While on final approach, a Bearskin Airlines Fairchild SA227-DC (C-GJVO/BLS318) from Winnipeg/James Armstrong Richardson, MB (CYWG) to Red Lake, ON (CYRL) had a landing gear indication. On a fly-over, a staff vehicle advised that the gear appeared to be down. The aircraft landed safely.

Incident at WINNIPEG / JAMES ARMSTRONG RICHARDSON INTL MB (CYWG)

TSB Report #A20C0019: C-GJVO, a Fairchild SA227-DC aircraft operated by Perimeter Aviation as Bearskin Airlines flight 312, departed the Winnipeg/James Armstrong Richardson Intl. (CYWG), MB. for Red Lake (CYRL), ON with a crew of 2 and 5 passengers on board. Shortly after takeoff, the crew received a fire warning indication on the right engine. The crew decided to shut down the engine and return to CYWG. An emergency was declared and the crew then completed a successful single engine landing. An inspection of the aircraft by the operators maintenance revealed a faulty fire probe in the right engine bay area.

Incident at 5NM WINNIPEG / JAMES ARMSTRONG RICHARDSON INTL MB (CYWG)

A Bearskin Airlines Swearingen SA226-TC (BLS312) from Winnipeg/James Armstrong Richardson, MB (CYWG) to Red Lake, ON (CYRL) was observed on radar climbing through the cleared altitude of 6000 feet. The aircraft was queried to confirm their altitude and did not respond until after descending back to 6000 feet. No impact to operations.

Incident at 35NM WINNIPEG / JAMES ARMSTRONG RICHARDSON INTL MB (CYWG)

A Bearskin Airlines Swearingen SW4 (BLS312) from Winnipeg/James Armstrong Richardson, MB (CYWG) to Red Lake, ON (CYRL) reported a fire indication and advised it was returning to CYWG. The aircraft landed without further incident at 0133Z. No impact to operations.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. Metro 23 flight BLS312 had an air return due to an engine fire indication. The right fire light on fire extinguisher panel illuminated dimly. Engine was shut down and fire extinguisher was activated. The no. 2 fire detector probe (450°F) on the lower left side of the right-hand (R/H) side of the engine firewall was found to be defective. No. 2 fire detector probe was replaced and the R/H engine fire extinguisher bottle was replaced.

Superior Airways vehicle towed aircraft off runway with escort. Airfield technician and NAV CANADA communicated via radio for removal of aircraft. NOTAM was not issued. A scheduled Bearskin Airlines flight circled for 5 minutes until runway was clear.

A Perimeter Aviation Fairchild SA227-AC (C-FMAV/PAG220) on a flight from Deer Lake, ON (CYZV) to Sioux Lookout, ON (CYXL) departed CYVZ whereas their flight plan indicated North Spirit Lake, ON (CKQ3) as the departure point. The pilot advised the Winnipeg area control centre (ACC) controller of the change. The Winnipeg ACC controller cleared PAG220 from CYVZ direct to CYXL, the pilot read back the clearance as present position direct.

Update TSB #A19C0145: C-FKAL, a Douglas DC-3C-BT67 operated by North Star Air Ltd, was conducting a cargo flight from Red Lake (CYRL), ON to Sachigo Lake (CZPB), ON, with 2 flight crew members on board. While on approach to CZPB, the aircraft collided with terrain approximately 500 metres southwest of the threshold of Runway 10. The 2 flight crew members received no injuries. The aircraft sustained substantial damage. The TSB is investigating.

Incident at RED LAKE ON (CYRL)

A Perimeter Aviation Fairchild SA227-AC (C-GWVH/BLS318) on a flight from Winnipeg/James Armstrong Richardson Intl, MB (CYWG) to Red Lake, ON (CYRL) initiated a missed approach due to a gear indication. The aircraft then entered a VFR hold to troubleshoot the issue and landed safely at 1339Z. No impact on operations.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. Metro 3 C-GWVH had a gear indication issue on final into Red Lake. Upon gear down selection, the pilot got 2 green and 1 black indication. 30 seconds later, the #1 gear indicated green. Aircraft was ferried back to CYWG with gear down. Maintenance found the #1 gear down lock switches to be intermittent, and so replaced the switches.

