaver PA 12 Replica; and a Djenss Holding Corp. Piper PA-24 (C-FLUI) on a flight to Sudbury, ON (CYSB).

The pilot of a Jazz de Havilland DHC 8 300 (JZA8605) from Toronto/Lester B. Pearson, ON (CYYZ) to Sudbury, ON (CYSB) reported that an upset passenger was causing a disturbance, and requested that the police be called. No impact on operations.

An Air Bravo Corp. Pilatus PC-12 (AB239) from Sudbury, ON (CYSB) to Toronto/Billy Bishop, ON (CYTZ) departed from Runway 22 IFR and reported a conflict with an inbound VFR aircraft conducting a simulated area navigation (RNAV) approach to Runway 04.

A Panorama Helicopters Ltd. Bell 205A-1 (C-GHUM) from Sudbury, ON (CYSB) to Winnipeg/James Armstrong Richardson, MB (CYWG) transited the Kenora, ON (CYQK) control zone with no contact on the mandatory frequency (MF).

UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. After C-FXUS/BLS377 landed in Sault Ste. Marie, the tower requested they carry out a runway inspection for birds while taxiing to the ramp. One dead bird was seen on the runway. After shutdown, the flight crew carried out a walk around inspection of the aircraft, with no signs of a bird strike. It was determined the dead bird was from a previous flight.

Approximately 8NM Southwest of Sudbury, ON (CYSB), a Sudbury Mens Health Center Inc. Found Brothers FBA-2C1 (C-GSXM) from Sudbury, ON (CYSB) to Sudbury, ON (CYSB) reported a hydraulic pump failure. The pilot indicated they did not need assistance, and had a manual hand pump for the gear if required. They requested to perform a "low and over" to verify that the landing gear was down. C-GSXM conducted a "low and over" at 0014Z, and the gear appeared down and normal. The pilot elected to continue the local flight. C-GSXM arrived at 0050Z without further incident. No operational impact.

At 1619Z, an unknown aircraft was observed landing at Sudbury, ON (CYSB) on Runway 30. Prior to this, the flight service station (FSS) had queried the Toronto (YYZ) area control centre (ACC) about an intermittent target observed on the NAV CANADA Auxiliary Radar Display System (NARDS) that was close to and possibly inside the CYSB zone after a report from a transiting float plane of a somewhat close encounter with an unknown aircraft estimated to be about 200 feet above. On the ground, the pilot of the aircraft, a Waterloo-Wellington Flying Club Diamong DA40 (C-GIXL) from Kitchener/Waterloo, ON (CYKF) to Sudbury, ON (CYSB), advised over the phone that C-GIXL had lost power and communications, and they were instructed by their flight school via cellular phone to get down safely. C-GIXL suffered a flat tire on its nose wheel upon landing. A Province Of Ontario, Ministry Of Natural Resources de Havilland DHC-2 MK. III (C-FOEU) that was inbound for landing was asked to orbit on thefor Runway 22 until it could be determined what C-GIXL was going to do after landing.

A privately registered Bellanca 7GCAA on a flight form Orillia Rama, ON (CNJ4) to Sudbury, ON (CYSB) made its first call at 8 miles South, estimating arrival in 5 minutes. An Aquila Flying Club Erco 415-D (C-FEHX) from Sudbury, ON (CYSB) to North Bay, ON (CYYB) was departing Runway 22, eastbound, at the time.

At approximately 2035Z, a 7506406 Canada Inc. Augusta AW139 (LF5) on a MEDEVAC flight from New Liskheard (Temiskaming Hospital), ON (Heli) (CNV3) to Sudbury/Health Sciences North, ON (Heli) (CSL8) reported passing overhead of a Gyrocopter within 3 statute miles (SM) of Sudbury, ON (CYSB), under the approach for Runway 22. The flight service station (FSS) had no contact with the Gyrocopter, and no "no radio" (NORDO) arrangements had been made. An Air Bravo Corp. Pilatus PC-12 (AB237) from Muskoka, ON (CYQA) to Sudbury, ON (CYSB) elected to land on Runway 30 to avoid a potential conflict. At 2100Z, a Thunder Airlines Limited Mitsubishi MU-2B (THU840) from Toronto/Billy Bishop, ON (CYTZ) to Sudbury, ON (CYSB) reported visual passage of a Gyrocopter over the Stinson Dam Head pond. The Gyrocopter appeared to be exiting the control zone (CZ). The pilots of LF5 provided photos and video of the aircraft and its proximity to the airport, available upon request.

Upon departure from Runway 30, a Perimeter Aviation Fairchild SA227-AC (C-GYRL) on an IFR flight from Sudbury, ON (CYSB) aborted its take-off. The pilot declined emergency assistance and returned to the main apron.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. C-GYRL had a rejected take-off due to the right-hand (R/H) engine torque reduced from 105% during take-off roll. Maintenance carried out the following: the Continuing Airworthiness Web Information System (CAWI) tested for operation on ground checks serviceable. Engine controls inspected, lubed, ground runs completed, with CAWI on and off, and checked for MAX power, no faults found. A successful test flight was carried out and the aircraft was released for service.
Update TSB Report #A20O0064: Perimeter Aviation flight JV377, a Fairchild SA227AC (C-GYRL) aircraft, was departing Sudbury (CYSB), ON, when the right engine (Garrett TPE331-11U-612G) lost power just before V1. The flight crew rejected the takeoff and declared a PAN PAN. The aircraft did not require assistance and returned to the gate. The aircraft was conducting a continuous alcohol water injection (CAWI) when the power loss occurred. Maintenance inspected the aircraft, tested the CAWI system for correct operation and performed max power grounds. Maintenance was unable to find any problems with the engine or CAWI system. A successful test flight was carried out and the aircraft was returned to service.

A Mag Aerospace Canada Corp. Diamond DA 42 (C-GCSO) from Sudbury, ON (CYSB) to Sudbury, ON (CYSB) had an engine failure while circling overhead CYSB at A040. They requested priority, descended and landed on Runway 30 without incident. A privately registered Piper PA-28-140 from Lachute, QC (CSE4) to Sudbury, ON (CYSB) did an orbit at the edge of the control zone (CZ) on final for Runway 30 to allow spacing.

A Morningstar Air Express Cessna 208B (C-FEXX/MAL8056) from Sudbury, ON (CYSB) to Timmins/Victor M. Power, ON (CYTS) did not make any calls from initial check-in until landing.

Air traffic control (ATC) cleared a WestJet Encore de Havilland DHC-8-402 (C-FTEN/WEN3516) from London, ON (CYXU) to Toronto/Lester B. Pearson Intl, ON (CYYZ) to descend to 8000ft. WEN3516 read back clearance to 8000ft. WEN3516 started to descend to 6000ft. No operational impact.
UPDATE AOR-263516-V3: WEN3516 read back a clearance that had been issued to a Morningstar Air Express Cessna 208B (C-FEXO/MAL7056) from Sudbury, ON (CYSB) to Toronto/Lester B. Pearson, ON (CYYZ) to descend to 6000ft. Separation was not assured between WEN3516 and a Delta Air Lines Bombardier CL600-2424 (N297PQ/EDV5024) from Toronto/Lester B. Pearson, ON (CYYZ) to Detroit/Wayne County, MI (KDTW).

A Morningstar Air Express Cessna 208B (MAL7056) from Timmins/Victor M. Power, ON (CYTS) to Sudbury, ON (CYSB) declared a PAN PAN due to an instrumentation issue. MAL7056 landed at CYSB at 2057Z without incident. No impact to operations.
Update TSB Report #A20O0041: Morningstar Air flight MAL7056, a Cessna 208 B (C-FEXX), was halfway between Sudbury (CYSB), ON, and Timmins (CYTS), ON, when the pilot noticed that the engine (PRATT & WHITNEYCAN, PT6A-114A) torque indicator was stuck at 1400 psi. On approach, the pilot declared a PAN PAN due to the faulty torque indication. The aircraft landed without incident. Maintenance determined the torque gauge was faulty and replaced it with a serviceable unit.

A Flightexec Piper PA-31 (FEX100) from Hamilton, ON (CYHM) to Sudbury, ON (CYSB) was cleared to climb to 6000 feet and read back the same. FEX100 was observed climbing through 6000 feet and, when queried, descended back to 6000 feet. No conflict or operational impact.

A Propair Inc. Beech 1900D (PRO4376) from Sudbury, ON (CYSB) to Wawa, ON (CYXZ) departed CYSB and turned direct CYXZ.

The pilot of a Sault College of Applied Arts and Technology Zlin Z242L (C-FVWT) from Sault Ste. Marie, ON (CYAM) to Sault Ste. Marie, ON (CYAM) called the London Flight Information Centre (FIC) at 1316Z to file a VFR flight plan from CYAM back to CYAM. The destination was filed as Sudbury, ON (CYSB) in error.