Incident at 15NM N From CYAG - FORT FRANCES MUNI - FORT

TSB Report #A19C0123: C-GYUA, a Mitsubishi MU-2B-60 aircraft, operated by Thunder Airlines Limited, was conducting flight THU890 from Fort Frances Muni (CYAG), ON to Red Lake (CYRL), ON. At approximately 15 nautical miles North of CYAG, climbing through 7000 feet, the right hand fire warning light illuminated. The flight crew stopped the climb and carried out the engine fire checklist. The flight crew did not observe fire or smoke from the engine and a decided to return to CYAG with both engines operating. During the descent, the fire warning light extinguished and a flight medic reported smoke from the right hand engine. An emergency was declared and the right hand engine was shutdown. The aircraft landed safely in CYAG. The operators maintenance inspected the aircraft in CYAG and a ferry flight was subsequently completed to Thunder Bay (CYQT), ON. The operators maintenance then replaced the right hand fire detector control unit.

After passing SILVU, the aircraft was observed to make a left hand turn. When queried by the controller on what the next point on their route was, C-FRCI responded with ROTMA. ROTMA ceased to exist on the last navigational update. There was no conflict or operational impact.

A KBM Forestry Consultants Inc. Piper PA-31-350 (C-FCAR) from Red Lake, ON (CYRL) to Red Lake, ON (CYRL) relayed a "MAYDAY" from a helicopter on fire in the vicinity of Dryden, ON. The Joint Rescue Coordination Centre (JRCC) Trenton reported that the helicopter was a Zimmer Air Services Inc. Aerospatiale AS 355 F2 (C-GCZS). Aviation Operations (AVOPS) was notified. No operational impact.
Update TSB #A19C0069: C-GCZS, an Aerospatiale AS 355 F2 helicopter operated by Zimmer Air Services, was conducting forestry application 20 nm north of Dryden/Best Western (Heli) (CKV3), ON with only the pilot on board. During the flight, there was a loud bang, followed by a number 2 engine (Rolls Royce 250- C20R) chip light. The pilot reduced the power, and began to manoeuvre to a cut block where a safe landing could be made. Approximately 10 seconds later, the pilot noted that the fuel filter warning light had illuminated. The pilot elected to jettison 150 litres of spray, and informed the company by radio that an emergency landing would be conducted. Approximately 30 seconds later, the engine fire warning light illuminated. Another helicopter in the area reported to the pilot that the aircraft was on fire. A safe landing was conducted, and the pilot exited the helicopter uninjured. The aircraft was subsequently destroyed by fire. Initial inspection revealed that the number 2 engine had sustained an uncontained failure of the 3rd and 4th stage power turbine blades.

UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. A Metro 3, C-GYRL departing Red Lake Ontario to YWG declared an emergency and subsequently cancelled the emergency. The Crew resolved the issue by properly setting the speed levers.

A Perimeter Aviation Fairchild SA227-DC (C-GJVW/BLS362) from Red Lake, ON (CYRL) to Sioux Lookout, ON (CYXL) declared an emergency with a right engine fire indication light warning. Emergency services were contacted and they continued the approach on to Runway 16. The aircraft landed without incident and there was no other impact on operations.
Update TSB #A19C0058: C-GJVW, a Fairchild SA227-DC aircraft operated by Perimeter Aviation (dba Bearskin Airlines), was conducting flight BLS362 from Red lake (CYRL), ON to Sioux Lookout (CYXL), ON with 2 crew members and 4 passengers on board. During cruise flight, the flight crew received a number 2 engine fire light on the fire panel. The engine operating parameters appeared normal, so the flight crew decided to keep the engine running and continued to destination. An emergency was declared, and the aircraft landed without further incident. After the landing, the flight crew stopped the aircraft on the runway and shut the number 2 engine down. The aircraft taxied to the apron, and there was no indication of any smoke or fire. A subsequent inspection of the system by the operators maintenance revealed loose terminal connections to the oil cooler fire detector. The loose terminals were repaired, and the aircraft was returned to service.
